研究背景: 暴露於高劑量放射治療所引起的皮膚傷口是臨床治療的重大挑戰。雖然目前有許多新的皮膚治療方式正在研究中,但尚未證實任何皮膚療法可用於治療放射性皮膚炎的損傷或是可以促進受損組織的修復。PRP是由富含血小板的血漿製成的具有生物活性的生物材料 (這篇文章寫做Plasma-based biomaterials, PBMs),可以製成固體和半固體形式,價格相對低廉,可作為現成的非冷藏產品。

Cutaneous wounds caused by an exposure to high doses of ionizing radiation remain a therapeutic challenge. While new experimental strategies for treatment are being developed, there are currently no off-the-shelf therapies for the treatment of cutaneous radiation injury that have been proven to promote repair of the damaged tissues. Plasma-based biomaterials are biologically active biomaterials made from platelet enriched plasma, which can be made into both solid and semi-solid forms, are inexpensive, and are available as off-the-shelf, non-refrigerated products. In this study, the use of plasma-based biomaterials for the mitigation of acute and late toxicity for cutaneous radiation injury was investigated using a mouse model. A 2-cm diameter circle of the dorsal skin was irradiated with a single dose of 35 Gy followed by topical treatment with plasma-based biomaterial or vehicle once daily for 5 weeks post-irradiation. Weekly imaging demonstrated more complete wound resolution in the plasma-based biomaterial vs. vehicle group which became statistically significant (p < 0.05) at weeks 12, 13, and 14 post-maximum wound area. Despite more complete wound healing, at 9 and 17 weeks post-irradiation, there was no statistically significant difference in collagen deposition or skin thickness between the plasma-based biomaterial and vehicle groups based on Masson trichrome staining nor was there a statistically significant difference in inflammatory or fibrosis-related gene expression between the groups. Although significant improvement was not observed for late toxicity, plasma-based biomaterials were effective at promoting wound closure, thus helping to mitigate acute toxicity.

研究目的: 使用 PRP 作為放療後皮膚毛髮損傷的預防。
研究方式: 使用12 隻 Wistar 大鼠進行雙盲試驗。由其中的2隻大鼠提供捐血獲得PRP,另外10隻用於臨床和組織學分析。
結果: 在放療前沒有使用 PRP 的區域:8 隻大鼠出現脫髮和 7隻大鼠發生局部色素減退,其中60% 出現完全脫髮,20% 出現部分脫髮,20% 未出現脫髮。在放療前有使用 PRP 的區域:4 隻出現脫髮和 3 隻色素減退,其中無完全脫髮病例,40%出現部分脫髮,60%未出現脫髮。從組織學分析中收集的數據顯示,有使用PRP 的那些區域中,100% 出現輕至中度萎縮;另一方面,沒有使用 PRP 的區域,80% 出現中至重度萎縮,20% 出現輕至中度萎縮。

Background and Objectives. Radiotherapy is usually associated with skin complications that affect the quality of life of patients and in some cases can also affect the timing of the reconstruction. The platelet rich plasma (PRP) is an emerging therapy; is autologous, safe and economic. Current use of PRP includes: efficacy in decreasing local inflammation, enhance wound healing, treatment of skin ulcers caused by radiotherapy and its use extends to the field of aesthetic plastic surgery improving the quality of the skin. The authors made an experimental trial to analyze the use of PRP as a prophylaxis method for skin damage after radiotherapy. We analyze the benefits of PRP as a prophylaxis method for skin damage after radiotherapy using an experimental model.
Methods. We conduct a double blind trial with rats. Twelve Wistar rats were used, 2 for obtaining the PRP and 10 for clinical and histological analysis.
Results. Data collected from the clinical analysis showed that those areas without PRP: 8 rats presented alopecia and 7 hypopigmentation. Total alopecia was present in 60%, partial alopecia in 20%, and 20% did not present alopecia. In those areas with PRP before radiotherapy: 4 presented alopecia and 3 hypopigmentation. There were no cases of total alopecia, 60% did not present alopecia and 40% presented partial alopecia. Data collected from the histological analysis showed 100% of mild-moderate atrophy in those areas with PRP. On the other hand, 80% of moderate-severe atrophy and 20% of mild-moderate atrophy was present on those areas without PRP.
Conclusions. The authors showed the benefit of PRP as a prophylaxis method for alopecia.

