Something I think a lot of people miss about Maxwell is that like. He is physically disabled. He doesn't look like it, but a lot of stuff points to that being true:
The most obvious being that he's literally described as frail and has equivalent HP to Wes, a character specifically unsuited for survival.
"Maxwell's extended reign on the Nightmare Throne altered him in ways that are not yet fully understood. He continues to rely heavily on his tome, the Codex Umbra, as a result." He's literally using a tool to make up for lack of basic capability.
Almost all of the quotes where he complains about doing work are specifically about work that is very physically taxing, barring some more related to disgust. However, he responds positively to work that isn't as hard on the body:
Cookbook- "Very well. If I can master the dark arts, how hard could cooking be?"
Cartographer's Desk- "Mapmaking is a soothing pastime. Methodical."
It's pretty clear that under the airs he puts on about work meant for servants, it's just that he can't do physical tasks effectively. I'd assume it's a case of chronic pain and a general physical weakness. It may even be in combination with mental symptoms, which he also very obviously has but could have different origins.
It's fun to rag on him for his behavior, but it's important to actually put that into context. He is incapable, he is incompetent, and it's silly that he pretends he's not, but all of that also means that he is disabled and coping with it as best he can.

I wrote a somewhat popular DST fic on AO3 years ago and just got an email someone's writing an alternate ending to it. which is super cool but also i hate everything i make soon after making it and am deathly afraid to ever approach it again due to Mental Illness
but I had to read it, because I want to give the author support from the original author, y'know? i know how meaningful that is. so i reached out to my system for support and help coping. do you know how weird it feels to have someone whose identity originates in a character, reassuring you about reading a successor to a fic you wrote about that character painfully dying? it's pretty weird.

As someone who was underweight for a long time I like to imagine the survivors being very excited about visible weight gain. Whether it's muscle from all the hard work they're doing or fat because with cooperation they're finally able to have stable food stores...
Being able to gain weight in their situation is an achievement. I can just imagine one of the less bound by social norms of them just lifting their shirt one day and going, "guys, look! Stretch marks!"

Wilson was still struggling against the Nightmare Throne in the cinematic, so I doubt it. Charlie's play also hints that Maxwell specifically hid the throne from her, so she probably took the chance as soon as she could to get it:
DOLL: The mirror brought me back to life. I can feel what it feels... We are going to help each other become whole again. But for that, I will need your power. KING: My power? Never! Mmmmagic away! DOLL: No! NARRATOR: And with that, the King used his magic to hide himself and his throne far away, where he thought the little doll would never find him.
So, Wilson canonically spent time on the nightmare throne, he, Maxwell, and Charlie are the only people observed to have ruled on the throne
So it's entirely possible
That Wilson brought in survivors
Particularly, I think he brought in Wendy

I've thought about doing research into the state of psychology in DST's time and making a psychologist OC who annoys everyone with psychoanalysis and. I gotta say. A Jungian psychologist would have a field day with Maxwell.
Two major ideas in Jungian psychology are the persona, or how one sees and presents themselves, and the shadow, the suppressed parts of the self that conflict with the idea of who they think they are.
Combine that with a man that very obviously puts up a front of superiority to hide his lack of capability. Who makes literal shadow beings from pieces of his own mind that oppose his own behavior. A Jungian psychologist could write a paper using him alone.

I think about this constantly and it's important to note that the survivors are from a range of dates that, notably, crosses over a major historical event. Maxwell would be stunned there was a world war at all. Many of the other survivors would be horrified that there was a second one.
At the very least, we know what PTSD is now. In a reasonable scenario, they'd likely end up in mental institutions anyway at least for a time. At least 13 human beings appearing to suffer from a shared delusion with no identification to speak of would be concerning on its own, and that's not even considering the robot, spider-child, fishchild, and plant man.
DST Modern AU except the survivors are still from 1920s they just get moved 1 century ahead when they escape and have a huge gap of missing knowledge of what happened the past 100 years
There are of course consequences from social points and the mental toll of “I just spent 100 years in a place adjacent to hell”

Wickerbottom - Orchidsexual
Aspec label meaning one experiences sexual attraction, but does not wish to pursue it.
Woodie - Objectum
A general label for experiencing attraction to objects.
Wurt, Walter, Wendy, Webber - N/A
While kids can have orientations, I'm not comfortable assigning sexuality to them.
