What is the difference between a tubal pregnancy and an ectopic pregnancy?

In ectopic pregnancy, implantation occurs in a site other than the endometrial lining of the uterine cavity, i.e., in the fallopian tube, horn of the uterus, cervix, ovary, or abdominal or pelvic cavity. Ectopic pregnancies cannot be carried to term and eventually rupture or involute.

Early symptomatology includes pelvic pain, metrorrhagia, and tenderness to the vaginal touch, especially when the cervix is displaced.

Syncope or hemorrhagic shock may occur upon rupture. Diagnosis is based on beta-hCG measurement and pelvic ultrasound. Treatment consists of laparoscopic or open surgical resection or IM methotrexate injection.

The incidence of ectopic pregnancy is approximately 2/100 diagnosed pregnancies.

Etiology of ectopic pregnancy

Factors that particularly increase the risk of ectopic pregnancy include

Tubal lesions

A history of ectopic pregnancy (risk of recurrence 10-25%)

History of pelvic inflammatory disease (especially Chlamydia trachomatis)

Previous abdominal surgery especially on the fallopian tubes, including tubal ligation

Other specific risk factors for ectopic pregnancy include

Intrauterine device (IUD) contraception

Infertility due to tubal damage

Smoking

History of induced abortion

Pregnancy is less likely to occur when an intrauterine device (IUD) is in place; however, about 5% of these pregnancies are ectopic.

Pathophysiology of ectopic pregnancy

The most common location of ectopic pregnancies is the fallopian tube, followed by the uterine horn. Cervical pregnancies, in a caesarean scar, the ovary, the abdomen or the interstitium of the fallopian tube are rare.

Heterotopic pregnancies (both ectopic and intrauterine) occur in only 1/10,000 to 30,000 pregnancies, but may be more common among women who have used ovulation induction or assisted reproductive technologies such as in vitro fertilization and gamete intrafallopian tube transfer (GIFT); in such cases, the reported rate of ectopic pregnancies is ≤ 1%.

The structure containing the fetus usually ruptures after 6 to 16 weeks. Rupture results in hemorrhage that may be progressive or rapid enough to cause hemorrhagic shock. Intraperitoneal blood irritates the peritoneum. The later the rupture, the more rapid the bleeding and the higher the mortality.

Symptomatology of ectopic pregnancy

Symptoms of ectopic pregnancy vary and are often absent until rupture occurs. Most patients present with pelvic pain (sometimes in the form of cramping) and/or vaginal bleeding. There may or may not be a delay or absence of menstrual bleeding, and the patient may not know that she is pregnant.

Rupture may be heralded by sudden severe pain, followed by syncope or symptoms of hemorrhagic shock or peritonitis. Rapid hemorrhage is more likely in ruptured ectopic pregnancies.

Pain on mobilization of the cervix, unilateral or bilateral adnexal tenderness on vaginal touch, or an adnexal mass may be present. The uterus may be slightly enlarged (but less than expected from delayed menstruation).

Diagnosis of ectopic pregnancy

Quantitative serum human chorionic gonadotropin beta subunit (beta-hCG)

Pelvic ultrasound

Sometimes laparoscopy

An ectopic pregnancy is suspected in any woman of childbearing age with pelvic pain, metrorrhagia, or unexplained syncope or hemorrhagic shock, regardless of contraceptive use, sexual intercourse, or delayed menses. The results of physical examinations (including pelvic touch) are neither sensitive nor specific.

A ruptured ectopic pregnancy is a surgical emergency because it causes maternal hemorrhage with a risk of death; prompt diagnosis is essential.

The first step is a urine pregnancy test, which is nearly 99% sensitive in diagnosing pregnancies (ectopic and others). When the urine beta-hCG test is negative and the clinical signs are not strongly suggestive of ectopic pregnancy, further testing is unnecessary unless the symptoms recur or worsen.

If the urine beta-hCG is positive or the clinical signs are strongly suggestive of ectopic pregnancy, a quantitative serum beta-hCG measurement and pelvic ultrasound are not indicated.

