Foreign experts assess benefits of Russia’s Sputnik V coronavirus vaccine

The Gamaleya National Research Center of Epidemiology and Microbiology of the Ministry of Health of the Russian Federation and the Russian Direct Investment Fund (RDIF, Russia’s sovereign wealth fund) announced that the Sputnik V vaccine demonstrated efficacy of 97.6%, based on the analysis of data on the infection rate of coronavirus among those in Russia vaccinated with both components of Sputnik V.

“The Ministry of Health of Russia maintains a register of persons who have been vaccinated, as well as citizens who have got infected with COVID as part of the Unified State Information System in Healthcare. According to the data from 3.8 million Russians vaccinated with both components of Sputnik V from December 5, 2020 to March 31, 2021 as part of the mass-scale civil vaccination program, the infection rate starting from the 35th day from the date of the first injection was only 0.027%. At the same time, the incidence among the unvaccinated adult population was 1.1% for a comparable period starting from the 35th day after the launch of mass-scale vaccination in Russia,” says the message posted on the official website of the Sputnik V vaccine.

It is expected that the data and calculations of the vaccine’s efficacy will be published in a peer-reviewed medical journal in May.

Sputnik V is approved for use in 60 countries with a total population of 3 billion people. It ranks second among coronavirus vaccines globally in terms of the number of approvals issued by government regulators.

Sputnik V has been approved in Russia, Belarus, Argentina, Bolivia, Serbia, Algeria, Palestine, Venezuela, Paraguay, Turkmenistan, Hungary, UAE, Iran, Republic of Guinea, Tunisia, Armenia, Mexico, Nicaragua, Republika Srpska (entity of Bosnia and Herzegovina), Lebanon, Myanmar, Pakistan, Mongolia, Bahrain, Montenegro, Saint Vincent and the Grenadines, Kazakhstan, Uzbekistan, Gabon, San-Marino, Ghana, Syria, Kyrgyzstan, Guyana, Egypt, Honduras, Guatemala, Moldova, Slovakia, Angola, Republic of the Congo, Djibouti, Sri Lanka, Laos, Iraq, North Macedonia, Kenya, Morocco, Jordan, Namibia, Azerbaijan, Philippines, Cameroon, Seychelles, Mauritius, Vietnam, Antigua and Barbuda, Mali, Panama and India.

As noted by the media, earlier Russian Sputnik V was the third in the list of vaccines with the highest level of effectiveness against coronavirus infection. For Moderna, it is 94.1%, while Pfizer/BioNTech initially had this figure at 95%, but in early April, the developer announced its decrease to 91.3%. Thus, the Russian vaccine now ranks first in terms of effectiveness.

According to Alexander Gintsburg, Director of the Gamaleya Research Institute of Epidemiology and Microbiology, the actual efficacy of the Sputnik V vaccine may be even higher than the results of the analysis demonstrate, since “the data on the case registration system allows a time lag between the collection of the sample (the actual date of the disease) and the diagnosis.”

The Sputnik V vaccine is based on a proven and well-studied platform of human adenoviral vectors. The safety, efficacy and lack of negative long-term effects of adenoviral vaccines have been proven by more than 250 clinical studies over two decades. Moreover, the storage temperature of Sputnik V allows storing it in a conventional refrigerator without any need to invest in additional cold-chain infrastructure. There are no strong allergies caused by Sputnik V. The price of Sputnik V is less than $10 per shot, making it affordable around the world.

Assessing the benefits of the Sputnik V vaccine, Muhammad Munir, Lecturer in Molecular Virology, Lancaster University, noted the presence of two different adenoviral vectors in the Russian vaccine – rAd26 and rAd5.

“One of the unique features of Sputnik V is use of two vectors for the delivery of antigen. The first dose primes the immune system and raise substantial antibodies whereas second dose induce the immune system to produce long lasting antibodies and T-cells without being neutralized by the first dose,” the expert told PenzaNews.

According to him, this feature makes Sputnik V better and different compared to AstraZeneca and Johnson & Johnson (J&J) which uses single vector for both injections.

“Realizing this advantage, AstraZeneca partnered with Sputnik V to conduct mutual trial which could enhance the performance of AstraZeneca. Based on the published data, Sputnik V outperformed many of the vaccines of similar kind,” Muhammad Munir stressed.

