to change NAME and/or GENDER on CT legal documents
A note - parental consent is needed for people under 18. For resources on name changes for minors, check out this resource...
STEP 1 - COURT ORDER
For name change only: file a petition and affidavit to the probate court in your district (find the court for your district here)
You will need:
2 forms of ID (at least 1 with a photo)
Long-form birth certificate
A social security number
$250 (or apply for a fee waiver)
STEP 2 - SOCIAL SECURITY CARD
Apply to update your social security card through the Social Security Administration (this is a federal process and is the same across all states).
A note - your social security card doesn't actually state your gender. But the SSA does keep a record of your gender, which you can change in their "database" (eerie, right?)
If you plan to change your name on your driver's license, you'll have to do this first. If you just want to change your gender marker on your driver's license, you can skip this step if you want.
You'll need to apply and submit certain documents for proof of identity and citizenship.
And for name and/or gender marker change, you can submit your court order (that's it!)
OR, for a gender marker change only, if it's easier for you, you can submit a signed letter from a health care provider saying that they have provided you with gender affirming treatment.
STEP 3 - DRIVER'S LICENSE
For name change, submit a Name Correction Request along with your social security card and court order reflecting your new name.
For gender marker change, submit a Gender Designation Change Form. As of January 27, 2020, the form includes a gender neutral (X) designation option.
(The form stipulates: "Employees shall not request additional gender-related information beyond that required on the applicable forms or otherwise inquire about the applicant’s private medical history or records.")
STEP 4 - BIRTH CERTIFICATE
First you have to request a packet from the Vital Records Office - contact department staff at 860-509-7956 or [email protected] to receive a copy by mail or in person.
Instructions will be included in the packet. But basically, you'll need to submit it along with:
A notarized affidavit on archival paper completed by you requesting a replacement birth certificate with your updated gender;
A notarized affidavit on archival paper completed by a licensed physician, a licensed advanced practice registered nurse, or a licensed psychologist, stating that you have undergone surgical, hormonal or other treatment clinically appropriate for the purpose of gender transition. 
A certified copy of a court order granting your legal name change, if you would like your amended birth certificate to reflect a new name;
A photocopy of a valid, government issued photo identification (e.g., driver's license, passport); and
$30 fee to receive a copy of the new birth certificate
