PR Internship Post #11
This past week was the first week where I literally didn’t receive any work from my internship. Things have been petering off slowing for awhile now, but this total lack of anything was a first. I found this especially unfortunate because I’ve reached the point where the usual ways that I’m entertaining myself during this quarantine are starting to grow stale. Some complex law-focused assignments from my internship would’ve been a lovely way to kill a few hours, but unfortunately, that wasn’t the case this week.
The main thing I’m worried about right now is still the summer internship search. Essentially every place that I applied to earlier in this spring is unresponsive, or has already outright cancelled their internship programs. I’m definitely not surprised by this, but it still feels pretty rough. I applied to a good amount of summer opportunities at companies of varying size, scope, and focus (my estimate would be 20-30 applications total), so this lack of communication from most places is tough. I believe I might’ve mentioned this already, but I was extremely hopeful when I started getting video interviews for major stuff in early March. It felt like my hard work was finally about to pay off with a big internship that might even eventually turn into a job. Now, my search has been reduced to long shot Instagram DMs, impassioned LinkedIn pitches, and praying to the gods of Los Angeles-based employment. At least I still have this stuff to fill some time. There are a few connections I’ve reached out to today that I hope will be fruitful, but as of right now, I would say my optimism from early this semester is long gone. I will keep working at the search though, and persist in the hopes of locking down something of value for this summer. One of the cool parts of remote work being essentially a requirement right now is that it can open some doors in terms of location. I recently found a great company in Nashville that aligns perfectly with my interests that I’ll be reaching out to later today to see if they have any remote work opportunities. It’s unlikely this company would’ve made as strong an impression on me if I was limited to LA opportunities.
Overall, I am still trying to make the best of the current situation. I am thankful to be safe and healthy. Things could be much worse, to say the least. On another note, I just really hope that once this is all over, America can head in a more positive direction. If there’s one thing that the coronavirus has been good for, it would be revealing the many glaring flaws in the current state of America’s bastardization of capitalism. This isn’t to say that these flaws weren’t obvious before, but this epidemic has, in my opinion, made these often heinous flaws more visible than ever. Unfortunately, my gut is telling me nothing will change. If Donald Trump manages to luck into a second term, things are practically guaranteed to get much worse. We’ll see. It’ll be tough to tell until maybe 10-20 years down the line.
This was definitely my most negative update yet. I promise I’m still a happy person! I just want to be realistic about the way I feel right now, and what I think is coming next.

Hospital at Home-An Idea Whose time has Come
Long before fears of infection from the coronavirus made people wary, hospitals were often not the best place to be treated and recover. Awareness of the risks of infections and the debilitating impacts of hospital stays, particularly on patients who are frail, have cognitive impairments, or other vulnerabilities, prompted Johns Hopkins University, to establish the first hospital-at-home program in the United States 20 years ago.
The model, which offers people who would typically require hospitalization to treat problems such as acute pneumonia, dehydration, or exacerbations of conditions like heart failure or chronic obstructive pulmonary disease (COPD) the option of receiving acute-level care in their homes, has been one of the most studied health care delivery reforms.
In recent years, a handful of U.S. health systems have launched similar programs, either because their hospitals are operating at capacity and/or they’ve taken on financial risk for the total costs of patients’ care and thus benefit from delivering services in a lower-cost setting. The Veterans Health Administration, which controls both payment and care delivery, has funded its own hospital at home program as have some health systems that have their own health plans and/or substantial numbers of patients in accountable care organizations or managed care plans.  
Several startup companies now offer logistical support and technology to facilitate hospital-at-home programs, as well as help contracting with commercial payers. Broader spread, however, has been stymied by the fact that Medicare does not pay for acute services provided outside of hospitals for fee-for-service beneficiaries.
As in so many parts of health care, the COVID-19 pandemic has catalyzed change: not only has it created a clear rationale for avoiding hospitals when possible, it has prompted Medicare to allow providers to bill for acute treatments delivered in surgery centers, hotels, or other non-hospital facilities. Some are hoping waivers tied to the Hospital Without Walls initiative will open the door for Medicare to pay for acute care in homes, too.
Several large hospital systems have begun their own hospital at home programs:
Mount Sinai Health System (NY)- Mount Sinai Health System launched a hospital-at-home demonstration in 2014, funded by a $9.6 million grant from the Center for Medicare and Medicaid Innovation. Leaders were drawn to the model as a way to relieve pressure on their inpatient wards, which even before the pandemic were routinely filled beyond capacity.
After the three year Medicare funded project ended Mount Sinai in 2017 set up a joint venture with Contessa Health, a Nashville-based startup launched in 2015 whose founders sought to make home hospitalizations financially sustainable by contracting with health plans willing to pay for them, namely Medicare Advantage plans, Medicaid managed care plans, and commercial health plans.
