Does the FDA think these data justify the first full approval of a covid-19 vaccine?
Wait, What? How Does This Abysmal, Shambolic Data Justify the Approval of the Pfizer Vaccine? I reiterate our call: “slow down and get the science right"

The FDA should demand adequate, controlled studies with long term follow up, and make data publicly available, before granting full approval to covid-19 vaccines, says Peter Doshi
On 28 July 2021, Pfizer and BioNTech posted updated results for their ongoing phase 3 covid-19 vaccine trial. The preprint came almost a year to the day after the historical trial commenced, and nearly four months since the companies announced vaccine efficacy estimates “up to six months.”
But you won’t find 10 month follow-up data here. While the preprint is new, the results it contains aren’t particularly up to date. In fact, the paper is based on the same data cut-off date (13 March 2021) as the 1 April press release, and its topline efficacy result is identical: 91.3% (95% CI 89.0 to 93.2) vaccine efficacy against symptomatic covid-19 through “up to six months of follow-up.”
The 20 page preprint matters because it represents the most detailed public account of the pivotal trial data Pfizer submitted in pursuit of the world’s first “full approval” of a coronavirus vaccine from the Food and Drug Administration. It deserves careful scrutiny.
The elephant named “waning immunity”
Since late last year, we’ve heard that Pfizer and Moderna’s vaccines are “95% effective” with even greater efficacy against severe disease (“100% effective,” Moderna said).
Whatever one thinks about the “95% effective” claims (my thoughts are here), even the most enthusiastic commentators have acknowledged that measuring vaccine efficacy two months after dosing says little about just how long vaccine-induced immunity will last. “We’re going to be looking very intently at the durability of protection,” Pfizer senior vice president William Gruber, an author on the recent preprint, told the FDA’s advisory committee last December.
The concern, of course, was decreased efficacy over time. “Waning immunity” is a known problem for influenza vaccines, with some studies showing near zero effectiveness after just three months, meaning a vaccine taken early may ultimately provide no protection by the time “flu season” arrives some months later. If vaccine efficacy wanes over time, the crucial question becomes what level of effectiveness will the vaccine provide when a person is actually exposed to the virus? Unlike covid vaccines, influenza vaccine performance has always been judged over a full season, not a couple months.
And so the recent reports from Israel’s Ministry of Health caught my eye. In early July, they reported that efficacy against infection and symptomatic disease “fell to 64%.” By late July it had fallen to 39% where Delta is the dominant strain. This is very low. For context, the FDA’s expectation is of “at least 50%” efficacy for any approvable vaccine.
Now Israel, which almost exclusively used Pfizer vaccine, has begun administering a third “booster” dose to all adults over 40. And starting 20 September 2021, the US plans to follow suit for all “fully vaccinated” adults eight months past their second dose.
Delta may not be responsible
Enter Pfizer’s preprint. As an RCT reporting “up to six months of follow-up,” it is notable that evidence of waning immunity was already visible in the data by the 13 March 2021 data cut-off.
“From its peak post-dose 2,” the study authors write, “observed VE [vaccine efficacy] declined.” From 96% to 90% (from two months to <4 months), then to 84% (95% CI 75 to 90) “from four months to the data cut-off,” which, by my calculation (see footnote at the end of the piece), was about one month later.
But although this additional information was available to Pfizer in April, it was not published until the end of July.
And it’s hard to imagine how the Delta variant could play a real role here, for 77% of trial participants were from the United States, where Delta was not established until months after data cut-off.
Waning efficacy has the potential to be far more than a minor inconvenience; it can dramatically change the risk-benefit calculus. And whatever its cause—intrinsic properties of the vaccine, the circulation of new variants, some combination of the two, or something else—the bottom line is that vaccines need to be effective.
Until new clinical trials demonstrate that boosters increase efficacy above 50%, without increasing serious adverse events, it is unclear whether the 2-dose series would even meet the FDA’s approval standard at six or nine months.
The “six month” preprint based on the 7% of trial participants who remained blinded at six months
The final efficacy timepoint reported in Pfizer’s preprint is “from four months to the data cut-off.” The confidence interval here is wider than earlier time points because only half of trial participants (53%) made it to the four month mark, and mean follow-up is around 4.4 months (see footnote).
