Richtofen is transgender . he most likely performed surgery on himself , but we'll never know .

Pre - MPD Richtofen was healthier than post - MPD Richtofen physically speaking ( because mentally is already obvious ) . this is because post - MPD Richtofen very often neglects his body's needs for the sake of his work — the apothicons aren't used to the needy demands of the human body !

Richtofen is a cannibal . he has eaten parts of Ultimis crew , during the time period in which they were his test subjects , as a way to keep a part of them with him , but there are times he does it simply to satiate his hunger , though it is less zombie - like hunger and more apothicon - like nutrition ( i don't know about you , but their mouths look too perfectly designed to chomp a human's head right off ! )

Richtofen has definitely thought about cannibalising Yena .

The MPD affected Richtofen's body in multiple different ways , some obvious — at least , if you were to strip him down — such as the mark in the middle of his chest , while others less .

Not even Richtofen can tell where he ends and where the Shadowman begins .

The beast mark on his thigh is a self - inflicted scar .

His surgery scars represent an Apothicon's tentacles .

Richtofen is the Shadowman's favourite .

Going off of particular voicelines in which Richtofen is heard worshipping a being he calls " my Lord " or " dark one , " Richtofen treats Shadowman like a god and himself as a one - man cult . this does not mean Richtofen doesn't have his own selfish goals , simply that he wouldn't mind dominating the world with the one who revealed to him the path to godhood .

One of Richtofen's aliases for Shadowman is " my Muse . "

There were times in which Shadowman was in control of Richtofen's body . no one ever really noticed , except Schuster . it was difficult to hide his unnaturally purple eyes from the man who had a crush on the body he was possessing . thankfully , Schuster never connected the dots .

Richtofen's hatred for Samantha was fueled by the voices of corruption ( the apothicons ) , labelling her an obstacle . even without , though , Richtofen would still feel a pang of hatred for her , even if he'd consider himself silly for his reasoning .

Schuster is his favourite colleague . he has had feelings for the man in the past , and while reciprocated , neither of them ever acted upon them more than they could have during that time period . he still considers himself superior , though .

Richtofen hated Dempsey from the moment the American was handed to him , though he was amused by his mannerisms under the influence of 115 , and would often mock him just to see him angry , even if it meant getting a beating .

While Richtofen likes the heightened sense of control he has over Nikolai compared to his other subjects , a part of him can't help but feel frustrated at how easy it can be to trick him .

Before Dempsey , Richtofen was most intrigued by Takeo , even though he wasn't the biggest fan of the 115 - induced vomiting .

While he didn't care for Pablo's incessant ramblings about the past and future , it was funny to hear his rantings while he was elbows deep into his organs . it was a shame he didn't get to hear his voice slowly die down as he passed away during their failed procedure .

Richtofen had a good relationship with Sophia . being the only ones who knew of Maxis' deal with the Nazi party , they often helped one another if they ever thought the other was in trouble with the few officers that sometimes patrolled the facility . of course , they had to fend for themselves once given different tasks from Maxis and forced to distance themselves .

Richtofen thought highly of Maxis during their early days . seeing the man he almost idolized put aside his morals for the sake of funding , despite knowing that Richtofen too could be a target , broke his entire world view . he hasn't felt an ounce of respect towards Maxis ever since .

Richtofen suspected Yena since they first met , and seeing Maxis treat the traitor with more respect and care than he had ever shown Richtofen while completely dismissing his claims about Yena's true nature only had him want both of them dead even more .

Stuhlinger reminds him of Schuster , in a way . their similarities in appearances , his mannerisms , his blind loyalty . . .

Sometimes , Richtofen wonders what it would've been like to live the life his Primis counterpart had . the same can be said for Primis towards Ultimis .

Richtofen didn't hear the Shadowman's voice often once he was imprisoned . this only led him further down the path to insanity , despite already being its epitome .
