A new landmark study has found that access to gender-affirming healthcare significantly reduces rates of depression, gender dysphoria, and suicidality among transgender people.

While it’s no secret that providing gender-affirming care to transgender individuals who ask for it can greatly benefit their well-being, an increase in transphobic rhetoric and bans on gender-affirming healthcare has prompted thorough medical studies into the impact of such care.

Now, brand new research conducted in Melbourne, Australia, has found that allowing transgender people to access the care they’re after can reduce suicidality by a stunning 55%.

As part of the first-ever randomized controlled trial (RCT) on gender-affirming care, researchers took 64 transgender and gender-diverse adults who had been looking to start testosterone therapy and randomly split them into a treatment group and a control group.

While the treatment group was allowed to begin hormone therapy that week, the control group waited three months for their treatment to begin.

Before the study began, both groups were evaluated on depression, gender dysphoria, and suicidality. Three months later, the two groups were evaluated again.

RCTs for medical care can often be hard to conduct due to practical and ethical concerns. However, researchers of this study found a way to hold an RCT for this study by incorporating a shorter follow-up period. Rather than giving the control group a placebo drug, or no treatment at all, they were simply given a longer wait time.

The results showed a notable decrease in gender dysphoria, depression, and – most significantly – suicidality.

The group that received gender-affirming care right away saw a 55% reduction in suicidality compared to a 5% drop within the control group.

Depression scores in the treatment group decreased by half, while gender dysphoria rates also significantly decreased.

Breaking down their findings, researchers Brendan J. Nolan MBBS, Sav Zwickl, PhD, and Peter Locke wrote: “There was a statistically significant decrease in gender dysphoria in individuals with immediate [access to gender-affirming care] compared with delayed initiation of testosterone therapy.”

“A clinically significant decrease in depression and a decrease in suicidality also occurred with immediate testosterone therapy.”

“The findings of this trial suggest that testosterone therapy significantly decreases gender dysphoria, depression, and suicidality in transgender and gender-diverse individuals desiring testosterone therapy.”

Of course, this isn’t the first time that research has shown significant drops in depression and suicide rates among transgender individuals who receive gender-affirming care.

A 2022 medical study showed that young transgender people who have access to puberty blockers are 73% less at risk of suicide and report improved well-being.

But, as anti-trans activists advocate for further bans on gender-affirming care, one of the key arguments is that the evidence in support of the care isn’t up to scratch with GRADE (Grades of Recommendation, Assessment, Development, and Evaluation) standards.

So research like this landmark RCT is so significant to the transgender community and its allies as the fight for their healthcare rights rumbles on.

Still, I have been writing. Since it’s been so long, here’s an excerpt from ATKM Chapter 17:

ALL THE KINGS’ MEN, CHAPTER 17 EXCERPT:

You weren’t quite sure how to react to Loki kissing you. The idea consumed most of your waking thoughts for so long, and now that it was happening, you froze. You froze. Soft lips were pressed against your own and you lay unmoving beneath the covers. The only man you could ever remember embracing you like this was James, and he was nowhere near as gentle. James kissed you to claim you- rough and cruel and in a way that almost always bruised your lips.

Loki was nothing like that. The warm caress of his mouth on yours made your head spin, and when he parted his lips, you allowed yourself to melt into him. You let him take control. He tilted his head to capture your lower lip between his and he did not try to shove his tongue into your mouth like James would have. Instead, he let the kiss progress naturally, even sliding an arm between your back and the bed to hold you closer. His other arm moved to support him on the bed.

“Are you okay?” He murmured against your mouth. “Is this too far?” You shook your head, and were rewarded with a kiss on your cheek. He moved to your jaw next, leaving soft pecks along your jaw until he reached your neck. The kiss he left there made you moan. Your eyes opened wide at the alien sound that escaped you, but your barely had time to process it before his lips moved to the shell of your ear, nibbling and licking in a way that had your cheeks heating.

“Loki…” your voice was breathless. “T-that feels wonderful.”