Tufts Medical Center (MA)- Like Mount Sinai, Tufts Medical Center in Boston was bursting at the seams, pre-COVID-19; it routinely had to turn down requests to accept patients from other hospitals because no beds were free. To help expand access, leaders had begun partnership discussions with Medically Home, another hospital-at-home company founded in 2017 after eight years of research and development. 
Medically Home partners with its clients’ clinicians and also hires physicians and nurse practitioners, who work from a central hub from which they use remote monitoring, video, and other telehealth tools to oversee patients’ care. They confer with Tufts Medical Center’s attending physicians to make sure patients are good candidates for the program. Advanced practice providers, paramedics, nurses, phlebotomists, technicians, therapists, and other staff visit acutely ill patients in their homes, providing infusion, lab tests, imaging, nutrition, translation, and other services. 
Medically Home draws on its health system partners and community resources, including local paramedics and equipment suppliers, to build a virtual hospital. 
Home-based models that offer coordinated episodes of care — including for emergent, acute, and post-acute needs — could be of particular benefit to patients who have frequent emergency department visits or hospitalizations.
CMS’s Hospital Without Walls initiative could provide a means by which hospitals can provide some care in homes, but it’s unclear how long the waiver will remain in place. To qualify as acute care, hospital providers need to deliver home services themselves and must deliver a certain level of services (e.g., oxygen, durable medical equipment, radiology, and meals).
In the short term, spread of the model may depend on the course of the coronavirus and hospitals’ desires to prepare for potential surges. In the longer term, it may find broader interest if value-based payments gain broader traction.

Will Coronavirus Close The Open Concept Kitchen?

For the past few decades, the sledgehammer has begun most remodeling projects. Countless design shows depict, early in the program, gleeful homeowners and builders knocking down walls as they transform yet another traditional home design into the open concept floor plan we are all said to favor. 
An open concept layout combines living, family and dining rooms with the kitchen, and the resulting space is often called a “great room.” We are said to want this configuration because it brings us together; the days of kitchens as separate work spaces are over, and now we cook, entertain, work, dine, do homework, watch TV and relax in the same space
The past year of Covid-enforced togetherness may have us looking at the open concept layout of homes in a new light.
Nicholas Potts, an architect working in Washington, DC, says he is rebuilding the kitchen wall removed years ago.
“When you have an open concept, you have to clean up all the time,” he says. “You don’t want to leave the dirty dishes because then you have to look at them. Nothing is more luxurious than not seeing everything.”

“As people continue to work from home and focus on their home as a safe sanctuary, there will be a renewed focus on rooms to serve a specific purpose,” says Keren Richter, co-founder of White Arrow, a New York-based design studio.
“I don’t think we’ll necessarily head back to the land of galley kitchens. Still, I believe lofted interiors, where all entertaining and eating happens in one room, will be less desirable. Open-plan spaces are problematic for noise mitigation — the more things happen remotely, the more we want to close doors and have a sense of privacy.”
Janice Costa, president and founder of KB Designers Network, agrees. 
“Layout wise, consumers are much more cognizant of the need for privacy within shared spaces. That means those open-floorplan kitchen spaces, while still popular, are seeing some refining, with niches added in to provide a bit of separation, allowing multiple family members to use the space at the same time without intruding on each other.”
Some designers don’t see us putting the walls back just yet.
“The open concept is not going away because our lifestyles have changed,” says Richard Anuskiewicz of Nashville, a member of the Consentino Design Alliance. “However, as kitchens are more exposed to other rooms, it spurs secondary spaces like pantries.”
Young Huh of New York points out that, when kitchens are open to the rest of the house, “People feel that they should eat all the time. When the kitchen was a closed room, it was customary to eat three meals a day, with maybe one snack.
“Where open kitchen plans work really well,” adds the interior designer, “Is in vacation homes. Life is more communal, more relaxed, and being together in one space does not interfere with someone’s ability to work.”

Nicholas Potts says here is another important reason for the wall.
“Walls are wonderful for storage. When you take down that wall to open things up, you’re going to have to find places to store whatever was in the shelves or cabinets that were built against that wall. 
“Now that we all see each other 24-7, we want less togetherness. We need walls, and to be able to get some privacy from each other.” 
The kitchen may be more than a functional workspace, but we still need privacy and quiet. And even live-in kitchens need lots of storage.

