When pathologist Amy Rapkiewicz began the grim process of opening up the coronavirus dead to learn how their bodies went awry, she found damage to the lungs, kidneys and liver consistent with what doctors had reported for months.
But something was off.
Rapkiewicz, who directs autopsies at NYU Langone Health, noticed that some organs had far too many of a special cell rarely found in those places. She had never seen that before, yet it seemed vaguely familiar. She raced to her history books and — in a eureka moment — found a reference to 1960s report on a patient with dengue fever.
In dengue, a mosquito-borne tropical disease, she learned, the virus appeared to destroy these cells, which produce platelets, leading to uncontrolled bleeding. The novel coronavirus seemed to amplify their effect, causing dangerous clotting.
She was struck by the parallels: “Covid-19 and dengue sound really different, but the cells that are involved are similar.”
Autopsies have long been a source of breakthroughs in understanding new diseases, from HIV/AIDS and Ebola to Lassa fever — and the medical community is counting on them to do the same for covid-19, the disease caused by the novel coronavirus. With a vaccine probably many months away in even the most optimistic scenarios, autopsies are becoming a critical source of information for research into possible treatments.
When the pandemic hit the United States in late March, many hospital systems were too overwhelmed trying to save lives to spend too much time delving into the secrets of the dead. But by late May and June, the first large batch of reports — from patients ranging in age from 32 to 90 years who died at a half-dozen institutions — were published in quick succession. The investigations have confirmed some of our early hunches of the disease, refuted others — and opened up new mysteries about the pathogen that has killed more than 500,000 people worldwide.
Among the most important findings, consistent across several studies, is confirmation the virus appears to attack the lungs the most ferociously. They also found the pathogen in parts of the brain, kidneys, liver, gastrointestinal tract and spleen and in the endothelial cells that line blood vessels, as some had previously suspected. Researchers also found widespread clotting in many organs.
But the brain and heart yielded surprises.
“It’s about what we are not seeing,” said Mary Fowkes, an associate professor of pathology who is part of a team at Mount Sinai Health that has performed autopsies on 67 covid-19 patients.
Given widespread reports about neurological symptoms related to the coronavirus, Fowkes said, she expected to find virus or inflammation — or both — in the brain. But there was very little. When it comes to the heart, many physicians warned for months about a cardiac complication they suspected was myocarditis, an inflammation or hardening of the heart muscle walls — but autopsy investigators were stunned that they could find no evidence of the condition.
Another unexpected finding, pathologists said, is that oxygen deprivation of the brain and the formation of blood clots may start early in the disease process. That could have major implications for how people with covid-19 are treated at home, even if they never need to be hospitalized.
The early findings come as new U.S. infections have overtaken even the catastrophic days of April, amid what some critics say is a premature easing of social distancing restrictions in some states, mainly in the South and West. A new modeling study has estimated that about 22 percent of the population — or 1.7 billion people worldwide, including 72 million in the United States — may be vulnerable to severe illness if infected with the virus. According to the analysis published this month in Lancet Global Health, about 4 percent of those people would require hospitalization — underscoring the stakes as autopsy investigators continue their hunt for clues.
Microclots in lungs
At their best, autopsies can reconstruct the natural course of the disease, but the process for a new and highly infectious disease is tedious and requires meticulous work. To protect pathologists and avoid sending virus into the air, they must use special tools to harvest organs and then dunk them in a disinfecting solution for several weeks before they are studied. They must then section each organ and collect small bits of tissue for study under different types of microscopes.
One of the first American investigations to be made public, on April 10, was out of New Orleans. The patient was a 44-year-old man who had been treated at LSU Health. Richard Vander Heide remembers cutting the lung and discovering what were probably hundreds or thousands of microclots.
“I will never forget the day,” recalled Vander Heide, who has been performing autopsies since 1994. “I said to the resident, ‘This is very unusual.’ I had never seen something like this.”
But as he moved onto the next patient and the next, Vander Heide saw the same pattern. He was so alarmed, he said, that he shared the paper online before submitting it to a journal so the information could be used immediately by doctors. The findings caused a stir at many hospitals and influenced some doctors to start giving blood thinners to all covid-19 patients. It is now common practice. The final, peer-reviewed version involving 10 patients was subsequently published in the Lancet in May.
Other lung autopsies — including those described in papers from Italy of 38 patients, a Mount Sinai Health study on 25 patients, a collaboration between Harvard Medical School and German researchers on seven and an NYU Langone Health study on seven — have reported similar findings of clotting.
Most recently, a study out last month in the Lancet’s eClinicalMedicine, found abnormal clotting in the heart, kidney and liver, as well as the lungs of seven patients, leading the authors to suggest this may be a major cause of the multiple-organ failure in covid-19 patients.
Heart cells
The next organ studied up close was the heart. One of the most frightening early reports about the coronavirus from China was that a significant percentage of hospitalized patients — up to 20 to 30 percent — appeared to have myocarditis that could lead to sudden death. The condition involves the thickening of the muscle of the heart so that it can no longer pump efficiently.
