Long rant post bc ugh doctors and weight.
I'm happy but also annoyed. So I stopped weighing myself as much this year because fuck it, I don't care anymore. I'm overweight and always have been and I'm pretty active and always have been, so what gives?
Every time I go to the doctors they go well you need to eat less (I eat 2 meals and a few snacks a day, just want me to do 2 meals?). Eat more veggies/leafy greens! "You can cook up spinach in some oil on the stove and that would be a good way to get more leafy greens in!" I'm a very picky eater, always have been, but I've been getting better as the years go on, but I'm not frying up spinach and eating just that. I snack on carrots, sometimes cauliflower, cherry tomatoes, etc. Not leafy greens but at least they are veggies. Work out: "have you tried working out? how much do you work out each day? you need to be working out every day". Last time I went to the doctor in June she went "so have you been working out?" and I was like "well no, but I haven't really had time or energy because I've been working 12 hour days a few times a week, but I'm literally always on my feet and moving at work" she literally looked at me with the most unamused/annoyed expression and was like "so that's a no to working out" and I was a bit pissed off. I work as a floor person at a pet store aka constant walking around, regular lifting and carrying heavy bags up and down ladders/out to people's cars. I work as a massage therapist aka constant movement there. I started working at a garden center where they have me do random odd jobs (moving clay pots, cleaning shelves, watering or moving plants, etc all without air conditioning) or just stand at a register occasionally. Well my cholesterol is high. Yeah I know, it's been high for a while but I stopped eating fast food except for rare occasions now so I was hoping it would go down (I did start eating more chips however after that so....). With the results of the test she was like "you need to be eating less, smaller portions, more veggies, and you need to be working out for an hour a day, every day." and I was like fuck okay fine. So I started going on the treadmill for like.. 15 to 30 minutes a day, but stopped again because I really don't have the energy or time. I also have depression that idk if we ever got fully under control because I struggle to get out of bed unless I have to go do something which is actually part of the reason I started working at the garden center because at least if I'm working a 12 hour day or morning shift somewhere I have to get my ass out of bed in the morning. I do not have the energy to work out, especially when I'm working. But I figured I would gradually start with 15 to 30 minutes a few times a week and then get to 60 a few times a week (yeah that lasted like maybe two weeks and I haven't worked out since). However, despite not working out still and going down to one day a week at the garden center (so only one 12 hour day a week), I've lost 12 pounds since June without working out. How you may ask? I cut chips and egg yolks out of my diet. Literally that's the only real difference. I still eat the same portions, I may have a few more veggies because it's summer and we have the garden going (so plenty of zucchini and cherry tomatoes), but literally I still eat just as much sugar as I have, I still snack quite a bit (just not on chips) AND I'VE LOST 12 POUNDS GRADUALLY IN TWO MONTHS. LITERALLY THAT'S ALL IT TOOK. THAT'S ALL I CHANGED! I've literally struggled with my weight for as long as I can remember, and while I will have to see how far it goes down if I keep this up/maybe also start working out, I've always been classified as overweight. I got the short and stocky genes from my mom. My mom works out 30 minutes to an hour a day almost every day, in addition to being on her feet from 6am to 2pm plus whatever she does at home, she is still classified as overweight. This lady is pure muscle, she doesn't eat a ton, but nope she's still overweight. I hate that even at my lowest weight as a pre-teen/teen (which is about 35 pounds lighter than I am rn), I was still overweight. I'm happy I've lost weight but also am sad and frustrated because even if I lost 40 pounds it wouldn't be good enough to not be overweight.

it’s hard to pinpoint exactly when it was he started tacking “my” in front of his pet names. “my angel,” “my love.”
even when she wasn’t in the room — it was impossible to know he was talking about a colleague with the way he spoke about her. “my woman’s always on my case abou’ shit like that — cholesterol levels, sugar intake. fuckin’ bullshit, but i do it to make ‘er happy.” or “can’t stay long, lads — got my lady waitin’ on me.”
in the summer months, her skirts get a bit shorter and her tops a bit tighter. he doesn’t blame her, the AC is shit and the heat can be suffocating. what does bother him, though, is the way his men ogle her as they stroll past her desk. how they’re coming up with excuses to visit her throughout the day.
it’s an easy enough fix. “why don’t ye come work in my office for the day, lovey?” he’s already collecting her paperwork. “keep an old man company, would ye? i’ve got a nice little fan too, keep ye nice an’ cool.”
