“The subject is restrained in the standard position on a cooled down-draft dissection table as previously described in earlier reports. Standard position for our purposes refers here to the extended and splayed positions of the arms and legs, with the head facing forward.

Unfortunately, early displays of resistance by the subject have resulted in minor bruising of the skin surrounding the wrists and ankles. Additional padding has been applied to these restraints to limit damage caused by voluntary and involuntary action of the limbs during the procedure.

Before beginning the main procedure we will prepare the subject by performing a devocalization. Devocalization is controversial in this area of study, but it is the opinion of this investigator that the reduction of undesirable and distracting vocalizations results in a superior quality of dissection.

Devocalization will be performed via the oral approach. This approach was selected to prevent disruption of the external tissues surrounding the larynx which may be dissected as part of this investigation. Two Molt-type mouth gags will be used to keep the subject’s mouth open. Electrical stimulation to the mandibular branch of the trigeminal nerve may be utilized to facilitate the opening of the jaw for placement of the gags. A standard tongue depressor can be used by an assistant to restrain the tongue ventrally. Electrocautery is utilized to trim away the vocal folds. Electrocautery is superior to standard surgical techniques as it prevents excessive bleeding into the airway which the subject could aspirate. Aspiration pneumonia is the most common cause to abort the study before completion so steps should be taken to avoid this at all costs.

During devocalization care should be taken not to remove the vestibular folds. These “false vocal cords” are responsible for some types of vocalization which the investigator may find useful in providing feedback from the subject. These vocalizations include growling (not useful), screaming (variably useful), and sonorous vocalizations (colloquially “moans and groans”; this is the most useful of all vocalizations maintained by the vestibular folds).

With the devocalization procedure complete the dissection can be performed. There is debate about the appropriate method of dissection as two methods have been described. The older method involves layer by layer dissection so that the body is dissected in its entirety layer by layer, from skin to subcutaneous tissues to muscle layers to veins and nerve fibers, on and on. The newer method involves performing the dissection “part by part” so that the entirety of one limb or part is fully dissected to its limits before the next part is explored. Commonly this dissection process begins with the limbs and ends with the chest cavities for reasons which may or may not be obvious to the junior investigator.

As for this investigator, I continue to review both methods and am currently unsure which would be of superior quality and value to this particular investigation. I may defer to the advice of my colleagues.”
