You sat comfortably on your boyfriend's bed, your back pressed against his chest as you typed away on your computer, taking notes for your psychology class. His chin rested on your shoulder as he peered at the screen, reading a bunch of words he didn't quite understand while his hand rested on your thigh, his thumb drawing soft circles.
"What are you doing?" He hummed, his fingers dancing slightly higher. He knew what you were doing, but he wanted to hear you say it. He loved how excited you got and how you rambled on and on when you were telling him something you were passionate about. If he was being honest, it was kind of a major turn on.
"I've got a test tomorrow on the biological bases of behavior, so I'm just refreshing my memory on the endocrine and nervous systems," you explained, too focused on your notes to register that his touch was slowly sliding closer and closer to your clothed core.
"Oh, yeah?" He murmured, dipping his head down, his lips brushing against your skin as he began to pepper soft, open-mouthed kisses down the column of your throat. His hand continued upwards, his fingers lightly brushing you over your already damp underwear. "Tell me about it, baby."
"Rafe," you gasped softly, biting your lip. You tilted your head to the side, your eyes fluttering shut as you tried to find your resolve and tell him to stop, but you couldn't help the way your legs parted for him. "I told you I have to study."
"You are studying. Cmon, tell me all about the nervous system while I touch you, baby. You can do both," he coaxed you, his long fingers expertly pulling your panties to the side and running along your wet folds. "Tell me everything that's in that beautiful brain of yours."
You let out a breathy moan, your breathing quickening as your head fell back against his shoulder. You racked your brain for any piece of information, his touch making your mind go blank. "The brain is um-" you drew in a sharp breath as his thumb nudged your clit. "The exterior brain structures are the cerebral cortex, cereb-bellum, occipital lobe, temporal lobe, frontal lobe, and parietal lobe," you managed to say.

"Rafe," you moaned, your lips parting in pleasure as his fingers worked expertly inside you. You couldn't focus when you could feel his long fingers dragging against your tight walls.
"Cmon, my smart girl," he cooed, his other hand snaking around your waist and splaying his large hand across your stomach to keep you in place. "You know it. I know you do." He continued drawing his fingers in and out of you as his thumb rubbed your puffy clit firmly.
You did know it. You'd even forced Rafe to help you make stupid flashcards, so you could memorize the parts and all their functions. But, he had a way of making you forget everything and turning your brain to mush when he had his hands on you.
Your voice was shaky and breathless as you spoke, feeling your orgasm building low in your stomach. "The interior brain structures are the-the amygdala, hippocampus, medulla oblongata-" you gasped, your back arching into his touch when his fingers hit that spot inside you that had you practically seeing stars. You hesitantly kept going, trying to focus because knowing Rafe, if you stopped, he would too. "P-pituitary gland, thalamus, basal ganglia, hypothalamus, midbrain, and pons." You were pretty impressed with yourself that you managed to remember all that while your boyfriend was knuckle deep inside your dripping cunt.
He groaned, feeling himself grow harder as he listened to your breathy voice tell him all about your studies. "You're so fuckin' smart," he found that sensitive spot on your neck, sucking gently. "My brilliant girl."
Your walls fluttered around his fingers at his praising words. He was always so interested in what you had to say, never dismissing you or brushing you off when you started rambling aimlessly and throwing random facts his way. He listened intently, showing genuine interest that made you feel so loved and seen.
"Keep going, baby," he rasped, his fingers speeding up in a way that had you gripping his bicep—needing something, anything to keep you present. "What do all those big words do, hm?"
You let out a breathy laugh, your chest rising and falling rapidly. "Well, the um- the hypothalamus controls the pituitary gland, which releases hormones that regulate bodily functions," you explained, smiling softly at how fitting that fact seemed to be.
"Mm, like the hormones that are rushing through your veins right now, making you all hot and bothered for me?" He asked, his voice low and taunting, his teeth lightly nipping at your neck. He pushed his fingers deeper, curving them up to hit that spot that always made your legs shake.
"Uh huh," you moaned, your nails sinking into his bicep through the soft material of his sweater. His fingers curling inside you as his thumb continued swirling around your sensitive bundle of nerves, coaxing you closer and closer to the edge.
He hummed against your skin, his tongue laving over the spot where his teeth had marked you. "Cmon, pretty girl," he whispered, feeling your body tense up. "You did so good. Let me feel you come apart on my fingers."
Your eyes fluttered shut, back arching into his touch as his words sent you over the edge. A whimper of his name fell past your parted lips amongst a sea of soft moans as you came, your walls clenching rhythmically around his fingers while he continued to drag them in and out of you, prolonging your high.
He groaned as he felt your velvety walls flutter and squeeze around his fingers, your sweet cries of his name spurring him on. He worked you through it, fingers pumping steadily, thumb circling your clit until you were a boneless, panting mess in his arms.
He peppered kisses along your jaw as he carefully pulled away from your weeping core. "Who knew you could make biology sound so sexy,"
"Technically, it's psychology," you corrected him, chest heaving. You turned your head to look at him as he brought his fingers to his lips, sucking your release off of them with a low groan. You bit your lips at the sight, his lips glistening as he pulled his fingers back.
"Yeah, yeah, whatever," he flashed you a grin before leaning down to capture your lips in a gentle kiss, slipping his tongue into your mouth to let you taste yourself on his tongue.
You moaned softly into his mouth, reluctantly pulling back after a moment. "If you keep distracting me, I'm never going to finish."
"Hm, that's weird because I'm pretty sure you just did," he hummed, feigning confusion as a cocky smirk tugged at his lips.
"Oh, shut up," you laughed, feeling your cheeks heat up at his innuendo. His hand slid up your arm, tracing soft shapes on your bare skin. You looked up at him, seeing the way his pupils dilated and feeling the evidence of arousal digging into you from behind. "Five more minutes and then I'm all yours, deal?" You raised a brow. You figured you had studied enough over the last couple days, and your oh so patient boyfriend deserved some of your attention.
A sly grin settled on his lips. "Deal, but don't think that I'll be keeping my hands to myself during these five minutes," he said, his eyes twinkling mischievously.
You turned back to your computer, giggling as he dipped his head back into your neck, already kissing and sucking at your delicate skin. His hands started to roam your body, and as you tried to focus, you found yourself wondering if you would make it five minutes before you surrendered to him.

