Do you mind saying anything about the level design and dream-like qualities of Dark Souls 2? I've heard a bit of it, but I don't know where to start looking it up myself (outside of getting my hands on a copy), but I'm so curious about bits like, apparently, going up an elevator in a wind mill and arriving in a volcano???

Yeah, so, from a Doyalist point-of-view, DS2 was a mess. Yui Tanimura was basically handed a director's seat and asked to salvage the botched mess that was the early build of the game, and tried his best to Frankenstein something together from the assets. The core team under Miyazaki was tied-up over in Bloodborne's development cycle.

From what people in the community (such as Lokey, SanaDSK, Dr. Pilbeam, RiverNyxx, et al) can piece together from cut dialogue and models, the original plot was a time travel story involving a scholar (called Vendrick, but otherwise dissimilar to the canon King Vendrick/Vanclad) in a more Nexus-like Majula who would send you off on a quest for two items "the Drop of Time" and the "Jewel of the Dragon" which power a magic pendulum. The Emerald Herald has a bit of Deja Vu upon seeing you, setting up a future/past meeting with a little baby Emerald Herald.

But apparently the time travel plot fell apart, and Tanimura was left having to kludge together locations which should have been past/future variants. The Windmill elevator (with a completely open sky) which leads up to a massive iron castle in Bowser's stock lava land is the most notorious example, but there are other oddities, like places that look like ruins standing right next to a pristine structure in the same architectural style, and an incredibly unnatural water-level.

You can see some of this in the introduction, as your character goes into the past using that watery vortex. However, a simpler version of time travel was used for the three "crowns" DLCs. The Watsonian explanation comes from Dark Souls 3 - when the world is about to die with the flame, the lands of the world converge on the kingdom of the heir to Gwyn. That explains, albeit somewhat unsatisfyingly, the warped spacetime of Dranglaic.