The antifibrotic potential of platelet-rich plasma (PRP) is controversial. This study examined the effects of PRP on in vitro transforming growth factor (TGF)-beta1-induced differentiation of fibroblasts into myofibroblasts, the main drivers of fibrosis, and the involvement of vascular endothelial growth factor (VEGF)-A in mediating PRP-induced responses. The impact of PRP alone on fibroblast differentiation was also assessed. Myofibroblastic phenotype was evaluated by confocal fluorescence microscopy and western blotting analyses of alpha-smooth muscle actin (sma) and type-1 collagen expression, vinculin-rich focal adhesion clustering, and stress fiber assembly. Notch-1, connexin 43, and VEGF-A expression were also analyzed by RT-PCR. PRP negatively regulated fibroblast-myofibroblast transition via VEGF-A/VEGF receptor (VEGFR)-1-mediated inhibition of TGF-beta1/Smad3 signaling. Indeed TGF-beta1/PRP co-treated fibroblasts showed a robust attenuation of the myofibroblastic phenotype concomitant with a decrease of Smad3 expression levels. The VEGFR-1 inhibition by KRN633 or blocking antibodies, or VEGF-A neutralization in these cells prevented the PRP-promoted effects. Moreover PRP abrogated the TGF-beta1-induced reduction of VEGF-A and VEGFR-1 cell expression. The role of VEGF-A signaling in counteracting myofibroblast generation was confirmed by cell treatment with soluble VEGF-A. PRP as single treatment did not induce fibroblast myodifferentiation. This study provides new insights into cellular and molecular mechanisms underpinning PRP antifibrotic action.

研究目的: 放射治療所引起的皮膚炎(以下簡稱放射性皮膚炎)是一種常見且有時很嚴重的副作用。本文章是隨機性研究,以了解使用蘆薈凝膠是否可以降低放射性皮膚炎的發生率及嚴重度。
研究方法: 100 名新診斷的乳癌患者在放射治療期間隨機接受蘆薈凝膠或不接受蘆薈凝膠,觀察並記錄其副作用。放射治療使用 6 MV 直線加速器的光子體外照射,總劑量為 50 格雷,每天 2 格雷,每週 5 次。
研究結果: 在第 2 週後兩組患者首次發現放射性皮膚炎。在蘆薈組和對照組中,2/8 名患者和 5/12 名患者在第 2 週後出現 第1級放射性皮膚炎(P 值 = 0.240)和第3級放射性皮膚炎(P 值 = 0.317)。 第 4 週後,數字為 18 例,23 例為 1 級放射性皮膚炎,只有 1 例為 3 級放射性皮膚炎(P 值 = 0.328)。 第 5 週後,蘆薈組有 31、12 和 2 名患者,對照組有 36、6 和 5 名患者分別出現 1、2 和 3 級放射性皮膚炎(P 值 = 0.488)。

Purpose: Radiotherapy induced dermatitis is a common and sometimes serious side effect. We conducted a randomized study to understand whether the adjunctive use of aloe vera gel might reduce the prevalence and/or severity of radiotherapy induced dermatitis.
Methods: One hundred patients with newly diagnosed breast cancer were randomized to receive aloe vera gel or nothing during adjuvant radiotherapy consisting of conventional external beam radiation using 6 MV mega voltage linear accelerator photons. The radiation portals were composed of breast fields in all patients and supraclavicular and posterior axillary fields in node positive cases. The total dose was 50 Gy with a daily fraction of 2 Gy, five fractions per week.
Results: Dermatitis was first found among patients of both groups after week 2. In the aloe vera and control groups, 2/8 patients and 5/12 patients had dermatitis grade 1 after weeks 2 (P value = 0.240) and 3 (P value = 0.317). After the 4th week, the numbers were 18 and, 23 for dermatitis grade 1 and only 1 for grade 3 dermatitis (P value = 0.328). After the 5th week, 31, 12 and 2 patients in the aloe vera group and 36, 6 and 5 of the controls had grades 1, 2 and 3 dermatitis, respectively (P value = 0.488).
Conclusion: Aloe vera exerted no positive effect on prevalence or severity of radiation dermatitis in this study.