This all happened because starting last December, Pfizer allowed all trial participants to be formally unblinded, and placebo recipients to get vaccinated. By 13 March 2021 (data cut-off), 93% of trial participants (41,128 of 44,060; Fig 1) were unblinded, officially entering “open-label followup.” (Ditto for Moderna: by mid April, 98% of placebo recipients had been vaccinated.)
Despite the reference to “six month safety and efficacy” in the preprint’s title, the paper only reports on vaccine efficacy “up to six months,” but not from six months. This is not semantics, as it turns out only 7% of trial participants actually reached six months of blinded follow-up (“8% of BNT162b2 recipients and 6% of placebo recipients had ≥6 months follow-up post-dose 2.”) So despite this preprint appearing a year after the trial began, it provides no data on vaccine efficacy past six months, which is the period Israel says vaccine efficacy has dropped to 39%.
It is hard to imagine that the <10% of trial participants who remained blinded at six months (which presumably further dwindled after 13 March 2021) could constitute a reliable or valid sample to produce further findings. And the preprint does not report any demographic comparisons to justify future analyses.
Severe disease
With the US awash in news about rising cases of the Delta variant, including among the “fully vaccinated,” the vaccine’s efficacy profile is in question. But some medical commentators are delivering an upbeat message. Former FDA commissioner Scott Gottlieb, who is on Pfizer’s board, said: “Remember, the original premise behind these vaccines were [sic] that they would substantially reduce the risk of death and severe disease and hospitalization. And that was the data that came out of the initial clinical trials.”
Yet, the trials were not designed to study severe disease. In the data that supported Pfizer’s EUA, the company itself characterized the “severe covid-19” endpoint results as “preliminary evidence.” Hospital admission numbers were not reported, and zero covid-19 deaths occurred.
In the preprint, high efficacy against “severe covid-19” is reported based on all follow-up time (one event in the vaccinated group vs 30 in placebo), but the number of hospital admissions is not reported so we don’t know which, if any, of these patients were ill enough to require hospital treatment. (In Moderna’s trial, data last year showed that 21 of 30 “severe covid-19” cases were not admitted to hospital; Table S14).
And on preventing death from covid-19, there are too few data to draw conclusions—a total of three covid-19 related deaths (one on vaccine, two on placebo). There were 29 total deaths during blinded follow-up (15 in the vaccine arm; 14 in placebo).
The crucial question, however, is whether the waning efficacy seen in the primary endpoint data also applies to the vaccine’s efficacy against severe disease. Unfortunately, Pfizer’s new preprint does not report the results in a way that allows for evaluating this question.
Approval imminent without data transparency, or even an advisory committee meeting?
Last December, with limited data, the FDA granted Pfizer’s vaccine an EUA, enabling access to all Americans who wanted one. It sent a clear message that the FDA could both address the enormous demand for vaccines without compromising on the science. A “full approval” could remain a high bar.
But here we are, with FDA reportedly on the verge of granting a marketing license 13 months into the still ongoing, two year pivotal trial, with no reported data past 13 March 2021, unclear efficacy after six months due to unblinding, evidence of waning protection irrespective of the Delta variant, and limited reporting of safety data. (The preprint reports “decreased appetite, lethargy, asthenia, malaise, night sweats, and hyperhidrosis were new adverse events attributable to BNT162b2 not previously identified in earlier reports,” but provides no data tables showing the frequency of these, or other, adverse events.)
It’s not helping matters that FDA now says it won’t convene its advisory committee to discuss the data ahead of approving Pfizer’s vaccine. (Last August, to address vaccine hesitancy, the agency had “committed to use an advisory committee composed of independent experts to ensure deliberations about authorization or licensure are transparent for the public.”)
Prior to the preprint, my view, along with a group of around 30 clinicians, scientists, and patient advocates, was that there were simply too many open questions about all covid-19 vaccines to support approving any this year. The preprint has, unfortunately, addressed very few of those open questions, and has raised some new ones.
I reiterate our call: “slow down and get the science right—there is no legitimate reason to hurry to grant a license to a coronavirus vaccine.”
FDA should be demanding that the companies complete the two year follow-up, as originally planned (even without a placebo group, much can still be learned about safety). They should demand adequate, controlled studies using patient outcomes in the now substantial population of people who have recovered from covid. And regulators should bolster public trust by helping ensure that everyone can access the underlying data.

