To put it bluntly, the only way to end homelessness is to house people or kill them.

That's it. Those are your options. Homelessness doesn't go away based on "crime crackdowns." You will imprison the unhoused, and when they are free, they will be unhoused again. Or they will die in prison.

When you burn encampments or force people out without their stuff, they remain unhoused or freeze to death.

After working with the unhoused, it's become abundantly clear that those who don't want to support them instead want them dead.

There is blood on the hands of every government and organization that controls the homeless population with force instead of compassion.

I had a patient come in the other day because she wanted her IUD replaced. It was at the end of it's life and she loved having her IUD, this was her second one. My MA let me know that the patient was very anxious about getting the IUD replaced, she'd had painful experiences in the past with her other replacements and was dreading this visit for that reason.

I spoke with the patient and she was literally shaking with anxiety. I asked her to describe her prior experiences as well as what she liked about her IUD and what she didn't like. She said that she wished she didn't have to get it replaced so often, so I recommended we place a Mirena instead of just inserting a new Kyleena IUD. She was nervous about this because she didn't want an IUD that was big. I explained that the Mirena and Kyleena are essentially the same size but the Mirena lasts 3 years longer and would likely bridge her to menopause given her age whereas with the Kyleena she'd probably need another replacement to get there. She was okay with trying the Mirena.

I then talked to her about pain control during the procedure as this was what she was most worried about. I asked about her prior experiences and then laid out what I wanted to do to try and improve her experience during this procedure. I told her I planned to give her prescription strength ibuprofen, a heating pad, and a very dense anesthetic block in her cervix to hopefully make it a better experience. If she had had someone to drive her home I would've also given her an ativan because we have studies that show patients who report higher rates of anxiety surrounding a procedure also report higher rates of pain associated with it.

She was down for this plan. I gave her a very dense block, she only felt three small injections and then nothing else. She was shocked when I told her that her old IUD was out and the new one was in. She didn't believe me when I told her it was over.

I don't tell this story because I wanna brag about how amazing of a doctor I am because I'm not. I tell this story because this is the way IUD insertions SHOULD go and I want people to know that IUD insertions do not need to be traumatic. And I want other providers who may insert IUDs to know that a paracervical block should be your standard when it comes to IUD insertions.

When people find out I'm an OBGYN, complete strangers, acquaintances, etc. , the two things they like to tell me immediately are their horrible birth trauma story and their terrible IUD insertion story and I'm trying to at least make the latter one a little less common.

If you place IUDs and aren't doing a cervical block, you need to start. This should be the standard but over 90% of OBGYNs in the US aren't doing them and it's unacceptable. We are traumatizing people and it's entirely avoidable. We are scaring people away from one of the most effective and long lasting forms of birth control in a time when people are losing their ability to end unwanted pregnancies all for no justifiable reason.

"It takes too long:" No it doesn't, that visit took me 20 minutes with a highly anxious patient from start to finish.

"It's not worth it for such a short/small procedure." It's worth it for the patient.

"It's too expensive." You can do a paracervical block with just normal saline. You don't even need lidocaine if you use a generous amount of volume. And if you place Nexplanons I know you stock lidocaine in your office, stop being fucking cheap at the expense of women's pain.

"Patient's don't need it, they'll get over it." I'm telling you they do need it and they aren't getting over it as evidenced by literally everyone wanting to tell me about their terrible IUD insertion experience as soon as they find out I'm an OBGYN.

We should do better. The cervix has nerve endings, stop acting like it doesn't.

Make cervical blocks your standard of care, there's no excuse not to.

someone got really mad at me for being pro-pornography so i'd like to be annoying for a little longer:

-there are enormous problems with exploitation in the porn industry that harm and endanger the people within it, and that harm is carried out mostly against women and minorities

-this is Bad

-however the last century of passing laws against pornography hasn't actually helped any of those problems, and what sex workers tend to advocate for is the legitimization of their labor, so that they can then access the same protections and regulations that people in other industries can access

-for instance football players, miners, roofers, and warehouse workers are also exploited and endangered by their professions, have to work long hours, and can end up traumatized and disabled by unregulated and unsafe working conditions. these people are used up and thrown away by powerful bosses they can't individually challenge.

-however because these industries are not de facto illegal to participate in, when these people form unions and demand better working conditions, they can at least fight for their rights.

