This is the report sheet I designed for my night shift med/surg position on a busy 48 bed neuro/pulmonary unit. I passed it along to my best friend (day shift) who utilizes it as well. She also gives a copy to all the people she orients. There are a lot of staff members utilizing a version of this sheet now.
Our nurse patient ratio is 1:5-6, so this allows me to see all my patients at a glance. I've been a nurse since July 2013 and have tried so many brains it's ridiculous. This is the one I constantly return to. I designed it a couple years ago on Excel. I use a separate paper for shift notes (meds, documentation, vital signs, assessment info, tasks, etc). Two (2) papers and I have all my info on 6 patients. I hated having 6-7 papers to give report from in the morning.
Translation time (some things are short hand in a way that makes sense to me. Let me know if you have questions on what anything means):
A/S/O/I: Age/Sex (ie 80F) inpatient or obs (written I or O)
Coverage: whose covering the patient. H-Smith translates to Hospitalist and name of attending, PA for private physician, Red/White for resident team, etc.

Not every breastfed baby latches and not every bottle has formula in it. I am a proud exclusive pumping Momma. My daughter has been feed exclusive breastmilk for 6.5 months by bottle only!
