Bashir does pad his uniforms- especially the shoulders. The first time he did it he just felt so good about his new body shape and those few people who still mistook him for a girl didn't anymore. He doesn't hate looking in the mirror anymore.

After the Dominion war, everyone goes their own ways. All except Julian who finds out that no one accepts his transfer request. Despite the fact he's stuck as CMO of DS-9, he gets the occasional promotion. Mostly so Starfleet can insist they aren't discrimating against him. While Julian has consisered leaving Starfleet abd simply taking off to do his doctoring as an individual, he hasn't. He is "The Augment Doctor of DS-9" and tgat means many augments have found him. They sometimes want the damage of their augmentation reversed and sometimes they just want to talk. Many aren't publically augments and Julian gives them tips to keep under the radar.

What even is the spleen?

Function

Similar in structure to a large lymph node

Acts primarily as a blood filter. 

Removes old red blood cells and holds a reserve of blood, which can be valuable in case of hemorrhagic shock

recycles iron 

as a part of the mononuclear phagocyte system, it metabolizes haemoglobin removed from senescent red blood cells (erythrocytes). 

 synthesizes antibodies in its white pulp and removes antibody-coated bacteria and antibody-coated blood cells by way of blood and lymph node circulation. 

Structure

The spleen contains two main regions of tissue called white pulp and red pulp.

Red pulp: Contains venous sinuses (cavities filled with blood), and splenic cords (connective tissues containing red blood cells and white blood cells).

White pulp: Mostly consists of immune cells (T cells and B cells).

Pathology

Enlarged Spleen (Splenomegaly): caused by eg viral mononucleosis (“mono”), liver disease, blood cancers (lymphoma and leukemia), etc

Ruptured spleen: can cause life-threatening internal bleeding 

Sickle cell disease:  inherited anaemia, abnormal red blood cells block the flow of blood through vessels and can lead to organ damage, including damage to the spleen. 

Thrombocytopenia (low platelet count): enlarged spleen sometimes stores excessive numbers of the body’s platelets. 

Accessory spleen: About 10% of people have a small extra spleen. This causes no problems and is considered normal.

It is possible to live without a spleen, but the individual will be more susceptible to various pathologies including infection.

What if the reason Julian requested to be on DS-9 was because he thought if he was ever revealed to be an Augment, someplace that practiced 'frontier' medicine would be more likely to keep a 'defective/illegal' doctor then a Starship which everyone is vying for a space on?  Do you think Julian regrets the fact he has to lie and keeps mentally comparing himself to his father, who he hates, and also lies for a living?

Those journals he gave Jadzia were mostly fabricated to make him look normal and none of its true to him but it's true to this idea a lot of people have of the wide eyed, naive, Starfleet doctor.  He hates them.

The reason he freaked out when Jadzia put his name in for that medical honor was b/c he doesn't like people paying attention/looking too closely.  And if he won, becoming the youngest ever to- well, his career could possibly end there.  There was no doubt in his mind that he’d never deserve that honor.

Julian has mastered the art of appearing to want to be in the limelight when he'd really rather be running the other way.  And he tends to chase unavailable women b/c then he doesn't have to worry about actual attachments where he could potentially hurt them with the fact he's a serial liar (and also probably terrified of having biological kids).
