The Myth That Refuses to Die: All Health Care is Local

In 1980, industry healthcare planners imagined a system where the centerpiece was a hospital in every community and a complement of physicians. Demand forecasting was fairly straightforward: based on the population’s growth and age, the need was 4 beds per thousand and 140 docs per 100,000, give or take a few.

In 1996, the Dartmouth Center for the Evaluative Clinical Sciences published the Dartmouth Atlas on Health Care quantifying variability in the intensity of services provided Medicare enrollees in each U.S. zip code. They defined 306 hospital referral regions (HRRs) that remain today as the basis for regulation of our healthcare system.

In the same timeframe (1980-2000), the ratio of doctors per 100,000 doubled as the number of medical schools increased from 75 to 126 leading health planners (Graduate Medical Education National Advisory Council) to predict a surplus of 70,000. Meanwhile, demand for hospital beds edged down slightly to 3.5/1000—the result of managed care efforts in certain parts of the country.

Today, we operate 2.4 beds per thousand and have 265 physicians per 100,000. But the bigger story is the widespread variability in the volume, costs and quality of care across our communities.  Across the 306 HRRs, bed supply ranges almost 250%; physician supply even more and costs as much as 400%.

For almost 40 years, we’ve operated the U.S. health system based on an underlying assumption that all healthcare is local. We’ve presumed that except in rare circumstances, patients stayed home for the care they need. But that’s changing.

Three trends are converging that are changing how we think of the markets we serve:

Virtual care: Goldman Sachs forecasts virtual care will be a $20 billion industry by 2020 as employers and insurers adopt lower cost options to local medicine. Investor-funded companies (American Well, Teladoc, 2nd.MD, Carena, Health Integrated, MD Live, et al) offer distance medicine that is convenient and less costly. 78% of consumers say they would be receptive to receiving care virtually (Carenet Healthcare Services) though only 14% of hospitals offer digital tools and only 23% offer telemedicine (Kaufman Hall). And funding for digital health is robust: per RockHealth, start-ups have attracted $18 billion in the last four years, with most applications targeting mechanisms whereby consumers can make better choices about the care they receive, where, from whom and at what cost.

Destination Hubs: Historically, individuals left their community when facing a complex diagnosis based on a referral from a trusted local clinician. For cancer, Hutchinson, Dana Farber, Memorial Sloan Kettering, Mayo, MD Anderson, Moffit and others were prominent. For hearts, Cleveland Clinic, New York Presbyterian, Mayo, Mass General, Duke, Northwestern, Brigham and Women’s and others attracted referrals. Most of these set-up referral management programs to accommodate out of town patients, offering lodging and other forms of assistance. But destination hubs have expanded beyond those that cater to physician referrals. Each of these is a business model that makes a big bet that consumers will leave their community for care elsewhere:

Founded in 1988, Cancer Treatment Centers of America (CTSA) operates inpatient and outpatient facilities in 5 markets. Its national ads promote its integrative clinical model and patient-friendly approach to diagnosing and treating the full continuum of cancers. On its website, patient satisfaction scores for each patient cohort are reported with scores averaging above 95%.

Founded in 2005, Laser Spine Institute operates surgery centers in 7 markets and advertises nationally. It offers a free MRI and requires candidates for their minimally invasive procedures to stay within 15 miles of their surgery center one day after the procedure for post-op follow-up. Per its website, the company has performed 75,000 procedures: 60% for patients who live outside its markets.

Large employers like Boeing, Wal-Mart, Lowe’s, Whole Foods are contracting directly with hospitals out of their local markets. Though only 3% of employers contract directly today, the National Business Group on Health estimates activity will increase as health costs escalate in coming years.

And medical tourism remains a factor: According to Patients beyond Borders, 1.4 million U.S. adults traveled outside the U.S. for medical care last year. The Joint Commission International accredits 458 hospitals off-shore that perform elective surgical procedures as 30% of the

So, destination hubs are no longer limited to tertiary care providers in urban settings.

Affordability: One in five Americans under the age of 65 say they are having problems paying their medical bills and 2 million will declare bankruptcy due to medical debt this year (Nerd Wallet). As prices charged by U.S. providers. Reported by the Federal Reserve last November, household debt hit an all-time high last year increasing 16% since the summer of 2013. Student loan, medical and mortgage debt are the culprits. As households face higher deductibles, affordable options will be on their radar. Out-of-market healthcare services offered at a lower price by a reputable provider will be attractive. And the entry of Amazon-JPMorgan-Berkshire Hathaway into the fray of employer benefits and health cost containment promises to spark increased consideration of new models and strategies.

