1. Nhồi máu cơ tim là gì?
Nhồi máu cơ tim là tình trạng cơ tim bị hoại tử do thiếu máu cục bộ, làm giải phóng các chất có trong tế bào cơ tim vào trong máu. Các dấu hiệu nhận biết nhồi máu cơ tim sẽ giúp cho bệnh nhân được cấp cứu kịp thời, còn các xét nghiệm sẽ giúp chẩn đoán chính xác bệnh nhân có bị nhồi máu cơ tim hay không và mức độ tổn thương như thế nào.
Do đó mọi người cần biết các dấu hiệu nhận biết nhồi máu cơ tim sớm. Các dấu hiệu cảnh báo nhồi máu cơ tim ở mỗi người sẽ có biểu hiện và mức độ khác nhau, bên cạnh đó các dấu hiệu cũng dễ nhầm lẫn với các bệnh khác như bệnh đường hô hấp, tình trạng suy nhược cơ thể. Tuy nhiên, các bệnh nhân nhồi máu cơ tim vẫn có những dấu hiệu chung, có hai dấu hiệu mà bệnh nhân nhồi máu cơ tim nào cũng có thể có, đó là:
Bệnh nhân đột nhiên cảm thấy mệt mỏi, cảm giác khó thở ngay cả khi làm những việc mà trước kia vẫn làm được bình thường.
Người bệnh cảm thấy bồn chồn, lo lắng, bất an.

Biểu hiện của cơn nhồi máu cơ tim
Ngoài ra, bệnh nhân có thể gặp một số triệu chứng sau:
Đau ngực trái có thể lan tỏa tới hàm, tới bờ trong cánh tay. Triệu chứng này đôi khi không điển hình, hoặc có người không có biểu hiện đau.
Chóng mặt, đau đầu.
Buồn nôn, nôn mửa, khó tiêu.
Rối loạn giấc ngủ.
Vã mồ hôi.
Rối loạn thị giác.
Tê ngứa ở bàn tay hoặc cánh tay, chán ăn.
Phụ nữ và nam giới sẽ có những dấu hiệu cảnh báo khác nhau như ở phụ nữ sẽ ít gặp triệu chứng đau ngực, nhưng các triệu chứng mệt mỏi, lo lắng, đau đầu, chóng mặt lại xuất hiện nhiều hơn.
Tuy nhiên, đây chỉ là những triệu chứng cảnh báo nhồi máu cơ tim, chứ không thể khẳng định bạn có bị nhồi máu cơ tim hay không. Để chẩn đoán xác định cần dựa vào các xét nghiệm phát hiện nhồi máu cơ tim.
2. Các xét nghiệm phát hiện nhồi máu cơ tim
2.1. Điện tâm đồ
Điện tâm đồ là một phương pháp cận lâm sàng rất có giá trị. Song nhiều khi điện tâm đồ không đặc hiệu và không rõ. Hình ảnh điện tâm đồ của bệnh nhân nhồi máu cơ tim sẽ có sự biến đổi như sau:
Đoạn ST chênh lên hoặc chênh xuống
Sóng T đảo ngược
Xuất hiện sóng Q mới
Hoặc block nhánh trái mới được phát hiện
2.2. Siêu âm tim
Siêu âm tim rất có giá trị trong chẩn đoán nhồi máu cơ tim, nó có thể giúp ta nhìn thấy những hình ảnh rối loạn vận động vùng liên quan đến vị trí nhồi máu như: huyết khối buồng tim, dịch màng tim,...
Đồng thời siêu âm tim cũng giúp đánh giá chức năng của tâm thất trái và các biến chứng cơ học của nhồi máu cơ tim như: hở van tim do đứt dây chằng, hay thủng vách tim gây thông liên thất.

