Diabetes rant
Been awhile since I posted but I'm just really sick of having to pay x amount to live. I need to vent or the urge to off myself will win and i'm not going to let the assholes win. So ignore if you want I just want to vent. My bg is five hundred currently so yeah. I'm just kinda pissed.
Diabetics in America, type 1 and type 2 have to make do by buying insulin from other companies, paying out of pocket prices that can range from 200 to 1500 for a month supply. Relying on Diabetic Facebook support groups where other Diabetics donate extra supplies, copay cards that only work half the time, and fiancal assistance programs that you only qualify if you're in the lowest possible income bracket. This is the fault of Pharmacy benefit managers, insurance companies, the top three pharmaceutical companies, Eli Lily, sanofi and novonordisk that own the patents. Then all three blame each other for the high price, blame the pbms and insurance companies who into blame each other as well. Fun fact Eli Lily came out with a generic insulin. Insulin lispro it's basically humalog. Not that they have been advertising it, the bastards. I've been told at my pharmacy an out of pocket price of humalog is $546 for a month's supply of vials, what's is insulin lispro's out of pocket price for a month's supply? $180 per vial. Had anyone had any luck finding out what it cost the companies to make their analog insulin? I've been looking for that data for 8 years still haven't come across a legitimate source. I heard once from a novonordisk drug rep that Eli Lily makes one vat of insulin for $50 dollars. Lol representative was trying to make the other company look bad but they all do it. So you have insurances that pick which insulins get to be on their formulary. BCBS for example favors humalog. So good luck unless you're allergic to humalog they will deny any prior authorization for another fast acting. My BCBS ins doesn't care that fiasp and novolog work better for me. Now I'm using humalog u 200 which is working for now thank God. Let's now get started on Continuous Glucose monitors, dexcom g6, freestyle libre and guardian sensors.
Having a CGM literally saved my life, I have a 'good insurance plan' I have an 800 dollar deducible, 5000 out of pocket. Met my 800. Dollar ded at the first of the year. Wanna know how? I had to drop $1000 to pay for a 3 month supply of dexcom g6. So I met my deductible so now everything is great? Wrong! I still have to pay %20 coins. So if I want 3 month supply I have to pay $367 dollars of my portion. I don't know about most people but I find it difficult to come up with that sort of lump sum of cash. It's September and I'm only 2000 in on my 5000 out of pocket. Why because I don't go to the doctor unless I have no other choice. I have enough trouble just paying for my diabetic supplies let alone the copays, and remaining balance left over at the end of doc visits. Only doc I see regularly is my Endo. Then BCBS had the gall to keep making me use dme (durable medical equipment) companies that charge a hell of a lot more than pharmacy benefits do. Supposedly bcbs is going to let commerical insurance patients use pharmacy benefits in October so if you have BCBS and use a dexcom CGM have your doc or Endo send prescription to community Walgreens who also has a $120 coupon.
Freestyle libre is at least trying to do something, funnily enough because they market their CGM as a meter and for type two diabetes. Out of pocket price is 75 using e vouch for a month supply, sensors last 14 days, 1 hour warm up time, no alarms but they are working on it. You can use a smart phone and save some money by not getting a reader. Guardian don't know much about but I tried one and sensor died on day one. If the senors work then it does have some advantages. transmitter lasts a year and is rechargable, and you can restart sensor fairly easily.
Pumps Omni pod, tandem and Medtronic.
Gotta Omni pod hands down even though I got a tandem tslim x2 pump. Omni pod is tubeless, Omni pod pods can be potentially pharmacy benefits. So go omnipod also because you can hack the classic pump with the open aes algorithm and get a artifical pancreas because #we aren't waiting. FDA approval takes forever!
Good news on dexcom front, G7 supposedly will come out in end of 2020. It will last fifteen days, 1 hour wait time and transmitter is inside senors so you throw whole unit away. Why is it going to be less expensive? They want to market it towards type 2s.
Did you know you can restart dexcom g6 transmitters and senors. Well you can check out YouTube for fellow Diabetics' instructions. It will save you money and stick it to the assholes trying to monetize diabetes.
It sickens me that Sir Fredrick Banting and his cohorts sold the patent for insulin for a dollar each, total sale being $4 dollars to Ontario college. They sold it so it would be affordable and accessible for everyone.
Back in 1942 Eli Lily and two other companies got in trouble for price fixing insulin. What's the difference between then and now? The government actually did its job and stepped into to stop them.
Everyone who price gouges insulin and Cgms and pumps should be ashamed.
I'm sick of being taken advantage of for an autoimmune disease I don't have a choice in. I'm pissed. I want justice.