Once upon a time a 17 year old English girl fell in love with a talented but relatively unknown country singer by the name of Taylor Swift. PLOT TWIST: 11 years later she is one of the most successful artists in history and I just spent my Friday night hanging out with her at her house. Wanna know how this happened? I'm going to tell you my story. Get yourself comfy (maybe some cocoa or a nice sweater) and listen up.  I don't claim to be the biggest Taylor Swift fan in the world, but like every Swiftie I would always like to think I am. I have loved Taylor for longer than I can remember and one of my biggest dreams in life has always been to meet her.  On 3rd October 2017 I woke up around 5am as I do every morning, and I turned off my alarm and turned my internet on. I had a few notifications, Facebook messenger, twitter, tumblr....but then there was a DM? I never get twitter DM's, I'm not remotely interesting enough for anyone to want to speak to me, so who was this messaging me? Half asleep and with my eyes still adjusting to the light I opened the DM screen and saw the words 'Taylor Nation'. My whole body froze...that's not what is says is it? I rubbed my eyes and looked again...it was. I clicked on the message and read it quickly. I didn't really take it in, something about a Confidential Event. I dropped my phone onto my chest and looked at the ceiling trying to take in what I had just read. Was I having another Taylor dream ? Somehow I knew I wasn't - the last Taylor dream I had we were eating banana splits in an old railway carriage turned into a diner...i don't even like bananas. Somehow this seemed more plausible.  I closed twitter and decided to ignore it for a second and look at my other notifications. There was a facebook message from my Swiftie friend Bethy telling me to have a good day and reminding me to keep my DMs on....wait, she had one to? I replied back saying 'I have, you too?' and for a solid hour we were messaging eachother in code without either of us admitting what we had received in text...but we both knew.  I sent my details over to Taylor Nation and tried to get ready for work...putting my jeans on back to front, loading the laundry into the machine without adding the washing liquid, walking around my house doing things which usually came naturally but for some reason took extra effort and concentration...something I was lacking.  It was another 10 hours before my DM was replied to saying they would contact me soon. Soon? How soon is soon? Bethy and I continued talking to eachother imagining what it could be. Would we be in a live stream? A music video? Are we being invited to a show? An interview? In the back of our minds we wanted it to be a secret session but it wouldn't be would it? People like us with a handful of followers whose idea of a great Friday night is to play Yahtzee while listening to Taylor Swift on repeat wouldn't get invited, would we?  Luckily soon was only 3 hours and I got a call from a New York number. I was at my parents house so I ran up the stairs to take the call. Some other Swiftie friends in a group chat were messaging me about stuff and I was trying to pick up but notifications were sending my phone into meltdown. Eventually I managed to pick up...then hang up on Taylor Nation. Yes, I hung up on Taylor Swift's management group!!! (Sorry Ali).  I tried to call back but it went to voice mail, so I sent them a DM and went for a wee. Ali called me again while I was on the toilet so I was nervously trying to finish peeing and then ran out to the top of the stairs to take the call. The signal in my parents house was awful and I kept shouting 'huh' and 'hello?' down the phone but Ali was so lovely. She told me I had been picked to be invited to a special event because I was a super fan and I could bring a +1. I knew Bethy was already going and I made a pact with Megan if ever we had an opportunity to meet Taylor we would take eachother. I hastily gave her Megan's details and then passed Alex's on to Bethy for her call before screaming at Alex to keep 13th October clear (for the record he is so difficult). On top of this I was also on holiday that day and so going to this event also meant cutting my holiday short...but this was Taylor Swift!  I found out on Sunday 8th October that we had to meet at a hotel and would be shuttled to the location via bus. Unfortunately my bladder condition flared up on 10th October and I nervously contacted Taylor Nation for details of restroom facilities, scared I wouldn't be able to go. I cried as I waited for an email back but Taylor Nation were so lovely....you could tell they worked for Taylor. I went to the local Dr on holiday and got antibiotics and instructions for bedrest for the 2 days before the event.  My bladder was still bad by early morning day of the event but luckily started clearing up as I travelled from Devon to London.  The day of the event we all met up at Waterloo Station in London before travelling to the hotel where we needed to meet Taylor Nation. I was so nervous that everything was going wrong and had been in so much pain and in denial I was going to be well enough to go that I couldn't believe it was happening.  On the way from the tube to the hotel we saw a funeral directors with 4 gravestones in the window - were they for us when Taylor killed us with her music? A bus went past...the number 13? So typically Taylor!  We arrived at the hotel and checked in. Taylor Nation were in the lobby (we didn't know it was them). The receptionist asked if we were going to the knitting convention? I was confused...was this a cover story? All I kept thinking was 'I knit sweaters yo'....'no, we are going to meet some friends' we told them.  We went upstairs to our room to get ready and when came down we didn't know where to go. There seemed to be some type of line forming around the building and the sheer quantity of red lipstick and floaty dresses told me we were in the right place.  