Classic myocarditis is typically easy to identify in autopsies, pathologists say. It occurs when the body perceives the tissue to be foreign and attacks it. In that situation, there would be large dead zones in the heart, and the muscle cells known as myocytes would be surrounded by infection-fighting cells known as lymphocytes. But in the autopsy samples taken so far, the dead myocytes were not surrounded by lymphocytes — leaving researchers scratching their heads.
Fowkes, from Mount Sinai, and her colleague, Clare Bryce, whose work on 25 hearts has been published online but not yet peer reviewed, said they saw some “very mild” inflammation of the surface of the heart but nothing that looked like myocarditis.
NYU Langone’s Rapkiewicz, who studied seven hearts, was struck by the abundance in the heart of rare cells called megakaryocytes. The cells, which produce platelets that control clotting, typically exist only in the bone marrow and lungs. When she went back to the lung samples from the coronavirus patients, she discovered those cells were too plentiful there, too.
“I could not remember a case before where we saw that,” she said. “It was remarkable they were in the heart.”
Vander Heide, from LSU, who reported preliminary findings on 10 patients in April and has a more in-depth paper with more case studies under review at a journal, explained that “when you look at a covid heart, you don’t see what you’d expect.”
He said a couple of patients he performed autopsies on had gone into cardiac arrest in the hospital, but when he examined them, the primary damage was in the lungs — not the heart.
Brain grid
Of all the coronavirus’s manifestations, its impact on the brain has been among the most vexing. Patients have reported a host of neurological impairments, including reduced ability to smell or taste, altered mental status, stroke, seizures — even delirium.
An early study from China, published in the BMJ, formerly the British Medical Journal, in March, found 22 percent of the 113 patients had experienced neurological issues ranging from excessive sleepiness to coma — conditions typically grouped together as disorders of consciousness. In June, researchers in France reported that 84 percent of patients in intensive care had neurological problems, and a third were confused or disoriented at discharge. Also this month, those in the United Kingdom found that 57 of 125 coronavirus patients with a new neurological or psychiatric diagnosis had experienced a stroke due to a blood clot in the brain, and 39 had an altered mental state.
Based on such data and anecdotal reports, Isaac Solomon, a neuropathologist at Brigham and Women’s Hospital in Boston, set out to systematically investigate where the virus might be embedding itself in the brain. He conducted autopsies of 18 consecutive deaths, taking slices of key areas: the cerebral cortex (the gray matter responsible for information processing), thalamus (modulates sensory inputs), basal ganglia (responsible for motor control) and others. Each was divided into a three-dimensional grid. Ten sections were taken from each and studied.
He found snippets of virus in only some areas, and it was unclear whether they were dead remnants or active virus when the patient died. There were only small pockets of inflammation. But there were large swaths of damage due to oxygen deprivation. Whether the deceased were longtime intensive care patients or people who died suddenly, Solomon said, the pattern was eerily similar.
“We were very surprised,” he said.
When the brain does not get enough oxygen, individual neurons die, and that death is permanent. To a certain extent, people’s brains can compensate, but at some point, the damage is so extensive that different functions start to degrade.
On a practical level, Solomon said, if the virus is not getting into the brain in large amounts, that helps with drug development because treatment becomes trickier when it is pervasive, for instance, in some patients with West Nile or HIV. Another takeaway is that the findings underscore the importance of getting people on supplementary oxygen quickly to prevent irreversible damage.
Solomon, whose work was published as a June 12 letter in the New England Journal of Medicine, said the findings suggest the damage had been happening over a longer period of time, which makes him wonder about the virus’s effect on people who are less ill. “The big lingering question is what happens to people who survive covid,” he said. “Is there a lingering effect on the brain?”
The team from Mount Sinai Health, which took tissue findings from 20 brains, was also perplexed not to find a lot of virus or inflammation. However, the group noted in a paper that the widespread presence of tiny clots was “striking.”
“If you have one blood clot in the brain, we see that all the time. But what we’re seeing is, some patients are having multiple strokes in blood vessels that are in two or even three different territories,” Fowkes said.
Rapkiewicz said it is too early to know whether the newest batch of autopsy findings can be translated into treatment changes, but the information has opened new avenues to explore. One of her first calls after noticing the unusual platelet-producing cells was to Jeffrey Berger, a cardiac specialist at NYU who runs a National Institutes of Health-funded lab that focuses on platelets.
Berger said the autopsies suggest anti-platelet medications, in addition to blood thinners, may be helpful to stem the effects of covid-19. He has pivoted a major clinical trial looking at optimal doses of blood thinners to examine that question as well.
“It’s only one piece of a very big puzzle, and we have a lot more to learn,” he said. “But if we can prevent significant complications and if more patients can survive the infection, that changes everything.”

