Get a Grip on Osteoarthritis

The online medical dictionary defines Osteoarthritis (OA) or degenerative joint disease (DJD), as a progressive disorder of the joints caused by gradual loss of cartilage and resulting in the development of bony spurs and cysts at the margins of the joints. The name osteoarthritis comes from three Greek words meaning bone, joint, and inflammation(1)
The common perception is that OA is caused by wear and tear of the cartilage in the joint. Eventually this cartilage is worn down and we get a “bone on bone” situation that we are told is very painful (2). Now, there are new ideas emerging on what helps to cause OA and what its relationship is to painful experiences (3). Instead of looking at OA as your cause of chronic pain, it may be that it is the chronic pain that comes first(4)(3). Lets say you get a RSI (repetitive strain injury) from doing a task without rest, such as using a mouse too often (5)(6). This injury will cause long-term inflammation chemicals such as a group called cytokines to hang out in the area where pain is felt. Over time these inflammation chemicals may cause the tissues in the area to degenerate or break down(7)(8). This is first felt as achiness accompanied with stiffness in movement, usually after waking up in the morning(2). Over time the OA has a central nervous system component(3). What that means is the brain and spinal chord play a major role in the pain you feel. There is an increase in cytokines in the spinal chord, which start to inhibit our natural pain dampening ability(7)(9). Also, some brain regions may have glitches in processing the signals between it and the painful joint(10). Studies show people with OA are more likely to feel pain with less provocation not just in the involved joint(s) but in areas away from the joint(s)(11)(3). There are also studies coming out that are showing that joint surgery for knee arthritis is no better than placebo surgery(12)!
The take-home message is that joint damage is a poor indicator of how much pain you actually feel.
If you feel pain in an area from doing repetitive tasks you should make an effort to permanently decrease that pain: Ask your RMT for help. If you already have bony changes from OA seeing a RMT should be part of your pain management program. This program should involve tweaks to make your activities easier, exercises and massage therapy, as well as any medications your doctor may prescribe. apparently, he gets really good grades in all of his classes. he's mysterious, and he leaves no trace. he bought his own laptop so the school couldn't get on his network.
but the only time he makes any notice of people is when he glances at them, and by now everybody can read him just from the way he glances.
the only time he's ever paid any notice to me is when he glances. but, for some reason, when he looked at me his glance lingered a little longer and i think he may have
smiled at me once. now i know why. but to use my name as a password- that's a little weird.

"let me see." i scrolled through and the file names were just the names of everyone at school, their friends, and their family, if they ever came to be known.
i looked my name, thinking it would just be a few minor things. i can't be that bad. right?

i looked up those files, and i couldn't believe it. they were a TON of drawings, and they looked like they took hours to make. they were just drawings of me on my phone, in class, at the beach... tons of drawings. "Webby... did you see... those files?" "no, i was texting Dewey back." that's funny. Dewey and Webby had a crush on each other but Webby didn't know about Dewey, and vis versa. i chuckle under my breath, but quickly remember those files. "hey, guys. how ya dewin'?" Dewey's been making puns with his name, but at this point they lost their touch. i hide the laptop under my backpack- if Dewey sees it he'll go nuts. we talk a little. time flies, and soon it's a little dark. "well, i'll see you guys later. i gotta go to school tomorrow- friday. i'll be able to hang out then." i walk home, but on the way the laptop starts beeping in my backpack. "FILES LOCKED. USER IS APPROXIMATELY 600 METERES FROM LOCATION." i leave it on a bench, and watch from a distance. eventually, Alex finds it, puts it in his backpack, and leaves. i get home and go to bed. ~~~~~~ :A:"Lena." who is that? where am i? wait... :A:"Lena.." that's Alex. am i in his room? how- :A:"Lena..." his voice is fading. what's wrong with him- whoa! he's collapsed on the floor, bleeding. but there's something weird about the blood- it's black... :A:"Lena..." i try to help him up. the bleeding is getting better for some reason, and it's turning red. and it's all over my hands. then it stops. :A:"Lena. i'm Alex. i'm connected to your mind with a neuromatrix i texted to your phone. i use technology, because while magic can upset magic easily- tech can withstand more attacks from itseltself. magic has talismans, but i have VPNs. i don't talk to people as a disipline unless i feel i really need to, so i'm talking to you in a neural emulation. i can control anything in this matrix i am aware of. but i will not hurt you, because although flesh wounds heal- mental wounds are much harder to repair." i don't know what a neural emulation is, but it sounds scientific. it makes sense, though- he studies physics, neuroanatomy, chemistry, and programming. :A:"Lena, i'm going to send you back. you'll wake up- but it's 7:00, so you should be fine. "wait, you're in my head?" :A:"actually, we're in each other's heads. bye, Lena. farewell." ~~~~~~ i wake up. it's 7:05. i make it to school early, having woke up at 7 a.m. i have all of Alex's classes, which makes the crush even more understandable. i get to pottery, and- like always- he's there first. he looks at me, and i can tell he's a little worried- so am i. he has no idea if i'll tell people about those files, and i have no idea if i'll recieve a "mental injury", like Alex says. classes go by with a sense of uneasyness- until lunch. "Lena." he's talking. he never talks unless, as he said, "when he really needs to". i suppose he really needs to talk to me. we're the same dimensions, but his backpack and oversized hoodie make him look a little imposing. for some reason, nobody notices the fact that the most silent kid in school is using his mouth for a purpose other than eating. his voice is a little lower and gravelly than i expect, but sounds like a quiet kid nonetheless. then it hits me- coincedentally, we're the same height. and age. he's wearing boots, flannel, jeans, and a sweater that's two sizes two big, with a calculator watch and something on his right wrist that looks like some sort of pencil-launcher. :A:"Lena, did you see those files?" he knows. it was inevitable- the kid who's more than likely to be smarter than everyone in this school would know if someone saw his files. there's no escape from this. "y-yes, i saw them." :A:"okay. bye, Lena." and he walked away. no anger, no expression, he just... walked away. i think about that for a while. this kid could probably be the most intimidating, powerful one here if he wanted to be. but he's soft-spoken, gentle, and- of course- quiet. he never disturbs the peace, and no one ever disturbs him. never. i go home, do my homework, and head over to the Boy's house. i