though the job came with benefits, perhaps more than an assistant should be getting, price didn’t think it was enough. when her phone started to slow and the screen cracked, he left a new one on her desk. didn’t bother mentioning it came out of his paycheque. if she complains about her outfit — all my good clothes are in the wash — he’ll take her shopping, doesn’t let her worry about the totals. and, hey, if they end up at a lingerie shop, no one has to know, right? he’s just being a good boss. it’s only crossing a few boundaries when he gets her to model it for him in the fitting rooms. when she disappears behind the curtain, john adjusts himself in his slacks — it’s a natural reaction. on that note, it would make too much of a fuss if he were to correct the worker when she asks if his wife needs any help.
when day turns to night and she’s refusing price’s suggestion of hitting another shop, he pulls into a nearby restaurant, insists on treating her to a glass of wine to end the night. finding out she’s a lightweight is a pleasant discovery — two glasses in and her skin is warm to the touch, she’s giggling and hanging onto his every word. he likes her like this, he decides — but it’s not safe to leave her alone. no, she should stay with him tonight. another few sips and she’s agreeing, changing into one of her new lingerie sets and falling into john’s bed, dozing off with his hand splayed over her tummy, beard tickling the back of her neck.
it’s been too long since he’s had a woman in his life. his wires have gotten a bit crossed. you can’t blame him, can you?

Wound Care
Ok so, take this with a BIG grain of salt, because I may be a medical doctor BUT you need to know how much wound care training we get in medical school: none. Zip. Zilch. There may be medical schools where you do, but mine wasn't a bozo factory and there was NO wound care training. Everything I know I learned from one of several sources: an intensive 2-day wound care course I did in residency (highly recommend), the local Home Health wound care nurse (highly recommend), a completely batshit insane old white male doctor who started our learning sessions by yelling Vietnam War stories at me (do not recommend), a hospital wound care nurse (highly recommend), and experience (oh god do not recommend).
The first thing you need to know is that wound healing varies dramatically across the course of a lifespan. Kids? Kids will heal. If they don't, get their ass to a pediatrician because there's something genetic going on. Young adults will heal. Middle-aged adults will heal. You know who doesn't heal for shit? The elderly, and people with severe illnesses, and people with uncontrolled type II diabetes.
Your body needs several things in order to heal. It needs macronutrients, so you need to be able to EAT protein, fat, and carbs. If you are on total parenteral nutrition, aka TPN, aka IV nutrition, you are going to be worse at healing. If you are starving yourself, you are going to be worse at healing. If your body is desperately funneling all the calories you take in to surviving your COPD or cancer, you are going to be worse at healing.
It also needs micronutrients. If your diet sucks, you won't heal. Take a multivitamin once in a while.
There are two CRITICAL skin components to healing: collagen and elastin. Guess what we stop making as we age. Promoting collagen isn't just good for "anti-aging," it's good for NOT ripping your skin apart. Taking oral collagen is probably bullshit because your body is going to have to disassemble it to get it across the intestinal membranes to absorb, but it's also harmless, and if your diet REALLY sucks, who knows. Give it a try. Collagen is made of amino acids; think protein.
Another absolutely crucial component is blood flow. As people age, they start to develop cholesterol plaques lining arteries that eventually pick up calcium deposits. This makes blood vessels less elastic, which is a problem, but eventually also blocks them off, which is a much bigger problem. If someone has the major blood flow to their feet decreased by 90% by arterial stenosis, they are not going to heal for shit AND their foot's gonna hurt.
One component of blood flow I hadn't thought about before going into medicine is fluid retention. The way your body works, blood exits the heart at a very high velocity, but slows to a crawl by the time it gets into capillaries, the smallest blood vessels in the body. Water is a very small molecule and can leave the blood vessel, especially if there aren't big, negatively-charged molecules like proteins like albumin in the blood vessels to hold the water there. And we're built for this--some water is supposed to leak out of our blood vessels when it gets to real little vessels. It gets taken back up by the lymphatic system and eventually dumped back into the bloodstream at the inferior vena cava. But if you aren't making albumin--for instance, in liver failure--you may leak a LOT of fluid into the tissue, so much that your legs get swollen, tight, the skin feeling woody and strange. This isn't fixable by drainage because the fluid is everywhere, not in a single pocket we can drain. And because it puts so much pressure on the tissues of the skin, it often results in ulcers. Congestive heart failure, liver failure, kidney failure--these are all common causes of severe edema, aka swelling due to fluid in the tissues. And they're a real bitch when it comes to wound care, because we have such limited resources for getting the fluid back out, which is a necessary first step to healing.
