I'm training him to take medicine from a syringe and he's really nonchalant and subtle about it

My current rig does essentially the same thing, Im using a 5 star y cinch that attaches to the rear dee at a similar angle. I don't see a problem with the rigging in the diagram, cowboys in my area call it a "v rig", you just need to have a long latigo on both sides to be able to do it