knock. "hello, it's Lena." Webby answers the door. "oh, hey Lena! come in!" Dewey's on the couch, on his phone. the other two apparently decided to go with Donald and Della for the weekend. we play some games, and go to bed. i think about today and last night. i fall asleep... ~~~~~~ i wake up. it's 4 a.m. i think of yesterday- i need to get my mind off of it so, of course, i go to the beach. it's early, so i leave a note. i didn't think it would be so chilly outside, so i put my sweatshirt on. when i get there, the sun is just a little bit above the horizon, and it's completely silent- until i hear someone. :A:"don't move! who are you? why are you here, and who told you where i am?" he had something on his wrist that he was pointing at me, and i saw a faint glint reflecting from the little sunlight there was. "i'm Lena!" :A:"oh... sorry-" "it's fine. but- what are you doing at the beach so early? :A:"i'm harvesting sand for purification into silicon, so i can make my own circut boards without publicly purchasing them." "okay... i just come here to get my mind off of stuff." :A:"i get that- would you like to walk across the shore?" i agree- it's better than that neuromatrix. we walk for a bit, and he's silent for a bit, but then he talks. :A:"you probably want to know why i never talk to people... right? i never thought about it, but now that he mentioned it, i did want to know. "sure, why?" he explained that he used to talk to everybody before he moved to Duckberg. he would talk about science, but nobody was intelligent enough to unnderstand what he was saying. so he stopped talking to everyone but his parents, but they divorced, so he talked to no one, because he felt that he caused his parents' divorce. by the time he was done, he was sobbing, and was so depressed that he didn't even look embarassed. "Alex... that's fine. i understand." he got up, and hugged me out of desperation. i realized that he really was my size- the sweatshirt hid it, which felt like a blanket. :A:"Lena... you saw those drawings. so i'm assuming that you know that i like you quite a bit..." he let go. no kidding. :A:"when i realized that you saw them, i knew that i could, and had, to talk to you. the reason i drew them was because i started noticing you more. you were always keeping to yourself. you weren't silent, but you were introverted. i noticed that apart from the talking thing, we were similar. i started taking a liking to you, and... well..." "well?" :A:"well, i suppose i grew a bit of an obcession... and you probably think i'm a creep..." he looked away, embarrassed. "Alex. i have had no one like me enough to draw even my face. if anything... thanks." he looked at me. then he just started laughing. i started laughing too, and he hugged me again. then he fell down, taking me with him. we fell apart laughing, and i actually started to like him. i didn't realize that he had a personality like this, but i liked it. eventually, i had to leave. :A:"bye, Lena." "bye, Alex. i thought about him. he was a pretty good artist. i saw a drawing of a cat that looked almost real. i fell asleep. ~~~~~~ BEEP! BEEP! BEEP! i wake up. i get to school and fly into algebra. the teacher gives us back our quiz. i wonder what grade i go- D-. i need a tutor... maybe Alex? he can do calculus and trig- he's more than qualified to help me. i run into him later. "hey, Alex. could you help me out? :A:"sure. what do you need help with?" "i don't get these standard deviations... i got a D- on my quiz." ~~~ :A:"well, if you want, you can come over to my place and i could help you out with it. i have a secret path, if you don't want to be seen with me." "okay... cool. where's your place?" :A:"it's on Loftwood. 3648 Loftwood." ~~~~~ :A:"so... this is my room. it's not exactly big, but i rent it out. i have a heater, a fan, and blankets, for whatever you feel like. feel free to take a seat." i see some needles, hooks, and yarn. "what are those for?" :A:"you see those blankets?" "yes." :A:"i made every single one of them by hand." sweet. we get started. ~2 hours later~ :A:"so you take alpha, beta, and gamma, add them up, get lambda, square root it, and then you get the standard deviation." oh jeeze... i'm done. 2 hours of math. 2 HOURS. :A:"well, Lena, i'm guessing that you don't want to do math anymore." no. no i don't. :A:"well, in that case, i suppose i should show you something." he opened the closet, and there was a 40" TV, and there was four game systems. :A:"if you want, you can play any of these. they're close to my heart, so be careful with 'em. but you can play them." "well, thanks, Alex." :A:"i suggest The 'Legend Of Zelda: Twilight Princess' personally, but you can play whatever you want. i have 4 copies of every game, so just use the empty disks." "thanks, Alex." :A:"would you like me to shut the door?" "sure." i put in Twighlight Princess, listen to the GC theme, and open a new file. ~~~~~ -Webby's POV- :W:"okay, guys. we need to keep an eye on this kid that Lena's hanging out with. Louie, i'll need you to follow Lena and Alex on the way from school. Huey, you're survellience. Dewey, you'll stay with me." :L: "Okay, they went inside of the house. i can't see them anymore. and he's wearing earbuds." :H: "i can see them. they're in the room, writing something down. i think they're just studying. wait- Alex just left." "okay. hey Dew- oh no. he's inside." Dewey. he managed to break into a house undetected, and now he's in Alex's room. Alex walks in. :A:"AH! you scared me! what's up?" "wh-what? you're not gonna beat me up?" :A:"no. i'd only destroy every bone in your body if you were an endangerment." wow. that definitely wasn't threatening. :A:"if you're looking for Lena, she's in the closet." we're all in the room by now, but he doesn't kn- :A:"oh. and hello to you three." what? he didn't even look at us, and we were completely silent! he turned around. :A:"i can tell when anybody shows up within 15 feet of my room, and 550 feet from my house. thanks, by the way." :LO:"for what?" "for proving my infared surveillance cameras worked. they're wirelessly connected to my wristpiece. when they detect something, they vibrate in binary." might as well introduce myself. "hi! i'm Webby!" he looks at me and smiles. :A:"hello." by now, Dewey opened the closet. :LE:"Louie?!??! why are you guys here?" :LO:"we didn't know about Alex. we weren't sure what you were doing here! we had no idea what was going on!" "so you're spying on us?" us? what? i knew Louie wouldn't like that- he has quite a bit of a crush on Lena. :LO:"well... yeah..." :A:"Lena, it's fine. really. it's fine." :L:"hey, Alex, what's a 'Nintendo'?" :A:"it's... from another time... i suppose you want to know about it." :L:"uh... yeah." :A:"okay, well, you might want to take a seat. what would you like to know first?" :D:"how many games do you have?" :A:"well, for the NES i have 8, for the Super Nintendo i have 10, for the Nintendo 64 i have 4, and for the GameCube i have 16." :D:"why only four for the 64?" :A:"the 64 is a delicately owned piece of hardware, and as such i only put 4 games through it. these games are: Goldeneye; Ocarina of Time; Perfect Dark; and Super Mario 64. it took me years to collect all these games, and now i have at least 3 copies of each game." wow. :Lena's POV: :A:"well, would any of you like a drink