I headcanon that when izuna gets very sick, he gets insanely affectionate and hugs and kisses everyone that appears infront of him while he lists his favourite things about that person, a trait that izuna never has when sober.

So one day, when he was 7, on December, specifically madadas' birthday, he wouldn't let go of his brother. He would also scream very loudly about how madara is the best brother and how much he loves him. Somewhere in between kissing his face, madara tries to move away, which ends up in izuna kissing him on the lips.

This was unacceptable, and madaras shocked yet terrified face meets that of izuna that is giggling at his brother funny expression.

Later, Izuna would learn that threatening to kiss on the lips was the only threat that worked on Madara.