2. My body is a Picasso
I’ve been incredibly fortunate in my journey to try some cutting-edge treatments in Australia, as well as having a very intelligent, compassionate and hard working CFS (chronic fatigue syndrome) specialist on my side. A specialist like that doesn’t come cheap either (very little if anything is covered by medicare so you have to be prepared to be out of pocket by tens if not hundreds of thousands over the years). He’s been my Dr for 8 years and yes he couldn’t get me better, but he has helped me in so many ways. 
He was able to diagnose a multitude of problems within my body through tests regular doctors would not have access to. His understanding and knowledge of CFS and what it does to the body on a cellular level is extraordinary. For those who know who I am talking about, they would agree that he is a brilliant man, who I owe my life to. He got me into private psych hospitals when I was suicidal, he got me in to see him on spur of the moment appointments to treat flu’s and tonsillitis so that a GP didn’t mess me up even more, he diagnosed me with Lyme Disease. Most of what I know about my body and these illnesses are because of him. 
My first 2 years of being chronically ill consisted of spending tens of thousands of dollars on testing and trialling supplements and medications. I did a tonne of research and learnt about “pacing” and other things I could do on my own to help my body repair. I did everything I was supposed to and my health slowly built back up to 80%. I went back to work part-time, back to Uni, I could leave my house almost everyday and things were getting better. Having a diagnosis of CFS without Lyme Disease (because it doesn’t exist in Australia), meant that the cause of my CFS was not being treated and therefore just a ticking time bomb. Of course I didn’t know this and neither did my doctor. 
So when I relapsed HARD, it was a complete shock to my doctor and myself. My heart became so bad I couldn’t shower standing up anymore, I couldn’t lift my arms above my head to wash my own hair, sitting up for longer than 10 minutes at a time was impossible, talking or paying attention to someone speaking to me was exhausting, looking at my phone or a computer made me feel incredibly ill and I was lucky to sleep 2 hours a night. 
It was terrifying to be this ill and not no why. I ended up on 5 injections, 52 tablets, oxygen therapy for 4 hours and drinking litres of special liquids every day. Without all these I would have been far worse than mentioned, but I wasn’t getting any better. 
You wouldn’t believe some of the tests I’ve done. I’ve peed into a huge jug over a 24 hour period and carried it into a pathology (multiple times), I’ve had to poo into a container and scrape pieces of it out to go into special containers and sent that off, I had to pee in complete darkness into a special container covered in foil because if any light hit my urine it would destroy whatever needed to be tested. Boy was that something I’ll never forget. I had to do it in my bedroom as I had blinds to make my room pitch black. Due to how sick I was, I wasn’t well enough to “squat” and hold my own weight, so I needed my mum in there to help hold me and being a girl you can’t really get your pee flowing unless you’re at least squatting. And how do you aim your pee as a female when you’re in total darkness? I’ll leave that one to your imagination. Being in my bedroom and not at a toilet wasn’t triggering whatever part of my brain was needed to tell my bladder to let it out and I possibly had stage fright from having someone standing there waiting for me to “go”. So it took some time and both my mum and I were getting quite frustrated. Then there was the logistics of finding the toilet paper, wiping, putting it into a bag once I’d finished and hoping I didn’t get any on the floor- all in pitch-black darkness. The things parents do for their kids huh? 
For you folks who hate needles, I could have 10 viles taken at each appointment if needed, I have permanent needle marks on my arm where blood gets taken because it happens so often. I also did a spit test, where I had to spit into this test tube and fill it to the top within a certain time frame. It was actually bloody hard to meet that deadline as I wasn’t allowed to drink for ages beforehand so I was dehydrated. My Dr also does a standing test for POTS (postural orthostatic tachycardia syndrome), which is one of my heart conditions. This is really horrible when you’re so sick as standing completely still for 20 minutes can be near impossible. Some patients faint before the 20 minutes are up due to lack of blood flow to their brain. I never fainted luckily, but my blood pressure would drop to something ridiculous like 45/60 and my heart rate would shoot up to 160bpm. I’d become very disoriented and brain-fogged due to oxygen not getting to my brain, my legs would shake with fatigue, I found it difficult to breathe let alone speak (the nurse would keep asking you how you’re feeling) as my heart was beating as if I was running.
Hydrogen breath tests suck too, for people that have done those. You go into a specific clinic, drink this disgusting liquid and then have to sit around for hours and have your breath tested every hour to see if you have an intolerance to foods like lactose and fructose. Being that I am intolerant, drinking a liquid containing things I’m intolerant to gave me gastro like symptoms, running to the toilet throughout my testing and feeling very bloated, nauseous and just yuck. Gastroscopy, colonoscopy, endoscopy, liver & kidney scans (very painful when they’re inflammed), I’ve been lucky enough to have all of those too!  