or something?" "actually, i'm really tired. i'm gonna go home." i walk home. it's a bit windy outside, and i get a chill. i'm a little mad at Webby for spying on us, but i don't blame her. Alex is a little bit mysterious, and i still don't know everything about him. i get home, eat, and go to bed. on a dark desert highway cool wind in my hair warm smell of colitas rising up from the air and still those voices are calling from far away BEEP! BEEP! BEEP! ugh... it's seven o' clock... i want more sleep... i get to school. again. ugh. "hey Alex." he took out his headphones. :A:"hello." "i think that your neuromatrix glitched or something. i can hear music all the time." :A:"oh. okay. one sec- BLEEOOP-okay, it should've stopped. yes. it finally stopped. "what are you listening too, anyways?" :A:"a CD that i made. just my favorite music. want to listen?" "sure." he hands me an earbud. all our times have come here but now they're gone the seasons don't fear the reaper neither do the sun, the wind or the rain we can be like they are come on baby (don't fear the reaper) baby take my hand wow. that's really good. "what is this?" :A:"it's Don't Fear (The Reaper) by Blue Oyster Cult. i like it. i have a copy of my CD if you'd like to borrow it." "sure." he hands me a double sided CD in a plastic case. the day flies by, and i get home. i pop the CD in my boombox and start the track. i listen for a while as i read the piece of folded paper that was inside of the case. there's a bunch of different artists- Blue Oyster Cult, Pink Floyd, Chicago, Led Zepplin, The Eagles, and a lot more. i like the music quite a bit, wondering where Alex gets his taste. i run into him the next day at school. "where... where did you get this music from?" "i got a little bit from my mom and then i just expanded from there. there's a lot more. i have about 20-25 CD's that i made." wow. i had no idea that he liked that much music. "you know, i never got your phone number. it'd be nice to have." :A:"sure. it's 528-934-1943. call me anytime." "okay. see you later, Alex. i go to food production. "okay, today's review!" this should be easy. i'm actually good at cooking. "remember to take notes...." class drifts by. i think of that music... 25 or 6 to 4... i get to English, and Alex is typing away at his keyboard, as per usual. he has his headphones on, but he notices me, and gives me i nod. i smile at him. the rest of the day is a breeze. i got a B+ on my algebra test. sweet. tommorow's Friday... :W:"hey Lena!" it's Webby. "hey, Webby." :W:"i thought that you'd like to go to the library. me and Violet are going." "sure." we walk there. it's warm inside, and Webby checks us in. :V:"hey, Lena." "hi, Violet." Webby goes to the mythology section. Violet starts reading a chemistry book. i look at a book with 'Nintendo' on the front of it, in big red letters. i open it and start reading. "sure. tomorrow at 1:00?" :A:"perfect." i go to bed. ~~~~~ "hey, Alex." :A:"hello. so... what would you like to do?" "how about some more music?" :A:"sure." he puts in a disk. Super Smash Bros: Melee? :A:"sure." he opens a case, puts in the game and sits on his beanbag. we play for a while, and i realize- i'm really good at this game. i actually beat him a couple times. :A:"wow. you're not that bad." he gets up. "would you like a drink?" "sure. what do you have?" :A:"Dr. Pepper, Co-" "what's Dr. Pepper?" :A:"it's a concoction i made myself. i grew the vanilla, infused the water with co2, canned it and made all the other stuff in this very room." "cool. i'll try it." he hands me a can. it has an interesting taste, and the only way i can describe it is that is tastes like Dr. Pepper. before i know it it's 5:00. "i got to go, Alex." :A:"okay. see you later." i go home, exhausted. my hands feel numb from the constant activity of the controller. the sky is colorful from the sunset. my eyes are heavy. i get home and listen to the other side of the CD. (this is what Lena's listening to) it's only 6:00. but i'm so tired... And no one showed us to the land i can stay up a little longer... And no one knows the where's or why's i'll read the book i got from the library... but something stirs and something tries so tired... And starts to climb toward the light i get lost in the guitar... i fall asleep... ~~~~~ school. again. we have a lecture in history, then i walk home. or maybe i should go to Alex's place... i get there and knock on the door. :A:"hey, Lena. need something?" "no, i just thought you might want to hang out for a bit." :A:"okay... well, come in." we go up to his room again. :A:"so i bought you something- i thought you might like it. i picked it up at a pawn shop, then fixed it up so it's basically brand new." he handed me something about the size of a phone, but way thicker. :A:"it's a DS. i got you a couple games, too. i thought you might like something like this." it was black, with a charger and a small case. he gave me a felt pouch with a few cards inside of it. :A:"i got the games i thought you might like. i also modded it to play newer games, so i got you Ocarina Of Time: 3D. i hope you like it..." "thanks! i- i didn't realize you'd get me a gift, but thanks. i wish i had something for you..." :A:"it's fine. i don't need more stuff- my room's crowded as it is." he's not wrong- he has a decent amount of space, but he has quite a few things. the room's only about 15x18, closet included, but he manages to get a lot of stuff in here by mounting it or putting it on a shelf. "well... thanks, Alex. this is nice-" :A:"you'll need the stylus for most games, and the two slots are for DS and GBA games. also, it has Picto-Chat. it's like a text service, but for a DS. i amplified the signal so i could chat with you on mine." "wow." it's 4:30. "well, i have to leave..." :A:"okay. enjoy your gift." i get home, and i turn it on. the screen lights up and i see Picto-Chat in the corner. i open it. i send a message to Alex. "if you got this, message me back." he does. :A:"got it." i open the pouch. there's a few games in it, and i put in Ocarina Of Time: 3D. i play for a bit... ~~~~~ i wake up. the game's still on and i'm still in my clothes... what happened... it's saturday, so i text Webby. "i'm bored. what's up?" she texts me back. :W:"nothing, really. what do you want to do?" "just hang out. Alex gave me something cool." "well, bring it. Louie wants you to come over." i head over with my DS. the battery is crazy strong- on all night and it's still at 98%. :W:"hey, Lena!" she lets me in. :LO:"so... what did Alex get you? i show them. "it's called a 'DS'. he gave me a few games, too." Louie looks at it. :LO:"cool! he just... gave it to you?" "well... yeah. i don't know why, really- i guess he just did it to be nice." :LO:"huh. i wonder why..." i knew that at any moment Louie was going to say something about a crush or whatever so i changed the subject. "so... what do you guys want to do?" :LO:"i don't know- what have you been doing lately?" really, the only thing i've been doing is playing my DS and sleeping. "not much. so... i guess we'll just watch tv." ~~~~~ :Webby's POV: Louie's a little sad. he doesn't know that i heard him in his sleep- i was awake and i walked down the hallway, but the door was open. of course, Lena has no idea. he's just dissapointed, i guess, since Alex showed up. Louie's still