The shaman is drawn to the pain.
There are 5 Elemental locations around the globe (for the purposes of Project ALTER MAN 47) and Treasure Island in San Francisco California has marked the Point of Alchemical FIRE since 2007. Last Monday (9/2/19, the very day after Burning Man, “Metamorphoses” was the theme this year 😉 marked the Sun’s annual alignment with the direct center of Sidereal Leo (Fixed Fire) according to the ancient original zodiacal reckoning of the Sumero-Babylo-Egyptian sidereal zodiac, and AM47 was there.


Treasure Island is currently under total redevelopment and the entire western shoreline where I needed to setup my altar was/is razed to the dirt and fenced off with absolute zero access. But there was one small space, and one small space only at the NWesternmost tip of the island to setup the AM47 ALTAER, and it was right next to the “radiologically controlled area” containing the radioactive nuclear waste leftover from when the island was a US Navy base.


AM47 first manifested as a Vision Dream in early 2007, and were money no object, the first “revolutionary cycle” of AM47’s global alchemical voyage would/should have commenced in late 2008. Looking back, I just realized that that also coincided with the start of Solar Cycle 24 in late 2008 (2008-2019). The “Solar Cycles” are the sun’s 11 year magnetic-pole-flip cycle caused in large part by the cycles of Jupiter and Venus, and the scientists say Solar Cycle 25 begins… NOW (late 2019).
There have been many catastrophic events and circumstances over the past 11 years which made the AM47 voyage impossible. GEIL invests $350 million in new wells, field development
Green Energy International Ltd
Green Energy International Limited (GEIL), operator of the Otakikpo Marginal Field, in oil mining lease (OML) 11, Rivers State, said it has secured a $350million financing package for the second phase development of the field.
GEIL said the fund was secured from a consortium of international companies, including an oil services giant, London-based International Bank, prominent crude oil offtaker, and EPIC contractor.With the deal, the Otakikpo field, a crude processing and exporting hub in the Eastern Niger Delta, would unlock the potential of other commercially-stranded marginal fields in the area, GEIL Chairman, Prof. Anthony Adegbulugbe, said in a statement.
He said the second phase development involves the drilling of additional wells at the Otakikpo Field, which would significantly increase the field production from the present 6,000 barrels of oil equivalent per day (bopd) to 20,000 bopd, and the expansion of processing facilities from 12,000 to 50,000 bopd to adequately handle production from other fields.
According to him, the project also involves the construction of 1.3 million barrels onshore terminal, and a 20-Kilometer export pipeline and crude oil loading system for efficient export of the expected production.GEIL had earlier secured the approval to construct (APC) for the onshore terminal, and the associated export infrastructure. The onshore terminal will significantly reduce the field production OPEX, increase operational efficiency, improve security and also contribute tremendously to the economy of the local communities.
The onshore terminal is expected to be located in the Atlantic Industrial Park, Ikuru Town, being also facilitated by GEIL, as a gas-powered commercial and high-value proposition anchored on the power to be generated from the field’s gas production.
Adegbulugbe said the deal remains a major milestone in the oil and gas industry particularly for the marginal field coming on the heels of a successful commencement of production in 2017, and recent conclusion of the interpretation of the 3D seismic data acquired by GEIL to maximise the Otakikpo field opportunities.
On gas monetisation, Adegbulugbe was quoted: “We are poised more than ever before to develop the oil and gas field with the Small Scale Gas Utilisation project ( SSGUP), as the backbone of field development activities to ensure compliance with the zero gas flare policy the Federal Government.”
He commended the Department of Petroleum Resources (DPR), for its prompt support in the conceptual designs and implementation of the projects for the development of the field, adding that the initiative project has the potential to bring over $350million direct foreign investment into the economy and bolster government’s drive for economic growth and investment inflow into Nigeria.

