Why does Carol' and Rudy's bed have only one pillow?

The only options I could see are:

1. They sleep in separate beds for temperature/comfort reasons or... other reasons.

2. Rudy took his pillow with him to the hospital (don't blame him, hospital pillows are not that comfy)

or

3. They share one regular sized pillow like psychopaths

Could this be indicative of them having relationship trouble? maybe

Could it also be something meaningless or something that was overlooked? probably.
