One way Sanderson could make a book real unsettling is to have Hoid not show up at all. Throughout the book you have moments where characters need support or a story told and it let's it linger on how no one is there, the characters only have themselves. At times a character might be described as hawkish and you get excited for a moments and it's just not him, every time. If Hoid doesn't show up you either know the characters plight isn't important enough to him or something has happened to him to make him unable to show up.