house md rewatch: 1x16, "heavy"

hard watch for chase enjoyers (me).
truly, this episode blows foreman's (thus far!) unexplainable dislike of homeless people out of the water. chase's absolute VITRIOL toward overweight patients is batshit crazy lol. i would be out of line not to foreground that in this little recap. he was also reiterating his behavior in self-preservation mode, and i like that this show isn't afraid to make the characters nasty when their put in positions of extreme stress. doesn't make it any easier to watch though lol.
since i'm me, and i always need to identify at least some attempt at a through-line in these episodes, i'd say that the idea of the Whole Person is the primary operating theme in this episode, manifesting in myriad ways. first and foremost ofc is the patient, jessica (WHO I LITERALLY JUST REALIZED IS THE JENNIFER STONE OF WIZARDS OF WAVERLY PLACE???), who is constantly ignored and dismissed by doctors on account of her weight. in a unique twist, i actually have more to say about the patients in this episode than usual!
her mom does her best to advocate for a deeper problem, despite all 3 of the ducklings at some point wanting to diagnose her with weight at the forefront of their minds. cameron is by far the most forgiving and sincere in this regard, and i LOVE that she's frank with jessica's mom that, by looking solely at jessica's weight, they could very well be wrong.

jessica's whole personhood is rejected because of one over-arching factor that controls the rest of her life, despite her best efforts to "fix" it. jessica resists what she feels is an immutable part of herself, her weight. meanwhile, in the clinic, house meets lucille hernandez, who's complaining of heartburn, and who he points at and says "fat woman? pregnant. the end." he sneakily gives her a pregnancy tests, learns that he's wrong, and, through some unnecessary plot complications, asks her to come back in because she very well might have cancer on account of the 30lbs tumor in her stomach.
but it's benign! and she likes how she looks! so she, at first, rejects getting the surgery done, arguing that house and wilson ("i too am in this episode") are reducing her to her visual appearance. and she's pretty much correct.

