SCIATICA – SCIATIC NERVE PAIN
The term “Sciatica” is quite commonly used among st doctors as well as patients, but often what the term implies can become unclear or is even misunderstood by many. The term “sciatica” is derived from the Sciatic nerve, a major nerve that starts in the lumbar spine (low back region of the spine) and gives rise to many smaller nerves that branch off further down both the legs. There are two separate nerves on either side.
The sciatic nerve is a large nerve created from many smaller nerves that come together after exiting the spine in the lumbar and sacral regions (lower back) and carries information of sensation, pain, and muscle function for the lower extremity to and from the brain.
The pain experienced by people suffering from sciatica can vary widely, from a mild ache to a sharp, burning sensation or intolerable pain. It can also be felt like a jolting pain, like an electric shock, at times. The pain is most often worsened by prolonged sitting or driving for long hours. Usually, one side of the body is affected. Many times, people also complain of numbness, tingling or crawling sensation, medically described as paraesthesia, accompanied by muscle weakness or occasional loss of balance on the affected side.
The sciatic nerve may be irritated due to several reasons, and only a slipped disc or herniated disc doesn’t need to irritate the nerve. Muscle tightness, and nerve entrapment elsewhere in the body, namely in the hip or thigh, may also cause similar symptoms.
Since starting work in the treatment of sciatica in Pune 15 years ago, educating people about the symptoms has been a major part of treatment. Pain in the low back is very common these days and it is a major hindrance in the daily lifestyle, work as well as fitness activities of thousands of people. It is important to understand that acute lower back pain and sciatica are usually self-limiting and will resolve over some time. So time is the best medicine. But steps need to be taken to address the cause as well as treat the problem itself, so back pain along with sciatica symptoms does not recur.
CAUSE OF SCIATICA PAIN:-
In the treatment of sciatica, a thorough history is important. Most often, the cause of sciatica is found in histories well taken, and then the diagnosis thorough physical examination can be confirmed. In places like Pune, where sitting jobs have suddenly risen over the past 15 to 20 years, long sitting hours in incorrect posture and sedentary lifestyles have caused a surge in the cases of sciatica to an extent that people have to stop work, and earnings and savings take a major hit because of the pain and radiation.
Sciatica most often results from a herniated disc (also known as slipped, ruptured, bulging or protruding disc). Discs are cushions between the bony vertebrae of the spine. Vertebrae are bones stacked one over the other in a specific curvature. Loss of this curvature due to muscle weakness leads to compromised posture and increases the risk of slipped disc. When a lumbar disc shifts out of its position, it can touch the nerve root and cause sciatica symptoms.
As people age, the discs lose their water content and thus lose their strength, reducing the space between two vertebrae, causing them to be compressed and thus can protrude out.
Poor biomechanics, straightened curvature of the spine, weak abdominal muscles, tight hip flexors can all increase the risk for disc herniation.
Spinal stenosis, or narrowing of the spinal canal, and spondylolisthesis (forward slippage of a vertebra) can also cause cord compression and resultant sciatica.
TREATMENT:-
As mentioned above, since working in the sciatica treatment in Pune for over 10 years, I have seen multiple cases recover with time. Pain usually subsidies with inactivity during the first few days and basic but important stretches need to be started during this phase. An extension position is generally recommended, which helps the disc to get re-absorbed and centralized into its own space and once the symptoms of sciatica reduce, or completely stop, further exercises must be started to keep the disc from squishing out again.
It is very important to seek professional help quickly. The quicker the diagnosis, the smaller is the recovery time and the higher are the chances of healing and permanent recovery. Centralizing exercises need to be started as soon as the initial response of pain is less. Lying in bed won’t help. One needs to start taking efforts and gradually start increasing the quality of exercises done, as well as the quantity of these exercises and stretches.
If it’s a case of disc herniation, applying ice for initial 48 hrs may be helpful, depending on the particular case, and then using heat packs can provide some relief in symptoms as well as discomfort and difficulty in walking. If there is a resultant spasm, heat application will help and provide relief over time.
Again, an early diagnosis is necessary and treatment must start soon. The sooner, the better. Remember, sciatica is not the cause, it is just a symptom, the cause is somewhere else, and it needs to be found out by a trained professional and treated accordingly. Most often, conservative management is suggested and the chances of complete recovery are high if a well designed primary care and exercise program are followed.

