Good and bad and better
I need you to read this through and not freak out because it has to go in order. A lot of you know that I was diagnosed in early 2022 with Stage 4A colorectal cancer which while not metastatic, was locally advanced. I went through chemo, radiation, and surgery that scooped my innards out like a pumpkin getting ready for Halloween. I entered remission on New Year's Eve of 2022 and maintained it until April of 2024.
I went in for a CT scan and they found nodule in my lung. These are nothing serious for the most part. Anyone who's had COVID, pneumonia, or bronchitis will have them and most of them go away on their own. This nodule first appeared in 2009 after viral pneumonia that knocked me out of work for a month. I am told that long-term granulomas can become cancerous or catch a stray cell from somewhere else in the body. In my case, it appears that this 'weak spot' caught a wandering adenocarcinoma cell.
Get your lungs checked, kids.
Since they could not biopsy before it grew to 8mm, they left it alone until April. It was biopsied and it was cancer.
It's been a long month. I had a PET scan and out of my whole body, from the top of my head to my purple painted toenails there is only that one little node of cancer. It has not spread anywhere. So, next steps. I will have to have it removed either by chemoembolization, laser, or resection. After that there's the possibility of prophylactic chemo and radiation for a few weeks, but not as hardcore as the first time.
The news is about as good as it gets.

Liver transplantation.
The one time you might actually want cancer.
Why? Because having a primary liver cancer bumps you up on the liver transplant list. But it can't just be any cancer - there's a criteria for acceptable cancer burden for adult liver transplant, which is known as Milan Criteria. Essentially one big tumor smaller than 5 cm or 3 small tumors less than 3 cm each. The tumor can't have left the liver either aka metastasis. However if it's too small (<2 cm ), you don't get priority listing for having that cancer. Kind of like Goldilocks - the tumor has to be just right.
Now first off, you almost never get a primary liver tumor aka hepatocellular carcinoma (HCC) without something wrong in the liver in the first place.
This is usually Cirrhosis aka fibrosis of the liver, which can be caused by Hepatitis C or more commonly drinking too much alcohol too often. The liver fails cause its cells are dead or dying, and the resulting scarring prevents blood from your GI tract from returning properly to your systemic blood. This pseudo-blockage results in fluid backing up in your body, resulting in ascites (fluid in the belly) and pleural effusion (fluid in the lung space) and generalized soft tissue swelling.
The body tries to compensate for this by making alternate paths. Problem is the liver is also a filter, and bypassing the filter through an alternate path affects the brain - a common symptom is confusion and sometimes coma in these patients. Official term is hepatic encephalopathy.
Cirrhosis patients can also literally turn yellow because bilirubin, which occurs from the normal breakdown of red blood cells, can't be processed well by a dysfunctional liver. Liver does a lot of other important stuff too, but I'd go on forever with that - so I'll end that here.
In any case, the only treatment for cirrhosis is getting a new liver - once cirrhosis occurs, it cannot reverse, only worsen. The liver transplant list uses its own criteria known as MELD which I also won't get into here, but generally the sicker you are, the higher you are on the transplant list.
Cirrhosis is essentially a breeding ground for primary liver cancer because cirrhosis is essentially repeated constant inflammation - and all it takes is one cell to heal in the absolutely worst way - and then you have a cancer. This is why cirrhosis patients get yearly liver imaging screening to detect that.
Milan Criteria was created because a good number of liver transplants in cancer patients turned out to have recurrent liver cancer in the new liver. After Milan was implemented, long-term recurrence-free survival improved from 30% to 75%. I believe the sizes used in Milan acts as a surrogate for the likelihood of microscopic metastasis, cancer that's left the liver but too small to be seen in imaging, but don't quote me on that.
Now why did I bring this up? One of the residents asked out loud about why we don't just cut out the cancer.
For one, cirrhosis ensures another one will pop up eventually, and for two, cutting a cancer out also means cutting out good tissue (or semi-okay in a cirrhotic's case) to create the disease-free margin. You don't have much good tissue left in a cirrhosis liver.
You could ablate the cancer by frying it with heat or freezing it, but you again have the issues of damaging normal tissue. Typically excision or ablation is done when there's no chance patient will get a donor liver.
There's also TACE (transarterial chemoembolization) which can be used to shrink tumors, usually back into the limits imposed by Milan criteria. Because if your cancer is too big for Milan, you don't get a liver. There are strict rules for that, and if you break them, your liver transplant service is not getting donor livers.

First performed by a Duke University faculty member in 2022, mitomycin intravascular chemoembolization (MICE) is a novel surgical technique that addresses the causes of corneal disease. In cases where lipid keratopathy threatens the visual axis, it eliminates corneal neovascularization (KNV), which represents a major reason for corneal blindness. The mitomycin has a direct cytotoxic effect on cultured endothelial cells of the pulmonary and umbilical vein. This results in the vascular endothelium’s irreversible destruction.
As described by MICE practitioner Kamran Riaz, MD, of the Dean McGee Eye Institute of the University of Oklahoma, the procedure is an effective alternative to treatment pathways such as antivirals, steroids, cautery, argon laser, and subconjunctival bevacizumab (Avastin). Unfortunately, these are either ineffective or have high recurrence rates.
MICE, by contrast, works well as a primary method of eliminating KNV or as a preparatory procedure for corneal transplant, conferring much higher success rates for the latter surgery when performed some months later. In many cases MICE, while technically challenging, saves vision and avoids the need for corneal transplantation altogether.

Been a minute, but I am taking a breath.
Side gig accomplished. I have some breathing room before the next one.
Also, some medical shit. People who have had lung infections - flu, COVID, bronchitis - commonly develop nodules. These nodules can resolve on their own or not. Other infections such as Valley Fever and tuberculosis can also cause them. I have had one of these since 2009, and it decided to suddenly grow after my last COVID infection. So, on Friday, after a month of waiting, I got up on a CT scan table and a very nice lung surgeon put very large needles into my upper left chest. These needles were full of lidocaine. He then took five samples from the 8mm nodule with a needle that I got one look at and promptly closed my eyes.
He said that he got around 90% just with the samples, and that the biopsy results will be in on Wednesday. He does not think that it is standalone lung cancer, but if it is, it was in situ and a chemoembolization will take care of it. It is unlikely to be related to my previous cancer because that DNA would have shown up in my monthly tests.
I've been sleeping a lot, catching up on lost sleep, and resting as ordered. No lifting anything over 10lb for a week. I'll post results as soon as I have them. Here, have a kitty.