as silly and over the top as it may be, lucille feels that they're forcing her to part with an immutable part of herself and that they won't see her for her Whole Person. the same applies for jessica, especially when they learn that it's a tumor on her pituitary gland that's causing so many of her medical issues. had they limited their insights to her weight, minimized her full personhood, they would never gotten there.
my attempt to translate this episode's ethos to the ducklings + house amounts to equating the diagnostics team as a Whole Person. with vogler's demand that house fire a member of the department especially potent right now, it's extra clear that they need their entirety to properly function, and that vogler, an outsider, can't see that entirety. furthermore, when any one team member is isolated by vogler, their dependence on one another is highlighted.
especially with chase, of course. the end-of-episode revelation that he's been vogler's informant, and that this has been to no avail because vogler won't promise him immunity shows how futile the department is when they try to act independently.

next, i like how house's concluding conversation with vogler pits their senses of absolutism against one another. house makes the decision to fire chase, which vogler (surprisingly, given how he just talked to chase) rejects. he changes the terms of his demand on a dime and without warning. house is not above doing this himself! which makes the fallout all the more fascinating.

the best line in this episode by far goes to cameron, however. he reminds her that "the world doesn't work that way, just 'cause you want it to," and she replies with: "figures you'd stall and refuse to deal with the issue. problem is the world doesn't go away just because you want it to."
JAW ON THE FLOOR! AND THEN HE IMMEDIATELY WALKS OUT! NOT ONLY THAT, HE THEN GOES AND TRIES TO IMPLEMENT CAMERON'S SOLUTION TO VOGLER SO THEY CAN AVOID FIRING SOMEONE, DESPITE MAKING FUN OF HER FOR HAVING THE IDEA.
@all-pacas has had some excellent commentary about house and cameron's dynamic so far, which has helped me respect this part of the show more, and i'm tagging you because i REALLY see the points you've been making manifested in this exchange. oftentimes i see hameron as reductive that house would kinda/sorta fall for one of his fellows, but this successful challenge/rebuke/fold in private is so excellent. it highlights cameron's capacity for ingenuity that's so similar to his (bending the rules) in a way that the text then celebrates by having him act on it. BUT, crucially (for me, at least), is that he acts on it in private. i like the resistance to taking cameron's advice here because it seems the most in-character response he's had to thus far.

and from a visual standpoint, i also like the privacy we experience with house in this moment. this singular expression does more work in convincing me of his inner conflict regarding cameron than any other scene/conversation of theirs so far. thank you to hugh laurie and his big ass eyes.
and this is to say nothing of how house threw her under the bus more than anyone else, again challenging her to fuck up, only to accept that she definitely makes the most compelling case, even under pressure.
finally, i want to make 2 wilson notes:
this was the first time we saw wilson buy house lunch, involuntary though it was.
wilson expertly highlights that house is making the ducklings play this game not because it will make a true difference in the end, but because he is selfishly avoiding the choice. being cruel with this game pushes his fellows away, which makes the inevitable outcome less painful for house, at least in his mind. self-sabotaging moment! without concern for collateral, either, a trend that will follow house ad infinitum.

foreman's conversation with house, where he's basically bullied into choosing who to fire, also has a subtle nod toward the selfishness that's driving house right now. when foreman finally says that he'd fire chase between the 3 of them, house is surprised, saying that he "didn't think [foreman] would pick at all." not only have we established that foreman is highly ambitious and pretty cutthroat thus far, but this is a moment wherein house tries to see himself/identify with another character and fails.
if foreman, who is so like house and who he loves to pressure, was unwilling to choose, it would give house some sort of validation to follow suit. but nope - foreman knows what he wants. he's uncompromising in a way that is stable and, for himself, productive.

really exciting episode overall. i had more to say about it than i thought! and i could unpack chase a lot more but, again, i like to keep my thoughts as limited to season 1 thus far as possible.

Scent Dynamics (Hormonal Mechanisms Edition)
An update of sorts to an earlier edition of scent dynamics that I wrote (and will reblog with this after it is published), which contains diagrams of the locations of various diagrams of scent glands. This edition talks more about the mechanisms that go into how the glands themselves work. Attached is a diagram I created for visual reference.

