Summary:

Upon fully presenting, most alphas go feral. Only a compatible omega can bring an alpha back to lucidity.

Some noble families cut alphas out of the line of succession, uninterested in conditioning the continuation of the family line on the possibility that the alpha will be brought back. Others are more romantic and developed traditions to find compatible omegas before their alphas fully present to bring their alphas back once they go feral.

And then there are those who simply purchase an omega at a brothel and toss them in with an alpha. The alpha might accept the omega and return to lucidity. Or they might reject the omega and attack them as an intruder.

When a noble cultivator comes to the brothel where Meng Yao lives to find an omega for his nephew, Meng Yao decides to take a gamble with his life to escape the brothel.