-sex workers, who engage in heavily stigmatized work that's also often illegal, have little recourse to demand better treatment.

-even if you don't like porn, and especially if you don't like porn, if you care about the women who are exploited in pornography, you need to advocate for the legality of pornography.

-the more illegal the porn industry is, the less safe and fair it is, and people will still be working in it, no matter how illegal it is.

-again: the porn industry should be regulated like any other industry and subject to laws guaranteeing fair compensation for labor, safe working conditions, and legal resources for workers suffering exploitation and abuse.

-once it is legal to do sex work, then women can bring charges against the men who have broken their contracts and abused them.

-and that is why i push back against posts saying that pornography is evil. it is an entertainment product, made by people, to meet an ongoing demand. criminalizing the consumption and production of it may slightly lessen the demand at the incredible cost of endangering everyone involved. and i think that is what's evil.

speaking of peeing the bed it's been long enough that i can tell this story publicly. in high school i went to a party at some house with no adults, as you sometimes would, and at the end of the night like 10 people all clonked out together in the same bed. fully clothed, one of those teenage moments where you're like wow heehee how rule-breaking, because sure a lot of our parents wouldn't like us sleeping in a bed with a bunch of other teenagers and no adult supervision blah blah. fond memories. anyway.

i'm an extremely light sleeper, so i barely slept, and sometime around 6 am, i woke up to a girl totally panicking, very quietly, because she peed the bed in her sleep. and listen. this wasn't a group of mean kids by any measure. but there's no level of kindness or understanding in the world that will make peeing the bed when you're 17, surrounded by people you only sort of know, a gentle blow.

so i sat up and she was like "oh my god" and I signaled at her to be absolutely silent and I said I'd be right back. And I crawled over everyone and out of the bed like a stupid cat.

and the thing is, by senior year i wasn't getting bullied much anymore. i was generally pretty well liked by my peers, but, if this makes sense, people still didn't always expect very much from me. i was still figuring out how to mask (autistic) and i still often said or did something that made everyone remember i'm weird and they'd just be like "well. that's story for you. i guess." and for the most part i'd become pretty secure in that.

so what i'm saying is i had nothing to lose and this girl had everything to lose.

so i went downstairs and i made tomato soup. and by "made" i mean i put a whole can of tomato soup in a too-small mug and microwaved it until it was lukewarm so as to be convincingly "made" but not so hot to burn someone.

and then i walked back upstairs, and no longer like a cat, i clumsily "attempted" to crawl back into bed, loudly lost my balance, and spilled tomato soup all over the girl and her lap and several other people's laps and heads and the mattress.

everyone woke up confused and anguished and i was like, "oh my god, I'm so sorry. I just got really hungry and it's all i could find."

and everyone immediately accepted with absolutely no further questions that I would go downstairs, make tomato soup at 6 am,and bring it back to bed. everyone just begrudgingly climbed onto the floor and went back to sleep while I put the bedding right into the laundry.

i don't even know this girl's name. i only remembered this story recently because i'm in my hometown for a few months and recently a high school acquaintance said, "hey. do you remember spilling soup on everyone after prom? why did you do that?" and for a moment i genuinely did not and i stared at them completely dumbfounded while the memory loaded and then i started laughing too hard to answer for 2 minutes.

the best part is i can tell this story, and even if it reaches the people who were there, none of them will know which one of them peed the bed. thanks to tomato soup.

bout to hit my bitch coworker with the 'do you treat everyone like this or only your gay coworkers?' because i know she'll bluescreen about it. and im gonna make sure to do it when no one else is around just like she does to me with her snide bitch-ass comments. i've literally never pulled the gay card before, but i talk about my wife often enough that everyone knows. I've also got a reputation for being very level-headed, straight-forward, and conflict-adverse. No One Will Believe Her.

Hey, if you did work over the past few weeks to try to stop the GOP's obscene and murderous budget: thanks. It ended up without the AI regulation ban, the trans healthcare ban, the western lands sell off, and without its stupid name being official.

It passing was a major loss for everyone in this country including the very wealthy and other people who believe they will be able to be better off from it.

But also, it passing was not as bad as it could have been and it took the ongoing effort of huge amounts of people to even make that a possibility, so if you were part of that, from the bottom of my heart: Thank you.