We checked in with Taylor Nation and as I gave Elise my ID she said she liked my passport cover. It's a picture of Taylor with 'grab your passport and my hand' written on it. She confirmed with me that she was the one I had been emailing and I thanked her for her help. I then signed a NDA and got my really cool wristband which has 'United Kingdom' in reputation font on it. And then we headed downstairs to wait for the bus.  I was on bus number 3/4 with Megan- Alex and Bethy had already gone on bus 2. I sat right behind the driver and he had his Satnav programmed with 3 different routes. He took the first route to somewhere random, loaded the second route and followed it, then loaded the third. It was clear he was trying to throw us off. As we entered part of the neighbourhood I felt very out of place. The cars had personalised plates, the houses were getting bigger, the hustle and bustle of London life seemed to getting further in the distance.  Suddenly the bus stopped in front of a beautiful house...this was it...this was Taylor's house? We were escorted off the bus and down to the side door which lead to a basement. The carpets were bouncy and the soundproof walls were so soft. We waited patiently for a few minutes and then suddenly the door opened and I caught a glimpse of a canvas of the New York skyline....this IS Taylor's house.  We were taken up the stairs to a central entrance hall with a staircase leading up to the top of the house and a corridor towards a large group of people with music playing - her Spotify playlist. The party was in the kitchen. Megan and I walked in to the kitchen, everything was so perfect. Ice buckets full of cans of soda and water, the best chicken bites I have ever tasted, cheese, vegetables and dip, reputation m&ms and cookies with REP on. There was so much I couldn't even see it all. We spotted our friend Bethy over in the corner by the French doors and headed over to speak to her. As we got there so did Scott Swift....the total legend and biggest fangirl ever. I told him I had briefly seen him in Nashville for 1989 and be told me the Mick Jagger story...the same one Taylor tells in interviews. Word for word and he beamed with pride as he told us stories about Taylor. There I was on a Friday night in Taylor Swift's house talking to her dad about 2 megastars as if we were all old friends. And he was really interested in us, not just polite conversation but so enthralled that we loved his daughter as much as he did. True to Scott Swift fashion he gave us some guitar picks as me made his way onto the next group.  I asked Alex where the toilet was and he took me out to the corridor...which hid a restroom behind huge grey panels. It was the nicest smelling toilet I have ever been in...and I have been in a lot! As I sat there peeing in Taylor's toilet and looking at the Jo Malone candle burning by the sink I wondered...how did I get here? Shortly after I left the toilet we were ushered across the entrance hall to a huge rectangular room. Opposite the door were large windows covered over by drapes and a single armchair - Taylor's seat. The floor was adorned with cushion after cushion and Megan and I ran to the closest cushion to Taylor. Megan sat right in front of her and I squeezed in behind. This was a SECRET SESSION!!!! A few minutes later I looked behind as the door opened and in walked a real angel to a round of screams and claps- red lips, natural 'I've just washed it' curly hair, snake boots and a huge smile. She sat down in front of us on the seat and said 'Hi I'm Taylor'. Just like at tour, only this time not to 70,000...to just 100.  And then the mystery was revealed. Taylor Nation hadn't just randomly picked us off of various social media sites - we had been hand selected by Taylor herself who had stalked us for over a year and sent TN our profiles to invite us. It hit me...Taylor picked me?!?! The woman I have been 'stalking' for 11 years had been stalking me too? Out of all the fans in Europe I was one she wanted to meet. I don't think that will ever fully sink in.  Taylor played us her album, telling us little stories about each song - the inspiration, the recording processes, the reactions from friends and family. It was so much to take in but watching Taylor mouth along to the words, act out different expressions and sitting-dance to all of the songs was hilarious - I wasn't in the presence of a celebrity, I was sitting with my best friend appreciating the biggest achievement she will ever have - true happiness. Taylor Swift is truly happy, about life, about her music, about her fans and it is nothing short of magical.  As each song played I really wondered how Taylor could top it, but she did...over and over again. Time went so quickly and it felt like the world's biggest and best slumber party, except we didn't get to sleep over.  At one point it was so hot that Scott Swift had to open all the doors to let the air in. As Taylor played his favourite song from the album she called for him to come and listen and he made a joke that he was actually handing out guitar picks to the neighbours.  After Taylor finished playing the album I looked around. Everyone was so Wonderstruck by what they had heard and I looked at Taylor and she looked so content. I don't know whether she was worried by what our reactions would be but all we had for this album was love...and I hope Taylor knows that.  