Prostate Cancer- Stages: PART 2
Tumblr media
Determining the stage of Prostate Cancer
The stage, or how far the cancer has spread, is an important part of assessing prostate cancer. When choosing therapies, knowing the stage helps define the prognosis. The four-stage TNM system, abbreviated as Tumor/Nodes/Metastases, is the most prevalent one. It is made up of the size of the tumor, the number of lymph nodes that are involved, and whether or not there are any other metastases.
Whether or not the cancer is still confined to the prostate is the most crucial distinction made by any staging system. Clinical T1 and T2 cancers are only found in the prostate in the TNM system, whereas T3 and T4 cancers have spread elsewhere. There are a number of tests that can be used to find signs of spread. When a doctor diagnoses early prostate cancer with a low risk of metastasis, medical specialty professional organizations advise against using PET, CT, or bone scans. When a CT scan looks for spread in the pelvis, a bone scan looks for spread to the bones, and endorectal coil magnetic resonance imaging looks closely at the prostatic capsule and seminal vesicles, those tests would be appropriate.
A pathologist examines the samples under a microscope following a prostate biopsy. The grade of the tumor is reported by the pathologist if cancer is present. The grade indicates the likelihood of the tumor growing quickly and how significantly the tumor differs from normal prostate tissue. Prostate tumors are graded using the Gleason system, which assigns a score between 2 and 10, with a score of 10 representing the most abnormalities. The pathologist gives the pattern that is seen most frequently under the microscope a number between one and five, and they do the same for the pattern that is seen second most frequently. The Gleason score is the sum of these two numbers. Another approach that is sometimes utilized is the Whitmore-Jewett stage.

Some Facts on Ureteric Reimplantation
Tumblr media
There are connecting tubes between the urinary bladder and the kidneys. There is difficulty in the passing of urine if there is a change in position of the tubes. A urologist in Kolkata fixes the positions of the tubes to prevent urine from receding into the kidneys with the help of Ureteric Reimplantation. The completion of the surgical procedure takes several hours. In normal cases, the flow of the urine is in one way from the kidney to the bladder through the ureter. The urine recedes to the kidney instead of flowing into the bladder when there is a bad connection of the ureter called redux. It leads to the scarring of the kidney and perpetual damage to the kidney.
During the surgery, the urologist changes the position of the tubes at the area where they join the bladder. This helps in preventing the urine from receding into the kidneys. While carrying out the Ureteral reimplantation procedure in children, they are given general anaesthesia. In order to give the children relief from pain in the waist area, they are also given caudal anaesthesia.

Radiation therapy by infusion
Tumblr media
Radium-223 (Xofigo): Castration-resistant prostate cancer that has spread to the bone can be treated with this radioactive substance. Alpha-emitting radionuclide radium-223 targets the areas of the bone where cancer is causing changes and mimics calcium. This therapy conveys radiation particles straightforwardly to growths tracked down in the bone, restricting harm to solid tissue, including the bone marrow, where ordinary platelets are made. For six months, radium-223 is administered intravenously (IV) once per month. A radiation oncologist or a nuclear medicine physician will administer this treatment. Throughout this treatment, your medical oncologist should continue to monitor your progress to ensure that the treatment is effective and that any potential side effects are managed. Radium-233 treatment does not always lower PSA, so patients should not anticipate significant PSA levels dropping during treatment; in fact, PSA levels may frequently rise.
This treatment should not be given to some people, especially those with prostate cancer that has spread to the liver, lungs, or both, or with lymph nodes that are three centimeters or larger in diameter. Radium-223 ought not be utilized in mix with abiraterone and prednisone since that blend builds the gamble of bone cracks. Check with your doctor to see if you might benefit from taking this medication.

4. BAMS:

This degree program is conferred to those students who have studied the modern medicines and traditional Ayurveda. Ayurvedic science is one of the ancient medical systems of the world. It traces its roots to the Vedic period. Also, BAMS is 5 and a half years course. Best ayurveda colleges in India are:
Shri Dhanwantri Ayurvedic College, Chandigarh

5. MD/MS:
MD is a post graduation degree, stands for masters in medicines and MS is a post graduation degree stands for masters in surgery. Both of them are a 3 years course for medical Graduates (MBBS) that deal with the branches which do not require surgical skills. MD is awarded in both non-clinical as well as clinical branches.
Likewise, the future prospects of MD and MS as medical courses in India are generally same. An individual who completes MS becomes a surgeon whereas the one who takes up MD is a physician. Finally, a surgeon can work in place of a physician with more study of medicine while a physician cannot be a surgeon. In contrast, financially, a surgeon can earn a lot more money than a physician, if he or she is skilled.
The Popular specializations in MS and MD are:
MD:
The obstetrics & Gynecology, Endocrinology, Orthopaedics, Neurology and the Anesthesiology, Cardiology, Internal Medicine, Paediatric, Psychiatry, Dermatology, Radio diagnosis and the Pathology.
MS:
Pediatric surgery, plastic surgery, cardio-thoracic surgery, Urology, Cardiac surgery, Cosmetic surgery, ENT, Ophthalmology, Gynecology, Obstetrics and the orthopedics.
So, usually it takes three years to complete the MD or MS but for s super specialization the student needs two years more after MD or MS.

Holmium Laser Enucleation of the Prostate (HoLEP)
Tumblr media
The men who have benign prostatic hyperplasia (BPH) may need to undergo Holmium Laser Enucleation of the Prostate (HoLEP). The removal of the blockage of urine is possible with the help of this laser surgery. As there are no incisions in the body in this treatment, many urology doctors in Kolkata suggest this procedure.
A number of urological surgeries are possible with the use of holmium laser, which is a type of surgical laser. The doctor removes the bulky prostate tissue that blocks the urine flow after he/she cuts and removes it using the laser in the HoLEP procedure. The doctor will go through your medical history and also suggests some diagnostic tests to check the severity of the condition. You may have to stop the consumption of blood-thinning drugs before the surgery. The anaesthetist will administer general or spinal anaesthesia depending on the situation. The doctor will insert a special instrument called the resectoscope including a camera through the urethra. He/she inserts the laser into the resectoscope to free up (enucleate) the tissue of the enlarged prostate. This tissue then gets deposited in the bladder. The doctor replaces the laser in the resectoscope with a morcellator in order to suction out the tissue deposit in the bladder. He/she puts a urine catheter in the place after removing the resectoscope at the end of the procedure.