Pressure is another common cause of wounds. Pressure forces blood out of those little capillaries, so you starve the cells normally fed by those capillaries, and they die. It's called pressure necrosis. Very sick people who can't turn themselves over--people in the ICU, people in nursing homes--are especially prone to these wounds, as are people with limited sensation; pressure wounds are common in wheelchair users who have lost some feeling in the parts of their bodies that rub against those surfaces, or diabetics who don't notice a rock in their shoe.
So, if you're trying to treat wounds, the questions to ask are these:
Why did this wound happen?
-Was it pressure? If it's pressure, you have to offload the source of the pressure or else that wound will not heal. End of story. You can put the tears of a unicorn on that thing, if you don't offload the pressure it won't heal.
-Was it fluid? If it's fluid, you have get the fluid out of the issues or else it won't heal. You can sometimes do that with diuretics, medications that cause the body to dump water through the kidneys, but that's always threading a needle because you have to get someone to a state where they still have juuuuust enough fluid inside their blood vessels to keep their organs happy, while maintaining a very slight state of dehydration so the blood vessels suck water back in from the tissues. You can use compression stockings to squeeze fluid back into the vessels, but if they have arterial insufficiency and not just venous insufficiency, you can accidentally then cause pressure injury. The safest option is using gravity: prop the feet up above the level of the heart, wherever the heart is at, at that moment, and gravity will pull fluid back down out of the legs. Super boring though. Patients hate it. Not as much as they hate compression stockings.
-Was it a skin tear because the skin is very fragile? This is extremely common in the elderly, because they're not making collagen and elastin, necessary to repairing skin. If this is the case, make sure they're actually getting enough nutrition--as people get into their 80s and 90s, their appetites often change and diminish, especially if they're struggling with dementia. And think about just wrapping them in bubble wrap. Remove things with sharp edges from their environments. I have seen the WORST skin tears from solid wood or metal furniture with sharp edges. Get rid of throw rugs and other tripping hazards. I had somebody last week who tried to a clear a baby gate and damn near destroyed their artificial hip.
The next critical question: why isn't it healing?
-Are you getting enough nutrients? Both macro and micro?
-Are you elderly?
-Are you ill?
-Do you have a genetic disorder of collagen formation?
Fix why it's not healing and almost anything will heal. If you're diabetic, find a medication regimen that improves your sugars and stick to it. If you're anorexic, get treatment for your eating disorder. If you have congestive heart failure, work with your doctor on your fluid balance. Wear the damn pressure stockings. Prop up your feet.
If, after those two unskippable questions are done, you want to do something to the wound--apply a dressing, do a treatment--that's a whole other kettle of fish. I'll write that later. The dryer just sang me its little song and I need to put away the laundry.

As usual, gym OOTD is: “Hasn’t accepted that he needs to size up.” 🐷 skipping cardio and letting my shirt ride up in front of everyone when I do my lifts.
I’m really being a fat slut recently. I’ve just been crushing my protein goals after a lengthy period of being too depressed to build muscle. I am feeling so BIG and loving the growing strength. I’m just totally embracing my appetite and the jiggles.
Oh course, I’m sure that it’s only a matter of time before I start freaking out again about the new highs on the scale and make another pathetic attempt at losing some of it. I’m not worried though. I know where I’m eventually going to end up and it’s going to make me so happy.
Standing at the precipice of stepping off the cliff of the 150 lb range is as thrilling and euphoric as it is genuinely terrifying. I can see myself and my path to 165 lbs (which would be my ideal next pit stop on my way to my goal of obesity at 180 lbs) but dang, I’m gonna be ruined physically and mentally if I’m not careful. I think that so long as I keep in shape I can mostly convince myself that my weight isn’t /too/ bad but it’s such a tight line to walk to not get fat too fast to stay fit (for me at least) and it gets more and more tempting to be lazy with every extra pound.
I’ve also basically decided that I’m gonna seek medication related to diabetes prophylactically when I reach that point. Like, I can eat as plant based as possible and work out to support my cholesterol and blood pressure but with my family history and signs of insulin insensitivity already appearing at a certain weight I’m gonna need some sort of pancreatic support - to minimize the squeeze from visceral fat if nothing else (besides more subq would be sexyyyyyyy) I’ve made a ridiculous delusional plan that I could access something like that and it could actually really support my gain by preventing damage from my obesity instead of reacting to it and hopefully minimize the chances of being in a situation where I have to lose weight for my health.
