SCIATICA is a medical condition in which pain going down the leg from the low back, Along the sciatic nerve pathway associated with weakness and numbness of the affected leg.
CAUSES:
· SPINAL DISC HERNIATION pressing on one of the lumbar or sacral nerve roots is the most frequent cause of sciatica, it occurs during heavy lifting.
· SPINAL STENOSIS is a condition in which the spinal canal, the space the spinal cord runs through narrow and compress the spinal cord, this narrowing because of bony spurs, spondylolisthesis, Inflammation or herniated disc.
· PERIFORMIS SYNDROME it contributes up to 8% of low back or buttock pain,the sciatic nerve runs through the periformis muscle, when the muscle shortens or spasms due to trauma or over use, this cause compression of sciatic nerve. PERIFORMIS SYNDROME has colloquially been referred to as “WALLET SCIATICA�� since wallet carried in a rear hip pocket compress the buttock muscle and sciatic Nerve, when it bearer sits down.

According to AMERICAN DIABETES ASSOCIATION it’s classified in to 3 Types.
1) TYPE 1 DIABETES MELLITUS

3) GESTATIONAL DIABETES
Pre-diabetic (border line diabetic) people are in hgh risk of heart disease,pre-diabetic people definitely land with diabetes if there is improper management.

PATHOLOGY:
Diabetes is a metabolic disease caused by the bodys inability to control chemical processes,The body either doesn’t produce enough insulin or the body does not use insulin properly .
Insulin is the hormone secreted by the islets cells of the pancreas that is necessary for the utilization of glucose.If the insulin is not secreted or used effectively by the cells ,glucose levels continue to rise in the blood stream thereby not fulfilling the body’s primary energy needs.
TYPE 1 DIABETES (INSULIN DEPENDENT DIABETES MELLITUS):
· Usual onset of type 1 diabetes is before the age 35 (generally during childhood)
· Destruction of the islets cells by the immune system may be caused by a viral infection with in the pancreas
· Type 1 is more serious than type 2 ,with sudden irreversible consequences.
TYPE 2 DIABETES (NON-INSULIN DEPENDENT DIABETES MELLITUS):
· In type 2 diabetes the insulin receptors on the cells become less sensitive to insulin ,this disruption of the body’s system of CHECKS and BALANCES in relation to glucose levels causes the rise of blood glucose level.
· Obesity is correlated to type 2 diabetes; lifestyle management during pre-diabetes can reverse the onset of diabetes.
GESTATIONAL DIABETES:
During pregnancy, some women are diagnosed with diabetes (gestational diabetes)these womens are at high risk of developing type 2 diabetes in future.
RISK FACTORS:
· Sedentary lifestyle
· Obesity or over weight(BMI >25)
· Over 45 years old
· Family history of type 2 diabetes
· High blood pressure (140/90mm of hg)
· Low HDL cholesterol levels(less than 35)
· High triglyceride level(250 or above)
· Cigarette smoking
SYMPTOMS OF DIABETES:
1) Polydipsia(increased thirst)
2) Polyphagia(increased appetitle)
3) polyuria (increased frequency of urination)

5)Fatigue (generalized weakness)

WHO SHOULD BE TESTED:
· Irrespective of gender,45 years or older and over weight
· Younger overweight people ,who have one or more risk factors
· If patients presents with symptoms of diabetes.
BLOOD GLUCOSE LEVELS:
· Fasting blood glucose test (blood glucose measured before eating breakfast)
· Normal blood glucose below 110mg/dl
· Pre – diabetic is 110 -125 mg/dl
· Diabetic is more than 126 mg/dl
· Oral glucose tolerance test (measures body reaction to sugar drink)
· Normal blood glucose is below 140 mg/dl
· Pre diabetic 140-199 mg/dl
· Diabetes is more than 200 mg/dl
PREVENTION OF DIABETES:
· Lose 5-10% of body weight.
· Exercise 30 min/day.
· Improve psychosocial health (stress/depression).
· Limit your carbohydrate rich foods.
· Increase your magnesium rich diet.
· Eat 5 serving of fruits/vegetables each day and at least 3 servings should be whole-grain.
· Get tested for diabetes periodically if your are at risk.
