There had been a few instances when Doflamingo considering abandoning his brother whenever they made quick runs into towns alleyways to scrum up whatever food they can come back with. These urges only got more intense after their mother passed and he was left with two of the weakest family members he's ever known.

If he had done so he would've justified it as making sure there was less mouths to feed. Having a little brother that did little more than make too much racket with his crying was quite burdensome on the older. But even still, perhaps he held off on doing so because it would've broken his mother's heart or even send her to her grave early had her baby boy gone missing. Him dissapearing on his own some time later however, was none of his concern and by far something he no longer remotely cared about.