PRP每月科技新知 (2022/10/31)- PRP可有效促進放射線皮膚炎癒合
研究背景:體外放射治療是癌症治療中非常重要的一環,但是,輻射線會導致嚴重的副作用,例如放射性皮膚炎,導致皮膚損傷與潰爛。此篇文章的研究目的主要是評估下列三種方式用於治療輻射引起的皮膚損傷的效果: 評估單獨使用微脂肪 (microfat)、微脂肪+ PRP、微脂肪+血管基質組份(SVF)。
研究方法:隨機使用5個實驗組,每組9隻小鼠:1個健康對照組和4個放射治療組(60 格雷)。在放射治療後3個月,使用下列方式來治療皮膚傷口;微脂肪、微脂肪+PRP(50%-50%)、微脂肪+SVF(90%-10%)、乳酸鈉溶液進行皮下注射。 在用藥後1、2和3個月評估���口癒合情況,並在第3個月安樂死小鼠進行組織學傷口分析。
結果:所有接受放射治療的小鼠均出現傷口。在給藥後,對照組的傷口面積增加了 91.1%、單獨使用微脂肪的傷口面積減少了15.9%、微脂肪+SVF的傷口面積減少了 27.3%、微脂肪+ PRP的傷口面積減少了76.4% (P=0.032)。

BACKGROUND: External radiotherapy has become indispensable in oncological therapies. Unfortunately, radiation is responsible for serious side effects, such as radiodermatitis. The skin is weakened and ulcerated. Our study aimed to evaluate the subcutaneous transfer of microfat (MF) alone and two mixes: MF+Platelet-rich plasma (PRP) and MF+stromal vascular fraction (SVF) to treat radiation-induced skin lesions.
METHOD: We defined randomly five experimental groups of nine mice: 1 healthy control group and 4 irradiated (60 Gy) and treated groups. The skin lesions were treated 3 months after irradiation by MF, MF+PRP (50%-50%), MF+SVF (90%-10%) or Ringer-lactate subcutaneous injections. Wound healing was evaluated at 1, 2 and 3 months post-injection and histological wound analysis at 3 months, after euthanasia.
RESULTS: All the irradiated mice presented with wounds. After sham-injection, the wound area increased by 91.1+/-71.1% versus a decrease of 15.9+/-23.1% after MF alone (NS), 27.3+/-23.8% after MF+SVF (NS) and 76.4+/-7.7% after MF+PRP (P=0.032). A significantly reduction of skin thickness in wound periphery was measured for the three treated groups compared to sham-injection (P<0.05) but not in the healed wounds (NS). The most important subcutaneous neo-vessel density was shown after MF+SVF injection.
CONCLUSION: The MF+PRP mix was the most efficient product to increase healing. The MF+SVF mix showed the highest rate of neo-angiogenesis but was disappointing in terms of healing.

研究背景:各種形式的放射線都會造成皮膚損傷。然而,針對嚴重、急性放射性皮膚損傷的治療選擇很有限。目前已知PRP可改善皮膚傷口癒合,但PRP對於高劑量放射線照射的效果仍知之甚少。

BACKGROUND: The skin is impacted by every form of external radiation therapy. However, effective therapeutic options for severe, acute radiation-induced skin reactions are limited. Although platelet-rich plasma (PRP) is known to improve cutaneous wound healing, its effects in the context of high-dose irradiation are still poorly understood.
METHODS: We investigated the regenerative functions of PRP by subjecting the dorsal skin of mice to local irradiation (40 Gy) and exposing HaCaT cells to gamma rays (5 Gy). The cutaneous benefits of PRP were gauged by wound size, histologic features, immunostains, western blot, and transepithelial water loss (TEWL). To assess the molecular effects of PRP on keratinocytes of healing radiation-induced wounds, we evaluated AKT signaling.
RESULTS: Heightened expression of keratin 14 (K14) was documented in irradiated HaCaT cells and skin tissue, although the healing capacity of injured HaCaT cells declined. By applying PRP, this capacity was restored via augmented AKT signaling. In our mouse model, PRP use achieved the following: (1) healing of desquamated skin, acutely injured by radiation; (2) activated AKT signaling, improving migration and proliferation of K14 cells; (3) greater expression of involucrin in keratin 10 cells and sebaceous glands; and (4) reduced TEWL, strengthening the cutaneous barrier function.
CONCLUSIONS: Our findings indicate that PRP enhances the functions of K14 cells via AKT signaling, accelerating the regeneration of irradiated skin. These wound-healing benefits may have merit in a clinical setting.

