While Steven Moffat came up with the idea for 'Into the Dalek', co-writer Phil Ford added the attack at the start.

He feared that one solitary Dalek made the episode too similar to 'Dalek', plus he wanted to make the premise "bigger".

Ford also revealed that, when he wrote the script, the Twelfth Doctor had not yet been cast; Moffat knew only the type of character he was hoping they would be.

Based on what he was told, Ford felt that the closest portrayal to the description was the Fourth, and penned a script based on that.

It’s not exactly cheerful and it’s full of things that are probably going to feel really fucking raw if you’re processing a fresh death.
