STATE OF FLORIDA

COUNTY OF VOLUSIA

BEFORE ME, the undersigned authority, personally appeared XXXX XXXXXXX who being first duly sworn, deposes and says:

I am at least 18 years old and am competent to testify.

I am a resident of Seminole County, Florida with an address of XXXX XXXXXXXX XXXX in XXXXXXX.

I am a former employee of Neo Matrix LLC. I was relieved of services on July 31, 2018.

I started working for Neo Matrix LLC on April7′h 2018. I was paid a salary of $60,000.00.

After I was hired, Mike Van Thielen (“Thielen”), the Chief Executive officer and Manager, said to me that I was hired so that I would be around for him to “fuck whenever he wanted”.

Thielen repeatedly texted me for sexual relations, including during working hours.

Thielen repeatedly sent me pornographic and sexually explicit texts during and after work.

Theilen told me that since I was salary, having to meet him for sex shouldn’t be questioned, and that he would alter his, my work, and patient schedules to make me available for him.

Thielen stated in front of Nicholas Sasso and Donald Bass, who are both Managers; Bobby Ardila, who is a Physician Assistant; and Maria Daily and Richard Williams, who are both employees, that “He (Thielen) was the fucking president and that he can do whatever he wants, and he never signed any papers that said he wouldn’t harass women.”

Thielen would hit or inappropriately touch my buttocks while at the office, including in front of other persons.

11. Thielen would make oral sexual gestures to me at the office, including in front of other persons.

At a meeting on July 5th, in front of the other Managers and the entire staff, Thielen said that I could be his “fluffer” and made numerous references to watching porn.

Thielen has continued to attempt to contact me even after my termination from the business.

The facts alleged in this affidavit are based on my person knowledge and I have reviewed the facts contained herein, and they are true and correct.