BACKGROUND: Platelet-rich plasma (PRP) is an autologous concentration of human platelets contained in a small volume of plasma and has recently been shown to accelerate rejuvenate aging skin by various growth factors and cell adhesion molecules. OBJECTIVE: This study was conducted to evaluate the efficacy and safety of intradermal injection of PRP in the human facial rejuvenation.
METHODS: This study was a prospective, single-center, single-dose, open-label, non-randomized controlled clinical study. PRP injected to the upper site of this right infra-auricular area and all face. Saline was injected to the left infra-auricular area. Histopathological examinations were performed before PRP treatment, 28 days after the PRP, and saline (control) treatments.
RESULTS: Twenty women ranging in age from 40 to 49 years (mean age, 43.65+/-2.43 years) were enrolled in the study. The mean optical densities (MODs) of collagen in the pre-treatment, control, and PRP-treated area were measured. They were 539+/-93.2, 787+/-134.15, 1,019+/-178, respectively. In the MOD of PRP, 89.05 percent improvement was found when MOD of PRP was compared with MOD of pre-treatment. The mean MOD of collagen fibers was clearly highest on the PRP side (p<0.001). The PRP-to-saline improvement ratio (89.05% to 46.01%) was 1.93:1. No serious side effects were detected.
CONCLUSION: PRP increases dermal collagen levels not only by growth factors, but also by skin needling (the mesotherapy technique 'point by point'). PRP application could be considered as an effective (even a single application) and safety procedure for facial skin rejuvenation.

Cutaneous wounds caused by an exposure to high doses of ionizing radiation remain a therapeutic challenge. While new experimental strategies for treatment are being developed, there are currently no off-the-shelf therapies for the treatment of cutaneous radiation injury that have been proven to promote repair of the damaged tissues. Plasma-based biomaterials are biologically active biomaterials made from platelet enriched plasma, which can be made into both solid and semi-solid forms, are inexpensive, and are available as off-the-shelf, nonrefrigerated products. In this study, the use of plasma-based biomaterials for the mitigation of acute and late toxicity for cutaneous radiation injury was investigated using a mouse model. A 2-cm diameter circle of the dorsal skin was irradiated with a single dose of 35 Gy followed by topical treatment with plasma-based biomaterial or vehicle once daily for 5 weeks postirradiation. Weekly imaging demonstrated more complete wound resolution in the plasma-based biomaterial vs. vehicle group which became statistically significant (p < 0.05) at weeks 12, 13, and 14 postmaximum wound area. Despite more complete wound healing, at 9 and 17 weeks postirradiation, there was no statistically significant difference in collagen deposition or skin thickness between the plasma-based biomaterial and vehicle groups based on Masson trichrome staining nor was there a statistically significant difference in inflammatory or fibrosis-related gene expression between the groups. Although significant improvement was not observed for late toxicity, plasma-based biomaterials were effective at promoting wound closure, thus helping to mitigate acute toxicity.

Psoriasis is a chronic immune-mediated disease affecting approximately 2–3% of general population. It is associated with significant negative impact on patient’s physical and psychological quality of life along with vulnerability to co-morbidities such as cardio-metabolic dysfunction, depression, and so on [1]. to evaluate the efficiency of platelet rich plasma (PRP) in the management of patients with chronic localized recalcitrant plaque psoriasis. This was a prospective pilot study conducted on 30 cases with chronic localized recalcitrant psoriasis attended Dermatology, Venereology and Andrology outpatient clinics, Benha University Hospital. This study was approved by Dermatology, Venereology and Andrology Department and Ethics Commitee in the Faculty of Medicine, Benha University. The study details were discussed with all patients and informed consents were obtained from them before the beginning of the study. PASI (Psoriasis Area and Severity Index) score had significantly decreased in group I after 8 and 16 weeks of treatment when compared to the baseline (p =0.001). There was also significant decrease in PASI score between week 8, 16 (p =0.0018). PASI score showed no significant difference in group II neither at week 8 nor 16 compared to baseline (p =0.894).
Conclusion: Platelet Rich Plasma (PRP) therapy represents a promising new therapeutic option for the patients with chronic localized recalcitrant psoriasis Plaques. The ability to use PRP could resolve several problems occurring with other treatment modalities of psoriasis. 文章出處:

Atopic dermatitis (AD) is a chronic inflammatory skin condition that most often presents in childhood but also affects many adults. Most patients with AD are treated with moisturizers and topical corticosteroids. However, these treatments do not always allow for an adequate control of the disease. Platelet-rich plasma (PRP) is used in treating many dermatologic conditions. It works by releasing bioagents, such as growth factors, from platelets, which induce a wound-healing cascade involving inflammation and tissue regeneration that promotes skin health. Here, we report a case of AD in which the patient successfully achieved an extended control of skin symptoms with PRP therapy.
A 44-year-old woman presented in May 2016 with a long-standing history of recurrent facial eczema. Six months later, the patient returned for a refill of the hydrocortisone cream and inquired about treatments for infraorbital dark circles and wrinkles. After discussing the treatment options, we decided to proceed with PRP injections, and informed consent was obtained. Two PRP therapy sessions were performed, spaced 6 weeks apart, using a 4-cc platelet-rich fibrin matrix kit (Selphyl, Aesthetic Factors, Inc). In addition to administration beneath the eyes, PRP was also administered into nasolabial folds for rejuvenation purposes.

