Abstract Medical Expenditures Understanding whether the type of primary caregiver and end-of-life (EOL) care location are associated with EOL medical expenditures is crucial to inform global debates on policies for efficient and effective EOL care. This study aims to assess trends in the type of primary caregiver and place of death stratified by rural-urban status among the oldest-old population from 1998-2018 in China. A secondary objective is to determine the associations between rurality, the type of primary caregiver, place of death and EOL medical expenditures. Methods A total of 20,149 deaths of people aged 80 years or older were derived from the Chinese Longitudinal Health Longevity Survey (CLHLS). Cochran-Armitage tests and Cuzick’s tests were used to test trends in the type of primary caregiver and place of death over time, respectively. Tobit models were used to estimate the marginal associations of rurality, type of primary caregiver, and place of death with EOL medical expenditures because CLHLS sets 100,000 Chinese yuan (approximately US$15,286) as the upper limit of the outcome variable. Results Of the 20,149 oldest-old people, the median age at death was 97 years old, 12,490 (weighted, 58.6%, hereafter) were female, and 8,235 lived in urban areas. From 1998-2018, the prevalence of informal caregivers significantly increased from 94.3% to 96.2%, and home death significantly increased from 86.0% to 89.5%. The proportion of people receiving help from informal caregivers significantly increased in urban decedents (16.5%) but decreased in rural decedents (-4.0%), while home death rates significantly increased among both urban (15.3%) and rural (1.8%) decedents. In the adjusted models, rural decedents spent less than urban decedents did (marginal difference : $-229 ). Those who died in hospitals spent more than those who died at home ($798 ). No difference in medical expenditures by type of primary caregiver was observed. Conclusions Over the past two decades, the increases in informal caregiver utilization and home deaths were unequal, leading to substantially higher EOL medical expenditures among urban decedents and deceased individuals who died at hospitals than among their counterparts who lived in rural areas and died at home. Because Adobe Director Projectors are 32-bit applications, SPR for Mac will not work on MacOS Catalina or Big Sur. This means that Flashpoint for Mac does not support Shockwave games on Catalina or Big Sur.
MacOS Big Sur (MacOS 11.0)
Flash applications have been successfully tested and are working. To use Flashpoint with Big Sur, a few modifications are required. Follow the steps below:

Apple Silicon Macs
To run Flashpoint on Macs with the new M1 chip, you'll need to install Rosetta by following these instructions. Once you've done that, follow the instructions in the previous sections to get Flashpoint working.
Installing Java
To add support for Java games, you'll need to install the Java Development Kit. Follow the steps below:

You can start Flashpoint without having to click on StartFlashpoint.command by using Automator using the below steps:

Drag the Run Shell Script action into your workflow.
Enter open /Users/[YOUR USERNAME HERE]/Downloads/Flashpoint Infinity for Mac Beta/StartFlashpoint.command in the command field.
Technologies
Currently, only Flashpoint Infinity is available for Mac. However, as explained here, both PHP and Apache are included with MacOS. This means it should be fairly simple to create a Flashpoint Ultimate package for MacOS as well. Let us know in our Discord if you can help!
No equivalent to the Flashpoint Redirector currently exists for MacOS. MacOS also does not provide any native way to set per-application proxy settings. In Flashpoint for Mac, the proxy settings for each supported technology must be set separately. This rules out using the Flash projectors, since they do not have configurable proxy settings. In Flashpoint for Mac, all Flash games run in a browser.
The Flashpoint FAQ contains a list of all web game technologies ('Platforms') supported in the Windows version of Flashpoint. The Mac version currently supports a small subset of these platforms. This is explained in detail below. The list of supported platforms includes an unscientific estimate of the percentage of games supported by our method.

Flash: Supported using the SeaMonkey web browser, with a Flash plugin included. Games that use a Flash Player version other than 32 are not yet supported.

Unsupported Platforms
Unity Web Player: Has not been attempted yet. We would need to track down the Mac builds of all 12 versions of Unity Web Player that we support on Windows. Needless to say, this would be a huge challenge.
PopCap Plugin: As far as we know, there is no version of PopCap Plugin for modern Macs.