After the listening session we were handed copies of the reputation magazines while we were waiting for photos. Megan and I were some of the first to go in. I was so focused on getting to the event that I hadn't even thought about what to say. Taylor ran over to Megan and hugged her and started talking about her bright pink hair. She loved it and it had been distracting her all evening. Then Taylor hugged me and I hugged her back like I had never hugged anyone before. She was so tall like a giraffe but with the grace of a swan...and I was me. I whispered 'thank you for inviting me' and she smiled. I didn't want to let go. As so many people were waiting they sort of rushed us with a picture. Taylor grabbed hold of us with her arms around our shoulders and smiled so sincerely at the camera. I wanted to talk to Taylor as I didn't get a chance but we were ushered off. As Megan walked out front if me I went to turn and talk to Taylor when Megan suddenly shouted 'we went to Nashville'. Taylor screamed back excitedly 'I know' with the biggest smile on her face. So...I didn't get to talk to Taylor but she knew we went to Nashville. She really did stalk me. When we left the room we were greeted by Mama Swift. I told her about my mum being terminally ill with Cancer and how she had gotten test results that day but she wanted me to be with Taylor instead. She gave me an extra hug and told me to hug my mum for her. I told my mum later and she started crying. Someone she has never met wished her well...it isn't just Taylor, it is her whole family.  We were taken back to the basement to collect Merch bags with a T-shirt, hat, sticker, pop socket and exclusive secret sessions keyring. It was so lovely of Taylor to give us something more to remember the day.  I got the bus back to the hotel and sat talking to Ali the whole way about the album and Nashville....I may have also invited her to CMA fest next year. By the time I got back to the hotel and collected my belongings (which had been taken from us earlier) I was shattered but I couldn't sleep. I posted on twitter and suddenly my phone couldn't stop vibrating...thousands of retweets and likes. I sat there trying to figure out what happened but I'm still not even sure I believe it myself. It took me 2 whole days to cry...not because I didn't care but because this wasn't the norm. I had stood outside countless radio stations and events before and not met Taylor...I had cried. I had come away from concerts and not been picked for Loft or Club Red etc and cried...but this was different. Not only had I met Taylor, she had picked me. I couldn't cry...I just felt a sense of fulfillment, of achievement, of love. By the time I did cry if wasn't because of what happened; it was because I missed her.  But this is not the end of my story with Taylor...I feel like this is the start of a new chapter. I came away with a whole mind of memories and countless new friends - affectionately known as FANtom Squad. Friday 13 October was the best night of my life; I will never be able to thank Taylor enough and I'll probably never have a chance to say what I wanted to say. But after years of hiding it & toning down this side of my life to please people I can honestly I am not ashamed of who I am because who I am is exactly how @taylorswift likes me.

Telemedicine appointments and drop-shipped home sleep tests surge during the pandemic. Some predict these changes may be incorporated into a new normal.

In the days before the novel coronavirus swept across the world, the medical community had already started to look to telemedicine as a viable way to reach patients. When the pandemic hit, many institutions throughout the United States were forced to urgently switch gears and adapt quickly to the new normal.
Sleep medicine is no exception. In March, many clinical sleep labs and private practices closed their doors to patients as public health officials called for social distancing to slow the spread of the virus. The regular rhythms of how sleep medicine providers conduct business and care for their patients changed in what felt like a blink of an eye. This new reality pushed telemedicine and other no-contact solutions, such as mail-order home sleep test delivery, to the forefront of sleep specialists’ minds.
A Sleep Review survey showed a large percentage of providers exploring new remote options or scaling up existing programs. Just under half of respondents, 46.6%, switched patients from in-person visits to telehealth visits for follow up appointments while 29.5% began offering virtual visits to new patients during the pandemic. About 44% changed patients from in-lab sleep studies to home studies and 20% reported ordering more mail-order home sleep tests.1
“The coronavirus pandemic has greatly accelerated the use of telemedicine,” says sleep specialist Kelly Carden, MD, MBA, who is the American Academy of Sleep Medicine (AASM) president. Will these sudden changes be woven into the “new normal” or will in-person visits soon return as the norm? And are sleep clinicians prepared?
Home Sleep Testing Surges
“Prior to the COVID-19 pandemic, adoption of telemedicine by sleep specialists had been slow, due largely to complex, fragmented, and inadequate reimbursement policies for virtual visits,” says Carden, who practices as a sleep specialist at Saint Thomas Medical Partners – Sleep Specialists in Nashville, Tenn. But, according to telemedicine and home sleep testing vendors, it ramped up quickly as government officials required residents to stay home for all except for the most urgent of tasks.
During the months of March and April, Cleveland-based home sleep testing company CleveMed saw a surge in demand for its mail-order service. At that time, its CEO and president Hani Kayyali told Sleep Review that the company had almost tripled its business. The company’s orders grew by approximately 85% in March alone.
“We have never, ever seen such growth in demand. We are seeing, over the past six weeks now, an unprecedented demand,” Kayyali said at the time. “We are trying to keep up.”
According to Kayyali, most of the calls that CleveMed received this spring were from large medical facilities that already have home sleep testing (HST) available but were looking to bulk up their programs.
Itamar Medical too received new calls from clinicians looking to beef up their home sleep study programs. Itamar Medical’s WatchPAT One home sleep test is fully disposable, making it an attractive option for some during the pandemic. The company also offers a WatchPAT Direct program, which handles the logistics of shipping for the medical practice or lab. The program allows providers to scale home sleep programs quickly with no upfront costs and with minimal effort from office staff, says Phillip Dukes, vice president of clinical sales at Itamar.
