Some early Predictions based on Episode 359
1) Guts party NEEDS to find a resolve around Casca not being able to be around Guts OR face division of the party.
How will Guts be able to protect his companions if Casca keeps getting triggered by his appearance?
This can lead to some VERY dangerous situations (think of Farnese being too busy to deal with Casca’s PTSD, or Casca running off right into enemy forces). It also means Casca won’t be able to fight alongside with the rest of the party - which kind of erases the point of her getting her own fighting gear. For this reason, they either may be forced to stay at Elfhelm for quite a while (causing a large timeskip to happen), or will have to depart early (likely to the Bakiraka Lands Rickert is heading to) to find time for recovery.
2) Casca will have to rediscover and reevaluate her relationship to her Comrades - and to Guts, too. She already started doing this in ep 359 and particularly noted Isidro, Farnese, and Schierke.
3) The clock starts ticking once the Moonlight Boy arrives in elfhelm, because that is when Griffith knows about Guts’ location. Griffith then may attack the island. There’s a couple of ideas here: does the time the boy can stay depend on fullmoon in the “real world”? or does a fullmoon on elfhelm prolong his stay on elfhelm with slower time flow, causing Griffith to be absent for a very long time in normal time flow (and causing the Fall of Falconia as well?!)
4) Guts’ contributions have not been brought up in any way yet, and neither have his motivations or feelings. The way we see the events unfold is also very disconnected from Guts’ perspective, to isolate the reader from his thoughts and feelings, to make him appear like he is going to leave.

I was thinking of this as well. After all, Guts was in full armor when he harmed Casca during volume 23…
Episode 359 in comparison:
Him dressed in his black shirt and pants would make him appear so much less intimidating - and also human and vulnerable.
It looks like the next thing to be dug out is Guts assaulting Casca back in volume 23…
But I can tell you that I do like the approach Miura chose to tackle this issue. Casca will have to relearn that Guts means no harm to her, and I believe if he works hard towards this, it will be possible for her to at least stop screaming on seeing him.