Authorware Web Player: Authorware is incompatible with all Intel Macs.
GoBit Plugin: As far as we know, there is no version of GoBit Games Plugin for modern Macs.
Silverlight: Silverlight for Mac is available here, but as explained in this StackExchange answer, Microsoft Silverlight for Mac is 32-bit only, even though the Windows version has 64-bit and 32-bit versions. This is a problem because no major browser for MacOS has a 32-bit version, and MacOS Catalina drops support for 32-bit programs entirely. As such, Silverlight is unusable on Mac.
3DVIA Player: As far as we know, there is no version of 3DVIA Player for modern Macs.
3D Groove GX: 3D Groove is incompatible with all Intel Macs.
ActiveX: ActiveX is a Windows-only technology.
ShiVa3D: The Windows version of the ShiVa3D Player is available in the free ShiVa Web Edition; this is where we obtained it. However, no Mac version of the ShiVa Web Edition is currently available, so we haven't been able to find a Mac version of the ShiVa3D Player. It's possible that we could extract a Mac version of the player from a Mac version of a ShiVa3D game. Or maybe we could obtain the Mac runtime from the Windows version of ShiVa3D Web Edition, since it has the capability of publishing for Mac.
Launcher Issues
A number of Flashpoint Launcher issues affect the experience of using Flashpoint Launcher on MacOS. Unfortunately, none of our Launcher developers currently have access to a Mac, so updates for the Mac version of Flashpoint Launcher will not be available in the foreseeable future. We will be unable to release a Mac version of Flashpoint 7.0 until a new version of the Launcher is built for MacOS. If you have access to MacOS and are able to build Flashpoint Launcher, please let us know!
The current issues with the Mac version of Flashpoint Launcher are listed below.
Errors are shown in the log when services are missing from the services.json. These services are unnecessary for Flashpoint on Mac, so the errors can be ignored.
Flashpoint Launcher cannot find its own Version file, even if it is present in the Launcher's folder and app package.
The Theme dropdown menu on the Config tab does not work properly. To select a theme, you must manually type the path to the theme's CSS file.
A popup appears if the user attempts to start a Windows executable from Flashpoint. This popup tells the user to install Wine, and points them to the Linux Support page, even though the Launcher is running on Mac.
If a shell script is specified in execs.json as an alternative to a batch script, the Launcher will attempt to launch the shell script defined for Linux, even if a different shell script is defined for Darwin (MacOS).
Flashpoint Launcher attempts to load the hidden resource forks of certain files as separate files. These files are not visible in the MacOS Finder, and have filenames beginning with ._.
If any Platform XML has a resource fork, all of the Platform XMLs will fail to load, causing a 'No games found' error.
If any localization JSON file has a resource fork, a 'NotValidJSON' error will appear in the logs.
When Wine is started from Flashpoint Launcher, it uses >70% CPU for several minutes without ever starting the intended executable. This is despite Wine working fine when started from the terminal. This issue needs to be investigated and solved before Flashpoint can support launching games through Wine.
By default, Flashpoint Launcher will attempt to read and modify config.json and preferences.json inside its own app package. Since Mac applications are not allowed to modify their own app packages, this will fail. For this reason, Flashpoint Launcher must be opened with a shell script command StartFlashpoint.command. This script sets the current directory to the Flashpoint Launcher folder so that the Launcher accesses the config.json and preferences.json in the correct location. However, this unusual way of opening the Launcher may be the cause of the previously mentioned Wine bug.
Other Notes
Some keyboard controls such as the spacebar or WASD keys may not work in some games. This is a known problem with the NPAPI Flash plugin on MacOS. We plan to resolve this problem in the next build of Flashpoint for Mac by using the PPAPI plugin instead.
Flash games with URLs pointing to filenames with special characters will not work. This is because nosamuEmbedSWF.sh does not properly handle special characters when it converts URLs to paths, so it cannot copy the game's HTML to the correct location in htdocs.
Some Shockwave games may not close when you quit them from the menu. If this occurs, click inside the Message Window next to the game. Then type quit and press Enter to close the game.