Then there’s the trialling of all the medications. Because when you have illnesses that are incurable and largely misunderstood, even regarded as not existing, you are essentially a guinea pig. It’s not only that, but every single CFS or Lyme patient has different symptoms and different biologies, which means all our treatments must be individualised and what works for one might not work for another. My body is also very sensitive to medications (it runs in my family) and having a bad reaction to things is common for my mum, brother and I. Every doctors appointment became a Russian roulette of new medications or supplements. “This test shows your body can’t absorb potassium, take 7 of these a day/ we found your stomach can’t produce acid to digest food, take these with every meal/ your mitochondria can’t produce energy, take this injection daily/ you’re chronically dehydrated, drink 2L of this a day/ you’re not producing the hormone that helps you sleep, take this highly addictive sleeping tablet plus these horrible tasting drops just before bed/ your blood pressure is too low, take this beta blocker.” The list just goes on and on. 
My body is like a Picasso- a jumbled mess, a masterful fuck-up, where down is up and up is down (actually this is sounding more like Dr. Suess). Nothing in my body makes sense or works how it should. So when I trial a medication that I react badly to, thats where you get side effects on top of chronic illness, I have to wean off it and start all over again with something different. I’ve tried hundreds of medications, supplements, tinctures, herbal remedies, whatever was recommended to me. Because I will do whatever it takes to get better and to just feel better. But the more I tried, the lower my success rate of finding things that actually worked. 
New doctors wouldn’t understand how seriously complex I am. A kinesiologist put me on some tinctures that made me seriously ill. My CFS Dr was furious because she wouldn’t have understood that putting me THAT b12 concoction would then irritate this part of my illness and he would never have let me take something like that. Then my Dr’s own colleague tried to treat me and she even made me really ill by telling me not to take certain things and trying others things, as she didn’t know my history for the last 6 years and wouldn’t have had time to read through my hundreds of tests and doctors notes over that time. Again my Dr was angry. This doesn’t make her negligent either, we are just so complex and confusing to the majority of the medical community, can you begin to understand why we are shunned as hypochondriacs or put in the “too hard” basket? 
This is what makes my CFS doctor so amazing, he never gives up on us. It can actually be traumatic for patients like myself throughout our fight to get better. We aren’t even heard, listened to or taken seriously. I had to yell and argue with my GP when I first got sick, just to get him to write me a referral to a CFS doctor because he didn’t “believe in it” and said I must just have low iron or something (he already tested it and it was within the spectrum). He even WORKED with a CFS doctor, so he had an easy connection and getting that referral was like extracting teeth. Years into my diagnosis he would then argue with me over prescriptions and why I needed to take certain things. “7 potassium tablets a day, you should be dead!” Yet I’d have my prescription bottles with me for proof. “Wow you’ve lost so much weight, you look great, are you working again now?” Would be his opening line to me as I slowly dragged myself into his office with a walking stick, barely able to stand or hold my own head up. I don’t think he’s a moron either (well he kind of is), but this is the general consensus of how people with my illnesses are treated by the people that should be helping us the most. So if I’m being frank with you here, the majority of the medical community are morons when it comes to illnesses like mine. They’re brilliant in many other ways, but that doesn’t help me or change my story. 
Can you imagine a cancer patient getting a positive test result or scan of a tumour and being told to just take some panadol (pain reliever), get out more and see a psychiatrist? Or you find a lump in your breast and want a scan, your doctor tells you you’ve imagined the lump, probably from stress or depression and that fatigue is because you need to exercise and lose some weight, then your imaginary lump will disappear. Later you find out your body is riddled with cancer, how angry would you be? My body is riddled with billions of deadly and destructive bacteria because of this ignorance and negligence towards certain illnesses. Can you imagine my anger when I was diagnosed with LATE STAGE Lyme Disease at age 24 and I should have been diagnosed in the hospital when I presented very clear symptoms at age 16. Instead the doctors found me a novelty, they kept coming in all night and gawking at me like a medical marvel. “What the hell is wrong with her? Those are some fucked up symptoms!” is what I imagine the talk outside my door would have been. Lyme Disease is more common than HIV/AIDS, it is not a unique or unheard of disease in the medical world. 
Anyway, so I was FINALLY diagnosed with Lyme Disease in 2013, better late than never, through overseas testing that costs upwards of $1,500-$2,000. So again, you better be rich if you want to get anywhere with treating this bastard illness. This is where the fun begins!