suspicious of Alex, but so am i. she just met him. and what he said to Dewey- "no. i'd only destroy every bone in your body if you were an endangerment." he could just not be used to people, though. i've never heard of him, and he seems a bit depressed- not enough where it's easy to tell, but where there's just a hint of it. Lena doesn't notice it, but she probably has a crush on him anyways. i know for a fact, though, that Huey likes Violet. no question. but Dewey- i don't know. :LO:"hey, Lena, what games did you get, anyways?" "let me look- okay, i have Ocarina Of Time, Tetris, Super Mario Advance 2, and Link To The Past. :LO:"cool." Louie's face blushes a tiny bit but he looks... dissapointed. "oh, guys, it's 4:30. i got to go-" "okay. see you later, Lena." :LO:"bye..." Lena goes home, and Louie stays there on the couch. "you okay, Louie?" "yeah... i'm fine..." he gets up and goes to his room. ~~~~~ :Lena's POV: i run into Alex on the way home. :A:"oh, hey, Lena. i made you something-" he took a CD out of his sweater pocket. :A:"it's a playlist i made for you. i thought you might like it." "thanks, Alex. but- you don't need to get me all this stuff." :A:"oh. sorry... i'm just not good at- well, people." "it's fine. i like the DS, though. that battery is crazy strong." :A:"yeah... i kinda supercharged it with a lithium-ion. about 50 ultrathin batteries in one case." "wow." :A:"yeah... well, i have to go." "okay. later, Alex." he walks away. i get home and put the CD in my player. There's a lady who's sure all that glitters is gold And she's buying a stairway to heaven When she gets there she knows, if the stores are all closed With a word she can get what she came for Ooh, ooh, and she's buying a stairway to heaven the DS is still charged, so i play a bit. i noticed that there was a file called 'Linkoid', so i opened it. the entire game was 100% done. all the temples, everything. i don't even have Midna yet. it's getting late, so i go to bed. ~~~~~ monday. i get to school. i get a call from Webby during lunch. "hey, pink." :W:"hey Lena! what do you want to do after school? i thought we could go to the mall." that sounds fun. "can Alex come?" :W:"uh... sure. i'll meet you there at 2:30." "okay. later, pink." ~~~~~ :Webby's POV: we meet up with Lena and Alex. Louie came, but he seems a little mad at Alex. :LO:"so... what do you guys want to do?" :A:"whatever you guys want. i have nothing but time." Lena and Alex go to the HotTopic. me and Louie walk around a bit- :IDIOT:"heh! look at the little couple!" some big guy was making fun of us- he had two friends next to him. :LO: "we're- we're not a couple." the guy looks at him and pushes him- he falls down. "Louie! you okay?" :IDIOT:"stupid litt-" the guy barely has a chance to finish his sentence. Alex seems to just barely move but he takes him down and he's on the floor, oblivious to what just happened. "wh- what the heck?" he got up. :IDIOT:"you piece of crap!" he tries to attack Alex, but he just dodges his hits. he strikes him at certain points- pressure points?- and he looks up at the cieling, and almost faints. he collects himself, and the guy goes to push him, but Alex grabs both his arms, looks him in the eye, and kicks him in the gut- launching him a couple feet. the idiot realizes that this is the wrong 'piece of crap' to mess with. his friends already left, so he stumbles away. :A:"you need help?" he doesn't seem to notice that everyone is in shock at the thought that Alex, of all people, would do what he just did. :LO: "uh... thanks, Alex..." he gets up. he had been mad at him earlier, but now it was kind of awkward. :A:"no problem. you alright?" :LO:"yeah..." Louie looks a little embarrased, but no one seemed to notice what was happening. :LE:"let's get out of here." ~~~~~ :Lena's POV: we walk home. what just happened? "hey, Alex-" :A:"yes?" "why doesn't anyone notice what we do?- and what did you do to that kid?" :A:"i have a device built in to my wristpiece that detects when people become interested in something, targets them, and releases mild magnetic waves that, for some reason, repel them." jeeze. he's got a lot of spare time. :A:"and me shaking his head is code for my wristpiece to do certain things. this time it vibrated his head just enough to trigger his glands and release melatonin." he pulls his shirt down his neck a bit. there's some sort of chip on his ear, going down with a wire. :A:"it's the motherpiece. it regualates everything else." jeeze. "so... what do you want to do now?" :A:"well, i really don't know. we could go to my house-" "wait- what if you came over to my place? i don't really have anything to do, though..." :A:"i could- no, that's stupid-" "what is it? it can't be that bad." :A:"i thought i could bring my keyboard. i can play a few beats, mostly lo-fi..." "we should at least try. you seem like a good musician." :A:"well, if you think i should, i'll go get it. where's your place, anyways?" "we could watch something on my computer. i have a few movies, but i don't know if you'd like them." Dewey had thought it was just a random book. i shouldn't of left it on the floor. :D:"i'm sorry! i didn't know! but- you like me?!" "well i..." he had a point- i did like him, but i didn't exactly want to tell him. too late. "well... yeah..." he blushed a bit. "Dewey, if you tell anybody about my diary-" :D:"i won't tell anybody! i promise! it was a mistake... i just read a little..." "okay. good. at least you promised..." :D:"but- you do like me?" "well- yes. i like you... i just never wanted to tell you, because i didn't know what would happen. we're just friends- or at least we were..." :D:"meaning?" "i... i don't know... just don't tell anyone." "i swear. really." "okay. good." he gets up and walks away. this is so messed up... "oh! sorry! i didn't mean to-" :A:"it's okay- i guess-" i try to get up but i fall of the bed, and as if this couldn't get worse, i land on top of him. i get off of him, and i see him blush a bit. :A:"you okay?" "yeah..." i blush quite a bit myself- this is awkward. :A:"well, the movie's over. you fell asleep about 20 minutes in, but i didn't want to wake you up." "i do like you, Dewey- i just didn't realize that you'd find out about it." :D:"well- here we are. so what do we do now?" "i don't know. this is so weird... i never thought this would happen." he chuckled under his breath. :D:"Dewey Vanderquack is kind of funny, though." "those balls at the bottom are Boba. they taste delicious." he's right- they do taste good. we finish our drinks. :A:"well, Lena, i have to go- i'll see you later." "later, Alex." i get home and play the CD again. it has a crazy amount of space- 18 songs and it's still going on the same side. i lost the other one, though. i don't know what happened to it. i listen to it for a bit... ~~~~~ :Webby's POV: :LO:"hey guys." Louie looks tired. he's been sad since Alex showed up, and hasn't talked much. "sure." i get there, and he opens the door. :A:"it's cold today. i didn't think it would rain so hard." we go in his room. "so- what do you want to do?" :A:"i got a few DVDs. i thought we could watch something together- i got what i thought you might like." he had about a dozen, but one of them looked good- Back To The Future. :A:"are you sure? it's a little nerdy..." "sure. it can't be that bad." he put a disc