Flashpoint for Mac uses exec mappings to determine what native executable to run based on the specified Windows application path. For example, flashplayer_32_sa.exe is mapped to nosamuEmbedSWF.sh for MacOS. To add more exec mappings, navigate to Flashpoint's Data folder and open execs.json in a text editor. The exec mappings use 'darwin' to refer to MacOS.
Flashpoint for Mac uses swfbbox to determine the width and height of an SWF file. swfbbox is part of the swftools homebrew package. To install swftools, first install homebrew. Then type brew install swftools in the terminal.
Flashpoint for Mac requires that PHP and cURL are enabled on your system. These components should be enabled by default.

Here are some steps to keep in mind when packaging Flashpoint for Mac. This assumes that you are using the current package as a base and modifying it.
You can follow these instructions to update Flashpoint's XMLs to the latest version.
You can follow these instructions to update Flashpoint Router to the latest version.
Use update-spr.sh in the FPSoftware folder to update SPR to the latest version. To get the latest version of SPR for Mac, see SPR for Mac.
If you would like to distribute a modified SeaMonkey profile, be sure to clear the cache, cookies, and other data. In SeaMonkey, click Tools -> Clear Private Data.
If you would like to add a plugin to SeaMonkey, first install the plugin normally. Then, follow these steps:
Navigate to /Library/Internet Plug-Ins in Finder and copy the files for the plugin you want to add.
Navigate to Flashpoint's FPSoftware folder. Right-click SeaMonkey.app and choose 'Show Package Contents.'
Inside the app package, navigate to /Contents/Resources/plugins. Paste the plugin files that you copied in Step 1.
Before running or distributing Flashpoint, you should run the dot_clean command inside the Flashpoint folder to remove resource forks. Flashpoint Launcher may not work properly if Flashpoint files have resource forks, as explained in Launcher Issues.

Upgrading from Amazon RDS for MySQL version 5.5
Amazon RDS for MySQL 5.5 major version is reaching end of life, and it’s recommended to upgrade to newer supported major versions. Amazon Relational Database Service (Amazon RDS) provides newer versions of databases so you can keep your DB instances up to date. These versions include bug fixes, security enhancements, and other optimizations. When Amazon RDS for MySQL supports a new version, you can choose how and when to upgrade your DB instances. Amazon RDS for MySQL supports major version and minor version upgrades. A major engine version upgrade can introduce changes that aren’t backward-compatible with existing applications, whereas minor version upgrades include only changes that are backward-compatible with existing applications. Although Amazon RDS for MySQL allows the option to automate some of these patching decisions, such as automatic minor version upgrades for performing minor version upgrades, major version upgrades must be requested explicitly. Therefore, it’s critical to be aware of common issues, steps involved, and best practices to upgrade your major version with the least impact on your applications. Amazon RDS for MySQL will continue to support until the community EOL date. MySQL 8.0 – Community end of life planned until April, 2026. Amazon RDS for MySQL will continue to support until the community EOL date. If you have any MySQL 5.5 instances in your account, you can upgrade them before February 9, 2021. For instructions on upgrading a database instance, see Upgrading the MySQL DB engine. Choosing a target engine version for upgrade We recommend you upgrade any of your RDS for MySQL 5.5 DB instances to 5.7 or newer versions because they are the long-term support releases. When upgrading any database engine’s major versions, checking for compatibility of the new version and its features plays a crucial role in the upgrade’s overall success. MySQL database versions and releases can differ in how they work and interact with applications, which may result in compatibility issues. Therefore, it’s crucial to upgrade your RDS for MySQL DB instances to a version that is compatible, certified, and supported by your application. In certain cases, feature improvements and performance optimizations in newer releases also play a pivotal role in picking a target engine version for your upgrade. For example, MySQL 5.6 introduced features such as multi-threaded replication, replication checksum, and persistent optimizer statistics. MySQL 5.7 introduced several new features, such as native support for the JSON data type and built-in JSON functions, and query performance improvements via optimizer improvement, resulting in better parsing and explain plans. It also introduced dynamic buffer pool resizing and improved tablespace discovery for minimizing crash recovery duration. In addition, Amazon RDS for MySQL supports the latest instance classes