Alphas and Omegas have very similar scenting dynamics in terms of how the gland works. In my omegaverse, betas do not have scent glands, but they do have a vestibular remnant structure that may give off a weak/negligible scent. The reason being that the sweat and scent glands are both exocrine glands (glands that secrete substances out of the body). The sweat and scent glands are independent structures, but share a common duct. 
    The scent glands are located in the dermis, a layer that is located between the epidermis and the subcutaneous (also called the hypodermis, or the fatty layer beneath the skin). Sebum, an oil secretion, can be used in combination with scent secretions. Sebum is primarily made of lipids and waxes, which aid in sealing the scent onto whatever surface the alpha or omega is trying to seal it on. This allows the scent to “linger” on a surface for extended times. 
     As shown in the first part of my scent dynamics, pups primarily have scent glands on their head/neck region. Pups do not yet have the ability to scent things themselves, but these primitive scenting glands allow the parents/packmates to scent their pups in locations that will stick. 
    Now comes everyone’s favorite part, hormones. The hypothalamus, a region of the brain houses three key locations (the reproductive nuclei) we’re going to focus on here; the paraventricular nucleus (PVN), surge center, and tonic center. Omegas have a paraventricular nucleus, surge center, and tonic center, while Alphas do not possess a surge center. Alphas do not have a surge center as only Omegas require the surge center.
    The surge center, a key reproductive nucleus of the female, is responsible for targeting the anterior pituitary gland. This stimulates the release of gonadotropins, which are hormones that specifically target the gonads (the ovaries and testes). Gonadotropin Releasing Hormone (GnRH) is a major hormone in both the surge and tonic center. The tonic center, located in males and females, also releases GnRH and targets the anterior pituitary. 
    The PVN is the nucleus responsible for targeting the posterior pituitary gland. It serves multiple functions, but our primary one for this analysis is that it stimulates the production of oxytocin. Whyever is that hormone so important? Well, oxytocin is a major hormone in the process of reproduction (especially labor and lactation), social bonding, and emotional regulation.  
    The pituitary gland is a small dual-lobed gland, located in the brain. It is divided into two lobes, the adenohypophysis (anterior) and neurohypophysis (posterior). The adenohypophysis is responsible for prolactin (a hormone that stimulates milk production), growth hormone. Adrenocorticotropic hormone (ACTH, regulates cortisol, a major stress hormone), follicle stimulating hormone (FSH), and luteinizing hormone (LH). Those last two, FSH and LH, are gonadotropins. FSH targets the gonads by triggering the producion of gametes (ova/sperm) and LH is the hormone responsible for the big hormones that many of us have heard of: testosterone in our alphas and estrogen/progesterone in our omegas. Estrogen and testosterone are steroid hormones that support the reproductive tracts, while progesterone is the hormone that maintains the uterus for pregnancies. The neurohypophysis doesn’t technically produce any hormones, but it does store oxytocin.