When the quantitative serum beta-hCG is < 5 mIU/mL, ectopic pregnancy is excluded. When ultrasound detects an intrauterine gestational sac, an ectopic pregnancy is highly unlikely except in the woman who has used assisted reproductive techniques (which increases the risk of heterotopic pregnancy); however, a cornual or intra-abdominal pregnancy may initially present as an intrauterine pregnancy.

Ultrasound images suggestive of an ectopic pregnancy (seen in 16 to 32%) of cases include complex masses (mixed solid and cystic masses), particularly in the adnexa, and fluid effusion from the cul-de-sac of Douglas.

If the serum beta-hCG is above a certain level (called the discriminatory zone), the ultrasound should detect a gestational sac in case of intrauterine pregnancy. This level is usually about 2000 mIU/mL. If the beta-hCG level is above the discriminatory zone and no intrauterine gestational sac is detected, an ectopic pregnancy is likely. The use of the transvaginal route or color Doppler can improve detection rates.

If the beta-hCG level is below the discriminatory zone and the ultrasound is negative, it may be an early intrauterine pregnancy or an ectopic pregnancy. If the initial clinical workup suggests an ectopic pregnancy (e.g., significant peritoneal irritation or hemorrhage), diagnostic laparoscopy may be necessary for confirmation.

When an ectopic pregnancy appears unlikely and the patient is stable, plasma beta-hCG levels can be monitored on an outpatient basis (typically every 2 days). Normally, the level doubles every 1.4 to 2.1 days until day 41; in an ectopic pregnancy (as in non-progressive pregnancies), levels may be lower than expected and do not double as rapidly.

If the beta-hCG level does not rise as expected or falls, the diagnosis of spontaneous miscarriage or ectopic pregnancy should be considered.

Default replacements for afbodysweatervest, efbodysweatervest and tfbodytech, each age group in two different palettes, dubbed “Maxis-match” and “vintage”. And custom versions of everything. All have fat morphs, adults also have a preg morph.

As always with default replacements, you can only have one per outfit, so here you’re gonna have to choose between several options.

★ For afbodysweatervest:

I replaced the skirt with @platinumaspiration’s Copperdale skirt, recolored a bunch and keeping the original Maxis shoes. The top textures for the vintage version were made an anonymous person whose files I found a direct link to on the ts2 default database (8th link here).

Download instructions: basically you’re supposed to end up with one 1, one 2, one 3 and one 4. I imagine most people would stick with either four from the Maxis-match set or four from the vintage set, so they’re in different folders, but they’re all on the same mesh and you can mix and match.

★ For efbodysweatervest:

I replaced the skirt with Kalynn06′s Dress Pants (last download link on this page), edited to make the legs a bit narrower and keeping the original Maxis shoes.

Download instructions: same as above, as long as you end up with one 1, one 2, one 3 and one 4, you’re good.

★ For tfbodytech:

Same credits as for AF, but I also replaced the shoes: for the Maxis-match set, Vans taken from this bottom by @deedee-sims​; and for the vintage set, Semller Acne Taurus shoes converted to TS2 by @alwayssims and converted to TF by @hemfbg here.

Download instructions: technically same as above too, but since the Maxis-match files and the vintage files use two different meshes, be sure to keep both if you want to mix and match between the two sets.

So, as a CC maker and a part-time TS2 CC historian, let me tell you something about CarnabySims.

CarnabySims was run by Berg at MTS. This site specialized in 60′s vintage clothing back in the day, which I thought was cool then, and it’s cool now! But the site has been dead for a long time. 

One anon asked me where I got the turtleneck I use in a lot of previews, and I was happy to reupload it. It’s a very old mesh from CarnabySims, and I’d hung onto it for a long time. I thought that I’d do something with it eventually.

Well, the day came that I thought I’d take a closer look at it, and you know what? The arms had the dreaded black spots! It was hard to tell that those were there with the original texture being so dark.  I’m no expert mesher, but I do know how to fix black spots. And as it was a mesh with one group, it was simple to throw a preg and fat morph on it too. So - this is a *NEW* mesh! 

