So Ichigo has certainly been in his fair share of scraps in and out of his career as a Substitute Shinigami. From dubious stab wounds, to multiple beatings and head trauma, all the way to point blank Cero blasts to the chest. In spite of the Anime / Manga never referencing any type of lasting scarring for him, My Portrayal will differ.
Ichigo’s most notable and prominent scar is the one given to him by Ulquiorra Cifer, a nasty looking gash in the middle of his chest where a hole was made by a Cero. This wound was healed by a combination of Orihime’s powers and the Hollow Powers inside of him, but the mark still lay bear upon his sternum. It is a constant reminder of the deadly battle between the two, and if not for his Hollow side, Ichigo would surely be deceased. ( It’s also a reminder of the time he lost control completely in the strife to protect everyone. It’s a blessing and a curse. ). It’s also worth noting that when Ginjou Kugo stabbed Ichigo to drain him of his Fullbring, he stabbed directly into that same scar still present upon his chest. It was like insult to injury.
Another notable scar lay upon his right shoulder, jagged and rough to the touch. This particular wound was given by Renji Abarai, in their second scuffle against saving Rukia Kuchiki. Zabimaru’s slice and followed up grind against flesh proved to be difficult to heal, and led to the prominent wound still showing today.
A smaller, fainter set of scars lay upon both of Ichigo’s palms. While fainter than most, these scars are still noticeable and commented upon most because of the open positioning. These scars were given to him by Grimmjow Jaegerjaques, by stabbing Tensa Zangetsu through both of his hands to stop mobility in their second fight.
Faint burn scars remain upon the boy’s upper left arm, residual and not as prominent but still present. These scars were given to him by Aizen Sosuke during their final bout. The power of Mugetsu soon coating it helped the wound in the long run, reiatsu healing it from the outside as all of his power burst forth from using Mugetsu.
And last but not least, a clean, calculated slice to his lower abdomen on the right side. While being notable in itself, the steadiness of the cut made was to be expected from the enemy who had struck, that being Byakuya Kuchiki during their first meeting at the arrest of Rukia Kuchiki. He had severed Ichigo’s Soul Chain with one strike, and that left a lasting reminder upon battle hardened flesh.
Most of these wounds could have claimed Ichigo’s life over the years, so viewing them all as reminders of what he has accomplished and who he has overcome is a somewhat comforting thought for the young Substitute. He does not abhor the wounded flesh, but more revels in his own victories.
          He is still Alive.