using AWS Graviton2 processors such as r6g and m6g starting from Amazon RDS for MySQL 8.0.17 and later, which offers better cost-performance for your RDS for MySQL DB instances. Downtime considerations for a major version upgrade Amazon RDS for MySQL lets you manually initiate a major version upgrade by modifying the DB instance—either via the console, AWS CLI, or Amazon RDS API. This is an in-place upgrade and requires downtime for your applications during the upgrade process. The duration of the upgrade process depends on the engine version and the DB instance class. The biggest determinant of upgrade duration when upgrading to MySQL 5.7 major version is if there is a need to convert date and time format columns to the new format, which can take hours for large tables. You can determine the amount of time it takes to upgrade by performing trial upgrade exercises with a snapshot restore in a staging environment. Upgrading your production database instance is an irretrievable process and if any issue occurs during or after the upgrade process, the only fallback option is to restore from the latest snapshot. So as a best practice, always take a snapshot before initiating the upgrade. If you’re running your DB instance in a Multi-AZ deployment, Amazon RDS for MySQL upgrades both the primary and Multi-AZ synchronous standby simultaneously. If your RDS for MySQL DB instance is in use with a production application, you can reduce the amount of downtime for your application by using a read replica to perform most of your maintenance tasks ahead of time. This method involves setting up a logical read replica that has the same major and minor version as the primary instance, then upgrade the read replica to a newer version and ensure that all validation steps for the application pass through. When this is done, make sure the replication has caught up completely by stopping application writes to ensure data integrity and then promote the read replica to a standalone instance and point the application to the upgraded promoted instance that has the up-to-date data. For best practices on achieving minimum downtime with a read replica, see Best practices for using a MySQL read replica to upgrade an Amazon RDS for MySQL database. How Amazon RDS for MySQL performs a major version upgrade When a major version upgrade is invoked on the console or via the AWS CLI or Amazon RDS API, Amazon RDS automation completes the following steps: Verifies if there are any read replicas that need to be upgraded. Runs a mandatory pre-upgrade check when upgrading from MySQL 5.7 to 8.0. This makes sure that there are no incompatibilities when moving to 8.0 and blocks the upgrade process if the pre-upgrade checks detect incompatibilities to avoid unplanned downtime. To find the incompatibilities the pre-upgrade checks detected, you can check the PrePatchCompatibility log of the DB instance. For a list of incompatibilities checked, see Prechecks for upgrades from MySQL 5.7 to 8.0. Takes a pre-upgrade snapshot (if backups are enabled on the RDS for MySQL DB instance). This snapshot can be used for rollback purposes if the upgrade runs into any issues. Shuts down the instance and prepares the instance for upgrade. Runs mysql_upgrade script to upgrade tables to the newer version. Takes a post-upgrade snapshot. Key considerations and best practices When performing an upgrade, consider the following: Take a manual snapshot using the create-db-snapshot AWS CLI command or via the console prior to starting the upgrade phase. If your RDS for MySQL 5.5 instances are running on one of the deprecated t1, m1, or m2 instance types, you have to change to a newer instance type prior to initiating the major version upgrade. You can identify any RDS for MySQL DB instances running the older instance types via the console or the describe-db-instances AWS CLI command. Amazon RDS truncates the slow_log and general_log tables during a major version upgrade. To preserve log information, back up the log contents before the major version upgrade. Take a table-level backup of the slow_log and general_log tables using the following code: mysqldump -h -u -p --databases mysql --tables slow_log --skip-lock-tables > filename.sql By default, when upgrading from one major version to next higher major version, Amazon RDS for MySQL chooses the default option group if the corresponding custom option group for the target version isn’t chosen. Always choose to have a custom option group created in the target version that reflects the options that you have from the version you’re upgrading from. Amazon RDS creates a new parameter group for the target upgrade version automatically during a major version upgrade if one isn’t provided. For RDS for MySQL DB instances using custom parameter groups, when upgrading, always choose a parameter group in the target version that matches your current version parameter group with similar parameters carrying similar