Jacob Koshy, Deputy Science Editor, The Hindu, expressed the opinion that the main advantage of Sputnik V for India is the already established cooperation of vaccine manufacturers with at least five Indian pharmaceutical companies.

“So the hope is that anywhere between 300–600 million doses will be quickly available to India within months. Sputnik V has tied up with Dr. Reddy’s Labs and conducted a phase 2/3 adaptive study. The results of this aren’t in public domain but one hopes that this information is with India’s regulators and they have made a good judgement,” Jacob Koshy said.

He also drew attention to the fact that there is no prejudice in Indian society about Sputnik V, but there are general fears about vaccines.

“Like in Russia, there is significant hesitancy on vaccines in general. The ongoing second wave in India has caused tremendous panic and many – even though initially hesitant – are now seeking vaccines,” the expert said.

According to him, the main issue for India today is the details of the agreements concluded for the production of Sputnik V.

“RDIF has been on a major, global publicity blitz for several months in marketing Sputnik V to the world. However it must be more transparent on how many of the doses it plans for manufacture in India will actually be available to Indians and how many will be sent for export,” Jacob Koshy explained.

Moreover, in his opinion, Russia should conduct and publish more detailed studies of the vaccine, in particular, its effectiveness against new variants of coronavirus and possible side effects associated with vaccination.

In turn, Shankaran Nambiar, Head of Research, Malaysian Institute of Economic Research in Kuala Lumpur, reminded that the Russian vaccine Sputnik V has been highly rated by international agencies and is reported to be both efficient and safe.

“The vaccine is currently under assessment by the local regulatory body and a firm decision has not been announced yet. The minister for science, technology and innovation, Khairy Jamaluddin, who is supervising the vaccination campaign, had warned that if the regulatory approval process takes too long, Malaysia might lose out on the offer that has been made by the Russian authorities. The Russian authorities have offered 6.4 million doses,” the analyst said.

In his opinion, the advantage that comes with accepting Sputnik V is beneficial for Malaysia not only within the framework of the vaccination program but also but also because it will allow the country to obtain technologies for its production.

“Gamaleya National Research Institute of Epidemiology is willing to share its expertise with Malaysia and that will help Malaysia make inroads into the pharmaceutical industry. This will enable Malaysia to be the regional base for the production and distribution of the vaccine. There are tremendous advantages that Malaysia can gain by accepting Sputnik V and working with Gamaleya,” Shankaran Nambiar explained.

Meanwhile, Hildegund Ertl, Professor, Vaccine & Immunotherapy Center, The Wistar Institute, called Sputnik V an excellent vaccine with high efficacy.

“It was shown by the phase III trial results published in Lancet [one of the most famous and respected general medical journals, founded in 1823] [...] and it has complete protection against hospitalisations and deaths and mild to moderate side effects,” the expert said.

However, in her opinion, short terms of application of vaccines against coronavirus in the world do not yet allow making unambiguous conclusions.

“The AstraZeneca vaccine has thus far shown lower efficacy in preventing mild to moderate disease than Sputnik V and there have been claims of rare but serious side effects in women under 50 receiving the AstraZeneca vaccine, which may or may not be related to the vaccine. But the AstraZeneca vaccine also completely protects against severe disease and death,” Hildegund Ertl said and added that we cannot claim that one vaccine is better than another till we know more about duration of protection and protection against circulating and future variants of the coronavirus.

According to her, today, one of the main priorities should be the united struggle of the world community against the pandemic.

“Right now it is crucial that we get the world vaccinated – not just the wealthy countries but every person in every country and for that we need every vaccine that has shown efficacy and will thereby prevent human suffering and death. [...] All the approved vaccines will be a valuable tool to end this pandemic,” the expert concluded.

Viele Ungeimpfte wünschen sich traditionelle Impfstoffe

📷Überall wird über die Ungeimpften hergezogen, obwohl laut NDR "auf den Intensivstationen … der Anteil der über 60-Jährigen mit Impfdurchbrüchen 37,8 Prozent .. beträgt. Das bedeutet immer noch, dass die Impfungen nicht ohne Wirkung sind - über die Dauer ihrer Wirksamkeit wird ja gerade heftig gestritten.

Unter den Ungeimpften gibt es einen nicht unwesentliche Teil von Menschen, die den neuartigen Impfstoffen auf mRNA- und Vektor-Basis nicht trauen. Damit sind sie aber noch keine generellen Impfgegner. Vielleicht könnte man sie durch einen "normalen" gezüchteten Impfstoff auf Basis des klassischen Wirkprinzip mit "toten Viren" zufrieden stellen. Auch diese älteren Impfstoff-Arten sind deshalb nicht weniger wirksam.