in the player and turned on the TV. :A:"any snacks?" he hands me a bag of Fritos and we sit down. :A:"this is actually 3 parts. it's quite long, over 3 hours." wow. we watch the movie for a bit, but my phone goes off- it's a text from Webby. :W:"Dewey found this. i thought it might be yours." there was a picture of a CD attached to it. i text her back. "yeah, it's mine." Alex is asleep. the movie ended already, and the credits are rolling. i get up to go, and i leave a note. i go to the shore, and Webby's there- :W:"here's your CD. i thought i would find you here." Dewey's a distance away, looking at the waves. "what are you guys doing?" :W:"i left, and Dewey wanted to go too. we're just walking around- then i remembered the CD." "thanks, pink. want to come over to my place?" "sure!" they come along. my room isn't that small, so we all fit. :D:"what do you guys want to do?" :W:"hey- does anyone have ideas for Halloween? it's only a week away..." Alex has his guitar, and he put together an outfit for me too. :A:"you look just like her..." "what's her name again?" :A:"Stevie Nicks." "and you're..." :A:"Jimmy Page." i have a black cloak, tophat, and a few bracelets. :A:"i made a map of houses to hit based on data i've collected over the years. we should get a massive amount of candy." we leave, and soon it's almost midnight. i don't know what Webby and :A:"well, it'd be cool to actually play something on this guitar." "yeah, we're fine." :A:"okay, good. get a flashlight- the power's probably going to go out soon." i just finished reading it when it did exactly that. everything shut off. :H:"what the heck?" Webby turned her phone flashlight on. :W:"i have an emergency radio in my room. we could hang out in there for a while." we head up there, and she pulls out an electric generator, boombox and a few games. :D:"what are we supposed to do now?" :H:"we should shelter in place. there's a lot of cumulonimbus clouds, so we'll be in here for awhile." :W:"i collected some cassettes just in case this might happen. we can listen to Fleetwood Mac, Led Zepplin, Chicago, Abba- :LO:"just play whatever." she opens the deck and puts something in. Got a call from an old friend we used to be real close Said he couldn't go on the American way Closed the shop, sold a house, bought a ticket to the west coast Now he gives them a stand-up routine in L.A... :W:"we have Scrabble, Monopoly, Yahtzee, and a few other games." :H:"i'm up for Yahtzee. anybody else?" we're bored, so we all nod our heads and get the game set up. ~~~~~ (two hours later) Webby's changed tapes to The Eagles and we finished Yahtzee twice- :D:"i win! again!" - three times. "jeeze... how much longer do you think this storm is going to last?" :H:"let's check the radio. it can't be much longer." we tune in on a station that's been updating every few minutes. :RADIO:"the storm should last for another day, at least, and power won't be back on 'til much later." i groan. we have to stay in here for another day. :L:"i think somebody's knocking on the door. i'll go get it..." we all go to the door, except Webby. she wanted to stay behind and keep track of the weather. Louie opens the door- :A:"hey, guys. i thought you might need some entertainment." he has a huge duffel bag. :A:"i brought a DVD player, a few game consoles and a folding TV i built." he takes off his raincoat and we go back to Webby's room. ~~~~~ :Webby's POV: Alex comes in the room, and i look at Louie. he's conflicted- the person who stole his crush also beat up the jerk that pushed him. "hey, Webby. i was thinking about you guys when i found out that the storm would last this long, so i packed up a few things and headed over here. i hope you don't mind me staying here, but i brought a sleeping bag." "that's fine, Alex. but how are we going to power all of it?" he pulls out a suitcase with outlets on the side. :A:"this is a HUGE battery pack i created. over 750 ultrathin lithium-ion batteries, all hooked up. this should give us about 55 hours of battery life- if we use all the outlets." he grabs something that looks like a laptop. he unpacks it, folds out the legs, and plugs it in, with a game console hooked up to it. :A:"who's up for MarioKart?" he hands me, and everyone else, a controller. they're kind of weird, but i get used to it after a bit. we pick our karts and characters, and then start the level. :LE:"yes! i won!" Lena's laughing, and Alex smiles. :A:"you know, you guys can play whatever. i have a lot of games." we pick Super Smash Bros: Melee, and Lena immediately starts destroying everyone. :D:"how in the heck are you so good?! :LE:"i played with Alex before. he taught me a lot of stuff." Louie cringes. i can tell that he's not exactly happy that Alex showed up, but he doesn't say anything. :H:"so... what do you do, Alex?" :A:"i just code, draw, play games like this, et cetera..." :H:"code what?" :A:"a few different things. i use JavaScript, C++, Python, all that. just little games and things." :H:"cool." it's hard to not be a little happy with Alex. it's like he's always happy when he wants to be, but he can definitely get depressed- he doesn't know that i saw him on the guitar with Lena. i had no idea he could play, either. "so... you play guitar, Alex?" :A:"yes i do. i brought it, actually." he pulls out a guitar and a small amp. :A:"i'll play something nice. one sec- i need to tune it-" he tunes it and plugs in the amp. :A:"you guys should know this song. i'll do the lyrics the best i can." he starts strumming. "THE STORM HAS SUBSIDED. RESIDENTS ARE NOW PERMITTED TO LEAVE THEIR HOUSES. POWER WILL BE TURNED BACK ON SHORTLY." :A:"oh. well... i guess i'll leave." :D:"we should go somewhere, though." :A:"we could go to the mall again. that guy might be there, though- but it shouldn't be a problem." he smiles at Louie. he has no idea- :LE:"sure. we could go." ~~~~~ :Lena's POV: we leave. the smell of rain is everywhere. :A:"do you guys want to split up again, or should we stay together- considering what happened last time." :H:"what happened last time?" :IDIOT:"it's him again! get him!" Alex just rolls his eyes and walks towards them. he looks irritated. :A:"for the last frickin' time..." he throws his hand out, and a staff comes out of his sleeve. what? :IDIOT:"ha! you think that'll do anything?" he pulls out a knife. Alex mumbles something and runs at him. the idiot slices his face. Alex steps back, feels the blood with his fingers and tightens his grip on the staff. he looks different than i've ever seen him before- completely enraged. he swings the staff up in the air and smashes him on the shoulder of the arm that was holding the knife. he drops the blade, and crumples to his knees. :A:"if you ever- EVER- f*ck with me or any of them again, just remember this." i expect him to say something, but instead his eyes grow wide and he cracks the staff against his head. he falls over- unconcious- and Alex picks him up by the shoulder, slams him against the wall to wake him up, and when he does- wondering where he is and what happened- Alex grabs the idiots' knife and shoves it in the guy's pocket. :A:"stay away from them, you pun-" one of the guys' friends slams a book against Alex' head. he falls over. the friend helps the jerk up and they start to walk away, but Alex murmurs something in pain- :A:"hypocrites... you