I'm guessing the doctor has a few methods (outside of the debt) of preventing Axel from strangling him. Either having a bomb implanted inside of Axel that will blow up if he tries anything hostile towards the doctor. Or, Axel is only able to stay alive thanks to a serum/chemical mixture that only the doctor can provide, without which he will die.

Why yes they do! There’s constant surveillance over Axel & there are small devices in his brain and ears that give off a high pitched sound & shock him. There’s basically a taser in his head. The doctor & staff members all have buttons to shock Axel if he’s acting with ill intent. If he’s acting extra bad the bands on his wrists will immobilize him or staff will tranquilize him.

Xander (The Doctor) knows Axel’s impulsive and nihilistic, so they’d rather torture him than threaten to kill him. Besides, why would they kill their most prized experiment?

Is axel's arm from an orca?

I’ve actually been on and off on whether orca people should exist or not in the bad lands world, it’s been leaning more towards no, but anyway-

The arm belonged to a great white shark who Axel knew for a short time.

Ruthless, stubborn yet cunning is Axel, a well known fighter for an illegal underwater arena. The blood sport had led to his death at 17, but a mad doctor brought him back to life & landed him in massive amounts of debt. He wants nothing more but to be free & probably strangle the doctor.