Referring to fat loss as weight loss is quite a mistake. When people refer to weight, they are actually talking about the aggregate body mass which consists of the weight of fat plus FFMI.
FFMI stands for fat free mass index. It is the weight of bones, skeletal muscles, connective tissue, organs, water, blood. Fat Weight is the weight of stored body fat, that which we are interested in losing.
Losing fat weight is what your target should be, while incrementing the body’s FFMI.
Have you ever weighed yourself multiple times in a single day? If you have, you might have noticed your body weight fluctuate quite drastically. The weight fluctuation can be up to the scale of 1 to 2.5 kg within the same day. Nearly all of this weight fluctuation is contributed to by water retention. Rest of it by undigested food in the stomach and a few other factors. Now if these weight fluctuations are misinterpreted to be fat loss and gain, it is a gross mistake and misunderstanding of how body burns fats.
People who weigh themselves multiple times a day also run into depressive phases because they think that their hard work isn’t paying off and that they should be beating themselves down more and more to achieve results. This is wrong and unnecessary thinking. It is a very misleading concept that a lot of health providers assort to, to misguide their clients into thinking that they are at fault because they aren’t dropping digits on the scale. Measuring body weight daily does not prove a thing as far as fat loss goes. Making a client weigh first thing at entry for exercise, then another weigh-in at exit is a classical practice followed by multiple health care providers to prove to the client that the workout session worked. This is because the exit weight has to be lower than entry weight since there is a loss of water during exercise, especially when people aren’t allowed to hydrate properly during workouts. The next day when the same person is back for exercise, he is again made to weigh-in prior to exercise and this time, his or her weight obviously records higher than the previous day’s exit weigh-in. Then the person is blamed for not following the day’s nutrition properly, leading to weight gain. This is highly uninformed and wrong advice, which is extremely misleading and psychologically detrimental to the daily exerciser.

Fat loss is never seen within a single day. Fat loss is achieved over a period of time. It takes weeks and months of consistent, high quality, scientific exercise and correct nutritional goals and strategies to achieve fat loss. On the contrary, one should be interested in increasing the body’s FFMI that is the weight of muscles, the health of connective tissue (ligament, tendons) and improving bone density. This can only happen with a well rounded nutrition plan and well-deigned, scientific and enjoyable daily exercise sessions. Incrementing muscle weight takes time and takes a lot of hard work, patience and perseverance.
I hope you have followed this. If not, do read again. You shall now understand the difference between fat loss and weight loss quite well. When you start to exercise, your goal should be to target fat weight and not body weight. Weighing scales should only be used as a proxy measure and a guidance feedback to keep an eye on which way you are proceeding. A downward trend in weight obviously indicates that you are losing weight and the hope is that the weight comes from fat. Whenever you watch for weight drop on the weighing scale, always remember that your are measuring “body mass” and not your fat. So if your weight refuses to reduce at a certain point for a particular reading, measure again a day or two days later. Chances are you are retaining fluid and so your weight doesn’t show a drop on the scale.
It is important to understand that you must observe a trend in weight loss and not obsess over reduction of weight at every consecutive reading. So if for instance you weigh 80 kg one day, and you observe that your weight in the next week reduces by 1 kg, it will show your weight down to 79 kg. This rate of reduction might carry on every consecutive week, or the rate might change, which is a high probability. But at times there will be periods when you will see no drop in weight or an increment in weight. For instance, if you see your weight drop to 74 kg at the rate of 1 kg per week, and the next week your weight shows a rise to 75 kg, it doesn’t mean that your fat weight has increased by 1 kg. Observe your weight again a couple of days later. And then check it directly in the next week on your regular weight check day. If in your fat loss program you see a trend of weight loss where multiple times your weight fluctuates and sometimes stagnates, it is important to check a trend of weight loss. So draw out your record of the past months that your have been exercising and check what your start weight was. And then check what your weight on the present day is. If you see that your weight drop has been from 80 kg to say 70 kg, but there are multiple times your weight has shown an increment and then decrements, and at times stagnation, those weight entries are not important. It is important that you have reduced 10 kilos. So it is essential to be happy about it and keep up with the efforts.