Based on our results and those of other studies, there is an indication for the potential use of PRP in the management of AD and other types of eczema. Two years of symptom relief is unprecedented. A randomized clinical trial with a greater number of participants is warranted to rigorously evaluate the seemingly positive effects of PRP in treating skin eczema, including AD. Further research may also shed more light on the pathophysiology of AD as well as the biochemical pathways through which PRP can exert its therapeutic effects.

Platelet-rich plasma (PRP) has gained popularity in facial plastic surgery because of its role in wound healing. PRP, having a higher concentration of platelets, allows for greater release of growth factors and biologically active proteins, which in turn activates the wound-healing cascade stimulating neoangiogenesis and collagen production. One of the most popular uses for PRP is for facial skin rejuvenation in the form of dermal injections and topical application during microneedling. The promising nature of PRP makes using it for injection and/or in conjunction with microneedling a good addition to any practice that deals with skin rejuvenation.

Platelet-rich plasma (PRP), a platelet concentrate contained in a small volume of plasma, has become a promising option in the last decade to treat different diseases related to the skin due to its high concentration of growth factors. When it is of autologous origin, it decreases the probability of suffering adverse reactions and transfusion-transmitted infections, thus it is an optimal and safe therapy for the patient. PRP has been used in the treatment of several dermatological conditions such as acne, alopecia, and skin ulcers. Its use has also extended to other skin conditions such as melasma, hyperpigmentation, and burns, where it stimulates tissue repair and regeneration. The purpose of this article is to review the management and treatment of different dermatological alterations with PRP.

PURPOSE: To evaluate the potential of heterologous platelet-rich plasma (PRP) gel for surgical skin wound healing in rabbits.
METHODS: Blood from a single healthy dog was used for PRP production, with calcium gluconate added to the PRP to form the gel. Two surgical excisions, one to the right and the other to the left of the dorsal midline, were made in six rabbits. One side was randomly allocated to topical application of a physiological solution, and the other was allocated to treatment with heterologous PRP gel. Clinical assessments (weight, pain sensitivity, coloring, edema, hyperemia, exudation, crust, and granulation) and morphometric evaluations were performed 0, 3, 7, 10, 14, and 17 days postoperatively. Histological analysis was performed on the 17th day.
RESULTS: With the exception of the presence of a crust at day 10, clinical variables did not differ significantly between the experimental groups. In both the control and PRP-treated groups, differences were identified when comparing time-points in terms of wound area reduction. Histological results indicated no significant differences between the control group and the PRP-treated group.
CONCLUSION: Heterologous platelet-rich plasma gel promoted dermal wound healing in rabbits with no adverse effects.

Objective: In this study, our aim was to investigate the clinical effects of platelet-rich plasma (PRP) on the skin of patients suffering from plaque psoriasis or atopic dermatitis.
Methods: Over a period of 53 months, we treated a total of 40 patients for inflammatory skin diseases with PRP. All of these patients were included in this study; 5-6 ml of PRP were prepared with the autologous-conditioned plasma (ACP) double syringe and injected subdermally. Follow-ups were conducted at three, six, nine, and 12 weeks after treatment. Besides the lesion size, Psoriasis Area and Severity Index (PASI) and Eczema Area and Severity Index (EASI) were also calculated. Data were evaluated statistically at a significance level of p≤0.05.
Results: A total of 30 patients were treated for plaque psoriasis. The elbow area represented the most common area of treatment (17 cases). The average lesion size decreased from 8.2 cm² to 0.3 cm² (p<0.00001). Of note, 80% of all patients achieved complete remission (PASI100) at the last follow-up. The remaining 20% reached at least PASI70. Ten patients were treated for atopic dermatitis. In six cases, efflorescences on patients’ arms were treated, and in four cases, patients' legs were treated. The average lesion size decreased from 8 cm² to 0.155 cm² (p<0.00001). Notably, 50% of all patients achieved complete remission (EASI100) at the last follow-up. The other half reached at least EASI70. In all cases, the lesion size decreased progressively. No adverse events were reported.
Conclusion: Our study revealed encouraging results for both psoriasis and atopic dermatitis. The autologous treatment was safe and effective in all patients. Further studies are required to validate these initial findings.