Medical device company BioSerenity also saw a large percentage of the medical centers it works with switch over to HSTs, says its CEO Pierre Frouin. “We have seen entire hospital systems switching over,” Frouin says.
Some in the home sleep testing space think this growth is a signal of a permanent pivot toward telehealth solutions. 
Vice president of sleep marketing at ResMed, Annie McBride, says many regions experienced a scarcity of beds before the pandemic. Since patients are likely putting off their sleep studies, there could be a backlog of people looking for these diagnostic tests.
The psychological strain of the pandemic could also leave patients unwilling to leave their homes for overnight sleep studies. “It will likely take some time for patients to be psychologically ready to pursue elective sleep lab and hospital visits,” says McBride.
A Bump in Infection Control Questions
Manufacturers report a rise in questions about infection control. Most companies follow the US Centers for Disease Control and Prevention guidelines for disinfection.
Since viruses and bacteria are only viable on surfaces for so long, the three-day turnaround time for CleveMed’s mail-order delivery program means there is less of a chance of transmission than if the devices immediately went back into circulation, says Kayyali.
All reusable components, the monitor, and the pulse oximeter are not in use for three days between patients (and are cleaned and disinfected first). Sensors that come with the test, including the nasal cannula and the respiratory belts, are disposable.
“We have updated our disinfection processes and continue to train our team on that,” says Kayyali.
Payors Expand Telemedicine Coverage
In response to the pandemic, the Centers for Medicine & Medicaid Services (CMS) loosened coverage guidelines for at-home sleep apnea treatments and devices.2 Though it is unclear which guidelines will return to pre-pandemic protocols and when, the changes could set new expectations and signal an industry shift in how care is administered. During the pandemic, CMS has given greater flexibility to patients and clinicians by waiving requirements for face-to-face and in-person encounters, such as those required for ongoing positive airway pressure (PAP) therapy.
“This change is going to make it less burdensome for Medicare patients to maintain coverage for PAP therapy,” Carden says.
CMS also responded to COVID-19 by making a temporary policy change to cover PAP devices based on the clinician’s assessment of the patient without requiring an in-lab sleep study or home test.2
“This change enhances the value of a sleep specialist’s clinical judgment and expertise, and it offers the opportunity for patients with suspected sleep apnea to receive therapy much more quickly,” Carden says. “However, it remains to be seen if this decision will have any unintended negative consequences for the quality of patient care.”
While there are still questions about the future of telemedicine, the demand for these services is skyrocketing. Due to this greater demand, the AASM released its telehealth platform AASM SleepTM to all members for a limited time. It also released its Sleep Telemedicine Implementation Guide, a 52-page document that is available to download for free on the AASM’s website, which covers how to start a telemedicine program.3
“Given the increasing demand for treatment of sleep-related disorders, the AASM leadership thinks that telemedicine is one of the mechanisms to address workforce challenges,” the guide says.
“Telemedicine utilization for sleep medicine is likely to expand rapidly, as are broader telehealth applications in general; further research into the effect and outcomes of these applications is needed.”
How to Start Remote Programs
Now, a number of sleep medicine providers are looking to scale up their home sleep testing programs and some are even beginning to look at starting programs from scratch. The AASM suggests that providers first should ask themselves if telemedicine is right for their practice.
The next step is to identify the target audience and then decide whether to invest in a mail-order home sleep testing program or to buy the equipment outright. An advantage to mail-order services is the patient never has to travel anywhere, and the home sleep tests can be mailed directly to their residence, explains Kayyali.
Another important step is to investigate the regulatory, legal, and ethical considerations of the implementation of telemedicine.
Data security is of utmost importance when evaluating equipment and software. When looking for software, providers should consider asking vendors questions about how the data is stored and transmitted. Is the patients’ information transmitted and stored in a way that is Health Insurance Portability and Accountability Act (HIPAA)-compliant? Are both live and stored information encrypted?
The upfront capital expenses will be another factor in determining if telemedicine is the right direction. For home sleep testing only, there may be minimal to no startup costs for a mail-order program, which may charge under a per-study contract. But for new patient and follow-up live telemedicine visits, a substantial investment in equipment may be required. Although some telemedicine companies offer comprehensive plans for equipment purchase and maintenance, others provide à la carte purchasing options.
Key start-up components may include a webcam for the provider site and the patient’s site and software for video transmission that is both HIPAA-compliant and encrypted. Depending on the particular setup, providers may want to invest in a telestethoscope and mobile examination camera. A telemedicine console, or cart, that combines these tools may be convenient, but expenses vary. According to the AASM, consoles range from about $2,000 to more than $20,000 depending on the vendor, brand, and features. 
Another option is equipment rentals, which may be available depending on the location and vendor.
