Young woman, COPD history, presents to the ER for atypical parasternal chest pain. Initial biology shows mild elevated leucocytes with negative CRP and DDimer levels superior to 10 x age. Normal clinical exam and ECG. Echo shows a floating mass visible at the level of the tricusid and pulmonary valve. What is your oppinion? I will present the diagnosis shortly in a additional clip.

Alexandru Mischie Obstructive Hypertrophic Cardiomyopathy with SAM (systolic anterior movement) from ALEXANDRU MISCHIE on Vimeo.

Alexandru Mischie, cardiologist, presenting a case of OHCM (Obstructive Hypertrophic Cardiomyopathy). Patient female of 60 of age comes to the ER fordyspnea. On echocardiogpraphy, i found a sub-valvular obstruction which involved the anterior mitral valve, creating a SAM (systolic anterior movement). Mild mitral insufficiency. There was no arrhythmia detected. Genetic testing undergoing. No family history of cardiac death.

What treatment should this patient have?