didn't think of who i was, just who you are..." he gets up and lines start glowing from underneath his clothes. the guy and his friends look in awe as he gets up and flies towards them. he grabs the third by the throat. :A:"you didn't hurt us. run." Alex' eyes turn black. what the heck is going on- he throws him to the floor, and he immediately gets up and starts running out of the mall. the first guy can barely move, so he goes for the guy with the book. he grabs his face and slams it against the floor. the lines start shining through his clothes and hurt my eyes to look at. :W:"did you know about this?" "i have no idea what's happening. you guys should leave." :D:"come on, Webby!" :W:"Louie!" he can't move. he's frozen. they go, and the guy with the book slides across the floor. the security guards don't notice- he's using that pulse thing- and he gets up. Alex isn't fazed and walks to him again. by then, the guy got up and tried to punch him. Alex catches the fist and seems to crush it. the guy screams in agony, and tries to kick Alex to escape. he just blocks it with his foot. :A:"you don't get it, do you? you don't get the fact that mre adrenaline has been injected into my bloodstream than you use in half a year, that i'm wearing robotic supports, that i've already noticed the idiot behind me-" the guy that left came back with a baseball bat and tried to hit him from behind. Alex caught it and threw it to the floor. he turns around and grabs the other guy by he shoulder. :A:"-that your friend here is about to have a dislocated shoulder and you a dislocated hip-" he barely moves as two sickening pops echo through the mall. :A:"-that i've calculated your future trajectory in my head already-" he throws one of them across the floor. the other one flies into the wall. Louie still hasn't moved, and the guy Alex just threw goes to attack him. :IDIOT 2:"this is your fault!" :A:"are you kidding? are you f*cking kidding me?" the guy goes to punch Louie in the face, but Alex flies across the floor, grabs his fist- the same fist he just crushed- and pulls it to the floor. :A:"Louie. leave. now." he does, and so does everybody but me. after the guy started falling, Alex jerks his elbow back and smashes his face. he starts bleeding everywhere. :IDIOT 2:"ugh... my nose..." :A:"i am Alex Featherwick." Alex grabs his leg and the other guy's shoulder. he pops their sockets into joint with a quick motion, and grabs them by the forehead. :A:"forget." with that word, his arms glow and they fall to the floor. his eyes go back to normal, and he looks at me. :A:"you okay? they'll wake up in about 20 minutes." i'm standing there, in shock. i can barely speak. "what... was that..." :A:"actually, i can barely remember the past 20 minutes. and what's this-" he touches his cheek. his face is bloodied, and his staff already retracted. :A:"oh jeeze... i'll clean that up in a bit. i'm probably going to pass out in 10 minutes, though, from the adrenaline rush's after-affects, so we should leave." "we can go to my house. it's closer, and i have some first-aid stuff. you can crash there." :A:"thanks. let's go..." we leave, and a few minutes later we get to my house. he cleans himself up and comes into my room. :A:"thanks, Lena, i really... appreciate... it..." he faints and falls to the floor. "Alex!" i drag him next to my bed and prop him up with a pillow. i roll up his sleeve. what was glowing... there's casings covering his arm. they're thin, and i can see through them a little bit. there are lines with LED's inside. /MOD. 1.9.6/A.D.M. 6/ there's cartridges that say ADRENALINE in blue letters. they're empty. i see the staff, so i pull it out. i press a small button, and two sections jut out of both sides. there's a ring on it, so i twist it. a spearhead comes out of one end. :A:"wh... what happened..." he's awake. i put the spear back and roll down his sleeve. he looks at me. :A:"wh- oh. i remember now." he rolls up the sleeve i just put down and ejects the cartridges. "what are those?" :A:"adrenaline cartridges. they were pumping stimulants into my bloodstream. i'm going to be sore for a week... but it won't be a problem. these should help me get along just fine, unless-" he feels the side of his neck. :A:"-unless my power ran out. how hard were my rods glowing?" "they were blinding. and your eyes turned black." :A:"oh crap... i went to the sixth... did hurt anybody?" "well, you knocked somebody unconcious- twice- and spared one of the guys. there were three of them. another one knocked you unconcious, and when you woke up, your eyes were black and your rods were glowing. everybody left except me, but nobody got hurt but them and you. and- you completely raged. and Louie almost got socked in the face." :A:"...almost?" you grabbed the guy's fist- which you crushed earlier- and slammed him to the ground. i think you broke his nose. also, you dislocated his leg- and the other guy's shoulder- and put them back into place." :A:"did the pulse device go off?" "the one where no one notices anything?" :A:"yes..." "yeah, it did. and you erased their memories. they passed out." :A:"oh crap... what time is it?" "it's 7:00. PM. it's dark outside- you can stay here for the night." :A:"thank you. well... what do you want to do now?" "i have my DS. i left it here before the storm." :A:"i have mine, too-" he takes it out and turns it on. :A:"MarioKart?" "sure." we play for a bit, and i notice he's quiet. "hey Alex-" he's asleep. i cover him up with a blanket, and turn his DS off. i'm tired too, so i go to bed. "night, Alex." silence. ~~~~~ so tired... "hey, Alex, you awake?" he's staring at the ceiling. :A:"ugh- Lena, i need you to help me." "with what?" :A:"i need you to reload the adrenaline cartridges in my arms and legs. i would be able to normally, but i'm in severe pain." "uh... how do i do that?" :A:"take my sweatshirt off. i have the cartridges in my shirt pocket. once you load them, i'll be able to get my legs." i take his sweatshirt off. Violet walked in. Alex looks at me for a sec. i'm about to panic. :A:"hello..." he forgets how sore he is and tries to get up. he winces and tries to relax. i forgot that Violet never met Alex. she comes in and closes the door. :V:"who are you?" :A:"i'm- ugh- i'm Alex. and you are?" :V:"Violet. Lena's sister." she looks at me. i completely forgot to tell either of them about each other. :A:"well, i know this must be awkward, but- nice to meet you." she looks at me. she's suspicious of him. :V:"what's on your arms?" :A:"robotic supports. i'm reloading my adrenaline capsules right now, or Lena is, anyways." :V:"interesting. well, i have to go- Huey wants to meet at the library." she leaves. Alex laughs. :A:"well, that was a bit of a suprise." "sorry... i forgot to tell you..." :A:"it's fine. now, back to the capsules." i take his sweatshirt off, after i get the capsules. :A:"do the right arm first. if you do the left arm, then it'll go to my heart too quickly and- and that won't be good." i take it off and grab the capsules. :A:"put the middle one in first, and then the left one. it should click." after i put the capsules in, he lifts up his arm. :A:"just a little bit will let me move around. it still hurts, but it'll be better." i finish the second arm. he does his legs, and i see a few scars. he sees me looking at them. :A:"i