If you happen to have MESH_CarnabySims_AF_Top_Rollneck in your game already, my mesh will override it. 

AND as long as I was at it, I gave this a new texture as well. Base turtleneck texture is by lilisims, and the patterns I’ve used are by All About Style.

Analyse av Volume 1 av Pink Sweat$

Jeg hører veldig mye på sangen Honesty av Pink Sweat$, og den har vært min go-to sang den siste uken. Derfor tenkte jeg å analysere albumcoveret Volume 1. 

Sangene i EP’en er veldig såre og man legger merke til at artisten har gått igjennom et brudd. Alle sangene i albumet er veldig rolige, og oppfattes som veldig såre. Han synger ofte med en høy pitch i refrengene som oppfattes som at det er veldig vanskelig å synge disse lyricsene. Lyricsene reflekterer over dette evt brudde som har skjedd noe man kan se foreksempel i Honesty der han synger  “I told her, "Don't rush girl, dont you rush Guess it's all a game of patience.".  Baktanken med dette er om det han gjør det hun spør om vill følelsene fortsatt være gjensidig.

Hovedinstrumentet i sangene er gitar og har ofte ekko effekt over stemmen for å skape en rolig og behagelig stemning over sangene. Stemningen gjør at man føler det rolige og begynner å reflektere over lyricsene han synger.

Det er flittig fargebruk i coveret i form av kontraster. Det grønne mot rosa og blå skaper en sommerstemning som gir et nostalgisk preg. Han har flere EP’er der hovedfargen i alle coverne er rosa. Albumcoveret spilller veldig på femininitet med tanke på bruken av rosa i både klær og blomster. Samtidig har han en orange stripe over den rosa genseren som er midstilt med hekken som skaper en sentrert og harmonisk cover.

Typografien har en tydelig kontrast ved at han har valgt å blade inn to forskjellige fonter. I neoen fonten kommer gjennomgangen av rosa fargen, som harmonierer selve coveret. Å velge en moderne font mot en eldre tegnet font syntes jeg funker veldig bra, med tanke på at fargebruken i bildet er sterkt. At han har valgt å ta sitt eget navn større enn EP tittelen syntes jeg funker veldig bra med at neoen fonten er en veldig kraftig og bold font som tar mye oppmerksomhet med tanke på den tynne hvite vintagefonten. Ved at den tengnede vintage fonten er på skrå skaper en vri på det midstrilte bildet som skaper en liten ubalanse som fungerer fordi det reflekterer over lyricsene der han beskriver ubalansen mellom han og en jente. Et eksempel på dette er ubalansen i livet til jenta som har gått over til rumiddler. “Cause you left your heart in LA. You left your mind in Paris. Traded emotions in for cocaine”

De har valgt å ta fotografi for albumcoveret som får frem de ekte fargene man kan finne i naturen og gjør det mer ekte. Ved bruk av fotografien får man et ekte innsyn på naturen med tanke på hvis man skulle ha illustrert bildet. Hvorfor jeg tror de har valgt å ha bilde over illustrasjon er fordi man skal få frem det ekte med lyricsene som han synger. Han synger om ekte følelser og dette synes også på coveret. 

Pink Sweat$ er plassert midt i bildet med en røyk med et helt rosa antrekk med orange stripe på genseren. Kameraet er vinklet litt underifra slik at man skal få med den blå himmelen og de rosa rosebuskene. Han ser opp på tittlelen som gjør tydeligere hva albumet og artisten heter. Fra et dramaturgisk perspektiv mener jeg at han ser opp mot himmelen som et tegn etter lengter etter denne jenta han synger om. Meningen med platecoveret er å vise det realistiske og følelser man får når du liker noen men de ikke føler det samme tilbake . 

Jeg mener dette albumcoveret er en ny måte å få vekk fokuset på artisten og få frem det idylliske med naturen som også reflekterer over lyricsene i sangene.

Why I Have a Beard
