Background: Although the effect of exercise in patients during the subacute and/or chronic subacromial impingement syndrome has been reported, only a few studies have examined the effect of pain-free exercise in the acute phase of this disorder.
Objective: To compare the effect of training using the Neurac Physical Therapy which combines body segments suspension and vibration vs. manual therapy, on various relevant outcome parameters in patients with acute subacromial impingement syndrome (PASIS).
Method: Twenty-six patients underwent a 4 week intervention program. Using random assignment, half of the patients were treated using the Neurac device while the other half was treated using manual therapy. The outcome parameters consisted of shoulder pain, shoulder function, range of motion (ROM) and the isokinetic strength of the external and internal rotators at 60 and 180∘/s.
Results: Following either modes of interventions, the pain, function, and ROM improved significantly compared to pre-intervention levels in both groups. Time-by-group interaction was observed for the rotational strength which increased significantly and exclusively in the Neurac group.
Conclusion: Given its positive effect on shoulder pain, function, ROM and the isokinetic strength of the external and internal rotators, we recommend the application of the Neurac technique in PASIS.

Neurac – a new treatment method for chronic musculoskeletal pain.
The effect of exercise therapy on chronic musculoskeletal pain is generally low and moderate at best. In this paper, a new exercise treatment method, Neurac, is described with theoretical background, development, main content, and documentation. Neurac Physical Therapy is a treatment method that aims at regaining normal functional movement patterns in patients with musculoskeletal disorders, by using high levels of neuromuscular stimulation This is an active treatment approach including four main elements: 1. Bodyweightbearing exercises utilizing the Redcord sling system, 2. Controlled vibration to selected body parts, 3. Gradual increase of resistance (work-load), 4. No pain or no increase of existing pain. A newly developed vibration apparatus, Redcord Stimula, is often used to augment neural adaptations by a potential increase in the activation of muscle spindles. The Neurac method also includes testing procedures for the neuromuscular function of the kinetic chains, and the integration of «local» and «global» muscle function. Theoretically, Neurac is based on supporting research on bodyweight-bearing exercises loading the biomechanical chains. Further, based on neuroscience and clinical trials the method utilizes vibration to increase neural drive and decrease pain. The development of the Neurac-methodology has emerged from the S-E-T concept (Sling Exercise Therapy). The new methodology involves the use of the Redcord Trainer (formerly called TerapiMaster), workstation, and vibration apparatus. Documentation: Systematic observations in the posture work physical therapy clinic show changes after implementation, but research is needed to evaluate the efficiency of the Neurac-method as a physiotherapeutic tool.

In 2017, 288.7 million people around the world reported neck pain (1). That does not include all the people who had pain but said nothing about it! Although most acute episodes resolve spontaneously, more than a third of affected people still have low-grade symptoms or recurrences more than one year later. For managing neck pain the strongest evidence is for exercise (2). A good assessment will ensure that the right exercises are selected. More importantly, the 3D avatar helps the person experiencing the pain or dysfunction to better understand their problem and how to deal with it. The Neurac® method uses the principles of suspension therapy to stimulate the neuromuscular system so that function and posture can improve. It stimulates the receptors in the muscles and joints to communicate with the nervous system, enabling the body to activate and relax the right muscles at the right time. It also helps to modulate pain.
When the neck is not aligned with gravity, the load on the neck and back is increased. This can cause pain and fatigue, as well as headaches and visual disturbances. Assessment of posture and movement is complex as it involves the interplay of multiple body parts. Physical therapists at Postureworks identify and measure ‘antalgic postures’ (painful postures) and ‘postural orientation errors’ (poor dynamic alignment of body párts associated with poor function) using Moovment® 3D digital technology to record, measure, analyse and identify the source of the problem – not just the symptoms. Дорогие любители циклических или экстремальных видов спорта, когда вы приходите ко мне на прием, я не хочу видеть легкую тоску в ваших глазах. Не хочу снова отвечать на ряд привычных вопросов: «Доктор мне нужно оперировать сустав?», «А может можно консервативно разобраться?», «Я вернусь в спорт?». Мне хочется дать совет вам и каждому: новичку, любителю или даже профессиональному спортсмену. Может быть это когда-нибудь станет началом книги о профилактике спортивных травм и заболеваний опорно-двигательного аппарата, приобретенных в результате регулярных занятий спортом. Начну с моего любимого и, наверное, самого наглядного примера. Наше тело, как автомобиль, у которого есть кузов (опорно-двигательный аппарат) и двигатель (сердечно-сосудистая система). Тренировки, в большинстве случаев, делятся на 2 типа: - первые направлены на развитие тех или иных качеств двигателя (выносливость, скорость, скоростно-силовые показатели и т.д). - вторые – на работу с кузовом. Это походы в тренажерный зал, где занимаемся с железом или посещение модного функционального тренинга. Все эти тренировки нужны и даже необходимы. Но до того, как ими заняться, нужно восстановить правильную биомеханику работы суставов. То есть нужны тренировки, на которых вы очень много думаете (осознаете) и мало напрягаетесь. Почему так? Потому что мышцы, отвечающие за безопасность сустава, маленькие, глубокие и с ними надо работать осознанно. Эти мышцы называют локальными стабилизаторами. Они являются «системой безопасности» сустава (обеспечивают его долговечность) и его «системой жесткости» (как карбон, современный тренд в автомобилях, они обеспечивают стабильность конструкции и инерционность). В странах с развитой спортивной медициной давно признали, что эти мышцы самые важные для спортсмена. Именно поэтому работать с ними нужно по специальным программам (PNF, NEURAC, Polestar PILATES и т.д.). Тренеры и врачи, которые владеют техниками активации локальных стабилизаторов, во время работы со спортсменом задаются единственной простой целью – активировать работу слабых мышц и выключить сильные мышцы (глобальные мышцы, которые мы обычно стараемся развить). Хоть цель довольно проста, она сложно реализуется. Основная задача специалиста – провести мышечное тестирование, которое поможет найти слабые места (суставы, мышцы, связки). Ведь именно эти места без активации локальных стабилизаторов во время суммирования нагрузок могут «лопнуть» и привести к травме. Из вышеописанного следует вывод: в процессе тренировки уделяйте время «истинной» проработке глубоких мышц, а не их компенсации (в базовый этап тренировки – 30% времени, в восстановительный – 70% времени). Тогда глобальные двигательные мышцы не будут брать на себя функцию локальных стабилизаторов. В результате вы получите: - здоровые суставы; - инерционность в движениях; - повышение выносливости; - улучшенную координацию; - улучшение техники двигательного или моторного контроля. P.S.: Не дожидайтесь травм, найдите своего ортопеда, который увидит дисбаланс, и опытного тренера, знающего техники хотя бы Пилатеса (со стажем не менее 10 лет).