Sleep Testing Evolves
More than a decade ago, awareness of sleep apnea home testing started to emerge. As the tests gained popularity with some dentists, cardiologists, and others, the AASM took note. Pressure mounted and the organization released guidance on the use of home sleep testing devices in a 2007 paper, published in the Journal of Clinical Sleep Medicine.4
The paper, a result of an AASM task force investigation, suggested that unattended portable monitoring for the diagnosis of obstructive sleep apnea (OSA) should be performed only in conjunction with a comprehensive sleep evaluation. It said that portable monitors can be used as an alternative to polysomnography (PSG) for the diagnosis of OSA in patients with a high pretest probability of moderate to severe OSA but is not appropriate for patients with significant comorbid medical conditions.
“That led us down the path of including home sleep apnea testing reluctantly,” says neurologist Jeffrey Durmer, MD, PhD, chief medical officer at Nox Health.
“Now, with coronavirus we are in a different place with home sleep apnea testing. First of all, it is well understood. It has some well thought out criteria developed around it,” says Durmer, who is a principal investigator on the National Institutes of Health-funded Sleep SMART trial.
Nox Medical has moved to expand the capabilities of home sleep testing by introducing the Nox A1 device, which is a PSG system with both in-lab and at-home functionality.  The device, which can perform Level I, Level II, and Level III sleep studies, was first released in 2014 in Europe and debuted in the United States and Canada in March of this year.
“We will be able to get more real-life data associated with their natural sleeping environment but also to bring sleep testing into the home,” says Durmer. “This way, people are not being pulled into a laboratory, which has high costs associated with it. So developing the Nox A1, which is the big brother of the Nox T3, to do home sleep testing, not just sleep apnea testing, that now is part of our program.
“There is now more acceptance of home-based sleep testing. There are people wearing wearables to track their sleep at home. I think it is a perfect time now to invest in a post-coronavirus telemedicine environment.”

The Women with Silver Wings The Inspiring True Story of the Women Airforce Service Pilots of World War II by Katherine Sharp Landdeck
When the Japanese attacked Pearl Harbor in December 1941, Cornelia Fort was already in the air. At twenty-two, Fort had escaped Nashville's debutante scene for a fresh start as a flight instructor in Hawaii. She and her student were in the middle of their lesson when the bombs began to fall, and they barely made it back to ground that morning. Still, when the U.S. Army Air Forces put out a call for women pilots to aid the war effort, Fort was one of the first to respond. She became one of just over 1,100 women from across the nation to make it through the Army's rigorous selection process and earn her silver wings. The brainchild of trailblazing pilots Nancy Love and Jacqueline Cochran, the Women Airforce Service Pilots (WASP) gave women like Fort a chance to serve their country—and to prove that women aviators were just as skilled as men. While not authorized to serve in combat, the WASP helped train male pilots for service abroad, and ferried bombers and pursuits across the country. Thirty-eight WASP would not survive the war. But even taking into account these tragic losses, Love and Cochran's social experiment seemed to be a resounding success—until, with the tides of war turning, Congress clipped the women's wings. The program was disbanded, the women sent home. But the bonds they'd forged never failed, and over the next few decades they came together to fight for recognition as the military veterans they were—and for their place in history.

New podcast: Stop and think. How will coronavirus affect nurseries, worship and last rites?

At this point, it’s clear that the coronavirus story has moved past concerns about whether members of ancient Christian churches can catch the disease from wine in golden Communion chalices.
People will debate that issue for one simple reason — people have researched that issue for centuries and argued about the results. That story is the tip of the iceberg, when it comes to reporting on how religious congregations — past and present — have reacted during times of plague.
So read up on the “common cup” issue and then move on. Oh, and resist the temptation to spotlight the inevitable proclamation from the Rev. Pat Robertson. And there will be more to this story than Episcopal bishops turning a scheduled meeting into a “virtual” gathering.
That’s the message at the heart of this week’s “Crossroads” podcast (click here to tune that in). And while many journalists tend to focus on Catholic churches — lots of people in sanctuaries that photograph well — I think that editors and producers need to consider how this crisis could impact highly independent Protestant megachurches and institutions linked to them. Mosques and synagogues will be affected.
Everyone will be effected. Reporters will need to focus on specific facts and broad trends.
While we were recording the podcast, I told host Todd Wilken that journalists may want to note that spring break is not that far away. In addition to sending legions of young people to jammed beaches and crazy watering holes, this is also a time when churches and colleges organize short-term mission trips to locations around the world. Sure enough, I saw this notice on Twitter a few hours later, from a campus in Arkansas:

That decision about mission trips was clearly linked to this earlier decision about an extension campus in Italy:
We are heartbroken to share the news that out of an abundance of caution related to the potential threat of coronavirus in Italy, the decision has been made to close Harding University Florence campus for the remainder of the semester, effective Feb. 29. Those students will be heading home to finish their coursework remotely. This is obviously a huge disappointment for these students in this very special program. The specifics of course completion and financial details are being communicated to the students. 