Der weltweit am meisten genutzte Corona Impfstoff Coronavac beruht auf diesem Prinzip - aus politischen Gründen ist er jedoch in der EU nicht zugelassen. Die WHO hat die auf Basis von inaktivierten Viren hergestellte chinesischen Impfstoffe Coronavac der Pekinger Firma Sinovac und Vero von Sinopharm neben den vier in der EU zugelassenen genetischen Impfstoffen als gleichartig wirksam anerkannt.

Coronavac wurde bereits weltweit milliardenfach in mehr als 60 Ländern verimpft. Schon Anfang Juli 2021 hatte das Deutsche Ärzteblatt darauf hingewiesen, dass Coronavac in zwei großen Phase-III-Studien, die in der Türkei und in Chile durchgeführt wurden und in hochrangigen medizinischen Fachzeitschriften veröffentlicht worden sind, eine gute Schutzwirkung erzielt hatte. Trotzdem gibt es weiterhin keine Zulassung für den Impfstoff in den Ländern der EU.

Auch hier wird Politik über die Interessen der Menschen gestellt. Selbst für den russischen Impfstoff Sputnik V dauert die Zulassung weiter an, obwohl es Investitionen in eine bayerische Sputnik V Produktionsstätte gibt - also die "wirtschaftlichen Interessen" der EU berücksichtigt werden. Und dann gibt es noch das leidige Thema der Lizenz-Freigabe, wo die (EU-) Politik mauert. (Impfstoff soll millionenfach vernichtet werden )