Obtaining and Filling Psychiatrists’ Medication Prescriptions Constitute Obstacles to Treatment of Bipolar Disorder (1) and (2). This post is to cooperate with my sister, Kim Holland, MD, in publicizing Mental Health Awareness Month. Many people who are diagnosed with Bipolar Disorder often need to maintain a measurable level of lithium and/or other substances in their blood in order to treat and avoid recurrence of bipolar symptoms. Consequences of failing to maintain blood serum levels of prescribed medications can include sleeplessness, emotional lability (instability), mood swings, depression and even psychosis (in those with Bipolar Type 1). In most countries many or all of the medications that treat Bipolar 1 and Bipolar 2 are available by prescription only.   In my case (Bipolar 2) case, my prescribed Quetiapine/Seroquel and Escitalopram/Lexapro are available in the Dominican Republic without prescription.   Xanax (crucial for sleep), however, is legally sold here and in most countries by prescription only.  (In the USA, all of my prescribed medications are sold by prescription only.) Even when bipolarity medications are available without prescription, they can cost hundreds of dollars per month, which will not be reimbursed by insurance companies unless prescribed by an authorized physician. I have no medical insurance.  I self-pay my medications in the Dominican Republic at an approximate cost of $100 USD per month (but the same medications might cost $500/month in the USA). Where medications are available by physician prescription only, the patient must have sufficient money and/or insurance to see a physician authorized to prescribe psychoactive medication.   Exceptions include socialized medicine countries such as Brazil, where government-proviided psychiatric visits are free but must be scheduled days or weeks in advance. In Brazil and in the Dominican Republic, only psychiatrists have the forms necessary to prescribe some or all psychoactive medicines, e.g. the Xanax that I take. Psychiatrists in the USA often have months-long waiting lists.  Patients are often required to see a social worker and/or primary care physician before seeing a psychiatrist. As a result, managing to schedule and pay for psychiatric visits in order to obtain medication can be as challenging as any other aspect of managing Bipolar Disorder. Money can be a significant obstacle to treatment.  In the USA, one visit to a psychiatrist can cost $200.00 USD (US Dollars).  That is how much a person with $660.00 USD of monthly income would be expected to be able to pay monthly for rent, or half the advisable rent of an individual living at the poverty level. In the Dominican Republic, a visit to a typical private-practice psychiatrist costs $60.00 USD, which is about half of the Dominican Republic’s minimum wage.  $60.00 USD is also about one-third of my monthly rent payment or a quarter of my monthly food budget. Bipolar Disorder, like diabetes, must be managed regularly by a physician because the medications used to treat it often cause obesity and diabetes, which can lead to liver damage and other irreversible illness if undiagnosed and  unmanaged.  Lithium, a common Bipolar medication (that I do not use) must be  blood tested at least monthly to avoid liver damage. Nonetheless, Bipolar Disorder patients in the USA regularly report being unable to schedule regular psychiatry visits, even when paying in cash.  Some rural and remote US communities with tens of thousands in population have no psychiatrists at all. Fortunately for me, I can schedule a visit with a private practice psychiatrist in the Dominican Republic within a three days, as long as I can self-pay. The obstacles I face in obtaining prescriptions and medication compliance are: Traveling five hours each way to my psychiatrist’s office and back. Scheduling the psychiatrist visits so that I can travel to the capital, visit the psychiatrist and return home in one 24-hour period. Paying $40.00 USD for transportation, not including the costs of owning and operating a motorcycle. Paying the psychiatrist’s $60.00 USD fee. Paying $100.00 USD for the medication.. Finding each of three medications in the capital during the one-day trip at the least cost. Coordinating with a friend to accompany and drive me to pharmacies in the capital. Preparing financially and scheduling a month in advance to visit the capital before medications run out. I am told there is a doctor an hour from me who will write a prescription in exchange for $10.00 USD, so long as I already know precisely what I need. I am also told that one psychiatrist in the public hospital an hour from me might write a prescription for free, if I know precisely what I need.  In Brazil, this service was so much in demand that emergency room doctors frequently refused to do this at all. Although it is not strictly lawful, it is reported that, in some cases and places, it may possible to find someone willing to sell a limited amount of a restricted medication without a prescription, but only if a legitimate emergency is demonstrated to the professional’s satisfaction on a one-time basis. Considering that the lack of medication may compromise a Bipolar person’s problem-solving skills, the financial and practical obstacles to simply obtaining prescriptions can easily prevent a Bipolar person from maintaining medication compliance, with serious consequences.

Pneumonia is not one of the more well-known diseases such as liver problems, a heart attack, or an accident that can cause patients to be admitted to intensive care. But it is different when pneumonia in aids patients happen. Pneumonia is one of the top reasons why patients with AIDS/HIV are treated in intensive care. More than 75% of Aids patients will develop Pneumocystis Carinii Pneumonia (PCP) unless action is taken to counter the disease. PCP can be a major threat to Aids sufferers as it takes advantage of a weakened immune system. HIV, the virus that causes Aids weakens the immune system.
Pneumonia
Pneumonia is often considered to be a disease that primarily affects elderly people, and in the general population that is certainly the case. Often an elderly patient sickens with other issues and once they are weak, along comes pneumonia to take advantage of their reduced immunity.
Tens of thousands of pneumonia sufferers in the USA die every year. The majority of these will be aged over 65.

The most common cause is the bacteria Streptococcus Pneumoniae (pneumococcus).

The symptoms of pneumonia include fever, chills, coughing that produces phlegm, short breath, and pains in the chest area.
In some cases, pneumonia can be treated at home with antibiotics. However, occasionally cases require hospital treatment.
Home treatment includes aspirin, ibuprofen, and Tylenol; together with cough medicine to make the patient more comfortable by reducing the coughing.
