Celebrating with colleagues at work my new job offer. It was very difficult to get a speciality in Sweden. Mainly because I’ve studied MD abroad and no one knows about me here. I applied many times to university hospitals and got rejected twice. It was hard for me to get over the job rejections. But after a while I applied to another city and finally got accepted to internal medicine speciality. Although I’m not looking forward to move far away from home, but I’m happy this time I don’t need to leave Sweden. Let’s hope for the best 🙏 Please wish me good luck.

I once told my husband, who works in the tech industry, “You have a bad day, something doesn’t get done, I have a bad day, someone dies.” I encourage everyone to remember this the next time they think doctors are paid too much or don’t care enough. Bad days are high stakes in this job, and after a bad day, we take a deep breath, adjust our hats, and go back and do it all over again. — Sarah Reck

Wellen's syndrome
*Wellen’s syndrome is a characteristic T-WAVE on an electrocardiogram during a PAIN-FREE PERIOD in a patient with INTERMITTENT CHEST PAIN. *T-wave changes represent reperfusion of the myocardium. *This finding suggests a high-degree stenosis of the proximal LAD that will soon result in an acute anterior wall myocardial infarction (MI). *Discovering Wellens’ syndrome is imperative as 75% of patients will develop acute anterior wall myocardial infarctions (MIs) within one week unless intervention is undertaken urgently- highly specific There are two variations of Wellen’s syndrome T-wave. V2 and V3 are diagnostic leads but the T wave changes can be widely spread in more anterior chest lead is occlusion is more proximal. Type A -75% of cases deeply and symmetrically inverted T-waves in V2 and V3.
Tumblr media
Type B- 25% of case biphasic, with the initial deflection positive and the terminal deflection negative T-waves in V2 and V3.

[Art in progress] • Inspired by a deep and emotional poetic line in Persian/Farsi language, goes as following: “نه در جان نه برون از جان كجايى" (Not inside my heart not even out of it Oh beloved, where are you?...)

There is nothing more magical than decorated trees and lights in the hospitals or family care clinics. Here’s our contribution. A special thanks to all the wonderful nurses who helped with the Xmas decoration.
Merry Christmas from Sweden

Today, I'll be assisting at the colposcopy clinic and before they open, I'm reviewing the staging of cervical dysplasia: (An intraepithelial lesion occurs when normal cells on the cervical surface are replaced by a layer of abnormal cells, and these changes are classified as high grade or low grade. 1) Atypical squamous cells of undetermined significance (ASCUS)—Borderline, some abnormal cells 2) Low-grade intraepithelial lesions (LGSIL)—Mild dysplasia and cellular changes associated with HPV 3) High-grade squamous intraepithelial lesions (HGSIL)—Moderate to severe dysplasia, precancerous lesions, and carcinoma in-situ (preinvasive cancer that involves only the surface cells)

This ECG looks abnormal. I see no P-waves 😱only some Q-waves before QRS-complexes or maybe its a "ventricular fibrillation". Anyway this patient needs further clinical evaluation.
