He just finished fighting a fire after more than 24h, probably thought that he missed the wedding and then immediately after being done with the fire, saw through texts that they're at the hospital now and rushes over to still be there.
From previous experience, he probably expects that Buck is at least a bit upset. After all, he knows that Buck was already at least slightly disappointed (but understanding) when he had to leave the bachelor party... And his previous partners were probably usually upset over the danger and long/unpredictable hours...
But then there's Buck, who just throws a compliment in his direction and shuts him up with a kiss.
You can just see Tommy's want/desperation... The way he squeezes his eyes while he cranes his neck to press more intensely against Buck's lips and tries to get more.
And it's so tender... They're not hungrily making out, no, it's just one kiss.
One kiss where Tommy shows how happy he is to be with Buck, how much he wants to be with Buck and also relief that Buck wants to be with him.