Since people carrying HIV already have compromised immune systems they should immediately seek medical help. Pneumonia in Aids patients is potentially fatal, as it is with sufferers of the following medical conditions: diabetes, asthma, and heart problems. Those over 65 are also at greater risk from the disease.

Vaccine shows promise against Klebsiella superbug
Scientists have produced and tested, in mice, a vaccine that protects against a worrisome superbug: a hypervirulent form of the bacteria Klebsiella pneumoniae.
Klebsiella pneumoniae causes a variety of infections including rare but life-threatening liver, respiratory tract, bloodstream, and other infections. Little is known about how exactly people become infected, and the bacteria are unusually adept at acquiring resistance to antibiotics. The prototype vaccine, the details of which appear in Proceedings of the National Academy of Sciences, may offer a way to protect people against a lethal infection that is hard to prevent and treat.
“For a long time, Klebsiella was primarily an issue in the hospital setting, so even though drug resistance was a real problem in treating these infections, the impact on the public was limited,” says coauthor David A. Rosen, an assistant professor of pediatrics and of molecular microbiology at Washington University in St. Louis. “But now we’re seeing Klebsiella strains that are virulent enough to cause death or severe disease in healthy people in the community. And in the past five years, the really resistant bugs and the really virulent bugs have begun to merge so we’re beginning to see drug-resistant, hypervirulent strains. And that’s very scary.”
Superbugs spread globally
Hypervirulent strains of Klebsiella caused tens of thousands of infections in China, Taiwan, and South Korea last year, and the bacteria are spreading around the world. About half of people infected with hypervirulent, drug-resistant Klebsiella die. Two types in particular—known as K1 and K2—are responsible for 70% of the cases.
The goal is to get a vaccine ready for human use before the hypervirulent strains start causing disease in even larger numbers of people.
The researchers decided to create a vaccine against the two most common strains of hypervirulent Klebsiella. The bacterium’s outer surface is coated with sugars so the researchers designed a glycoconjugate vaccine composed of these sugars linked to a protein that helps make the vaccine more effective. Similar vaccines have proven highly successful at protecting people against deadly diseases such as bacterial meningitis and typhoid fever.
“Glycoconjugate vaccines are among the most effective, but traditionally they’ve involved a lot of chemical synthesis, which is slow and expensive,” says Christian Harding, a cofounder of VaxNewMo, a St. Louis startup. “We’ve replaced chemistry with biology by engineering E. coli to do all the synthesis for us.”
The researchers genetically modified a harmless strain of E.coli, converting it into tiny biological factories capable of churning out the protein and sugars needed for the vaccine. Then they used another bacterial enzyme to link the proteins and sugars together.
To test the vaccine, the researchers gave groups of 20 mice three doses of the vaccine or a placebo at two-week intervals. Then they challenged the mice with about 50 bacteria of either the K1 or the K2 type. Previous studies had shown that just 50 hypervirulent Klebsiella bacteria are enough to kill a mouse. In contrast, it takes tens of millions of classical Klebsiella—the kind that affects hospitalized people—to be similarly lethal.
Fighting antibiotic resistance
Of the mice that received the placebo, 80% infected with the K1 type and 30% infected with the K2 type died. In contrast, of the vaccinated mice, 80% infected with K1 and all of those infected with K2 survived.
“We are very happy with how effective this vaccine was,” says first author Mario Feldman, associate professor of molecular microbiology. “We’re working on scaling up production and optimizing the protocol so we can be ready to take the vaccine into clinical trials soon.”
The goal is to get a vaccine ready for human use before the hypervirulent strains start causing disease in even larger numbers of people.
“As a pediatrician, I want to see people get immunity to this bug as early as possible,” Rosen says. “It’s still rare in the United States, but given the high likelihood of dying or having severe debilitating disease, I think you could argue for vaccinating everybody. And soon we may not have a choice. The number of cases is increasing, and we’re going to get to the point that we’ll need to vaccinate everybody.”

Legal Rights for Lake Erie? Voters in Ohio City Will Decide
Lake Erie has faced a string of environmental calamities — poisonous algal blooms, runoff containing fertilizer and animal manure, and a constant threat from invasive fish, many of them attributed to business. If the lake gets legal rights, the theory goes, people can sue polluters on its behalf. Should Lake Erie, which supports the economies of four states, one Canadian province, and the cities of Toledo, Cleveland and Buffalo, have the legal right “to exist, flourish, and naturally evolve”: (1) Yes, (2) No? Why? What are the ethics underlying your decision?
The failing health of Lake Erie, the world’s 11th largest lake, is at the heart of one of the most unusual questions to appear on an American ballot: Should a body of water be given rights normally associated with those granted to a person?
Voters in Toledo, Ohio, will be asked this month to decide whether Lake Erie, which supports the economies of four states, one Canadian province and the cities of Toledo, Cleveland and Buffalo, has the legal right “to exist, flourish, and naturally evolve.”
