Even when he was only 7 months old, Adam (AJ) Stapleton’s mother, Jennifer Davis, noticed her son was leaning to one side with one shoulder lower than the other. By age 2, AJ was visibly rocking from side to side as he walked. That year, he was diagnosed with infantile congenital scoliosis. The principal suggested Davis call the hospital for an appointment. “I made the call, and three days later we had our appointment,” said Davis. “He had an MRI, and when Chief of Staff Henry Iwinski, M.D., saw the results, he placed an arm on AJ’s shoulder and said, ‘I believe I can help you.’ We have been on the road to hope ever since we heard those words.”
AJ was placed in halo traction, which helps manipulate bones so they will start growing in a straighter pattern. Halo traction is a method used in severe cases of scoliosis to slowly and gently stretch the soft tissues connected to the spine in order to obtain some correction of the curve before surgery. When AJ started the traction, his scoliosis was at 150 degrees, but after several weeks the traction had decreased his curve to 80 degrees.
“AJ has been such a pleasure to work with,” said his physical therapist, Shelly Ryan. “He has been very motivated by his progress during the process and is looking forward to seeing his friends at school with his 'new’ look. This is a life-changing event for AJ, and I am proud to be here to help him.”
Shared Experiences Following the halo traction, AJ, who is 15 now, had two surgeries scheduled for September 2013. The two surgeries would correct his spinal curve from the front (an anterior fusion), and later correct the curve from the back (posterior fusion). AJ was nervous about his upcoming procedures until he met fellow scoliosis patient, 20-year-old Ethan Hensley. Ethan had the same surgery several years ago and was at the hospital for a short stay. Ethan was able to give AJ insight about what to expect with the surgery and advice on how to recuperate after the procedure. “They really connected, and Ethan was such a blessing for AJ,” Davis said. “They plan to stay in touch after AJ leaves the hospital.”
On September 10, 2013, two months after his first conversation with Dr. Iwinski, AJ had a successful spinal fusion. The halo was removed, and he wore a brace for six weeks. After six weeks of homeschooling, AJ was anxious to see his classmates, who immediately started planning a “Walk for AJ,” with the proceeds to be donated to Shriners Hospitals for Children. “I plan to walk with my classmates the entire way,” AJ said, “ and thanks to my surgery, I can.”

An Introduction to Lateral Lumbar Interbody Fusion Procedures

Based in Silicon Valley, California, SpineEX, Inc. is an innovative medical device company specializing in minimally invasive implants and tools for spinal fusion surgeries. In March 2019, SpineEx Inc. launched the commercialization of one of its products, the Sagittae lateral interbody lumbar device used during lateral lumbar interbody fusion (LLIF) procedures. The LLIF procedure is a relatively new, minimally invasive technique for spinal fusion surgeries. Unlike other surgical methods that access the spine from the back (posterior lumbar interbody fusion) and front (anterior lumbar interbody fusion), LLIF approaches the spine from the side, reducing lower back muscle disruptions. The procedure is also associated with reduced neural damage and less blood loss. During the procedure, a patient who has been administered anesthesia lies on her side. A surgeon makes two incisions on her side—one for a tubular retractor and the other for a guiding probe. The tubular retractor allows the surgeon to gently push aside muscles to reveal the area where the surgery will be done. The probe guides the surgeon during the procedure alongside intraoperative imaging like fluoroscopy. LLIF is primarily performed on people who have herniated discs, degenerative disc disease, degenerative scoliosis, and spondylolisthesis (when one vertebra overlaps another). Once performed, the procedure can relieve pressure on the spine, and stabilize it. These are both beneficial results for patients with spinal injuries, especially since the lower back carries most of the body’s upper weight.

Foot and Ankle Surgery

The foot and ankle are intricate musculoskeletal structures. They are extremely vulnerable to injuries.
Metatarsal surgery
Metatarsal surgery, also known as osteosynthesis, is a procedure that addresses problems with your feet’s five metatarsal bones (long bones behind the toes). When foot or toe pain does not respond to non-invasive treatment, such as physical therapy or therapeutic foot supports, you may need a surgical intervention.
In most cases, your podiatrist makes a small incision in the front of your foot and cuts and repositions your metatarsal bone to fix it. A metal pin or screw is used to hold your bone in the corrected position until it heals.
Typically, after the surgery, your foot is put into a cast or splint. You may need to use crutches or other aids for a few weeks to keep weight off the foot and prevent pressure from causing the pins to shift.
Morton’s neuroma
A Morton’s neuroma occurs when the tissue next to a nerve thickens and is irritated by pressure. This may happen because of the way you walk or by wearing certain shoes that put pressure on your feet.
Symptoms often start with burning or tingling in your foot or toes. They may get stronger as the neuroma gets bigger, and pain may last for days or weeks at a time.
If you have a tingling, burning or numbness sensation in your foot or toes, it is important to see a doctor. If left untreated, it can lead to permanent numbness in the affected area.
Treatments for Morton’s neuroma typically include pain-numbing medications, cortisone injections, and sometimes surgery. Nonsurgical treatments usually provide some relief in about 50% of patients with this condition.
Tibialis posterior tendon surgery
If your ankle pain is due to a problem with the posterior tibial tendon, you may be referred for surgery. This operation can relieve your pain and restore the normal shape of your foot.
The tibialis posterior tendon is the main tendon that supports the arch of your foot. If the tendon becomes inflamed or torn, it can no longer provide support for your arch and you will develop a flat foot (pes planovalgus).
A doctor can treat your tibialis posterior tendon condition with a variety of methods, including nonsurgical treatments. This can include orthotics and braces to support your foot.
Occasionally, surgery is required when nonsurgical treatment has been ineffective or the condition worsens. This often involves a tendon transfer and bone reshaping.
During your surgery, your doctor will make a small incision through the skin and muscle of your lower leg. He or she will then use small tools and a tiny camera to remove the damaged part of your tibialis posterior tendon. Then, your doctor will replace the injured portion with another tendon or a synthetic tissue from your calf.
Ankle replacement
If your ankle pain does not improve with conservative treatment options such as exercise, footcare or drug therapy, surgery may be an option. However, your podiatric foot and ankle surgeon will conduct a number of tests before recommending the surgery to you.
During ankle replacement surgery, your doctor makes an incision over the front of your ankle. The muscles are retracted and tendons and ligaments moved away to expose the damaged parts of your tibia, fibula and talus bones.
The damaged parts of the bones are removed and replaced with artificial joints or prostheses. A bone graft is then inserted between the tibia and fibula to create a fusion of the two bones and prevent loosening of the artificial ankle.
The surgery can be performed under general anesthesia or with a nerve block. Recovery takes several weeks and includes using crutches for walking. You are often referred to a physical therapist to regain movement of your new ankle.