#2 Coca-Cola: Coca-Cola Life
When Coca-Cola launched a new set of soft drinks known as Coca-Cola Life, a month long campaign followed. The drink is a health drink in the same genre as Diet Coke and Coke Zero. It was a healthier option compared to their sugary ones. The campaign was rolled out in a whopping 7 thousand locations worldwide. And that’s not all. The integrated marketing services utilised during the launch of this campaign saw billboard and bus hoardings blending with digital screen ads. These were further supported by digital, print, experiential and PoS activity. Television not being used was a surprising twist. Along with this, the company also launched a pop up store in the UK to give a people a chance to win a trip to NYC. Those who could not visit the pop-up shop could enter the competition by sharing a Coca-Cola Life pic online using hashtags like #CocaColaLife. The marketing mix was just right and the media choice at the forefront of the campaign was perfectly judged. Integrated marketing combines traditional and outbound marketing in new and exciting ways.
#3 Porsche: Popup Autocross Event Series
Porsche is one of the leading automotive brands in the world. Synonymous with luxury, class, elegance and speed, it is a premier brand in the automotive sector. Porsche is also a successful integrated marketer and the launch of its Popup Autocross Event series was an example of this.
Adverts on social media announced big events like a drive at Chicago’s Soldier Field. Additionally, there were 360 videos of the driving experience. It was a real integrated campaign with the videos adding an element of personalisation and giving it a voice that defined the luxury car brands intention to stand out in a market dominated by hoardings and clever taglines.
#4 Southwest Airlines: Transfarency
Southwest Airlines has always been an airline which has been seen as a budget brand offering low fares and unquestionable value for money. As the airline has grown, it is facing considerable competition from others in the industry, offering brand amenities, and luxury services. To re-establish itself as the leader in the airline industry, Southwest launched the integrated marketing campaign entitled Transfarency. The campaign used radio, print, television and digital assets to communicate value propositions of brand authenticity and value for cash.
The campaign also boasted a micro site, which exposed hidden airline fees charged by competitors such as American Airlines, Delta and Spirit Airlines. There was even an interactive component which had visitors take a quiz called fee or fake to highlight the massive charges by competitors, and of course, the montages of dancing Southwest Airlines pilots in the quiz only added to the fun!
The site also had a Dear Southwest Ad libbing feature, where users chose predetermined words in a letter to Southwest complaining about treatment in other airline brands. The fee hacker helped users to minimise fees while forced to fly on brands other than Southwest.
The buzz this out-of-the-box campaign created on Facebook and Twitter was massive. So was the impact on Southwest’s sales.
#5 Nike: R2SPECT
Taking on a rare sports player and legend like former Yankees player Derek Jeter on their integrated marketing campaign proved to be a major success spinner for sportswear brand Nike. So, what happens when sports legends are given a hashtag (#R2SPECT) to honour their efforts by a sportswear giant? Pure social media frenzy! The R2SPECT campaign commenced with a video showing New Yorkers tipping their hats to Jeter for the Yankees success. In the final season of his Major League baseball, the sports legend received tributes on TV, print, outdoor media and digital media too.
Consider that Nike’s campaign reached more than 44 million viewers online and trended everywhere from Instagram to Facebook and Twitter and you’ll understand the power of this integrated marketing campaign. The video also inspired hundreds of followers to put their videos of the player on social media platforms and create a viral brand story for Nike that was as relevant as it was compelling.
Thus, integrated marketing gained traction as against individually singled out traditional or digital media campaigns. Ansat Helicopters Approved to Carry Neonatal Medical Module

Kazan Helicopters has received permission from Rosaviatsiya (the Russian Federal Air Transport Agency) to equip Ansat helicopters with a special module for transporting neonatal patients. The equipment can be used to provide evacuation and airborne medical care to infants. The new module was developed together with the Ural Optical and Mechanical Plant (part of Shvabe holding company) specifically for neonatal air transport.

The manufacturer of the Ansat medical module, Kazan Aggregate Plant, developed a special platform for the incubator to replace the more traditional stretcher design. "The capability to install a neonatal medical module for Ansat had been in demand for a long time. Thanks to cooperation of Rostec holding companies and other partners, the helicopter now comes with upgraded equipment, allowing medical teams to continuously monitor the condition of a child, maintain the vital body functions and conduct intensive therapy during the flight," said Managing Director of Kazan Helicopters Yuri Pustovgarov. The incubator comes with a set of medical equipment, including an artificial lung ventilator, a monitoring unit, an aspirator, and an infusion pump. The neonatal module can be installed optionally on all Ansats equipped with base medical modules. No redesign of the helicopter interior is required. Stphane Gras has been appointed General Manager of the Four Seasons Hotel Megve and Les Chalets du Mont dArbois, Megve, a Four Seasons Hotel. Born in Saint-Etienne, France, Stphane began his career at the Waldorf Astoria in New York before joining Four Seasons Hotels and Resorts in 1999 as Assistant Room Service Manager at the Four Seasons Hotel New York.