Upper Crossed Syndrome
Today’s life is heavily driven by technology. And technology is quite the weapon that must be used to stay afloat in this fast paced world. But technology comes at a cost. At the very basic level, today we all are constantly glued to our cellular phones, tablets and computers. We type, we read, we communicate almost entirely through technology these days. There is very little time spent in the day without the presence of some sort of communication equipment in our hands.
This constant use of devices added on to daily stressors like seated jobs, long distance drives and degraded lifestyles has led the human body to make compromises it is not expected to make. To meet the demands of today’s life, people tend to forget that there is only so much the body can take before it starts to give up. And this, among other things is seen in today’s society’s most common postural compromise – forward head posture accompanied by rounded shoulders and a weak back called Upper Crossed Syndrome.
A person’s posture is the product of balance (or imbalance) between opposing (antagonistic) muscle groups. When the body is in perfect balance, it habitually sits, stands and moves around in the way that it was deigned to. However, due to repetitive misuse, and constantly maintaining a bad posture, an imbalance sets in because muscles are constantly trying to counterbalance the wrong posture. Additional stress placed on the tissues can eventually lead to pain and injuries.
UPPER CROSSED SYNDROME
Comfortable positions for repetitive actions like those mentioned above force the body stay in a position that leads to tightness in the chest (Pectoralis Major/Minor) and shoulders (Upper Fibres of Trapezius), accompanied by weakness in the upper back (Rhomboids/Lower Fibres of Trapezius) and neck (Cervical Spine Flexors). The combination of tightness and weakness in these opposing muscle groups can lead to many chronic issues such as Impingement Syndrome and Thoracic Outlet Syndrome.
SYMPTOMS
Upper Crossed Syndrome is commonly characterised by:
a chronic forward head position with a jutting and dipping chin
Increased cervical lordosis – anterior or forward curvature in the levels of the cervical (neck) spine
Increased kyphosis – posterior or backward curvature in the levels of the spine that of the upper
elevated, protracted, or rounded shoulders, where the muscles are in a continuous state of being pulled or stretched forward
Scapular winging – the visible portion of the shoulder blade protrudes way out instead of laying flat
THE SOLUTION
Theoretically, correcting this postural compromise is simple but it is a lengthy process. Imagine the muscles of the chest and back playing tug of war. With a constantly forward head posture, the muscles of the back are chronically lengthened and thus are at a significant mechanical disadvantage in this game. Lengthening the tight muscles of the chest using a combination of stretches and soft tissue release, and strengthening the muscles of the back while habitually trying to keep your posture more erect, will eventually level the playing field. In less severe cases, this issue can be resolved relatively quicker, but in more severe cases, it can take quite a bit longer to correct the dysfunction.
Slouching and head jutting forward is a posture that becomes comfortable to be in over time after repetitive load being put on the body in the same manner. Slowly, as you start to lengthen the muscles of the chest and strengthen the muscles of that back, over time, the correct upright posture will become more comfortable to be in and finally become habitual.
EXERCISES
There are a certain few things you can follow at home to keep things simple yet achieve some benefit and with time reduce the inappropriate load put on the body. Start these exercises right away and if done the right way, feel the difference in a next few days.
Lying down
Place a foam roll or a narrow pillow upright on the floor or mat and lie down on it with the head slightly in a tilted up position – as if to look backwards. Let your arms and legs relax by your sides without letting any muscles in your body tense up. Feel the stretch in the muscles of your back chest and shoulders. Breathe deep and learn to relax in this position daily for at least ten minutes twice a day. Slowly increase the time as days pass.
Standing
Stand upright. Stretch your hands behind your back and join both hands with finger interlocked. Start to slowly stretch both arms towards the floor and feel the stretch in the muscles of your chest and shoulders. Try to bring the shoulder blades (scapulae) as close to each other as possible.
Stand upright in a door way and stretch both arms downwards to the floor. Now place palms of both hands on the frame on the door on either side and start to slowly step forward. There will be a limited motion forward before your body starts to resist the motion. At this point, start to tilt the body forward as if you were falling in front of you. Be mindful that the tilting happens through the ankles and the body tilts forwards as a single unit. It is not just bend ahead through the waist. Be sure to look upwards as you perform this stretch. Hold position for at least ten to fifteen seconds and stand back up. Repeat this exercise at least five times at one time and perform as many times as possible throughout the day.
It is very important to remember that posture is the product of daily habits and repetitive actions and must be motioned closely.
