Small Bowel Transplant
A small bowel transplant, also known as an intestinal transplant, is a surgical procedure in which a
damaged or non-functional small intestine is replaced with a healthy donor small intestine. This
procedure is typically reserved for individuals who have severe intestinal failure or dysfunction that
cannot be managed effectively through other medical treatments.
The small intestine is responsible for absorbing nutrients and fluids from the food we eat, so when it
becomes damaged or fails to function properly, it can lead to malnutrition, dehydration, and other
serious health complications. Common reasons for needing a small bowel transplant include:
The small bowel transplant procedure involves removing the damaged intestine and replacing it with a
healthy small intestine from a deceased donor. The surgical team carefully connects the blood vessels
and intestine of the donor organ to those of the recipient. After the surgery, patients require intensive
medical management to prevent organ rejection and manage potential complications.
Short Bowel Syndrome: This occurs when a significant portion of the small intestine is missing or non
functional due to surgical resection, congenital anomalies, or diseases like Crohn's disease.
Intestinal Failure: Individuals with intestinal failure are unable to absorb enough nutrients and fluids
from their diet to sustain themselves, often requiring intravenous nutrition (parenteral nutrition) for
prolonged periods.
Small bowel transplants are complex procedures with significant risks, and the decision to undergo one
is typically made on a case-by-case basis after a thorough evaluation by a transplant team. The success
rates for small bowel transplants have improved over the years, but it's important to understand that
the procedure is not without challenges.
Dr. Karthik Mathivanan is a medical stalwart with more than 15 years of experience as a multi-organ
transplant surgery specialist specializing in liver, small bowel, kidney, and pancreatic transplantation and
rehabilitation.
Although it is possible for a living donor to donate an intestine segment, most intestine transplants
involve a whole organ from a deceased donor. Intestinal transplants is a very challenging procedure. It is
the least frequently performed vascularized intra abdominal organ transplants.

Liver cirrhosis is a chronic and progressive condition characterized by the scarring and damage to the
liver tissue. It is typically the result of long-term liver diseases, such as hepatitis B or C, alcoholic liver
disease, non-alcoholic fatty liver disease (NAFLD), or autoimmune hepatitis.
When the liver is injured, it tries to repair itself by forming scar tissue. Over time, this scarring can lead
to the development of cirrhosis. The scar tissue disrupts the normal structure of the liver and impairs its
ability to function properly. As cirrhosis progresses, it can lead to serious complications, including liver
failure, portal hypertension (high blood pressure in the liver), liver cancer, and even death.
The symptoms of liver cirrhosis may vary depending on the stage and severity of the condition. In the
early stages, there may be no noticeable symptoms. However, as the disease progresses, common
symptoms may include fatigue, weakness, loss of appetite, weight loss, jaundice (yellowing of the skin
and eyes), abdominal pain or discomfort, fluid retention (edema) in the legs and abdomen, easy bruising
and bleeding, and mental confusion or changes in cognition.
Diagnosis of liver cirrhosis involves a combination of medical history, physical examination, blood tests,
imaging studies (such as ultrasound, CT scan, or MRI), and sometimes a liver biopsy to confirm the
presence of scar tissue.
Although there is no cure for liver cirrhosis, treatment aims to slow down its progression, manage
complications, and improve the quality of life for affected individuals.