Stphane now manages the destination's two hotels (Four Seasons Hotel Megve and Les Chalets du Mont d'Arbois, Megve, a Four Seasons Hotel) as well as the Golf du Mont d'Arbois and the mountain restaurants in the Edmond de Rothschild Heritage Group (Le Club du Mont d'Arbois, La Taverne du Mont d'Arbois, La Pizzeria de la Taverne, L'Idal 1850, L'Auberge de la Cte 2000). Stphane has a Bachelor's degree in International Hotel Management from the Ecole Htelire de Lausanne, Switzerland. Outside of work, Stphane and his wife are interested in art and regularly visit his two daughters in the United States. He also plays golf and enjoys downhill skiing - two disciplines that will enable him to make the most of Megve in winter as well as summer. Marriott Opens First Aloft Hotel in Bali, Indonesia
Marriott has opened the first Aloft hotel in Bali, Indonesia. Located within walking distance of the beach, the Aloft Bali Seminyak features 80 guest rooms, of which eight have direct access to a lap pool. Each guest room features free Wi-Fi, 55-inch smart TV, Bluetooth speakers, and a mini bar, while bathrooms have a walk-in rainfall shower.

A variety of dining and social spaces are available to guests, including its main attraction: The Kahuna rooftop restaurant, which serves up a fusion of eclectic fare with a playful twist on international and local cuisine complemented by mesmerizing sea views as a backdrop. The hotel also features a W XYZ bar, 24/7 fitness centre, a rooftop infinity pool that features live music and DJ’s spinning until late, as well as two multi-functional meeting spaces which can be transformed into an intimate event venue accommodating up to 66 people. “We are thrilled to open the first Aloft hotel in Bali and to welcome the next generation of travellers to Indonesia’s most famous island destination,” said Kristanti Tannady, General Manager, Aloft Bali Seminyak. “Aloft Bali Seminyak offers a fresh hospitality experience, which brings a contrast between contemporary and traditional designs as well as a blend of both lively and serene lifestyles for guests. Accor has expanded its portfolio of Mövenpick hotels in Vietnam with the opening of the Mövenpick Resort Waverly Phu Quoc on Ong Lang Beach. The hotel features 305 guestrooms, while the adjacent Mövenpick Residences Phu Quoc offers 329 apartment-style residences and 79 pool villas.

The resort features a variety of F&B outlets, a beach club, six swimming pools - including two kids’ pools, as well as an Elements Fitness Center, Spa & Salon. For events, the 720 square metre Coral Ballroom features a column-free space with 6.8-meter-high ceilings and a large 324 square metre foyer that can host up to 860 guests. Aman to Launch New Hotel Brand

Aman, one of the most exclusive and luxurious brands in the hotel industry, has unveiled plans to launch Janu. The Janu brand will launch in 2022 with three hotels which are currently under construction: Montenegro, Al Ula in Saudi Arabia and Tokyo. While some purists will shudder in horror at the thought of such a highly respected hotel brand diversifying its portfolio in this way, others will suggest that the two may complement one another. So how will they differ? Here is how Aman compares the two brands, “While Aman is synonymous with its Sanskrit translation, ‘peace’, Janu stands for ‘soul’; Aman is a sanctuary, whereas Janu is connectedness; Aman offers refined respite, whereas Janu offers an energetic vibe seeking greater purpose with inner contentment. British Airways Launches ‘Book with Confidence’ Policy

With so much uncertainty surrounding the COVID19 outbreak, booking a trip away, whether it be for business or leisure, is a much more stressful decision that it normally would be. To allay at least some of the concerns that travellers may have, British Airways has launched a ‘book with confidence’ policy that gives customers the flexibility to delay travel that is booked during the next two weeks without any charge.