got those from... the run." "the what?" "i was walking home one time, when a few kids started chasing me. they were faster, and when the caught up to me, they slashed the back of my legs. i couldn't walk home, so i lay there for a few hours. someone- i don't know who, exactly- helped me up and took me to my house. after that, i built these supports to help me in case this ever happened again." he sat up. :A:"i should head home." he gets up, and walks out the door. :A:"bye, Lena." he goes to cross the street. "later, Alex-" a car hits him, flinging him across the pavement. the car backs up and speeds away, but i got a pic of the license plate. :A:"don't bother. he'll pay soon enough." he's alive! :A:"ugh... Lena..." "Alex! are you okay?!" i get my phone to call the ambulance. :A:"Lena... rip the cord under my sleeve.." i rip it, and he spazzes out for a few minutes. "what's going on?! Alex?" :A:"that was... a formula... i'll be better now. i'll heal. i'll be fine." he faints. on the street. the ambulance's sirens surround me, and the paramedics take him into the ambulance. ~~~~~ :Webby's POV: (two weeks later) we're in the waiting room. Alex has been in a coma for two weeks, and everyone's anxious for him to wake up. he has a few broken ribs, but he's doing fine now. everyone came to the hospital when they found out, even Scrooge and Mrs. Beakley. :MB:"i'm so sorry, Lena. i never even met him." Scrooge isn't sure what happened, but he still has sympathy to some degree. :SC:"i never met him, you know. but he seems like a nice guy, coming over when the storm hit." Louie feels bad. the guy that took his crush, saved him- twice- just got hit by a car. :LO:"i can't sleep. i don't know why, but i'm so exhausted..." :NURSE:"you can see him now." we head up to his room. the sterile smell is everywhere, but i get used to it after a while. he has a tube down his throat, and the monitor beeps in rythym with his breathing. the guy who hit him got caught, but Alex's aunt decided not to press charges after Lena told them what he said. Lena's sitting next to the bed, on a chair from the cafeteria. :LO:"he's a good guy. i'm... i'm sorry, Lena-" Alex starts moving. he coughs through the tube and the beeping gets faster. a nurse comes in. :NURSE:"move. we need some space." she tells Alex to stop moving, mutters something about "deflating the cuff", and takes the tube out. :LE:"Alex?" he coughs for a little bit, and turns to look at us. :A:"well... hi." :LE:"Alex! you're awake!" she hugs him. he hugs her back- a little shocked at first, but he smiles after a few seconds. :A:"uh... how long was i out?" "two weeks." :LE:"after you got hit by the car-" :A:"i got hit by car? i can't even remember what happened..." they left his robotic supports on after realizing that they just do not come off. they couldn't figure out how anything worked, so they just had to leave it on- it was too risky to try anything. :A:"well, i can take these off now." he unlatches something, and it comes off of his arm. there's a ton of super- short needles, and marks all over his skin. :LE:"well, i guess you won't be sore anymore." he gets up. :A:"well... who else is here?" Scrooge comes in. :SC:"i thought you'd like some company." :BE:"hello. i don't think we've met." Alex shakes their hands and tells them what happened. :BE:"say, what's on your arm?" :A:"robotic supports. they pump adrenaline into my blood, or whatever else i might need. when i got hit, Lena pulled the cord under my leg, and it released a formula i made. it heals fast, but it's extremely painful. that's why i fainted." ~~~~~ :Lena's POV: Alex got out of the hospital a few days later. i haven't slept for a while, so i head home. i turn on the CD that he gave me, and i go to bed. and everything is submarine, and green so tired... ~~~~~ :Webby's POV: we all get home from the hospital. :D:"so... what do you want to do? it's quite a bit boring..." :SC:"i didn't know Alex. you said he came over when the power went out?" :H:"yeah. he brought a ton of stuff to do, even a TV and a generator." :BE:"that was nice of him."

Российские производители получили 20 млрд рублей на создание новых процессоров

Российские производители получили 20 млрд рублей на создание новых процессоров 23.12.2020 [21:30], Владимир Фетисов
Стало известно о том, что Минпромторг определил исполнителей опытно-конструкторских работ по созданию трёх новых микропроцессоров. Как сообщает издание CNews, более 20 млрд рублей достались компаниям МЦСТ, «Модуль» и «Элвис». Эти деньги министерство распределило между производителями в рамках тематических тендеров, итоги которых были подведены в середине этого месяца.

В конце прошлого месяца стало известно о том, что Минпромторг планирует потратить на процессорные проекты около 21,1 млрд рублей. Эта сумма получилась в результате сложения начальных цен контрактов в конкурсах на тематические ОКР, которые министерство объявило 20 ноября. Они были организованы в рамках реализации подпрограммы «Развитие производства вычислительной техники» государственной программы «Развитие электронной и радиоэлектронной промышленности» на 2020-2025 годы.
Согласно имеющимся данным, 7,63 млрд рублей было зарезервировано на ОКР «Разработка системы на кристалле для встраиваемых вычислительных комплексов на базе процессорных ядер с архитектурой NeuroMatrix нового поколения» (шифр «Нейро-Б»). Ещё 7,49 млрд рублей выделено на реализацию проекта по созданию процессора «Эльбрус-32С» для высокопроизводительных серверов (шифр «Процессор-21 СРВ»). Кроме того, 6 млрд рублей планировалось выделить на ОКР «Разработка доверенного многоядерного процессора для программных маршрутизаторов защи��ённых сетей» (шифр «МАРКО-240»).
Исполнителями контрактов по двум первым тендерам станут НТЦ «Модуль» и компания МЦСТ, соответственно являющиеся разработчиками известных на рынке нейропроцессоров и процессоров «Эльбрус». Что касается контракта с шифром «МАРКО-240», то он достался научно-производственному центру «Элвис». Таким образом, сумма предстоящих к заключению государственных контрактов составила 20,22 млрд рублей. Сроком их исполнения в соответствии с техническим заданием является 31 января 2025 года.
Стоит отметить, что по решению Отраслевой комиссии Департамента инвестиционной и промышленной политики города Москвы крупнейшему производителю и экспортёру микроэлектроники в России «Микрону» одобрена субсидия в размере 51,8 млн рублей. Благодаря этому предприятие сумеет частично компенсировать затраты на лизинг оборудования для сборки чип-модулей смарт-карт и производства RFID меток.

Coherence (Part 1 of 4): What is the Sense of All of This?
This is a long overdue follow-up series on a post on “Confident Ambiguity” from 2016.

Most of you reading this post will have some background in the biopsychosocial model, pain science, and movement science. Based on this premise, my assumption is what I am about to say rings true with many of you:
Despite all the knowledge we have gained, the data we have scoured and synthesized, little of what we have learned “makes sustainable sense” when you throw it all together in effort to make it workable. With increasing knowledge, more gaps are inevitable and gaps in knowledge are never ending. Somewhat haphazardly, we patch the gaps as quickly as we can but the patches we use to bring them together are often mismatched.
