Catheterization in Parkinson's
In a small number of patients, detrusor overactivity is not the underlying issue, but rather, detrusor underactivity, with incomplete emptying of the bladder. This issue, if not treated, can predispose a person with PD to frequent urinary tract infections. In this situation, intermittent catheterization may be required, or for some, a permanent catheter may be advised. There is also an internal prosthesis for women that can help an underactive bladder. Your doctor will be able to discuss and determine the most suitable option for you, or your loved one.

Long Term Catheters
I have used Foley Catheters since the summer of 2021 when my Parkinson’s and a somewhat enlarged prostate began to badly affect my bladder continence and the rest of my urinary muscles. I have a new one inserted by a health professional every 28 days or so. Sometimes I manage my own insertions which I prefer.
I am most often fitted with a Bardia 14 French Infection Control (I.C. silver coated) 10 cc Balloon Foley Catheter by the doctor but will drop to a size 12 French I.C. when I anticipate going for a long bicycle ride. When I absolutely cannot afford any leaks, I wear a size 16 French Bardia Infection Control Foley. If I get sick or otherwise am going to be housebound, I use an 18 French I.C. Foley. The rationale according to Bardia is:
12F
Ideal For: Pediatric patients or adults with narrow urethras.
Usage: Suitable for patients requiring a smaller diameter catheter to prevent urethral trauma during vigorous activity
14F
Ideal For: Adult females and some males.
Usage: Commonly used for routine urinary catheterization, balancing comfort and drainage efficiency.
16F
Ideal For: Most adult males and bedridden females
Usage: The most commonly used size for general purposes, providing efficient drainage with minimal discomfort and very low rate of around-catheter leakage.
18F
Ideal For: Bedridden Adult-males with a higher urine output.
Usage: Used when larger volumes of urine need to be drained, or for patients with larger urethras. From time-to-time, I will wear a Bardia or Medline 14 or 16 French 100% silicone Foley for long-term (60-90 days without a change) management of my incontinence. I often wear this type of catheter during the summer months while I am on the go. Silicone catheters are bio-neutral and have a larger effective drainage lumen than latex catheters so that they are less likely to become blocked or develop biofilms. The Bardia brand is for the most part the most comfortable and reliable catheter brand I have worn and continue to use. Medline catheters are a close second choice and are less expensive.