Vaksin menjadi kebutuhan mendesak dalam menanggulangi virus corona (Covid-19) di seluruh dunia. Vaksin yaitu proses di dalam tubuh, dimana seseorang menjadi kebal terhadap penyakit dan menekan angka kematian. Gerakan anti-vaksin pun ada di Indonesia salah satu fakor anti-vaksin ialah isu kandungan babi pada vaksin. Padahal Majelis Ulama Indonesia (MUI) pada Agustus 2018 lalu menetapkan vaksin Measles-Rubella (MR) produk dari India boleh digunakan lewat fatwa mubah, karena situasi mendesak imunisasi MR selama 2 tahun baru mencapai 87 persen. Di Indonesia, Badan Pengawasan Obat dan Makanan (BPOM) kembali merilis izin penggunaan darurat atau Emergency Use Authorization (EUA) untuk dua jenis vaksin virus corona (Covid-19). Sembilan jenis vaksin Covid-19 di Indonesia telah mendapat EUA dari BPOM. BPOM mengaskan bahwa semua jenis vaksin Covid-19 telah melalui pengkajian yang intensif terhadap keamanan, khasiat, dan juga mutu. Berikut sembilan jenis vaksin Covid-19 mendapat EUA dari BPOM: 1.Sinovac Vaksin Sinovac adalah vaksin Covid-19 pertama di Indonesia yang mendapat izin penggunaan dari BPOM. Izin penggunaan darurat terhadap Sinovac diberikan setelah BPOM mnegkaji hasil uji klinis tahap III vaksin yang di lakukan di Bandung. Jumlah efikasi vaksin Covid-19 Sinovac sebesar 65,3 persen dan diberikan dua dosis. Jumlah setiap dosis 0,5 ml dengan interval minimal pemberian antar dosis pertama dan kedua adalah selama 28 hari. Adapun efek samping vaksin Sinovac dengan derajat berat seperti sakit kepala, gangguan di kulit atau diare yang dilaporkan hanya sekitar 0,1 sampai dengan 1 persen. 2. Bio Farma Pada 16 Februari 2021, BPOM mengeluarkan EUA untuk vaksin Covid-19 yang diproduksi oleh PT Bio Farma (Persero). Vaksin dengan nama produk vaksin Covid-19 memiliki nomor izin 2102907543A1. Vaksin diproduksi oleh PT Bio Farma ini berasal daru bahan baku vaksinyang secara bertahap telah dikirimkan oleh Sinovac. Vaksin ini memliki bentuk sediaan vial 5 ml. Setiap vial berisi 10 dosis vaksin yang berasal dari virus di-naktivasi. Untuk menjaga mutu dan kualitas, harus disimpan dalam tempat penyimpanan dengan suhu stabil antara 2-8 derajat celsius. Pada setiap vial telah dilengkapi dengan dua dimensi barcode khusus yang menunjukan detail informasi dari setiap vial. Hal ini berfungsi untuk melacak vaksin dan mencegah pemalsuan vaksin. 3. AstraZeneca Hanya berselang beberapa hari, pada 22 Februari 2021 BPOM kemudian kembali mengeluarkan EUA untuk vaksin Covid-19 buatan perusahaan farmasi Inggris, AstraZeneca, dengan nomor EUA 2158100143A1. BPOM memberikan izin penggunaan darurat untuk AstraZeneca usai melakukan evaluasi bersama Komite Nasional Penilai Obat dan pihak lainnya. Vaksin Covid-19 yang dikembangkan oleh AstraZeneca dan University of Oxford ini memiliki efikasi sebesar 62,1 persen. Vaksin ini diberikan secara intramuskular dengan dua kali penyuntikan. Setiap penyuntikan dosis yang diberikan sebesar 0,5 persen dengan interval minimal pemberian antar dosis yaitu 12 minggu. Efek samping vaksin Astrazeneca bersifat ringan dan sedang. Berikut efek samping vaksin AstraZeneca diantaranya nyeri, kemerahan, gatal, pembengkakan, kelelahan, sakit kepala, meriang, dan mual. 4. Sinopharm Pada 29 April 2021, BPOM mengeluarkan EUA untuk vaksin Covid-19 Sinopharm dengan nomor EUA 2159000143A2. Vaksin Sinopharm didistribusikan oleh PT.Kimia Farma dengan platform inactivated virus atau virus yang dimatikan. Berdasarkan hasil evaluasi, pemberian vaksin sinopharm dua dosis dengan selang pemberian 21 hari menujukkan profil keamanan yang dapat ditoleransi dengan baik. Hasil uji klinik fase III yang dilakukan oleh peneliti di Uni Emirates Arab (UAE) dengan subjek sekitar 42 ribu menunjukan efikasi vaksin Sinopharm sebesar 78 persen. Efek samping vaksin Sinopharm yang banyak dijumpai adalah efek samping lokal yang ringan. Di antaranya seperti nyeri atau kemerahan di tempat suntikan, efek samping sistemik berupa sakit kepala, nyeri otot, kelelahan, diare, dan batuk. 