If knowledge feels patched together to you, what does it feel like for our clients/patients?
Think about it. We’ve got this biopsychosocial model (framework!), the neuromatrix, the needless distraction of predictive processing, the sensation versus perception arguments, and all these other cool neuroscience things. But what about consciousness, what is it??? There are also aspects of contemporary biomechanics and loading capacity that need to be understood and incorporated. Then we’re dabbling with psychology, we’ve got expectancy violations, graded exposure, fear avoidance, yellow flags, resilience…. But wait, what about social and cultural implications? How can we be so cruel as to expect someone in the worst socioeconomic status to be anything other than trapped, they could never develop resilience and be another self-help success story because nothing of their environment supports it! Then there are arguments of logical fallacies, continuing battles of epistemology and ontology, and, wait is there a value to philosophy? But what about the person in front of us? Their story, their narrative! Surely we shouldn’t forget the person! But what about the new graduate navigating the whizz bang shiny objects excited that by finding that “dysfunction”, poking, scraping, corrective exercising, or constricting the circulation of their client into oblivion hoping for that magical, “that feels better!” verbal response to be provided. What about our patients’ autonomy? And our science! What about our science? Outcomes measure outcomes not interventions, the limitations of the peer review process, the poorly (sometimes fraudulently) performed systematic and meta-analysis, the lack of disclosure of conflict of interest, poor blinding and lack of bias observation in much of everything that is available. Oh. and don’t forget, what about our own self care? Don’t look now, there’s the next social media post and the next article to argue about, wait what are we arguing about? Are we arguing?
….If you are reading this paragraph several times, you may wonder, like I have, how most clinicians who fell into this curse of wanting to learn more and do better have not all gone mad. It is no wonder the transference of this information has been poor and slow to take on culturally, it’s like we don’t even have ground. We’re taking on all this information but we have no idea where we stand, how to make sense of it, and not just how to apply it, but how do we meaningfully share it with others?

ACE Fitness | The 4 Seasons of Chronic Pain
For individuals’ living with chronic pain, the long-range forecast is often filled with metaphorical rain storms, treacherous winds and long, dark nights. When pain is present, the most noticeable characteristic of a client’s changing climate often revolves around biomechanical restrictions and movement limitations. Perhaps not so obvious (yet often just as burdensome) are the emotional and psychological factors involved with experiencing pain on a regular basis.
To create a truly unique, multidimensional strategy for individuals in pain, it is imperative that you expand beyond the optics of biomechanics and connective tissue principles alone. Let’s take a walk through the four seasons of chronic pain and examine how you can help your clients transition smoothly through each one.
The First Season – Winter (fear)
Winter is the first season of chronic pain, where the radical change in climate significantly impacts an individual’s emotional and psychological well-being. The predominant characteristic of this season is usually fear—fear of movement, fear of pain worsening, fear of the unknown. This fear can reach such heightened states that just the anticipation of pain is enough to steer an individual away from doing the things that matter the most. Imagine avoiding an activity because of the anticipation of pain, rather than in response to it.
When such avoidance behaviors manifest, it’s clear that trying to address chronic pain solely from a biomechanical perspective is an incomplete approach. The neuromatrix theory of pain proposes that the output of pain is regulated by afferent sensory mechanisms in conjunction with cognitive inputs (Melzack, 2001). These cognitive inputs have the capacity to upregulate and exacerbate states of anxiety, apprehension, depression, self-doubt; all of which fall into the category of psychological/emotional stressors.
It’s these stressors that contribute significantly to winter’s burdensome climate. To clients in pain, winter’s dark and ominous atmosphere can sometimes seem to endure indefinitely. Successfully helping clients transition out of winter requires an understanding of the difference between a client’s “external” and “internal” problems.
The Second Season – Spring (awareness)
The melting of snow, dissipating precipitation and the awakening from hibernation are all welcomed signs that winter is nearly over. To the health and exercise professional, guiding clients toward a more desirable climate requires an understanding of each client’s internal problem.
All clients in pain have two global problems. The “external problem” is the biomechanical or anatomical concerns each client reveals during their initial consultation. Consider, for example, the client who has been experiencing knee pain for years. The knee pain is the external problem. The “internal problem(s),” however, may be the areas in life that hold the most meaning to individuals negatively impacted by chronic pain. The internal problems are the emotional, psychological and social/environmental stressors that are unique to the individual.
For example, consider a husband and wife who spend meaningful time together each day walking their dog. However, in recent months the husband’s knee pain (external problem) has become so problematic that it’s preventing him from participating in the most meaningful aspect of his day, which is connecting with his wife via their evening walk (internal problem).
A client’s emotional transition from winter into spring begins with his or her health and exercise professional gaining awareness into the client’s internal problem(s). This awareness then provides an opportunity for both the coach and the client to begin scripting a new, more desirable narrative based on what the individual client values most.
The Third Season – Summer (possibility)
For any seed to blossom and reach its full potential, a conducive climate is required. To clients in pain, fear, anxiety, feelings of hopelessness and negative self-talk are the metaphorical weeds of cognition. If these weeds are ignored, they can uproot any forward progress. During the summer months, the seeds of possibility must be nurtured and the cognitive weeds must be pulled on a regular basis.
As with any journey, minor setbacks and moments of self-doubt are to be expected (particularly when chronic pain is present). Because of this, including clients in the process of constructing future-oriented, growth-focused possibilities becomes essential. The Yellow Brick Road refers to this process as a “Possibilities Paradigm” and it involves four steps, each designed to amplify and reinforce a clients’ emotional and psychological resiliency and well-being. When successful, these 4 steps can help stir hope and bring optimism back into each clients’ current and future outlook.
The Fourth Season – Fall (self-regulation)
The fourth and final season is a time when clients return to pain-free living. And while there are a host of factors influencing each individual’s pain-free transformation (biomechanics included), a client’s capacity to accurately assess and regulate his or her own emotional state (self-regulation) is a primary contributor in overcoming his or her internal problem(s).
Consider the experiential difference in self-regulation in the following two statements: “Oh no, I just threw my back out again!” vs. “My back tightened up, but I know it’s just my body protecting me.” These are two completely different emotional reactions, the former laced with fear and the latter signifying the perception of safety and protection.
Preventing clients from experiencing negative emotions is, of course, not possible. However, as health and exercise professionals, we can strive to cultivate a climate that enhances each client’s self-regulation competence and help them identify and overcome the emotional and psychological stressors that contribute to their pain. Importantly, you can begin this process with your client even before you have conducted his or her biomechanical evaluation.