2.3. Chụp động mạch vành (Coronary Angiography)
Phương pháp chụp động mạch vành là tiêu chuẩn vàng cho phép khẳng định chắc chắn bệnh nhân có nhồi máu cơ tim. Sau khi chụp mạch vành và phát hiện vùng tắc động mạch gây nhồi máu cơ tim, các bác sĩ sẽ tiến hành đặt stent để tái thông dòng chảy mạch vành và cứu sống vùng cơ tim đang bị nhồi máu. Đây là thủ thuật xâm lấn tối thiểu và khá an toàn cho bệnh nhân.
2.4. Các xét nghiệm hóa sinh
Các xét nghiệm hóa sinh rất cần thiết và có giá trị cao trong việc chẩn đoán và theo dõi nhồi máu cơ tim. Cho đến nay có 3 thế hệ các xét nghiệm hóa sinh được áp dụng để chẩn đoán nhồi máu cơ tim đó là:
Thế hệ 1:
Creatin kinase (CK) toàn phần
Aspartat amino transferase (ASAT)
Alanin amino tranferase (ALAT)
Lactat dehydrogenase (LDH)
Thế hệ 2: CK-MB và Myoglobin
Thế hệ 3: Các protein đặc trưng của cơ tim (Troponin)
Các xét nghiệm này lại được chia thành 2 loại:
Các xét nghiệm enzyme và isoenzyme: CK, LDH, ASAT, ALAT, CK-MB,...
Các xét nghiệm protein: CK-MB mass, Myoglobin, Troponin T, Troponin I,...
Việc chẩn đoán và đánh giá mức độ nhồi máu cơ tim căn cứ vào động học của các chất này trong huyết tương:
Giải phóng ra khỏi tổ chức: lúc đầu giải phóng chậm, sau nhanh, kích thích ổ nhồi máu, nồng độ, độ hòa tan và nơi khu trú của các chất quyết định thời gian xuất hiện của chúng ở trong máu.
Khuếch tán vào máu: do vị trí của cơ tim không gần với lưới mao mạch cho nên thời gian kể từ khi tế bào bị tổn thương cho tới khi các chất xuất hiện trong máu phải mất hàng giờ.
Thải trừ ra khỏi máu: tốc độ thải trừ của các chất trên là khác nhau.

2.4.1. Các xét nghiệm enzyme và isoenzyme
2.4.2. Các xét nghiệm protein
2.4.2.1. Myoglobin
Myoglobin là một protein có trong bào tương của cơ tim, ngoài ra nó cũng có trong cơ xương. Myoglobin có các đặc điểm sau:
Là protein chứa Hem gắn O2, có khối lượng phân tử là 17.800 Da, nó có vai trò vận chuyển và dự trữ oxy trong tế bào cơ.
Là protein bào tương, chiếm khoảng 2% toàn bộ protein tế bào.
Có khối lượng nhỏ, do đó khi có tổn thương nhồi máu cơ tim thì myoglobin sẽ sớm xuất hiện ở huyết thanh nên có thể sử dụng để chẩn đoán sớm.
Thời gian bán hủy nhanh, chỉ từ 10 - 20 phút.
Để xác định Myoglobin người ta sử dụng phương pháp miễn dịch sử dụng kỹ thuật điện hóa phát quang (electroluminescence).
Bình thường Myoglobin có nồng độ trong huyết tương < 70 - 110 mcg/L. Nồng độ Myoglobin trong huyết tương cũng phụ thuộc vào độ lọc cầu thận, khi lượng Myoglobin huyết tương tăng > 200 mcg/L sẽ xuất hiện Myoglobin niệu.

