When acid reflux back from stomach to esophagus due to lax valve between esophagus and stomach and produce damage to the inner lining of esophagus called GERD (Gastro Esophageal Reflux Disease).
What are the Symptoms of GERD (Gastro Esophageal Reflux Disease)?
Heartburn: this is the most common symptom of GERD (Gastro Esophageal Reflux Disease). This is a burning feeling which rises from the upper abdomen or lower chest up towards the neck.
Pain abdomen or chest pain, dyspepsia
Some uncommon symptoms of GERD: these may occur and can make the diagnosis difficult.

Investigations Required are GERD (Gastro Esophageal Reflux Disease)?
Tests are not usually necessary if you have typical symptoms. You need to have upper GI endoscopy to confirm and establish the extent of damage. There are GERD where endoscopy may be negative. There are other investigations; like 24 hour pH analysis which is highly sensitive test to confirm GERD. Other tests like manometry, chest x ray, ECG might be needed.
What can we do to reduce the GERD (Gastro Esophageal Reflux Disease) symptoms?
The following lifestyle changes may help to reduce the reflux:
Stop smoking
Some foods and drinks – Foods and drinks those might be responsible for reflux are peppermints, chocolate, spicy foods, hot drinks, coffee, and alcoholic drinks. Also, avoiding large-volume meals may help. Certain medications like pain killers may worsen the symptoms
Weight loss – If you are overweight, losing some weight may help to improve the symptoms.
Posture – Lying down or bending forward may increase reflux. Wearing tight belts may increase intraabdominal pressure and increases the reflux.
Bedtime
Avoid sleeping within 2 – 3 hours of meal.
Raise head end of the bed by 1- 20 cm.
What are the treatment options for GERD (Gastro Esophageal Reflux Disease)?
Medications – antacids, H2 blockers, proton pump inhibitors, prokinetic agents
Surgical treatment – laparoscopic fundoplication
Choice of treatment depends on severity and duration of symptoms, presence or absence of complications.

ERCP is a procedure that uses an endoscope and X-rays to look at your bile duct and pancreatic duct. ERCP can also be used to remove gallstones or take small samples of tissue for analysis (a biopsy).
What happens during ERCP
The endoscope also has a ‘side channel’ down which various tubes or instruments can pass. These can be manipulated by the doctor who can do various things. For example:
Inject a dye into the bile and pancreatic ducts. X-ray pictures taken immediately after the injection of dye show up the detail of the ducts. This may show narrowing (stricture), stuck gallstones, tumours pressing on the ducts, etc.
Take a small sample (biopsy) from the lining of the duodenum, stomach, or pancreatic or bile duct near to the opening called the papilla. The biopsy sample can be looked at under the microscope to check for abnormal tissue and cells.
If the X-rays show a gallstone stuck in the duct, the doctor can widen the opening of the papilla to let the stone out into the duodenum. A stone can be grabbed by a ‘basket’ or left to be passed out with the stools (faeces).
If the X-rays show a narrowing or blockage in the bile duct, the doctor can put a stent inside to open it wide. A stent is a small wire-mesh or plastic tube. This then allows bile to drain into the duodenum in the normal way. You will not be aware of a stent, which can remain permanently in place.
The endoscope is gently pulled out when the procedure is finished. An ERCP can take anything from 30 minutes to over an hour, depending on what is don

April is Esophageal Cancer Awareness Month. Let’s talk about risk factors of Esophageal Cancer.
Individual’s risk of developing esophageal cancer during his or her lifetime is 0.5 percent. Five-year survival rate for esophageal cancer is an average 20 percent.
Types of esophageal cancer:
Squamous cell carcinoma, which typically affects the upper and middle part of the esophagus
Adenocarcinoma, which usually occurs in the lower part of the esophagus
Esophageal cancer often does not cause symptoms in its early stages. However, as it progresses individuals can experience:
Difficult or painful swallowing
Weight loss
Hoarseness
Cough that does not go away
Indigestion or Heartburn
Vomiting
Because symptoms usually do not appear until the disease is advanced, esophageal cancer is also one of the most difficult cancers to treat.
Risk factors for esophageal cancer includes:
Smoking
Drinking alcohol
Having Long-term acid reflux or gastroesophageal reflux disease (GERD)
Barrett’s esophagus
Obesity
Poor nutrition

Men are also approximately three times more likely to develop esophageal cancer than women.
Prevention:
Avoid using tobacco products
Stop consuming alcohol
Treat obesity
Get treated for reflux or Barrett’s esophagus
Eating non-starchy vegetables and whole fruit probably lowers risk.
Regular physical activity lowers risk

Having one or all of these risk factors does not mean that one will develop esophageal cancer.  Also, not having any risk factors does not mean that one will never develop esophageal cancer. Raising awareness of risk factors associated with esophageal cancer is one way to help reduce the incidence of an esophageal cancer.  People who are considered to be at an elevated risk should consult their doctor immediately about getting screened as early as possible.
Early detection can improve the chances of survival if cancer is found in the esophagus.   There are also more treatment options available for early esophageal cancer and better outcomes.
