Meiko Kaji (梶芽衣子) in a couple screencaps for the TV movie Crazy Signal (Kurutta Shingo, 狂った信号) which aired on Nippon TV in Japan on May 15, 1984.

Meiko plays Hatsuko, a mother whose son was killed in a traffic accident and something happens (the perpetrator gets off lightly and/or there’s some conspiracy) that motivates Hatsuko to seek revenge.

There’s not a whole lot of info on this readily available on the net (and I haven’t spent too much time looking yet), but TVDrama-DB and this site have some info if you’re curious. That second link also has a small screencap with Meiko in it.