“Piggy boy is such a fatass that he needs meds because he just can’t say no to ice cream, doc. That’s why his belly is spilling out of his shirt and he’s up 10 pounds year over year for the past 5 years. You better do something quick before he starts tipping past 200 lbs and the gains accelerate until he can barely waddle back into this office.”

Long-lasting 'forever chemicals', which can cause damaging health issues, found in 18/20 brands of paper straws
In the first analysis of its kind in Europe, and only the second in the world, Belgian researchers tested 39 brands of straws for the group of synthetic chemicals known as poly- and perfluoroalkyl substances (PFAS).
PFAS were found in the majority of the straws tested and were most common in those made from paper and bamboo, the study, published in the peer-reviewed journal Food Additives and Contaminants, found.
PFAS are used to make everyday products, from outdoor clothing to non-stick pans, resistant to water, heat and stains. They are, however, potentially harmful to people, wildlife and the environment.
They break down very slowly over time and can persist over thousands of years in the environment, a property that has led to them being known as "forever chemicals."
They have been associated with a number of health problems, including lower response to vaccines, lower birth weight, thyroid disease, increased cholesterol levels, liver damage, kidney cancer and testicular cancer.
"Straws made from plant-based materials, such as paper and bamboo, are often advertised as being more sustainable and eco-friendly than those made from plastic," says researcher Dr Thimo Groffen, an environmental scientist at the University of Antwerp, who is involved in this study.
"However, the presence of PFAS in these straws means that's not necessarily true."
A growing number of countries, including the UK and Belgium, have banned sale of single-use plastic products, including drinking straws, and plant-based versions have become popular alternatives.
A recent study found PFAS in plant-based drinking straws in the US. Dr Groffen and colleagues wanted to find out if the same was true of those on sale in Belgium.
To explore this further, the research team purchased 39 different brands of drinking straw made from five materials -- paper, bamboo, glass, stainless steel and plastic.
The straws, which were mainly obtained from shops, supermarkets and fast-food restaurants, then underwent two rounds of testing for PFAS.
The majority of the brands (27/39, 69%) contained PFAS, with 18 different PFAS detected in total.
The paper straws were most likely to contain PFAS, with the chemicals detected in 18/20 (90%) of the brands tested. PFAS were also detected in 4/5 (80%) brands of bamboo straw, 3/4 (75%) of the plastic straw brands and 2/5 (40%) brands of glass straw. They were not detected in any of the five types of steel straw tested.
The most commonly found PFAS, perfluorooctanoic acid (PFOA), has been banned globally since 2020.
Also detected were trifluoroacetic acid (TFA) and trifluoromethanesulfonic acid (TFMS), "ultra-short chain" PFAS which are highly water soluble and so might leach out of straws into drinks.
The PFAS concentrations were low and, bearing in mind that most people tend to only use straws occasionally, pose a limited risk to human health. However, PFAS can remain in the body for many years and concentrations can build up over time.
"Small amounts of PFAS, while not harmful in themselves, can add to the chemical load already present in the body," says Dr Groffen.
It isn't known whether the PFAS were added to the straws by the manufacturers for waterproofing or whether were the result of contamination. Potential sources of contamination include the soil the plant-based materials were grown in and the water used in the manufacturing process.
However, the presence of the chemicals in almost every brand of paper straw means it is likely that it was, in some cases, being used as a water-repellent coating, say the researchers.
The study's other limitations include not looking at whether the PFAS would leach out of the straws into liquids.
Dr Groffen concludes: "The presence of PFAS in paper and bamboo straws shows they are not necessarily biodegradable.
"We did not detect any PFAS in stainless steel straws, so I would advise consumers to use this type of straw -- or just avoid using straws at all."

Hey support. I gave my phone to my dad for safekeeping while I went to a doctors appointment. During the appointment, I started feeling strange, like I was getting dumber? Also my shirt feels tight, don’t know if that’s something.
It's just a screening appointment. I'm here because I need to get my blood levels checked. Of course, I know for a fact that the doctor will say something about my cholesterol levels again. He will say something about the fact that I should do more sport. What should I do? I'm a junior in college. It is extremely challenging to keep up with the curriculum.
The shirt is not only tight, it is also itchy. It would be nice if the doctor came soon. Then I can finally take it off for the examination. I don't have forever. I have to be back on campus by 4 p.m. at the latest. Otherwise I won't have enough time to warm up before training.
I hope da doc isn't uh wimp. Yes, I cud have taken uh shower before going to da doctor. But before da doctor wuz gym, afta da doctor is football training. Ha much sense does uh shower make? let me check my armpit. Yes, gud jock sweat. Da doctor will like that.