I want a healthcare system that won't bankrupt me. I want a system that actually focuses on preventive care and not being for profit. Fun fact our healthcare has not always been for profit. You can thank president Nixon for that.

Also fun fact that is when the big top three insulin producers starting raising the prices of insulin and they haven't stopped since.
So the solution to fix our healthcare system is pretty damned simple. Go back to having it be non for profit.

My yearly doctor’s appointment is something I kind of dread, because it’s generally bad news. Blood sugar’s still too high, oh hey your eye’s getting worse, the rest is all good but please be careful...all that.
I had this year’s appointment last Friday, and the news is as follows:
1. thanks to the Freestyle Libre sensor system, my blood sugar is finally starting to lower (went from 89.2 to 81.9, which would be 10.2 and 9.6 in the old system) after literal years of being stuck in the 89s;
2. my right eye is actually getting better, which I already knew, but my doc was happy to hear it. My right eye is the one that’s been getting injections and laser treatment, but for the next 4 months I don’t have to get any injections, and then with the check-up in august we’ll see if my eye doesn’t worsen again. I hope not. The cataract on the left is still so severely underdeveloped no one’s worried about that yet (though I, a perpetually stressed person, am stressed out about the actual operation to remove it);
3. My feet are also still fine, as expected, but it was funny that my doctor prodded my toe and asked if I could feel it with me very blatantly watching him do that, which kind of defeats the point. But after closing my eyes, I still could feel all the things;
4. I’ve been taking a pill called enalaprilmaleaat since I was 12. There’s a little leak in my kidneys that cannot be from anything diabetes related, as I’d only had it for 5 years when it was discovered, and as a kid my diabetes was a little easier to handle (damn hormones). But now my doctor wants to see if I can perhaps stop with that treatment, and see how that goes. For the next three weeks I’m going to go from 20mg a day to 10, checking my blood pressure all the while, and if that goes okay, I can try to stop. We’ll see what that does with some further tests along the road, but it’s nice! And...
5. I’ve been given information about research that’s going to be done on type 1 diabetics, centred on mobile monitoring. The team wants to build an app that doesn’t just allow patients to set alarms for stuff like checking blood sugar or eating things on time, but also allows them to seek advice for specific problems without having to visit the hospital or wait for fridays to call the office. I’d love to do it, as my phone has almost atrophied with me at this point (I use it to check my blood sugar), but I selfishly hope I can get into the test group that uses the app they’re building because you’ll get fitted with a FitBit x) x) x) I love gadgets, what can I say
So for the first time in a while, I’ve left the hospital with nothing but good feelings, which was odd. Usually I cheer myself up afterward with a cappuccino in the hospital restaurant, or go to the Body Shop right next to it to buy myself nice smelling soaps and shit. But this time I just meandered back to my car and drove home, signing along (badly) to Imagine Dragons in my car.
Kinda hoping this upward line continues.

Tens of thousands of type-1 diabetics in England are being denied access to a "life-changing" blood sugar monitor due to a postcode lottery, data suggests.
The Freestyle Libre, used by Prime Minister Theresa May, was made available on the NHS last November.
But an investigation by the BMJ found a quarter of local NHS groups in England were not recommending the devices for patients.
Health bosses said the "NHS does not have unlimited resources".

Flash glucose monitors work via a sensor attached to the skin and allow people to check their blood sugar levels by using the device with a portable reader or a smartphone app.
They reduce the need for finger-prick blood tests and can make it easier for diabetics to manage their condition.
But Freedom of Information requests made by the BMJ found that while some clinical commissioning groups have made the devices available to hundreds of patients, others have not made them available to anyone.
Based on the data received by the BMJ, Partha Kar, NHS England's associate national clinical director for diabetes, estimates that just 3-5% of patients with type-1 diabetes in England are currently able to get the monitors on the NHS.
If CCGs were following NHS England guidance correctly, this figure should be closer to 20-25%, if not higher, he said.
Mr Par said some CCGs were merely paying "lip service" to offering the monitors.
About 400,000 people in the UK are estimated to have type-1 diabetes. About 29,000 of them are children.
'It is life changing'

Victoria Hill, 24, who was diagnosed with type-1 diabetes when she was six, is not able to get a Freestyle Libre monitor through her CCG, despite the fact her neighbouring CCGs offer it.
She started paying for the monitor herself after benefiting from the device during a trial. But she found herself unable to afford the monitor while she was saving to buy a house.
A Freestyle Libre starter pack, which comes with a reader and two sensors, costs £159.95. Sensors cost £57.95 and need to be replaced every two weeks.
After buying a house, she has since started paying for the monitors again.
She said: "I'm absolutely chuffed - it is life changing.
"I've been on multi-syringes and then moved to [insulin] pens but this is the one thing that has made a difference to my life overall.