“Good,” he whispered in your ear. “I always want you to feel wonderful around me.” He kissed your ear, then the spot below it, lips trailing down until he reached your neck. You shivered when his lips applied more pressure there. A resultant tremble built in you, followed by another, and another as he repeated his motions. They shot straight through you and down to your core, making you press your thighs together.

“Oh!” You fisted his shirt. His mouth curved into a smile. You noticed that he took great care not to climb on top of you. He simply held you close, allowing you the safety of having him nearby without the suffocation of another body holding yours down. He-

“My king!” The call from outside was followed by a sharp knock on the door. “My king!”

A noise of annoyance erupted from the back of Loki’s throat. He stood up abruptly, marching over to the door to grip the handle and pull it open. “What?” Fear bled into the face of the soldier standing before him. You couldn’t blame the younger boy, for Loki’s irritation was coming off of him in waves. “What do you want?”

“They found the room,” announced the boy. “They found the room with the flowers-”

“They did?” you asked. “That was quick.” The boy’s face darted to yours and he promptly flushed. He must have noticed your quick breathing, along with the rumpled bed sheets and the king’s behaviour, and realized what he interrupted.

“My armies are very efficient,” Loki told you. “Heimdall would have transported them to where they needed to be, and since the Rogers’ household is empty-”

“It wasn’t,” the young soldier interjected. “It wasn’t empty. There was someone in there trying to steal the flowers.”

James, you thought. It must be James. He must have seen the soldiers and figure out what was happening.

Loki must have had the same suspicions, seeing as his eyes flickered to you. “Who was it?” His tone was rough. “Was it Commander Barnes?”

“It was a woman.”

Day 2/85

I studied almost the entire day today with bouts of breaks to sleep in between. I have my mbbs semester 6 end exams this week and I am kind of stressed because I haven't really prepared anything for it since I was busy preparing for step 1.

But having given so many exams, I know that term exam results won't matter in the long run.

Currently I am just preparing enough to pass, or in case I fail, to fail with decent marks.

The main goal is step 1 lads! 💪

i'm going to have to totally disagree with that poll about el's sexuality. the issue isn't her orientation or ability to have sexual attraction, but rather her emotional support system. a healthy understanding of the physical and emotional consequences of sex is an important learning curve for anyone, but for her, who has been abused and grown up in a lab, much of her formative years being skewed and used for ulterior purposes?

she's got a lot to learn, and s3 was clearly a breakout moment for that teen girl normality with her and max. they brought this back for s4 too, with the memory callbacks and max being the memory that was associated with el at the end. stupid boys, etc. this surely is what el's whole emotional arc/family arc/autonomy arc is about.

we also have el responding in a very child-like manner to max's comment about happy screams. yes, it's girlhood naivete, but MBB plays up the slowed-speech with El at times, and i think it's to show that certain topics really go over her head.

but sexuality is also innate and can appear very early, so i don't think it's a case of her needing to be older or whatever. its just that the duffers arent exploring el's sexuality in a way that's about discovery, like mike and will's, but rather about a girl who was sexualised too young (mainly by brenner, accidentally by mike - i mean he kissed her out of the blue for god's sake and she looked shocked af), and who needs to now move away from sexuality and romance for a while.

whether she's ace etc is part of her self discovery, and i think the show wants you to root for her taking her time to function as an emotionally stable young woman, not care about romance and love. that's a joy that can come later for her, but it's not going to be her endgame.

i really dislike it when mi-levens say its ableist to suggest el has special needs, meaning literally has special requirements to function in society. of course she does, and why is that bad? if anything, to remove her access to specialist support after her traumatic upbringing is toxic and damaging. mi-levens deny el the support she needs in order to fulfill their ship, saying that it's rude to suggest love and romantic companionship is weak. of course it's not weak - all humans need love and wanting romance is not weakness. but el needs time to figure out what she wants and who she is outside of the lab. she even says in s3, mike is just her first boyfriend. the show could not have worked harder to show us that her and mike don't work (which is a way of saying that el+romance doesnt work right now). what else do they have to do to convince people that el's arc isn't about romance or her sexual orientation??