or same values. If the parameters don’t exist or have their default values changed between versions, carefully test out and validate the results and behavior prior to upgrading the instance. Disable any jobs prior to upgrading and ensure you enable them post-upgrade. Also test job behavior in a lower environment prior to performing the actual database upgrade process. Perform an overall benchmark for the application to understand if there are any dependencies and application behavior deviation in a lower environment prior to performing the major version upgrade in the production environment. Keep a list of clients and their drivers used along with the appropriate version numbers. Identify that these clients and their drivers work with newer versions of MySQL. Post-upgrade, we strongly recommend you test your automated backup restore process, disaster recovery, or production to non-production refresh procedures. summertiiime and the living’s [independent research]
this post is officially written by a second-year!!  that’s right!  i finished the first year of my phd!!!  i have absolutely loved it so far and have enjoyed it beyond measure.  things are actually (strangely) lining up REALLY well…  i earned straight “a”s this semester.  i look back on this year and realize i’ve learned SO MUCH in such a short time.  i do not remember the last time i truly felt i’d learned such a great deal.  probably my first year in french class in eighth grade.  i really enjoyed all of my professors.  data analysis II was my favorite class (seriously, who ever thought i of all people would become a quantoid?).  research methods II was also a good class.  the exams and projects for that class are SO LABOR-INTENSIVE and also really good for building a solid foundation of knowledge about research.  practicum was very challenging (quite honestly, most of the time) and i think we have built a solid base to go off of.  our class just submitted the irb this week and is completing the pilot testing before we go live.  exciting things!
speaking of exciting things… i was selected into the agesw pre-dissertation fellows program!!!  who ever would have thought?!  i began my application in december while at sara’s over break, so i am elated that my hard work appears to have paid off.  i rewrote—i’m not kidding—probably 8 drafts of my personal statement for cassey and sara.  once they signed off, i sent my cv and personal statement through charlotte.  once she signed off, i sent the same materials through nancy kusmaul, who was in the first cohort of fellows and now teaches at umbc.  once she signed off, i sent it through joan davitt, who was really great at helping me tighten it and pull it all together.  apparently it worked?  this is such a great step because, in the last third of the semester, i was lucky enough to have a skype call with dr. tracy schroepfer, who teaches at wisconsin and who is one of the co-directors.  she is one of the pioneers of the body of research on the wthd.  i first read her research in sara’s death and dying class in spring 2009 and marveled at how truly amazing it was.  when i got into my msw, i continued to love the content, though i couldn’t shake how hard it would be in the logistic sense to carve out a space for myself in that area (e.g., irb concerns, people wanting to participate, people’s ability to engage, people’s priorities at their eol, etc.)…  so, when i framed my research interests in my application materials, i’d simply framed them around a close secondary interest.  once i got into the program and dove into the literature, i kept coming back to this content and sort of allowed myself to keep wading deeper and deeper under the premise of, “i’m going to keep going until i see a reason to stop.”  so far, it’s paid off!  so, forward.  onward and upward!  as a benefit of my selection, i will attend gsa (this year, in austin, tx).
similarly, the research i’m doing with caroline is expected to wrap up somewhat soon!  we were selected to present at cswe in denver!  we were also selected to present at inccip in huddersfield, england!  i am hoping we will be selected to present at sswr in dc!  i just need to figure out funding… i’ve applied for the travel fellowship through the phd program for cswe.  caroline will use $500 of her faculty funding for my travel to england.  she has requested the dean match that amount and that is still tbd.  the only other thing currently on the docket is the secondary data analysis project i did for data analysis II with cagle…  working with the HRS was a VERY laborious endeavor but it gave me really good data!!  i seriously thank joon for helping me run the data and statement files in a way to bring about a workable data set and then helping me merge the six data sets i had into one final product.  seriously—we met approximately seven times and every time i was throwing coffee at him, as a thank-you/i’m sorry you have to do this.  