"It is the most frustrating thing in the world that in Gloucester, which is five miles down the road, they are getting it on prescription and I'm not.
"Why are they more deserving than me? I am able to afford this but there are people out there who can't and they are really missing out."
Consultant diabetologist Emma Wilmot said Freestyle Libre, the only flash glucose monitor available in the UK, was one of the biggest "life-changing" advancements in type-1 diabetes care for many years.
She said some of her patients were considering moving to a different GP practice a few miles down the road to get the monitor, while others were making "huge sacrifices" to pay for it themselves.
"My worry is the people who aren't in a position to self-fund it," she said.
"The most deprived people in the population often have the least access."
'Limited NHS pound'
Meanwhile, official prescribing data obtained by the diabetes campaigner Nick Cahm and shared with the BMJ suggests that, as of July 2018, just 2% of patients with type-1 diabetes in England were getting Freestyle Libre on GP prescription, compared with 11% in Scotland, 16% in Wales, and 35% in Northern Ireland.
Nikki Joule, from Diabetes UK, said patients were facing an "unfair postcode lottery".
She added the charity wanted to see all areas agree on policies for accessing the monitors.
Julie Wood, chief executive of NHS Clinical Commissioners, which represents CCGs, said: "Clinical commissioners have a responsibility to consider the needs of their whole populations, reduce inequalities, and improve quality of care while living within the funding they are given and it is right that they should follow a due process when considering new medicines and technologies to ensure they are making the most effective use of the limited NHS pound."

Investor Updates: The Latest in Diabetes Tech (Spring 2017)
To help keep you all up on what's new and hot in diabetes technology, we continue to follow the industry's quarterly "earnings calls" -- webcast sessions designed to update investors on product pipelines and business projections.
Heading into mid-2017, the recent calls we've listened to haven't provided any big "Oh Boy!" moments, but rather a lot of promising "wait and see" on exciting stuff in the hopper for our D-Community -- from useful incremental updates, to next-generation devices and expanded access to this type of technology.
On these recent calls, a huge elephant in the room, so to speak, were the big legacy players and how so much is in flux right now -- from Medtronic's plans with its 670G hybrid closed loop being launched gradually this year, the stand-alone CGM they're developing, how quickly the Abbott FreeStyle Libre Flash Glucose Monitoring (FGM) system may gain FDA approval this year, and the future of diabetes tech from legacy companies Roche and JnJ.
Here's a rundown of what we learned in recent earnings calls from Dexcom, Insulet and Tandem Diabetes Care:
Dexcom
The California CGM company held its first quarter earnings call on May 2. A full transcript is available at Seeking Alpha, at least for the time being.
Touchscreen Receiver Approved! Biggest news is that on March 8, the FDA approved Dexcom's new, next-gen touchscreen receiver -- just nine months after submission. During the call Dexcom said it plans to launch that "later this year," timeline TBD. How fast the launch will happen is anyone's guess, but we do think we'll at least see this new receiver on display at the big upcoming ADA Scientific Sessions conference starting June 9.
We requested photos of this new device, but haven't yet received those. We did spot this image from documents filed with the FCC last year, to at least give us an idea of what it may look like -- a different orientation that more resembles a smartphone than the existing little iPod-ish horizontal receiver with round scroll button wheel.
Dexcom spokesman Steve Pacelli says, "it's more durable and has a color touchscreen, otherwise it looks like the G5 mobile phone app." He also says this new receiver fixes the audible alert issues that the past receivers have been plagued with. As to whether this will work with both G5 and the future G6 that will likely be submitted to FDA by the end of September 2017, Pacelli tells us it will work with both CGM generations but it's not yet known how the new receiver's firmware updates between the G5 and G6 will be handled. It will also still have a one-year warranty for commercial payers and 3-year warranty for Medicare.
Medicare Coverage: A big chunk of the call focused on the Centers for Medicare and Medicaid Services (CMS), which is able to cover the Dexcom G5 system and those final details are being worked out. Dexcom admits there's some confusion and delay based on the lack of a national coverage decision by CMS, but regular interactions are happening and the company hopes that can all be finalized and clarified soon.
No Android Yet... We're all still waiting on the Android mobile app capaility that's pending before the FDA, as well as the new one-button insertion system and G5x transmitter that will be 50% smaller than the current G5 transmitter. Pacelli said on the earnings call that FDA review of those is moving slower than initially expected, and depending on timing, the company could decide to hold off on even launching the new G5x transmitter to release it in tandem with the G6 platform expected in 2018.