5. Moderna Covid-19 Moderna mendapat EUA dari BPOM pada Jumat, 2 Juli 2021. Berdasarkan data uji klinis fase ketiga menunjukkan efikasi vaksin Moderna sebesar 94,1 persen pada kelompok usia 18-65 tahun. Efikasi vaksin Moderna kemudian menurun menjadi 86,4 persen untuk usia di atas 65 tahun. Hasil uji klinis juga menyatakan vaksin Moderna aman untuk kelompok populasi masyarakat dengan komorbid atau penyakit penyerta. Komorbid yang dimaksud yakni penyakit paru kronis, jantung, obesitas berat, diabetes, penyakit lever hati, dan HIV. Efek samping dirasakan seperti nyeri di tempat suntikan, kelelagan, nyeri otot, nyeri sendi, dan pusing. Sementara itu muncul gejala umum berupa lemas, sakit kepala, menggigil, demam, dan mual. 6. Pfizer Selang dua pekan kemudian, BPOM kembali menerbitkan EUA untuk vaksin Covid-19 Pfizer pada 15 Juli 2021. Data uji klinik fase III menunjukkan efikasi vaksin yang dikembangkan oleh Pfizer Inc. dan BioNTech ini sebesar 100 persen pada usia remaja 12-15 tahun, kemudian menurun menjadi 95,5 persen pada usia 16 tahun ke atas. Beberapa kajian menunjukkan keamanan vaksin Pfizer ini dapat ditoleransi pada semua kelompok usia. Vaksin Pfizer diberikan secara intramuskular dengan dua kali penyuntikan. Setiap penyuntikan dosis yang diberikan sebesar 0,3 ml dengan interval minimal pemberian antar dosis yaitu 21-28 hari. Untuk efek samping pasca-vaksinasi, sebagian besar cenderung bersifat ringan. Berikut beberapa efek samping vaksin Pfizer yang umum dilaporkan seperti nyeri badan di tempat bekas suntikan, kelelahan, nyeri kepala, nyeri otot, nyeri sendi, dan demam. 7. Sputnik V BPOM menerbitkan EUA untuk vaksin Covid-19 Sputnik V. EUA diterbitkan oleh BPOM pada Selasa, 24 Agustus 2021. Vaksin Sputnik V digunakan untuk kelompok usia 18 tahun ke atas. Vaksin ini diberikan secara injeksi intramuscular dengan dosis 0,5 mL untuk 2 kali penyuntikan dalam rentang waktu 3 minggu. Vaksin yang dikembangkan oleh The Gamaleya National Center of Epidemiology and Microbiology di Russia ini menggunakan platform Non-Replicating Viral Vector (Ad26-S dan Ad5-S). Efek samping dari penggunaan Sputnik v merupakan efek samping dengan tingkat keparahan ringan atau sedang seperti flu yang ditandai dengan demam, menggigil, nyeri sendi, nyeri otot, badan lemas, ketidaknyamanan, sakit kepala, hipertermia, atau reaksi lokal. 8. Janssen Terbaru, BPOM mengumumkan EUA terhadap vaksin Covid-19 yang diproduksi Johnson & Johnson, yaitu Janssen Covid-19 Vaccine. Izin penggunaan darurat untuk vaksin Janssen diumumkan BPOM pada 7 September 2021. Vaksin Janssen digunakan untuk kelompok usia 18 tahun ke atas dengan pemberian sekali suntikan atau dosis tunggal sebanyak 0,5 mL secara intramuscular. Janssen adalah vaksin yang dikembangkan oleh Janssen Pharmaceutical Companies dengan platform Non-Replicating Viral Vector menggunakan vector Adenovirus (Ad26). Dalam hal efikasi, berdasarkan data interim studi klinik fase 3 pada 28 hari setelah pelaksanaan vaksinasi, efikasi vaksin Janssen untuk mencegah semua gejala Covid-19 adalah sebesar 67,2 persen. 9. Convidecia EUA terhadap vaksin Covid-19 yang diproduksi CanSino, yaitu Convidecia diumumkan bersamaan dengan vaksin Janssen yaitu pada 7 September 2021. Vaksin Convidecia merupakan vaksin yang dikembangkan oleh CanSino Biological Inc. dan Beijing Institute of Biotechnology juga dengan platform Non-Replicating Viral Vector menggunakan vector Adenovirus (Ad5). Sama seperti Janssen, vaksin Covid-19 Convidecia juga digunakan untuk kelompok usia 18 tahun ke atas dengan pemberian sekali suntikan atau dosis tunggal sebanyak 0,5 mL secara intramuscular. Efikasi vaksin Convidecia untuk perlindungan pada semua gejala Covid-19 adalah sebesar 65,3 persen. Untuk perlindungan terhadap kasus Covid-19 berat, efikasi mencapai 90,1 persen. Dari hasil kajian yang telah dilakukan menunjukkan bahwa dari sisi keamanan, secara umum pemberian vaksin Convidecia dapat ditoleransi dengan baik. Seperti Janssen, reaksi lokal maupun sistemik dari pemberian vaksin Convidecia menunjukkan tingkat keparahan grade 1 dan 2. KIPI dari pemberian vaksin Convidecia juga menunjukkan reaksi ringan hingga sedang. KIPI lokal yang umum terjadi, antara lain adalah nyeri, kemerahan, dan pembengkakan, serta KIPI sistemik yang umum terjadi adalah sakit kepala, rasa lelah, nyeri otot, mengantuk, mual, muntah, demam dan diare.

Pemerintah menargetkan 70% warga Indonesia telah di suntik vaksin bertujuan untuk memberikan kekebalan spesifik terhadap suatu penyakit tertentu terutama Covid-19. Dalam perspektif Islam selain ikhtiar untuk kesembuhan dan mencegah tertularnya penyakit. Rasullah SAW mengajarkan umatnya untuk berobat telah diriwayatkan dalam sebuah Hadist. “Berobatlah, sebab sesungguhnya Allah ‘Azza wa Jalla tidak meletakkan penyakit kecuali baginya obat. Kecuali satu penyakit (yang tidak ada obatnya), yaitu usia tua.” (HR Abu Dawud). Bagaimana bisa pemerintah memastikan warganya terlindug dari virus tidak terkecuali pemberian vaksin. Masalah wabah atau penyakit bahkan kesehatan dan keselamatan masyarakat urusan dan tanggung jawab pemerintah.