this project examined the predictive properties of healthcare encounters at different levels of care (i.e., hospitalization, er admission, nursing facility, and outpatient visit) on death anxiety.  the death anxiety variable was ordinal, so my options were (a) keep it ordinal and run a multinomial logistic regression, (b) use it as continuous and run a multiple linear regression, or (c) dummy code it and run a multiple logistic regression.  (have i mentioned before how much i love regression?)  i had originally wanted to run an anova but then cagle pointed out the possible link between hospitalization and er admission would violate the  assumption of independence of observations, so he encouraged a regression model.  i ran the linear and found it legit violated 3/4 assumptions, so i ran the logistic and got some serious SS results!  WAHOO!  (i’d be lying if i said i didn’t tear up at my first “big kid” project working “successfully”—yes, i’m totally acknowledging the publication bias.)  so, i’m now just waiting on cagle’s comments on the manuscript we handed in as our capstone project before i can begin revisions.  i met with him before i presented my paper to the class and asked if he would be willing to mentor me through the publication process, as i had never published before.  he was kind enough to agree, so once i make his edits, he and i will connect to discuss next steps.  bruce also agreed to review.  since his edits are always so solid, i think i will stand a good chance at getting my first publication!  the plan is to target death studies, so we shall see!  i am also planning on submitting that as a late-breaker session for gsa.  this would bring me to four conferences in the very near future.  i am hoping this gets selected for gsa partly because my travel is and registration is already covered through the fellowship.
speaking of cagle, he will be my gra this upcoming year and i could not be more excited!  i am sad to leave caroline, though i also know i will learn and grow A LOT with john.  here’s hoping for a productive year!!  in her letter congratulating me on my fellowship (for which she wrote my letter of recommendation), joan apologized for not getting matched with me as a gra for the upcoming year.  she explained she had no funding, so here’s hoping our timetables will align in the near future!  in the meantime, she is in sweden presenting with rajean and has assured me they plan to complete their coding of our conference project upon their return.  this will yield my second publication!  i also need to check in with her about whether she has any other projects going on that i can help out with.  when i met with her and brought it up, she told me she would check and see if she has any additional funding.  i never inquire about funding because it truly is such a privilege to publish with her and it is honestly so wonderful that she always makes it a point to look into.  she has no need to do that for me, but she does, and it is so admirable.
sara’s also agreed to let me jump on one of her projects, so that would give me another publication.  i’m also currently working on a scoping review with the gerontology group on social isolation in informal caregivers of older adults with dementia.  it’s actually going pretty well!  we just wrapped the title/abstract screen.  there are four of us on the project.  the initial search across three databases yielded 301 articles.  once duplicates were discarded, there were 182.  so, we divided this into halves.  ji and i took one half, while joon and sol took the other.  those halves were then halved again.  i screened one of these quarters as p1 and ji screened the other as p1.  then we switched and reviewed each other’s as p2.  joon and sol did the same.  we will meet this wednesday to resolve discrepant opinions on whether articles should pass this stage of the screen or not.from there, we will begin the full article screen.
all in all, things are going pretty well!  i just finished spending a considerable amount of money i do not have on clothes to present.  the irony of spending money i do not have on clothes to go to conferences the ssw or gsa or faculty will pay for me to travel to in order to present research is not lost on me.  thankfully, cassey is financially keeping my head above water because, if not for her, i’d surely be drowning…  speaking of drowning, cassey and i will go to ocnj this weekend and then, in a few months, puerto rico!  we had actually originally scheduled our trip to pr during (incidentally) the same exact weekend of the inccip conference in england.  fate, huh?  all things considered, i am hopeful for a productive and successful summer!

Innovation Amidst the Crisis: Health IT and the Opioid Abuse Epidemic | Part 4 – Resource Allocation and Access

The opioid crisis in the United States is having a devastating impact on individuals, their families, and the health care industry. This multi-part series will focus on the role technology can play in addressing this crisis. Part one of the series proposed a strategic framework for evaluating and pursuing technical solutions.
A Framework for Innovation
In part one of our series, we declared the opioid crisis an “All Hands-On Deck” moment and made the case that health IT (HIT) has a lot to offer. Given the many different possibilities, having a method for organizing and prioritizing potential IT innovations is an important starting point. We have proposed a framework that groups opportunities based on an abstract view of five types of functionality. In this article, with an assist from Dr. Marv Seppala, Chief Medical Officer at the Hazelden-Betty Ford Foundation and Dr. Krista Dobbie, Palliative Care physician at the Cleveland Clinic, we will explore allocation of resources and access to care and the role that technology can play.
