HAPTOGLOBIN – THE BIOMARKER FOR HAEMOLYTIC ANAEMIA
Haptoglobin is a protein produced by the liver that the body uses to clear free hemoglobin (found outside RBCs) from circulation. It’s an iron-containing protein complex that transports oxygen throughout the body. It functions to bind the free plasma hemoglobin, which allows degradative enzymes to gain access to the hemoglobin, at the same time preventing loss of iron through the kidneys & protecting the kidneys from damage by hemoglobin. Haptoglobin-hemoglobin complex is quickly cleared from the bloodstream & removed from the body by your liver.
Haptoglobin normal range in serum is 32-205 mg/dL. If your levels are lower, it means you may have haemolytic anemia, in which your RBCs are prematurely destroyed. An undetectable level is almost always due to haemolytic anemia. Autoimmune haemolytic anemia where RBCs are breaking apart or being destroyed prematurely. A condition is considered idiopathic when its cause is unknown. Such chronic intravascular haemolysis causes persistently low haptoglobin concentration.
Decreased levels of haptoglobin are also found in sickle cell anemia & thalassemia, liver disease, congenital deficiencies, acute malaria, severe liver disorder. Haemolytic anemia is a disorder that happens when your RBCs are destroyed faster than they can be replaced.
Besides, regular strenuous exercise may cause sustained low haptoglobin, presumably from low-grade haemolysis.
Elevated haptoglobin levels are seen in diseases associated with elevated ESR such as infection, trauma, hepatitis, amyloidosis, collagen diseases, coronary diseases, TB, Hodgkin’s disease, nephritic syndrome, lymphoma & leukaemia, obstructive or biliary diseases as well as prolonged steroid use.
Clinicians will ask for the test when you have symptoms like pale skin, cold hands & feet, jaundice, or yellowing of the skin & whites of the eyes, upper abdominal pain, dizziness, light headedness, shortness of breath, or abnormal heartbeat etc.
Jaundice occurs as a result of high bilirubin, the yellow pigment that forms when RBCs are broken down & eliminated from the body. When RBCs are destroyed at an increased rate, it can lead to a build-up of bilirubin in the blood, causing skin or eyes to appear yellow. Higher levels of bilirubin can also indicate gallstones, the hard deposits formed in the gallbladder obstructing the bile ducts.

WORLD HEART DAY – ‘USE HEART TO CONNECT EVERY HEART’
World Heart Day is celebrated annually on September 29, to increase the awareness of cardiovascular diseases (CVD) & its potential menace; it’s promoted by World Heart Federation in collaboration with WHO.
Globally, heart disease cases nearly doubled from 271 million in 1990 to 523 million in 2019 & heart disease deaths rose from 12.1 million to 18.6 million, mostly from ischemic heart disease & stroke, claiming around 32% of deaths in toto.
As per reports, Global Burden of Disease study estimate of age-standardised CVD death rate of 272 per 100 000 population in India is higher than the global average of 235 per 100000 population. CVD risk appeared to be highest in South India including Goa. Smoking, diabetes, high blood pressure, obesity are the underlying reasons.
This burden can be attributed to underlying genetic predisposition, metabolic deregulation, cardiomyopathy & increasing consumption of red meats/saturated fats & trans fats/junk foods as well as higher stress in sedentary lifestyle. It’s estimated that people with heart failure lose nearly 10 years of life compared to those without heart failure.
In India, certain facets of the CVD epidemic are real causes of great concern, owing to its accelerated occurrence, early age of disease onset, the higher fatality rate. The epidemiological transition from primarily infectious disease to noncommunicable diseases has occurred over a short time. In India, more than 17 Lakh people die every year due to heart diseases & by 2030, the figure is expected to increase with 2.3 crore. Around 26 percent of total deaths in India occur due to non-communicable diseases, largely referred to the heart diseases & diabetes.
Fortunately, the principal causes of CVD are modifiable factors like lack of exercise, smoking & a poor unbalanced diet. WORLD RABIES DAY – VACCINATION PREVENTS RABIES 100%
World Rabies Day is observed to create awareness on rabies prevention. It’s observed globally to mark the death anniversary of Louis Pasteur, who developed the first rabies vaccine.
The scare for preventive vaccination is galore in the communities still leading to vaccine hesitancy & the misconceptions about vaccination. It’s our duty to share the facts to ensure post-exposure prophylaxis, to protect everyone & to eliminate the menace of rabies by 2030. Mortality & morbidity is seen in humans & animals, but the impact is highest in human beings.
This year’s World Rabies Day theme is: “Rabies: Facts, not Fear”. The theme helps us to emphasise the necessity of vaccination. We need to convince the fact that 99% of dog bites are fatal, yet it’s a 100% preventable disease through vaccination.
On a dog/cat/bat bite, wounds should be washed & flushed immediately with soap & water for 10–15 minutes. If soap not available, flush with water alone. This is the most effective first-aid treatment against rabies. Wounds be cleaned thoroughly with 70% alcohol/ethanol or povidone-iodine, if available.
The incubation period of rabies ranges from a few days to several months. it is compulsory to start treatment, keep the biting dog/cat under 10 days of observation. Single-dose vaccines are available, but they only provide immunity for a limited period.
Till late 90s, goat brain vaccines were used, which manifested rare reactivities. Modern rabies vaccines are as human diploid cell vaccine, purified Vero cell rabies vaccine, purified chick-embryo cell vaccine and purified duck embryo vaccine, which are very safe. The standard schedule is five doses on days 0, 3, 7, 14 and 30. All modern rabies vaccines can be given to pregnant women or lactating mothers also. Immunosuppressive drugs (steroids, chloroquine, anti-cancer drug) are contraindicated during vaccination.
For all category III bites, Rabies Immunoglobulin (RIG) should be given immediately after the incident. RIG should be infiltrated as much as possible in and around all wounds. After infiltration of the wounds, if there is any remaining RIG, it should be given intramuscularly.
TAILPIECE: Be responsible by vaccinating pet dogs or cats against rabies. Pets and livestock should be vaccinated against rabies at age of 3 months, 9 months, then yearly once. Puppies, dogs must be given anthelmintic drugs regularly before vaccination. If any animal bites you, keep the animal under observation for 10 days.