The ‘book with confidence’ policy covers all British Airways routes with changes available right up to departure. The policy is available for new bookings made between 3 and 16 March 2020. Andrew Brem, British Airways’ Chief Commercial Officer said, “As some customers may choose to change or delay their travel plans at the moment, it’s important we offer them greater flexibility. By waiving our change fee our customers can have the added confidence to book a trip with British Airways that suits them.” It should be noted however, that any fare difference due to a date change will still apply. Aerobility to Expand Fleet with Former MOD Vigilant T1 Gliders

The MOD’s Defence Equipment and Support (DE&S) organisation has sold 63 decommissioned Vigilant T1 aircraft to Aerobility, an UK-based charity which provides flying experiences to those with disabilities and injured ex-military personnel. The first batch of 10 motor gliders will be modified and refurbished by German company GROB Aircraft SE – the original manufacturer and Design Authority – to meet civil certification standards. The remaining aircraft will be engineered and recertified in the UK, where the charity has identified a qualified engineering partner in Southern Sailplanes, based in West Berkshire. Hampshire-based Aerobility will initially more than double its fleet with eight Vigilants thanks to a grant from the Department for Transport (DfT); meaning they will be able to help about 2,600 people into the air every year compared to 1,000 currently.

Clive Walker, head of the Defence Equipment Sales Authority (DESA) in DE&S, said, “I am very pleased to see the RAF Vigilant T1 gliders move on to a new lease of life, in particular supporting the excellent work of Aerobility in creating flying experiences and opportunities for those who might otherwise never get the chance to fly.” Glider modifications will include new engines, propellers and refurbished cockpits, and the charity hopes the first glider will be ready to fly in the summer of 2021. Aircraft that aren’t used for flights will be refurbished and sold to generate revenue for the charity, pay for the ongoing costs of their operational fleet and help them branch out into other parts of the British Isles. Aerobility’s CEO, Mike Miller-Smith MBE, said, “Acquiring these aircraft will help us transform the lives of an even greater number of disabled people by giving them the unrivalled sense of freedom through the magic of flight. We are extremely grateful to the Ministry of Defence, Department for Transport and our various partners for supporting us in this ambitious project. Not only will it help us build capacity for our future operations, it will enable us to expand the charity to support more disabled people and to do so at additional locations around the UK.” The sale will create four full-time engineering jobs, one project management role and one administration position at the charity. They will also need a full-time co-ordinator and a part-time flying instructor, with all positions being filled by candidates with a disability where possible. Bombardier Offers 4G In-Cabin Wi-Fi as Retrofit on Learjet 40, 45, 70 and 75 Aircraft

Bombardier is now offering customers Gogo Business Aviation’s next-generation, 4G air-to-ground (ATG) internet system, Gogo AVANCE L5, as a retrofit for in-service Learjet 40, Learjet 45, Learjet 70 and Learjet 75 aircraft. Customers can have the system installed at all Bombardier Service Centres and Authorized Service Facilities throughout the United States. The high-performance 4G in-cabin Wi-Fi system has been an available option on new Learjet aircraft since last year.

The new and significantly faster 4G service uses Gogo AVANCE L5, which is lightweight and compact, accesses the Gogo Biz 4G ground network of more than 250 towers and delivers seamless connectivity over the continental U.S. and large parts of Canada and Alaska. Designed specifically for business aircraft, the system offers audio and video streaming as well as faster web surfing for a vastly improved user experience over previous-generation technology. “Learjet aircraft are renowned productivity tools, and this enhancement ensures that our customers have the connectivity they need to maximize their time in the air,” said Michael Anckner, Vice President, Worldwide Sales, Learjet Aircraft and Corporate Fleets, Bombardier Business Aircraft. “With various upgrades available for our large fleet of in-service aircraft and the recent unveiling of our value-added Learjet 75 Liberty aircraft, there’s never been a better time to own a Learjet.” In addition to providing this 4G connectivity upgrade to the Learjet fleet, Bombardier is also offering the latest update to the popular Garmin G5000 avionics suite aboard Learjet aircraft, which was recently certified by the U.S. Federal Aviation Administration and brings many workload-reducing improvements to the cockpit.