Resource Allocation and Access for Opioid Management
Supply and Demand
There is almost always an imbalance between what we need (demand) what we have (supply) in healthcare. This ever-present imbalance between supply and demand inevitably and regularly leads us to grappling with how to wisely allocate precious resources. The opioid crisis is no exception and is a good window into the general challenge of resource allocation in healthcare. Fortunately, technology (both information tech and life sciences) can help navigate these challenges efficiently and effectively.
What exactly do we mean by “resources”? For the purposes of this paper we break this down into two groups: people and materials. As we will see, when it comes to people, this is mostly about access to care in a timely, effective and efficient manner. Materials can encompass many things, but we primarily will focus on drugs, testing and the like. In both cases, there are instances of underuse, overuse, shortages and misallocation which point towards ample opportunity for improvement.
Access to Care
Timely access to appropriate care is essential in dealing with the opioid abuse crisis – just as it is for many diseases. The vast majority of this care involves patients having access to and interacting with a variety of providers. “The first thing that comes to mind when I think about shortages and access is providers” says Dr. Seppala. This shortage includes primary & specialty care, physicians and other licensed professionals. There is a heavy emphasis on Medication Assisted Treatment (MAT) for addiction these days and this requires MD oversight. Physicians who specialize in addiction medicine are highly qualified to provide this oversight but are in short supply. Most primary care physicians, who could in theory provide this care, are reluctant to do so either due to lack of knowledge or concerns about the clinical and medical-legal consequences of bad outcomes like overdose or death. To make matters worse. Addiction specialists often have a personal preference for a broad-based practice that includes services besides MAT – services that could be provided by either primary care physicians or other clinical professionals. So, we have too few specialists practicing too broadly and too few primary care providers who are properly trained and confident in providing these services.
The potential solutions to these challenges can be drawn from similar ones in other areas of medicine:
Optimize and maximize the impact of experts like addiction medicine specialists.
Expand the capacity and capabilities of primary care providers.
Enhance the referral and consultation process in ways that maximize the collaboration and impact of a combined primary-specialty care approach. The mantra here should be: everyone at ‘top of licensure’ so each professional is mostly doing things that only they are qualified to do.
Obviously, some of these challenges are more “geopolitical” than technical. For example, tech likely has little to offer when it comes to a medical specialist’s preference to practice broadly. However, there are a number of promising health IT opportunities here:
Leverage analytics and scheduling applications to optimize scheduling to maximize access to the right provider at the right time.
Enhance communication between various providers to support provision of primary care and facilitate the timely identification and referral of patients who require higher levels of care.
Expand educational opportunities for all providers to “raise their game” and comfort level in caring for these patients.
Telehealth
Telehealth is a proven way to extend access to care and holds great promise in addressing this crisis. According to Dr. Seppala, “Telehealth definitely helps but is limited to 1-to-1 interactions between the provide and patient. What we need now is a 1-to-many solution that supports group visits. “ This is a major tech gap that, assuming it is HIPPA and CFR Part 42 compliant, could have a major positive impact that goes beyond simply expanding capacity.  As Dr. Seppala pointed out, “There is a proven, special value in group visits. This disease undermines brain’s ability to recognize that one has the disease. This goes far beyond simple denial and is a function of the disease itself. But these patients can recognize it in other people in the group and that can lead to the important breakthrough of self-recognition.”
Efficient Use of Material Resources
Material resources refers to things like drugs, diagnostic testing, etc. They are the non-people part of our resources and they too are subject to misallocation, overuse and underuse. As in many other industries, enhanced supply chain management applications can play a transformative role in allocation of materials. But that’s just the tip of the iceberg in this area.
Fortunately, with rare exceptions, the main drugs used to treat opioid addiction are readily available and covered by insurance. However, as is often the case, newer drugs tend to be in shorter supply and much more expensive. They are also subject to stricter control by insurance companies and usual require prior authorization. Lack of insurance coverage or self-pay situations can place these drugs out of reach. This isn’t unique to the opioid crisis. Regardless, more efficient authorization processes, easier access to drug payment programs and provider adherence to best-practice treatment protocols backed up by clinical decision support – all of which can be IT-enabled – provide opportunities for further improvement.