The bottom line is this: healthcare is no longer about competition between insurers, physicians and hospitals operating in a relatively confined hospital referral region. It’s about health facilities and services offered by parties that bet local consumers will respond favorably to their pitches.

The reality is that consumers are finding alternatives to local care: some to obtain services they can’t get at home and many for care they think better, cheaper or easier to access elsewhere.

For the least fortunate in our communities, staying at home for care is reality. For others, out of town options are expanding.

Strategies that presume all healthcare is local are bound to the same fate as Borders, Blockbuster and 10,000 other retailers who’ve ceased operations in the past decade as their markets changed (National Retail Federation). They recognized the retail apocalypse too late to respond.

So, at best, some healthcare is local, but not all.

Crisis: Why We Can’t Ask For Help

Anyone who has ever dealt with emotional distress in Berkshire County has likely had to deal with the dreaded “Crisis Team” at least once in their life. I’ve been involved in the “Psych System” since a young age. I’ve spent my transition into adulthood working hand-in-hand with these organizations. I can tell you that my treatment hasn’t been great. For those who do not know, those who are experiencing an emotional disturbance are encouraged to call Crisis or go to the ER to speak to them. I’ve had such appalling treatment by them in the past that I’ve grown to both fear and despise anything to do with them. I try to avoid them at all costs but these week, we crossed paths again and my experience was quite sickening.

Delving into my life story isn’t relevant to the experience that I am going to talk about but I can say that I have been struggling a great deal lately and that lead to the act that initiated my entire experience. I tried to commit suicide on Saturday. To take one’s own life is a scary act so part way through, I backed out before I could do any permanent damage. Though severely depressed, I didn’t tell anyone and went on with my life. Over the next few days, I attended doctor’s appointments and meetings and went out to eat. On Tuesday, I met with my therapist. In confidentiality, I told her what I did that weekend. Concerned for my safety, she urged me to go to Crisis. Because of my past experiences, I was very reluctant to go but agreed. My therapist told me that she’d be by my side the entire time and she wouldn’t let anything like what had happened to me in the past happen again.

My therapist and I went to the emergency room and were brought back to talk with Crisis. At that point, she did want me to receive inpatient level of care and advocated for that. Between talking to clinicians and psychiatrists, things went quite smoothly. Our hope was to get me into a specialized psych unit at the Brattleboro Retreat but after all the discussions, we found out that they didn’t have a bed available. We had to decide what to do with me for the time being until a bed opened up. They wanted to send me to Jones 2, one of Berkshire Medical Center’s psych units. I wasn’t too keen on going there because of negative experiences in the past but then I had a bomb was dropped on me. One thing to know about me is that I have multiple disabilities and I work with a service dog who is with me 24/7. Berkshire Medical Center told us that they only allow hearing and guide dogs on their units. That is a huge violation of the Americans with Disabilities Act (ADA). A venue cannot decide which disabilities they want to accommodate for and service dogs can work with people with any type of disability, not just deafness or blindness. Both my therapist and I voiced our shock and legal disagreement with what was told to us but the clinician told us that it was hospital policy and that we couldn’t fight it. My therapist and I pledged to each other that we’d take legal action against the hospital as soon as we could. I told the clinician that I refused to be separated from my service dog and Jones 2 would not be an option for me. Somehow, sending me home with a “safety plan” in place and coming back in the morning was brought up as an option. A safety plan was written up, detailing who I could contact if a crisis situation arose and actions to do to prevent me from hurting myself, such as handing over any pills or ropes and leashes to my mother. At this point, my mom had met us at the emergency room and was fine with letting me go home for the night. I had my safety plan and an appointment with Crisis in the morning to continue to pursue getting me admitted to the Brattleboro Retreat. I went home for the night and went to sleep.

