Its called Unveiling the Unknown. You can choose at the beginning whether the main character is a transgender woman, or one of several slurs. But besides that, the writing in the visual novel gameplay is pretty good, and the optional dungeon-crawling gameplay is actually kind of addicting.

Desperate enough to consume some snack, drink, etc that’s really just a laxative, appetite suppressant, or dehydration agent to lose weight as quickly as possible.