The Other Side of the Coin: A Palliative Care Perspective
It’s important to remember that opioids do have an important and valid place in patient care. To learn more about this perspective, we spoke with Dr. Krista Dobbie an experienced palliative care physician at the Cleveland Clinic. Dr. Dobbie of course is concerned about the opioid abuse epidemic but also pointed out that use of opioids is a mainstay of effective pain management in palliative and end-of-life (EOL) care. As is often the case, it’s about context, clinical knowledge and the specific patient and their needs; In a word: judgement.
When it comes to access and allocation, she notes a couple of challenges that have arisen in providing palliative care as a result of well-intended attempts to address opioid abuse. For example, it has become more difficult to get cancer and other palliative care patients the medications they need.
Some of the new hurtles are administrative. For example, morphine for cancer patients previously did not require prior authorization since this it is considered first-line treatment in this population. Some payers now require prior authorization which increases administrative burden and can result in delays in care. In addition to the direct clinical impact on the patient, this can lead to extended hospital stays as inpatients wait for prior-authorization of these vital medications before they can be discharged. Dr. Dobbie states there are, “More hoops than ever – even with cancer patients” and notes that pharmacists and some insurance companies refuse to fill prescriptions out of, in these cases, misguided concerns about dose, duration, or amount dispensed.
From a materials perspective, there have also been reports of outright shortages of vital pain drugs due to production issues. Dr. Dobbie recalled previous periods of time when there were shortages of intravenous pain meds for hospitalized patients which forced providers into tight restrictions and careful allocation of the limited supply.
Heightened concerns about opioid abuse may be leading to increased resistance of clinicians and others to engage palliative medicine services because of undifferentiated concerns about addiction and misuse. This, in turn, may result in under-treatment of pain in vulnerable patients with terminal illness.
On the plus side, Dr. Dobbie sees that heightened awareness has increased screening for palliative medicine patients who may be at high risk for misusing or abusing opiates. Most patients are signing treatment contracts outlining opiate risks. These contracts also state that there must not be any misuse or diversion of drugs. Routine drug screening is utilized to ensure patients are using medications appropriately. If the clinician is concerned, the situation can be investigated further, and team conferences can be leveraged to evaluate and plan interventions.
Another resource to consider is diagnostic testing. Dr. Seppala pointed to urine drug screening (UDS) as an area ripe for improvement. To start with, UDS tests have known limits. Lower cost, urine screening can only detect some drugs for a short time after they’ve been used leading to false negative results. Other UDS methods only test for prescription drugs missing a major cause of the opioid crisis – the migration use of street drugs like heroin. More sophisticated tests can overcome these limits, but are more expensive and time consuming. Adopting a tiered approach based on evidence is best. Better education, distribution of best-practice protocols coupled with decision support are potential avenues where health IT can contribute. Advances in clinical testing hold out hope as well.
According to Dr. Seppala, UDS also provides an interesting window into overuse, another form of inefficient allocation. This is mostly a story of unscrupulous providers taking advantage of patients and arguably, committing insurance fraud. The typical scenario plays out in low cost, “sober housing” settings. In a kind of bait-and-switch, these providers set low base fees to attract patients. They then overuse tests like UDS by ordering too many and directing the samples to their owned labs where they charge high fees. Dr. Seppla says insurance companies and State attorneys general have woken up to this and are beginning to pursue.
In the face of deliberate fraud, testing protocols and CDS will be of limited value. Of course, in value-based reimbursement models the incentive is to avoid wasteful overuse since that decreases provider profits.
Big data analytics may hold some answers to these behavior-based challenges. It is not hard to envision a future where the same AI techniques that look for potentially “bad” credit card charges and flag them for further attention could be applied to many of the challenges outlined above. This kind of profiling has the potential to find unusual or suspicious patterns while also recognizing legitimate differences in practice between say, an advance pain expert like Dr. Dobbie and the garden variety primary care physician.