As we head into spring break, we encourage you to stay informed of ongoing developments of the spread of the virus as many will be traveling throughout the U.S. and the world.
By the way: The number of American students in study-abroad programs? It’s way higher than you think (and I say that as a professor with more than two decades experience in work linked to that trend).
In addition to looking at Catholic responses to coronavirus trends, I would suggest that reporters pay attention to what leaders will attempt to do in religious bodies that have — #DUH — much weaker national leadership infrastructures.
In other words, what’s going on right now at the Southern Baptist Convention’s headquarters in Nashville? What is happening in totally independent megachurches from coast to coast?
If you have a megachurch nearby with 5,000-plus members, stop and think how many babies and toddlers there are in the church nursery and kids involved in “children’s church” worship services. If church leaders shut those down, how will that affect services in all of those theater or even sports-complex-sized sanctuaries? At what point do they simply cancel coffee hour and every other form of fellowship and ask the grown-ups to sit quietly in the pews wearing masks?
Here’s another story: Will those tech-friendly churches simply go virtual? Let’s say you’re covering Second Baptist Church in Houston — which has 60,000-plus members. Does the Rev. Ed Young just tell everyone to stay home and stream the services? How many other large churches will choose that option?
Meanwhile, the normal American church is nowhere near that large and they do not have that option ready to go. The vast majority of religious congregations have somewhere between 50 and 150 people gathered for worship. How many of those will attempt to move to a “virtual worship” option?
Our own Julia Duin has been following this situation closely in the newspapers on the West Coast — since she lives very close to the crisis center in Seattle and Kirkland. She sent me a link from The Seattle Times pointing to a very interesting worship angle in the Jewish community there.
Temple De Hirsch Sinai, a Reform congregation in Seattle’s Capitol Hill neighborhood, will stream its Shabbat services Friday night and Saturday morning, rather than have congregants come in person.
That’s not an option for all synagogues: Orthodox congregations abide by religious rules that restrict them from using technology on the Sabbath, the day of rest. Capitol Hill Minyan, a Jewish Orthodox prayer group that meets at a retirement community, announced on its website that it was suspending services as of March 4.
But the changes are discordant with the current the lunar month of Adar, one that’s traditionally marked by joyous celebrations — including the Jewish holiday of Purim, which this year starts Monday night and lasts through Tuesday.
Whatever religious groups try to do to cope, one simple fact remains. At some point, people are going to get sick and head to hospitals and some will die.
Will clergy go to visit the sick? In ancient traditions, will they anoint the sick and pray for healing? Will priests hear final confessions and take Holy Communion to the dying?
Of course they will. Clergy have, for centuries, been risking their lives to provide comfort and save souls.
My “On Religion” column this week focuses on one such story from the horrific “Spanish flu” outbreaks of 1918-20 — when it is estimated that a third of the world’s population got sick. Somewhere between 50 and 100 million people died. Here is the top of that column, focusing on a priest in my own global communion. This is long, but I think readers will see the links to the current crisis:
The second wave of influenza in the fall of 1918 was the worst yet. By the time Father Nicola Yanney reached Wichita, Kansas, a citywide quarantine was in effect.
A 16-year-old girl had already died, creating a sense of panic. The missionary priest -- his territory reached from Missouri to Colorado and from Oklahoma to North Dakota -- couldn’t even hold her funeral in the city’s new Orthodox sanctuary. As he traveled back to his home church in Kearney, Nebraska, he kept anointing the sick, hearing confessions and taking Holy Communion to those stricken by the infamous “Spanish flu.”
After days of door-to-door ministry in the snow, Yanney collapsed and called his sons to his bedside. Struggling to breathe, he whispered: “Keep your hands and your heart clean.” …
A century later, many Orthodox Christians in America -- especially those of Syrian and Lebanese descent -- believe Yanney should be recognized as a saint. And now, as churches face fears unleashed by the coronavirus, many details of his final days of ministry are highly symbolic.
”Father Nicola got the flu because he insisted on ministering to people who had the flu,” said Father Andrew Stephen Damick, creator of “The Equal of Martyrdom,” an audio documentary about the man known as “The Apostle to the Plains.”
”For priests, there are risks,” said Damick. “But you cannot turn away when people are suffering and they need the sacraments of the church. You go to your people and minister to them. This is what priests do.”

Reports of the FDT hack have come from LPFM broadcasts all over the country, from a regional Mexican channel in Nashville, Tennessee to a Catholic programming station in Evansville, Indiana.    
Notably, YG offered to give the Trump inauguration some street cred by showing up for a set — on the condition that he would only perform FDT, for $4 million.