When I woke up Wednesday morning, I was just very anxious about what was to happen that day. I still wasn’t guaranteed a bed at Brattleboro and I worried about what would happen when I got to the actual psych unit. I packed some clothes for myself and supplies for my service dog and my mom brought me to my appointment at the Crisis office. I was alone without my therapist. When I arrived at the Crisis office, I was sitting in the waiting room when I received some very upsetting news that wasn’t related to the reason that ended me up in that situation. I was crushed by the news and my feels of worthlessness came flooding back and I felt devastated. At the same moment, I was called back to speak with a new clinician. I presented to the clinician that I was feeling quite distraught at the moment and I was trying to process the bad news that I had just received. She spoke to me but I was soon left alone in the room while we waited for an ambulance to come transport me to the Crisis section of the emergency room. Because in that moment, I was in such a bad place, I decided to take some steps detailed in my safety plan the night before. One thing mentioned in my safety plan was to remove any ropes from the area so I removed the leash from my service dog (who was also wearing a traffic lead, a very short leash) and placed it outside the door. When the clinician came back, she asked about the leash and I told her that I was not in a good state and didn’t want it in the room with me. Soon, the ambulance arrived to transport me. I arrived back in the Crisis section of the emergency room. This time I didn’t have anyone to advocate for me. I spoke with the clinician again and then I spoke with a psychiatrist. The psychiatrist brought up past misdiagnoses as being an option for what I was dealing with. By many mental health professionals and doctors, I have been told that I am neither of those things but living with my misdiagnoses was a traumatic part of my past and is still very hurtful to think about. To have it be suggested once again was an upsetting experience. Fear took ahold of me and I worried about all that could go wrong. I had only started to receive more accurate treatment once my misdiagnoses had been removed from the books. The next person to approach me was a medical doctor and I told him of my worries. I told him that I just wanted to go home because I was scared of becoming a human guinea pig again. The next time the clinician came in the room, she told me that I had been “Sectioned” and going home was not a possibility. For those that don’t know, a Section 12 means that a person is involuntarily being treated and under a doctor's orders, they legally cannot leave. A Section is put into place when they find a patient to be a serious harm to themself or others. The Section had been put into place when I was still at the Crisis office and no one had informed me. I was told that it was due to what I had done with the dog leash. In other words, I had been Sectioned for following my safety plan and taking proper steps to keep myself safe. At that point, I was also told that my health insurance was no longer active. Another panic came across me as I worried that I cannot afford thousands of dollars in medical bills. I was left in the room again to only have the clinician to return to drop some more unpleasant news on me. She told me that my insurance was active but Brattleboro would not accept it because they are now considered out of network. She also told me that Brattleboro would take me if I paid out of pocket but only service dogs that are “ADA Certified” are allowed. I do not have “ADA Certification” for my dog because such a thing does not exist. Online shops sell “certification documents” online but they are a scam and not supported by the ADA. I voiced my legal knowledge but didn’t argue much because I knew that I couldn’t afford to go to Brattleboro anyway and it would be a battle to fight once I regained my freedom. During this whole process, I was calling my therapist to try to get help in navigating this complete disaster that was unfolding. The clinician returned to me again and told me that the decision was made. I was being sent to Jones 3, Berkshire Medical Center’s most intensive psych unit, and there was nothing that I could do and there was nothing that my therapist could do. In other words, I was being hospitalized against my will and being separated from my service dog. I was presented with some legal document that they wanted me to sign. I didn’t really understand what was going on but my interpretation of the document was that if I signed it, I was agreeing to being involuntarily hospitalized. It made no sense to me but the clinician told me that I was being hospitalized against my will either way but if I signed this document, I would look better on paper, therefore getting a quicker release. I reluctantly signed the document.

I sat in my room waiting for my mom and my therapist to arrive to try to defend me but my saving grace came with the change of shift. New staff had arrived for the Crisis Team and I was assigned a new clinician. She came in the room to introduce herself and asked how I was doing. I gave her a recap of my entire experience over the past two days with Crisis. The whole situation confused me so much. I’m no saint and I have had times when at Crisis where I had been crying, yelling, and making suicidal statements yet I hadn’t been Sectioned. This time, I didn’t do any of that and I had remained calm. Not only that, I wasn’t even actively in a crisis situation. I had just opened up about an act that I had committed earlier in the week but I had gone about my daily life safely and compliantly ever since then. Crisis had sent me home the night before and I had kept myself safe. Nothing made any sense at all. How did I end up in a position where I was being sent to the most intensive psych ward against my will? Thankfully, my clinician heard me out and she was as confused as I was. She didn’t understand why I was in that situation and she didn’t want me to be in that situation. Her and my therapist went to bat for me against the authority, fighting for my rights over the phone. The new clinician said that we were going to plan as if I was going to be discharged and that I would have a chance to speak to another psychiatrist who would be able to remove my Section and release me. Meanwhile, my mom and my step-dad showed up to support me. My mother was concerned about my well-being and wanted me to receive help but this was certainly not how she envisioned it. Not long after she arrived, the new psychiatrist came to speak to me. In an anticlimactic end to my roller-coaster ride of an experience, I was released. I went home and went to bed.

I’m writing this to let the public know that this is how people who are dealing with trauma and emotional distress are treated. We are encouraged to seek help when we aren’t in a good place but this is the “help” that we receive. Our rights are violated and we receive anything but help. We learn to fear and distrust the system. We don’t feel that the programs in place are a safe place to turn to and often feel alone. There are alternatives to traditional treatment, such as peer support communities and open dialogue groups where taboo subjects can be spoken about without judgement, but the accessibility of these options is lacking. As a community, we need to back these support systems and give those dealing with trauma a safe place to turn to.
