Basically sakuya plays a sick kid that wishes to see life outside of the hospital and the spring fairies help him to fulfill that wish.

Along the way, sakuya finds that the fairies’ village is on the brink of destruction and uses his wish to save them instead of himself

Whether he lives or not is up to your imagination ;))
