A NEW FORMAT OF SURGERY, HIPEC, E HELPS FIGHT OVARIAN CANCER
for 51-year-old Bharati Gawli, an abdominal swelling in December 2017 She had been diagnosed with issues in her stomach due to borewell water and then fever "It had been a rough year, and the swelling followed by fatigue and irritation topped up a tough year for my mother," says her son Siddhant. Blood tests and CT scans led to the diagnosis of stage 3 Ovarian Cancer and it had luckily not spread to other parts of her body Post 2 chemo cycles, on March 13, last year. Bharati underwent HIPEC surgery for Ovarian Cancer. A year after, she is now able to do her daily chores and is finding her feet.
The story is not very different for Alok Kumar's sister. The 32-year-old Puneite found his sister grappling with swelling and sudden fatigue post her son's birthday on August 2. "Her stomach in eight days had swollen as it would be in the case of a woman in an advanced stage of pregnancy She couldn't eat. She was tired. When we saw a gynecologist in Raipur, he recommended that we get CA 125 test done and we immediately came to Pune. Many recommendations pointed us to Dr. Snita Sinukumar Cancer Specialist in Pune. Why Is Ovarian Cancer called silent cancer?
Ovarian Cancer does not present with any specific signs and symptoms. Most symptoms mimic common complaints like indigestion, bloating, urinary tract symptoms, thus leading to delay in diagnosis. Seventy percent of cases are diagnosed in the late stage three or four of the disease.
How common is Ovarian Cancer among women?
Ovarian Cancer is slowly emerging as one the most common cancers among Indian women with the highest incidence being noted in Pune and Delhi. Research states that the age-standardized incidence rates (ASR) for ovarian cancer varies from 0.9 to 8.4 per 100,000 person-years amongst various registries.
Can you throw some light on the newer treatment modality?
For Ovarian Cancer, treatment entails:
a combination of surgery and intravenous chemotherapy However, most times this is not enough and cancer comes back early. So to decrease the chances of cancer coming back early recently a new treatment modality has emerged, called HIPEC or Hyperthermic Intraperitoneal chemotherapy. It involves heating the chemotherapy and instilling directly into the peritoneal cavity at the time of surgery and the advantage of this is that the chemotherapy comes in direct contact with the cancer cells and is a one-time application at the time of surgery. However, it can only be administered after a good surgical removal of all diseases from the abdomen. This procedure is a highly specialized procedure called cytoreductive surgery and HIPEC and requires special surgical training.
The two parts to HIPEC are...
Part A: Surgical cytoreduction:
This requires removal of the tumor within the peritoneal cavity with no visible residual disease or residual disease less than or equal to 2.5mm. This is termed complete or near-complete cytoreduction. To achieve this state of complete cytoreduction, multiple extensive surgical resections might need to be performed. such as peritonectomy multiple bowel and visceral resections.
Part B: HIPEC: Only after achieving all complete or near-complete cytoreduction can HIPEC be administered. If gross residual disease is left behind, HIPEC is not justified. During HIPEC, a special machine, similar to a dialysis machine heats the peritoneal dialysate fluid and the chemotherapeutic agent to a temperature of 42-44 degrees which is then pumped into the abdominal cavity@of 1ltr/min. Around 4-6 liters of fluid is circulated within the abdominal cavity over 60 minutes, through intrabdominalnally placed drains.

21-year-old treated for rare desmoid tumour in the city
A 21-year-old was treated for a desmoid tumour at a city-based hospital. The patient noticed a swelling behind his collarbone and neck on the right side. The patient's family physician referred him to an orthopedic doctor and an MRI of the shoulder and neck was advised. The MRI revealed a tumour behind his collarbone in direct contact with the nerves supplying blood to his right arm, forearm, and hand.
However, after an Internet search, he reached Cancer Specialist in Pune Dr. Snita Sinukumar at Jehangir Hospital. Dr. Sinukumar said that desmoid tumours' are extremely rare tumours arising from the connective tissue. "It can occur in any part of the body but most commonly occurs in the arms, abdomen, and legs. Although these tumours do not spread, they are known to recur locally. Hence complete and meticulous surgical resection is the mainstay of treatment," said Dr. Sinukumar. The case was unique because of the challenging location of the tumour and the implications of preserving the function of the right hand.
Dr. Sinukumar said that tumor was biopsied and the diagnosis of a spindle cell malignancy was made.
"After a thorough explanation and perusal of all reports, the decision to operate and remove the tumour was made. The surgery was complex owing to the location of the tumour which was in direct contact with the nerves supplying the right hand called the brachial plexus. Any injury to the nerves would result in weakness of the hand and affect function. The tumour was also found to completely encase the vein supplying the arm," said Dr. Snita Sinukumar. The operation took four hours. The patient was discharged on the fourth day of the surgery.