The peculiar ballot question comes amid a string of environmental calamities at the lake — poisonous algal blooms in summer, runoff containing fertilizer and animal manure, and a constant threat from invasive fish. But this special election is not merely symbolic. It is legal strategy: If the lake gets legal rights, the theory goes, people can sue polluters on its behalf.
The proposed Lake Erie Bill of Rights is part of a growing number of efforts to carve out legal status for elements of nature, including rivers, forests, mountains and even wild rice. The efforts, which began decades ago but have gathered momentum in recent years, seek to show that existing laws are insufficient to protect nature against environmental harm. Under current law, lakes and deserts do not have legal standing, so people cannot sue on their behalf.
In Toledo, residents and elected officials say they believe the initiative has a good chance of being approved, but there is a catch: The measure’s own backers acknowledge that it is likely to be challenged in court as having little or no legal footing, and that it could ultimately be invalidated as reaching beyond the scope of city law.
The initiative’s main opponents are the owners of area farms, where much of the agricultural runoff that feeds the lake’s toxic algaeoriginates. Farmers say that if the measure passes thousands of small farms could be sued for damages for polluting the lake and driven out of business.
Thomas Linzey, executive director of the Community Environmental Legal Defense Fund, a nonprofit group based in Pennsylvania that helped write the measure, said existing environmental laws were inadequate.
The intent of the initiative, Mr. Linzey said, is twofold — to send a warning that the community is fed up with a lack of state and federal action to protect Lake Erie, and to force the courts to recognize that ecosystems like the lake “possess independent rights to survive and be healthy.”
In other words, that rivers have a right to flow, forests have a right to thrive and lakes have a right to be clean.
Even if that concept never becomes the law of the land, the group says that its efforts are meant to make it clear that places like Toledo will oppose what they see as environmental degradation, sending an unsubtle message that certain companies might want to look elsewhere to do business.
The broader idea, environmentalists say, is a rethinking of nature and an individual’s place in it.
“There’s no precedent for any of this,” Mr. Linzey said. “It is almost a new consciousness — that a community is not just Homo sapiens.”
As the Feb. 26 Election Day approaches, some Toledo residents say their dependence on Lake Erie has made the question of the lake’s rights more than theoretical.
In 2014, the city went without drinking water for three days when the lake became so fouled by phosphorus runoff from upstream farms that household water was fit only for flushing toilets.
Stores closed. Hospitals accepted only the most seriously ill patients. Restaurants were empty. And some 500,000 people depended on bottled water in the middle of a brutally hot August.
“The city of Toledo shut down,” said Crystal Jankowski, 31, who was in a hospital delivering her daughter during the water crisis. “They were having to cancel surgeries because they couldn’t sterilize equipment.”
While the idea of nature having rights has been around for centuries, environmental advocates point to a 1972 United States Supreme Court decision, Sierra Club v. Morton, as providing much of the impetus for current efforts.
In that case, the court ultimately rejected the notion of nature’s rights, but Justice William O. Douglas wrote in a dissent that “contemporary public concern for protecting nature’s ecological equilibrium should lead to the conferral of standing upon environmental objects to sue for their own preservation.”
The recent push started in earnest about 15 years ago, when Mr. Linzey’s group began working with towns and cities to protect local natural resources. The concept has won support in dozens of municipalities — from cities with long histories of environmental activism to unexpected places.
Tamaqua Borough, Pa., in the center of the state’s historic coal-mining region, was the first place in the nation to approve a rights-of-nature ordinance in 2006 after it banned companies from dumping dredged minerals and sewage sludge into open pit mines.
The bill approved by the borough council included language that said corporations could not “interfere with the existence and flourishing of natural communities or ecosystems, or to cause damage” to them within the township.
Four years later, Pittsburgh approved a rights-of-nature ordinance that prohibited fracking in city limits.
Santa Monica, Calif., has since passed an ordinance that requires the city to “recognize the rights of people, natural communities and ecosystems to exist, regenerate and flourish.”
And earlier this year, the White Earth Band of Ojibwe in Minnesota announced that it had granted wild rice its own legal rights, including “the right to pure water.”
The Community Environmental Legal Defense Fund, which has been involved in each of the efforts, said similar laws were now on the books in municipalities in Maine, Maryland, New Hampshire, New Mexico, New York, Ohio and Virginia.
In Toledo, much of Lake Erie is frozen over this winter. But nearly every summer for the last several years, water runoff from large farms flowed into the lake and produced a slick green coating of toxic algae so expansive that it could be seen from space.
The 2014 water crisis failed to spur a state or federal crackdown on large farms in the Maumee River basin, where fertilizers used for crops contain phosphorus, which is washed into streams and eventually the river during rainstorms.
Once in the lake, the phosphorus feeds the algae, which produces microcystin, a toxin that causes diarrhea, vomiting and liver-function problems in humans and can kill small animals — including dogs — that ingest it.
Yvonne Lesicko, vice president for public policy at the Ohio Farm Bureau, acknowledged that farms were the primary culprit, but also pointed to other sources of harmful runoff like lawns, septic systems and golf courses.