Next-Gen G6: As we've reported before, the G6 will be a true leap forward in CGM tech, with 10 days of wear instead of the current 7 days, only one daily calibration required instead of two, and improved accuracy and reliability. Dexcom plans to file that by the end of Q3 2017, and is anticipating a 2018 launch. We asked about ADA's Scientific Sessions right around the corner, but were told not to expect any new groundbreaking data on this model to be presented there.
Verily Collaboration: Dexcom is also continuing its work on the first-generation tech with Verily (formerly Google Life Sciences) that will be a miniaturized, dime-sized version of a CGM sensor that requires no calibration. Since it's based on the G6 planned for launch in the first half of 2018 (depending on regulatory review), Dexcom hopes for commercialization at the end of 2018. The second-generation, referred to as the "little bandaid sensor," will be the two companies' expected home-run product, planned for roughly 2020 or 2021.
Dexcom's Future? Oh, and just in case you were wondering if Dexcom has been shopping around to possibly acquire a pump company such as the beleaguered Animas, Roche or Tandem it has partnerships with... Nope. This quip from Pacelli on the earnings call flat-out denies it:
"No, we're not looking to buy a pump company... You could see us become a little more involved with one or more of our partners if some of them seem to fall off. But at this point in time, we're going to continue with our partnership strategy, both on the pump side and on interconnected pens and other software platforms. So, not interested in buying a pump."
Insulet
The Boston-based OmniPod patch pump company's recent first quarter call on May 8 didn't bring much new to the table. To compare, see the mid-2016 update and full-year 2016 updates reported a couple months back. They basically reiterated:
OmniPod DASH: As we reported last Fall, the big news is Insulet's development of its next-gen, touchscreen OmniPod DASH platform that will have a new touchscreen PDM and Bluetooth-enabled Pod. This has not yet been submitted to the FDA, but they expect to do so sometime by this Fall. Insulet's been very excited to promote this new platform and show it off at the upcoming ADA meeting, and as of now they predict a phased rollout once it's approved and launched.
Closed-Loop Tech: Early clinical work continues on Insulet's Horizon closed-loop technology, which will first bring us a hybrid closed loop system at some point in late 2019, and eventually a more fully automatic closed loop system after that. Some clinical trial data should be presented at ADA in June, and Insulet plans to begin its pre-pivotal trial work by the end of this year.
Access Improvements: Insulet says it's expanding the state Medicaid program coverage for OmniPod, and that it continues to work improving access through Medicare too.
Tandem Diabetes Care
There's been a lot of uncertainty for Tandem on the business side of late, with many folks wondering if the San Diego insulin pump company will be able to survive long-term. While their finances haven't been reassuring, they are apparently raising more money and keep pushing out the marketing on how excited they are for the future. They've just distributed invitations for a pre-ADA "Media Day," presumably to let the press know that they're in good shape.
Blame Game? During the Q1 earnings call on April 27, we had to scratch our heads over how much Tandem CEO Kim Blickenstaff focused on Medtronic and its new hybrid closed-loop 670G system as a reason for much of the rumor mill on Tandem's business. Seriously, he said "670G" a total of 14 times on the call, "Medtronic" was at 15, and reading through the transcript it appears he only said "Tandem" less than a dozen times. Hmmm. Tandem's leader says: "We are not hiding from the competition."
New Infusion Set Connector: As reported in March, Tandem's planning a new infusion set connector dubbed the t:lock. It will use a Luer-Lock tube connector that's a universal design and works for most insulin pump models, except the proprietary ones made for Medtronic pumps. Tandem currently offers four types of infusion sets (t:90, t:30, Comfort, and Contact sets; the Cleo was phased out last year) and to date, users could also choose other Luer-Lock infusion sets -- but after the end of this year, anyone using t:slim pumps will be "locked in" (so to speak) to these new t:lock connectors. On the recent earnings call, Tandem said it plans to launch that in Q3 of 2017.
Dexcom G5 Integration: Tandem's still waiting on the FDA to approve its integration with the G5, allowing for its Remote Updater launched in March 2017 to give pumpers access to G5 data directly on their t:slim X2 devices. The company's preparing for a Summer 2017 launch, and expects to be ready get this on the market within 30 days of regulatory approval. Tandem also points out the Remote Updater will be free of charge for those using the X2 platform already.
Closed-Loop System: Tandem too is working on its first automated insulin delivery system, recently filing an investigational clinical pivotal trial protocol with the FDA and expecting updates on that in May. As detailed before, this first-gen system will have a Predictive Low Glucose Suspend (PLGS) feature that would automatically shut off insulin delivery when a hypo is predicted based on the Dexcom CGM. That's expected in early 2018. For its second generation, Tandem plans to integrate an algorithm created by TypeZero Technologies for a "treat-to-target" system to adjust insulin doses to keep users continually aiming for a particular blood sugar number, as well as correction dosing. That is also expected by the end of 2018.