Across the United States, thousands of parents have consented to having their children’s puberty stopped with a class of drugs called gonadotropin-releasing hormone agonists. Known colloquially as “puberty blockers,” these drugs overstimulate the pituitary gland to the point of preventing it from sending signals to the ovaries or testes to start producing the hormones responsible for puberty.
Parents who have consented to these drugs for their children love their kids dearly, but they’ve consented under entirely false pretenses. The doctors who’ve advised them say that puberty blockers are known to improve mental health — that they are even life-saving — and that they are fully reversible and just give kids “time to think.” None of this is true.
Major American medical associations say that “gender-affirming care” for kids is “medically necessary” and “life-saving.” Health authorities Finland, Sweden, Norway, Denmark and the U.K. disagree. Last month, the National Health Service of England decommissioned puberty blockers as a treatment of adolescent gender dysphoria. “We have concluded that there is not enough evidence to support the safety or clinical effectiveness of [puberty blockers] to make the treatment routinely available at this time,” the NHSE explained.
Imagine if American doctors told parents the following truths. The mental health benefits of puberty blockers are highly uncertain, according to multiple systematic reviews of the evidence, the bedrock of evidence-based medicine. The World Health Organization says the evidence is “limited and variable.” There is no research into long-term harms, but some evidence suggests decreased IQ and brittle bones. Permanent sterility is guaranteed for minors who go through full hormonal “transition.” Sexual dysfunction appears to be extremely common as well. Over 93 percent of kids who take these drugs go on to cross-sex hormones, which lead to permanent physical changes including excruciating genital growth, vaginal atrophy and tearing and much higher risk for cancer and cardiovascular disease.
There is no credible evidence that puberty blockers function as suicide-prevention measures. Finland’s top gender clinician has called the suicide narrative “purposeful disinformation” and “dangerous.” For all these reasons, health authorities in a growing number of countries, including some of the most LGBT-friendly, are now prioritizing talk therapy.
How many parents would consent to puberty blockers under these circumstances? Very few, if any.
It is common for drugs to enter pediatric use after evidence of their success in adult medicine. The opposite happened in gender medicine. It was the failure of “sex reassignment” in adult men to achieve satisfactory cosmetic outcomes and improve life functioning that led a group of clinicians in the Netherlands to propose starting the “reassignment” process in childhood.
Their hypothesis was as technologically appealing as it was ethically dubious: since males could not reverse the effects of testosterone-fueled puberty to pass as women, it would be beneficial to these men to have their puberty bypassed altogether.
The Dutch recognized the dilemma but thought they found a way around it. Relying on their experience using puberty blockers to treat a condition known as central precocious puberty (CPP), they argued that blockers were fully reversible and thus part of the diagnostic process. If it turned out that the kid wasn’t “truly trans,” the drugs would be discontinued and puberty allowed to resume.
Their argument was dubious from the get-go. First, CPP has an objective diagnosis, based on a blood sample, whereas gender transition is based on the adolescent’s feelings and experiences, which are subject to change. In a political climate such as ours, in which mere exploration of the reasons for rejecting one’s body can be labeled “conversion therapy,” differential diagnosis becomes impossible.
As Dr. Jason Rafferty, author of the American Academy of Pediatrics’ current policy statement on “gender-affirming care,” has put it, “the child’s sense of reality and feeling of who they are is the navigational beacon to sort of orient treatment around.” The AAP statement has been witheringly critiqued, and Rafferty and the AAP are now defendants in lawsuits by former patients.
Second, in CPP puberty suppression is by definition temporary; the goal is to delay puberty to its appropriate developmental window. In gender dysphoria, a “successful” prescription is where puberty is bypassed altogether. The assumption about reversibility, never tested and highly questionable form the start, proved to be the ethical foundation for the entire Dutch experiment, and it quickly crumbled. Over 93 percent of adolescents who are put on puberty blockers for gender issues continue down the medical pathway to cross-sex hormones. Some go on to surgeries.
Gender clinicians do not see this suspiciously high figure as a reason to rethink their approach. They see no possibility of iatrogenesis — a medical intervention that unintentionally induces harm, in this case by causing gender distress or confusion to persist artificially. On the contrary, they regard the high persistence rate as proof of their own remarkable diagnostic abilities.
More modest and scientifically-minded clinicians and researchers see things very differently. “Blocking puberty,” writes Sallie Baxendale, a professor of neuropsychology and author of an important new study on puberty blockers, “prevents the critical rewiring in the brain that underpins the ability make complex decisions. Puberty blockers may give children time to think but they simultaneously rob them of their developing capacity to do so.”
What is likely happening is that an ongoing youth mental health crisis whose origins predate and have little to do with gender is being misdiagnosed and mistreated with harmful and experimental drugs. Puberty blockers are the definition of a “quick fix” solution.
Researchers incorrectly refer to what the Dutch did as an experiment. In an experiment, falsifiable hypotheses are proposed, alternative interventions are tested, outcomes are monitored and competing explanations for observed results are thoughtfully ruled out.
The Dutch did nothing of the sort, according to a comprehensive scholarly examination of their study. Further, the only attempt to replicate that study, which was done in the U.K., failed. The researchers had to be forced to disclose their disappointing findings. Any scientific-minded person willing to put in the effort and read the literature will come to the same conclusion: Pediatric gender medicine is an industry built on fraud.
During the 2000s and 2010s, the Dutch pseudo-experiment with puberty blockers “escaped the lab” and became entangled in a fast-growing international social movement for transgender recognition. In the U.S., the drugs are being prescribed at numbers far exceeding anything the Dutch could possibly have imagined. Most adolescents referred to pediatric gender clinics are teen girls who have no history of dysphoria in childhood but who do have other mental health challenges that predate their distress with their bodies.
American medicine is no stranger to scandal — lobotomy, “recovered memory” and OxyContin are just a few examples. What makes pediatric gender transition unique is that it has been framed as a nonnegotiable civil right and defended by powerful civil rights groups, the Democratic Party and their ideological allies in the mainstream media.
A key reason for the divergence between U.S. and European medical authorities, as I’ve explained in a previous essay, is the latter’s greater willingness to follow principles of evidence-based medicine, including reliance on systematic reviews. Jack Turban, a prominent American gender clinician, revealed in a deposition that he seems not to know what a systematic review of evidence is.
Another reason is that in the U.S., doctors who practice child “transition” demand and often receive deference as the experts on the evidence for their practices; abroad, such clinicians are seen as having conflicts of interest. When the National Health Service of England appointed the highly respected Dr. Hilary Cass to lead its review of its youth gender service, it did so precisely because she was “a senior clinician with no prior involvement or fixed views in this area.” Sweden and Finland delegated the evaluation of evidence to experts with no personal involvement or stake in pediatric gender medicine.
Parents should never have been put in the position of having to decide whether to “allow” their kids to go through puberty. Those who would put the onus on parents are letting charlatans in the medical profession off the hook. Puberty is difficult for all teens, and it is not a disease. Puberty blockers offer teens in distress — especially girls with history of sexual abuse, autistic kids and gay kids — false hope by casting puberty as optional.
Puberty is a rite of passage from childhood into adulthood, responsible for the development of the body’s major organs and systems and not just its external sexual features. Puberty blockers rob children of their right to an open future.