Ms. Lesicko said farmers have sought to limit the amount of runoff, but estimated that measures like using less fertilizer and building berms may take as long as 15 or 20 years to produce results.
The ballot initiative, she said, was not a reasoned response to a complex problem.
“We care very much about the lake,” Ms. Lesicko said. “But this is not a solution. In fact, it is counterproductive. This is going to lead to lots of lawsuits and stress.”
The issue has also put Toledo’s elected officials in an unenviable position. They say that if they oppose the measure, they fear they will appear to support polluting the lake.
But backing the initiative could be worse, some reason, because the measure could cost the city tens of thousands of dollars to defend in court with no guarantee that it will survive legal review. Toledo, still heavily reliant on the automobile industry, has struggled economically in recent years.
“I think everybody is frustrated,” said Nick Komives, a City Council member, who said he supported a clean lake, but opposed the measure because it would most likely drain city finances. “The initiative is a powerful tactic. And for me, coming from an activist background, it is important to send messages. But this is probably unconstitutional.”

In most countries many or all of the medications that treat Bipolar 1 and Bipolar 2 are available by prescription only.   In my case (Bipolar 2) case, my prescribed Quetiapine/Seroquel and Escitalopram/Lexapro are available in the Dominican Republic without prescription.   Xanax (crucial for sleep), however, is legally sold here and in most countries by prescription only.  (In the USA, all of my prescribed medications are sold by prescription only.) Even when bipolarity medications are available without prescription, they can cost hundreds of dollars per month, which will not be reimbursed by insurance companies unless prescribed by an authorized physician. I have no medical insurance.  I self-pay my medications in the Dominican Republic at an approximate cost of $100 USD per month (but the same medications might cost $500/month in the USA). Where medications are available by physician prescription only, the patient must have sufficient money and/or insurance to see a physician authorized to prescribe psychoactive medication.   Exceptions include socialized medicine countries such as Brazil, where government-proviided psychiatric visits are free but must be scheduled days or weeks in advance. In Brazil and in the Dominican Republic, only psychiatrists have the forms necessary to prescribe some or all psychoactive medicines, e.g. the Xanax that I take. Psychiatrists in the USA often have months-long waiting lists.  Patients are often required to see a social worker and/or primary care physician before seeing a psychiatrist. As a result, managing to schedule and pay for psychiatric visits in order to obtain medication can be as challenging as any other aspect of managing Bipolar Disorder. Money can be a significant obstacle to treatment.  In the USA, one visit to a psychiatrist can cost $200.00 USD (US Dollars).  That is how much a person with $660.00 USD of monthly income would be expected to be able to pay monthly for rent, or half the advisable rent of an individual living at the poverty level. In the Dominican Republic, a visit to a typical private-practice psychiatrist costs $60.00 USD, which is about half of the Dominican Republic’s minimum wage.  $60.00 USD is also about one-third of my monthly rent payment or a quarter of my monthly food budget. Bipolar Disorder, like diabetes, must be managed regularly by a physician because the medications used to treat it often cause obesity and diabetes, which can lead to liver damage and other irreversible illness if undiagnosed and  unmanaged.  Lithium, a common Bipolar medication (that I do not use) must be  blood tested at least monthly to avoid liver damage. Nonetheless, Bipolar Disorder patients in the USA regularly report being unable to schedule regular psychiatry visits, even when paying in cash.  Some rural and remote US communities with tens of thousands in population have no psychiatrists at all. Fortunately for me, I can schedule a visit with a private practice psychiatrist in the Dominican Republic within a three days, as long as I can self-pay. The obstacles I face in obtaining prescriptions and medication compliance are: Traveling five hours each way to my psychiatrist’s office and back. Scheduling the psychiatrist visits so that I can travel to the capital, visit the psychiatrist and return home in one 24-hour period. Paying $40.00 USD for transportation, not including the costs of owning and operating a motorcycle. Paying the psychiatrist’s $60.00 USD fee. Paying $100.00 USD for the medication.. Finding each of three medications in the capital during the one-day trip at the least cost. Coordinating with a friend to accompany and drive me to pharmacies in the capital. Preparing financially and scheduling a month in advance to visit the capital before medications run out. I am told there is a doctor an hour from me who will write a prescription in exchange for $10.00 USD, so long as I already know precisely what I need. I am also told that one psychiatrist in the public hospital an hour from me might write a prescription for free, if I know precisely what I need.  In Brazil, this service was so much in demand that emergency room doctors frequently refused to do this at all. Although it is not strictly lawful, it is reported that, in some cases and places, it may possible to find someone willing to sell a limited amount of a restricted medication without a prescription, but only if a legitimate emergency is demonstrated to the professional’s satisfaction on a one-time basis. Considering that the lack of medication may compromise a Bipolar person’s problem-solving skills, the financial and practical obstacles to simply obtaining prescriptions can easily prevent a Bipolar person from maintaining medication compliance, with serious consequences.