Xét nghiệm Myoglobin tăng sau nhồi máu cơ tim
Ý nghĩa lâm sàng của chỉ số xét nghiệm Myoglobin:
Nồng độ Myoglobin tăng rất sớm, khoảng 2 giờ sau khi nhồi máu cơ tim cấp, nhồi máu cơ tim thứ phát hoặc khi tưới máu lại thành công sau liệu pháp streptolysin.
Nồng độ Myoglobin trong huyết tương đạt cực đại sau 4 - 12 giờ, trở về bình thường sau 24 giờ.
Chỉ số này giúp chẩn đoán sớm nhồi máu cơ tim nhưng không đặc hiệu vì nó còn tăng trong các tổn thương cơ xương.
2.4.2.2. CK-MB mass
CK-MB mass là một isoenzym của CK, được cho là CK đặc hiệu cho tim.
CK-MB có thể được xác định bằng phương pháp miễn dịch sử dụng kỹ thuật enzyme, hóa phát quang, huỳnh quang, hoặc điện hóa phát quang. CK-MB được xác định bằng sự kết hợp của nó với các kháng thể đặc hiệu CK-B và CK-M hoặc với kháng thể đặc hiệu của CK-MB.
Có hai phương pháp xác định CK-MB đó là:
Phương pháp đo hoạt độ (UZ/L)
Phương pháp đo khối lượng (mg/mL): phương pháp này đặc hiệu hơn phương pháp đo hoạt độ.
Bình thường nồng độ CK-MB trong huyết tương là < 5,3 - 8 mcg/L, tùy theo test kit.
CK-MB có ý nghĩa lâm sàng như sau:
Nồng độ tăng 4-5 giờ sau cơn đau thắt ngực.
Đây là một xét nghiệm được sử dụng trong chẩn đoán nhồi máu cơ tim và tưới máu lại.
2.4.2.3. Troponin
Troponin là một phức hợp protein có hình cầu nằm trong các sợi mảnh của cơ tim, nó tham gia vào quá trình điều hòa sự co cơ tim. Phức hợp này gồm ba thành phần đó là Troponin C, Troponin T, Troponin I. Trong đó Troponin C gắn kết với canxi và nó có cả ở cơ tim và cơ xương, còn Troponin T và Troponin I là dạng đặc hiệu của cơ tim.
Khi cơ tim bị hoại tử, Troponin sẽ được phóng thích vào trong máu, do đó xét nghiệm phát hiện Troponin I hoặc T trong máu đóng vai trò quan trọng để chẩn đoán chính xác nhồi máu cơ tim.
Troponin I (cTnI)
Nồng độ troponin I trong huyết tương bình thường là < 0,1-0,2 μg/L.

Ý nghĩa lâm sàng của chỉ số Troponin I:
Khi bị nhồi máu cơ tim:
Troponin I tăng cùng với Troponin T sau cơn đau 3-4 giờ và đạt giá trị cực đại ở 12 - 24 giờ, điều này còn phụ thuộc vào sự tái tưới máu của động mạch vành.
Độ nhạy của Troponin I giống Troponin T trong pha đầu của nhồi máu cơ tim cấp, tăng cùng với CK-MB, CK isoenzym, Myoglobin.
Troponin I chỉ có một đỉnh tăng ở pha đầu tiên.
Trong các bệnh tim khác:
Với bệnh nhân sau ghép tim nếu không có sự loại trừ quả tim gáp thì troponin I sẽ trở về bình thường rất nhanh, khoảng 2 - 3 tuần.
Cơn đau thắt ngực không ổn định: nồng độ troponin có liên quan đến biến chứng và tỷ lệ tử vong.
Troponin T (TnT)
Bình thường nồng độ Troponin T trong huyết tương < 0,03 ng/ml.
Ý nghĩa lâm sàng của chỉ số Troponin T:
Xét nghiệm Troponin T có độ nhạy 100% (10 giờ - 5 ngày).
Độ đặc hiệu lớn hơn CK-MB mang Myoglobin.
Chu kỳ bán hủy của troponin < 2 giờ.
Sau nhồi máu cơ tim, troponin T xuất hiện sớm trong huyết thanh, đạt giá trị cực đại thứ nhất ở ngày thứ 4 và sau đó giảm dần cho tới ngày 12, do đó có khả năng chẩn đoán trường hợp nhồi máu cơ tim đến muộn.
Troponin T nhạy cảm như CK-MB, Myoglobin ở giai đoạn sớm, khi vùng tổn thương được tái tưới máu thì troponin T huyết thanh sẽ có cực đại thứ hai ở giờ thứ 14. Chính vì vậy troponin T có khả năng giám sát tác dụng điều trị làm tan huyết khối.
Troponin T có thể đánh giá sơ bộ kích thước và mức độ nhồi máu cơ tim trên giá trị đo được ở ngày thứ 3 - 4 sau cơn đau.
Chẩn đoán xác định nhồi máu cơ tim khi TnT > 0,1ng/ml.
Chẩn đoán phân biệt nhồi máu cơ tim với chấn thương và các phẫu thuật khác, đặc biệt khi TnT tăng hằng định sau 3- 4 ngày chấn thương hoặc phẫu thuật.