Doctor finally called back. My TSH has not gone up by as much as I expected (3.11 last time, 4.8 this time) but it still shouldn't be increasing by that much in six weeks, or at all, and it shouldn't be that high in the first place. But my T4 has dropped from 19 to 13 in that time!! Normal range is 12-22.
TSH isn't actually a thyroid hormone; it's a pituitary hormone which 'asks' the thyroid to produce more thyroxine. T4 is thyroxine. So basically my brain is starting to raise its voice more and more at my thyroid to get more thyroxine, but thyroxine production is dwindling anyway.
No doctor has ever told me that btw I had to find all that out myself lol.
Essentially, because we know that I have antibodies attacking my thyroid, the drop in T4 shows that they are doing so quite successfully and having an impact on its function (i.e. reducing it so that I feel like garbage).
Doctor has booked me an appt for tomorrow afternoon so we will see what happens. 4.8 is only slightly out of range for TSH, and T4 is not yet out of range, but obviously will be soon if it continues at that rate. Like I'll just be making lunch for example and all of a sudden my arms will start to feel heavy and I'll start feeling lightheaded. Then my legs will get that heavy and weak feeling, then my hands will start to shake and my vision takes on this kind of... idk, hyper sharpness that also makes things feel out of focus? Think like when you mess with the sharpness of a photo too much and it starts making things look distorted by just how sharp it is. And then the edges of my vision will get kind of fuzzy and dark, and I'll start dropping things and knocking things over, banging my knees into cabinets without meaning to. People will be talking to me and it becomes hard to answer them because I have hard time processing what they're saying and how I want to answer. Usually by then I'm shaking so badly I have to just stop and go collapse on the couch and curl up tightly into a ball until the symptoms fade. And it usually leaves me feeling drained for hours afterward. And I never know when it's going to happen and when it won't. It changes from day to day, hour to hour. I can wake up feeling great and by the end of the day feel awful. I can have a week of barely any symptoms and then two weeks of every day being hell. It's completely random.
My doctor - I think - is convinced it's some kind of neurological thing happening. Or possibly a really hard to detect/unknown pituitary disorder of some kind, but again there's just nothing as of right now that's making sense or is showing up in tests.
